1.Application of"integration of four dimensions"teaching mode in the un-dergraduate compulsory education on"Healthcare-associated Infection Control"based on KANO model

Objective To investigate the application of the"integration of four dimensions(mainline teaching-on-line course-medical case-mind map)"teaching mode in the undergraduate compulsory teaching course"Health-care-associated Infection Control",and provide reference for further improving the design of undergraduate compul-sory course on infection control.Methods A questionnaire survey on undergraduate students' satisfaction for com-pulsory course"Healthcare-associated Infection Control"was conducted using KANO model.A total of 4 dimen-sions and 21 quality indicators were set up.KANO attribute classification,satisfaction degree,and importance coef-ficients etc.were analyzed,and curriculum design was optimized.Results The overall questionnaire reliability Cronbach's a coefficient was 0.915,and the validity analysis Kaiser-Meyer-Olkin(KMO)measure of sampling adequacy value was 0.867.Among the 21 quality indicators,12 were charismatic attributes,which accounted for the largest proportion(57.14%)of the total indicators.Most quality indicators received high student satisfaction ratings.The indicators with the highest satisfaction coefficients were"playing teaching videos in class"(4.73),along with"in-tegrating typical healthcare-associated infection cases into the curriculum for relevant teaching""maintaining a relax-ed and pleasant teaching atmosphere",and"humorous and witty teaching style of the teacher"(all scoring 4.71).Four important but currently with low satisfaction indicators were"combining course content with utilitarian exam preparation""adopting a completely offline teaching format""adopting relatively strict assessment methods",and"reflecting differentiation based on difficulty coefficient in final assessment".Conclusion This course has achieved certain efficacy in undergraduate compulsory education,but there is still room for improvement in the setting of cur-riculum assessment methods.In the future,the course system should be integrated,the assessment mode of combi-ning theory and practice should be optimized,and course improvement and innovation should be promoted.
2.Molecular epidemiological characteristics of clinical isolates of CRKP resistant to ceftazidime/avibactam

OBJECTIVE To investigate the molecular epidemiological characteristics of clinical isolates of carbapen-em-resistant Klebsiella pneumoniae(CRKP)resistant to ceftazidime/avibactam(CZA).METHODS From Jan.2022 to Dec.2023,totally 63 strains of non-repetitive CZA-resistant CRKP that were isolated for the first time from hospitalized patients of the First Affiliated Hospital of Nanchang University were enrolled in the study,and 50 strains of CZA-sensitive CRKP were randomly chosen as the research subjects.The drug susceptibility rates of the strains were observed.The drug resistance genes,virulence genes and capsular serotypes of the strains were detected by means of polymerase chain reaction(PCR).The molecular epidemiological characteristics of the strains were observed by using molecular biological techniques such as pulse field gel electrophoresis(PFGE)and multilocus sequence typing(MLST).RESULTS Among the CZA-resistant CRKP strains,45(71.43%)were iso-lated from sputum.The result of drug susceptibility testing showed that the drug resistance rates of the CZA-re-sistant strains to amikacin,aztreonam and minocycline were lower than those of the CZA-sensitive strains(P<0.05),and the drug resistance rates of the CZA-resistant strains to tigecycline was higher than that of the CAZ-sensitive strains(P<0.05).The carrying rates of blaKPC,blaNDM,blaSHV-1,blaTEM-1,blaCTX-M,blaqnrS,blaacc(6')-Ib and blarmtB genes of the CZA-resistant strains were relatively high.Among the detected capsular serotypes,K64(n=18,28.57%)was dominant.ST11(n=25,39.68%)was predominant strain among the CZA-resistant CRKP strains.CONCLUSIONS ST11 is dominant among the CZA-resistant CRKP strains.The strains carry with multiple drug resistance genes and virulence genes.The drug resistance rate of the CZA-resistant strains to tigecycline is higher than that of the CZA-sensitive strains,and it is necessary to attach great importance during the clinical treatment.
3.Economic burden of healthcare-associated infection in patients with severe acute pancreatitis:a study based on propensity score matching

Objective To study the economic burden caused by healthcare-associated infection(HAI)in patients with severe acute pancreatitis(SAP),and provide theoretical basis for formulating HAI prevention and control measures.Methods Patients with SAP discharged from a tertiary first-class hospital in Jiangxi Province from July 1,2023 to June 30,2024 were selected as the study subjects.Information including demographic characteristics,clinical data,and hospitalization expense were collected.Patients were divided into a HAI group and a non-HAI group according to HAI occurrence.A propensity score matching(PSM)method was used to conduct a 1∶2 ma-tching,and differences in the length of hospital stay and hospitalization expense between the two groups of patients after PSM were compared.Results A total of 709 patients were included in the analysis,out of which 65 cases ex-perienced HAI,with a HAI incidence of 9.17%.After PSM,all 65 patients in the HAI group were successfully matched.The length of hospital stay,total hospitalization expense,expenses of medication and hygiene product of patients in the HAI group were all higher than those in the non-HAI group,and differences were all statistically sig-nificant(all P<0.001).Patients who experienced≥2 episodes of HAI had a higher economic burden than those who experienced only once(P<0.05).HAI of bloodstream,abdomen,digestive system,and respiratory system significantly increased the economic burden of patients(all P<0.05).Conclusion HAI in SAP patients can extend the length of hospital stay and increase economic burden of patients.Targeted infection prevention and control mea-sures should be formulated to reduce the incidence of HAI and save medical resources.

7.Clinical evaluation of efficacy and safety of omadacycline in the treatment of patients with community acquired pneumonia

Objective To evaluate the efficacy and safety of the omadacycline-based therapies in patients with community-acquired pneumonia (CAP).Methods The clinical data of CAP patients treated with omadacycline (±β-lactam/β-lactamase inhibitor) in the First Affiliated Hospital of Nanchang University from January to May 2023 were reviewed and analyzed.The patients were assigned to omadacycline alone or omadacycline plus β-lactam/β-lactamase inhibitor treatment group.The clinical efficacy,microbiological efficacy,and drug-related adverse events were summarized and compared between groups.Results A total of 135 adult patients with CAP were enrolled,including 23 (17.04%) patients with chronic liver disease,25 (18.52%) patients with chronic kidney disease,and 64 (47.41%) patients treated with omadacycline alone.The overall clinical efficacy rate was 81.48% (110/135),specifically 79.69% (51/64) in the patients treated with omadacycline alone and 83.10% (59/71) in the patients treated with omadacycline plus β-lactam/β-lactamase inhibitor.The clinical efficacy rate was 86.36% (38/44) in the patients treated in the general ward.Overall,21 cases of drug-related adverse events (mainly nausea) were reported in 15 patients,all of which were mild to moderate.Conclusions Omadacycline has shown good therapeutic effect in treatment of CAP in Chinese adults,especially for the patients treated in general ward.Most of the adverse events of omadacycline were mild to moderate and tolerable.
8.Study of new ultrasound technology combined with cluster analysis on prediction method of liver-heart integration in patients with liver fibrosis

Objective:To evaluate the early assessment of hepatocardiac integration based on ultrasonic elasticity and blood flow vector imaging (VFM) technology, in conjunction with unsupervised cluster analysis and supervised machine learning methods.Methods:An observational research design without any intervention was adopted from December 2021 to September 2022, 45 patients with liver cirrhosis, 43 patients with liver fibrosis, and 42 healthy volunteers were selected from the First Affiliated Hospital of Harbin Medical University. Liver combined elasticity technology and VFM technology were used to obtain information on the liver and heart of the subjects, respectively. The acquired data were standardized, and then clustered using topological data analysis (TDA) technology on the processed data. Subsequently, the clustering results were evaluated based on statistical analysis, and finally, supervised multi-classification tasks were realized through machine learning methods.Results:Patients were stratified into five distinct groups based on a network of patient similarities. The average characteristics of each group were as follows: Group 1 exhibited the most severe hepatocardiac conditions relative to the other groups. Groups 2 and 3 displayed moderately severe conditions.In contrast, Group 4 comprised entirely of healthy controls, all of whom presented with normal hepatocardiac function. Group 5 presented a unique case among the categories.Participants in this group showed poor liver conditions. However, according to the guidelines for cardiac diastolic function assessment, their heart function was generally unremarkable, with only a minority of indicators deviating significantly. Support Vector Machine (SVM), Random Forest Tree (RFT), and Multilayer Perceptron (MLP) were employed for multi-classification tasks on the test dataset. The average accuracies achieved by these models were 70%, 81%, and 84%, respectively.Conclusions:By combining liver combined ultrasonic elasticity, cardiac VFM technology and TDA technology to construct a patient similarity network, we successfully identified patients with liver fibrosis who did not show abnormalities in conventional cardiac indicators but may have potential abnormal cardiac function, which has important implications for guiding the selection of clinical intervention measures, and optimizing patient management stratification.
