Objective To evaluate the feasibility and safety of cap-assisted endoscopic nylon loop ligation (C-ENLL) as a new and simple method on gastric fundus submucosal tumors. Methods 74 cases with small gastric fundus submucosal tumors ≤2.00 cm in diameter were reviewed between January 2015 and June 2016. All cases were treated by C-ENLL. The clinical efficacy was analyzed. Results All the 74 patients underwent endoscopic ultrasonography before operation, 70 cases originated from the muscularis propria, 3 cases originated from the muscularis mucosae, 1 case originated from the submucosa. The average diameter of the lesions ranged 0.50 ～ 1.80 cm. C-ENLL achieved an en bloc resection rate of 100.0％, with a mean total procedure time of 26 min. Two patients developed delayed perforation, were treated with nylon rope and metal clip purse suture wound. All of whom were managed successfully. There was no delayed bleeding after operation. Pathological examination showed that 66.2％ (49/74) of the tumors were gastrointestinal stromal tumors. No tumor recurrence was observed during the follow-up. Conclusion The C-ENLL may be a feasible and safe method for the treatment of small gastric fundus submucosal tumors.

Objective To retrospectively evaluate the clinical and radiographic outcomes of immediately loaded full-arch fixed prostheses with All-on-4 concept after 1 year and 3 years of function.Methods A total of 40 dental implants were placed in ten edentulous patients(3 in maxilla,7 in mandible).Follow-up examinations were performed at 6 months,12 months,and thereafter every 12 months.Radiographic assessment of the marginal bone level was performed after 1 and 3 years in function.The marginal bone resorption was compared between tilted and axial implants.Results The success rate of implants was 100％ (40/40) and 97％ (37/38) after 1 and 3 years,respectively.The success rate of prostheses were both 100％.37 after 1 and 3 years.The marginal bone resorption was,on average,(0.96 ±0.25) and (1.48 ± 0.20) mm after 1 and 3 years,respectively.The difference of marginal bone resorption between tilted implants and axial implants was not statistically significant (P ＞ 0.05) after 1 year or after 3 years.Implants presented heahhy peri-implant soft tissue conditions.Conclusions The clinical outcome of edentulous immediate rehabilitation supported by 4 implants was satisfactory after 1 and 3 years in function.

Objective To explore the correct treatment methods after indwelling catheter incarceration in burn children.Methods Twenty-six burn children with indwelling catheter were analyzed according to their conditions and locations of incarceration,classified and treated effectively.The two cases catheterization incarcerated in the membranous urethra,twenty-two cases incarcerated in the bladder and two cases incarcerated in the internal orifice of urethra.The different catheter incarcerations were respectively handled with guide wire penetration method,acupuncture and surgical operation method.Results Twenty-six burn children could removed catheter after treatment,among them,nineteen cases with guide wire penetration method,four cases with acupuncture,three cases with surgical operation method which was removed of foreign body through perineal urethrotomy.Twenty-six burn children were better recovery after operation.Again indwelling catheter happened in one patient after operation,indwelling catheter was removed after two weeks,and the patient was discharged.Conclusions Nursing staff should skillfully master physiological and anatomical characteristics of male patients,the structure,know performance,operation and notes of balloon urethral catheter,and correctly grasp the decannulation method of the balloon catheter.

OBJECTIVETo explore the correlation between single nucleotide polymorphisms (SNPs) of interleukin-28B (IL-28B) gene and the susceptibility to primary hepatocellular carcinoma (HCC).

METHODSA total of 300 histologically confirmed HCC cases (from November 2001 to April 2010) and 310 healthy controls with no history of chronic hepatitis B or hepatocellular carcinoma (2009-2010) were selected from a hospital in Guilin and a hospital in Beijing for this case-control study.139 HCC patients in the case group had complete clinical tracking data. All the subjects were Han Chinese, with no age or gender restrictions.2 ml peripheral blood samples were drawn from each subject with informed consent. SNP of rs12972991, rs4803223, rs8099917 and rs12979860 four loci in IL-28B gene were analyzed by matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF).

RESULTSThe frequencies of C allele at rs12972991, G allele at rs8099917 and G allele at rs4803223 were 6.7% (40/598), 7.9% (47/598) and 10.0% (59/588) respectively in case group; all higher than the corresponding frequencies in control group, separately 2.9% (18/618), 4.1% (25/616) and 3.6% (21/608). The differences were statistically significant (χ2=9.542, 7.858, 20.736, P values all<0.05). The above alleles could increase the risk of HCC, and the OR (95%CI) values were separately 1.67 (1.13-2.46), 1.49 (1.08-2.06) and 2.91 (1.79-4.72). The genotype frequencies of AC+CC at rs12972991, GT+GG at rs8099917, GA+GG at rs4803223 were 13.0% (39/299), 14.7% (44/299) and 19.0% (56/296) respectively in case group; while the frequencies were lower in control group, separately 5.8% (18/309), 8.1% (25/308) and 6.6% (20/304). The differences were statistically significant (χ2=9.319, 6.557, 20.948, P values all<0.05). These genotypes may increase the risk of HCC, and the adjusted OR (95%CI) values were 2.24 (1.31-3.83), 1.81 (1.14-2.88) and 2.90 (1.78-4.70), respectively. The stratified analysis of the clinical data indicated that the frequency of genotype GA+GG at rs4803223 was 50.0% (13/26) in patients of tumor thrombosis in portal vein (TTPV), higher than the frequency of genotype AA (21.1%, 23/109). The difference was statistically significant (χ2=8.965, P=0.003).

CONCLUSIONThe results suggested that IL-28B gene polymorphisms was correlated to the susceptibility to HCC in Chinese Han ethnic population. Among them, GA + GG genotype at rs4803223 could increase the risk of TTPV in HCC patients.

Objective To investigate the effect of sterilization and cost of different packaging materials and packing patterns,with a view to provide evidences for looking for packing materials with low-cost and perfect sterilization effect.Methods Five kinds of packaging materials and eight kinds of packaging methods were used to pack goods which was sent to sterilize after packing.Then examined part of packs on 14,21,30,60,90,150,180 days after sterilizing,respectively.The effect of sterilization,preservation time and cost were compared among the eight groups.Results All of the eight packing methods had reached the sterile effect.Preservation time of medical creps paper,medical nonwoven fabrics,medical three-dimensional paper bag and paper plastic package all of which had sterilized was longer than cotton package.One time packing of doubledeck cotton was compared with monolayer medical crepe paper,one time of packing of medical nonwoven fabrics,medical three-dimensional paper bag and paper plastic package for the cost,respectively.the differences were significant (t=27.9,25.3,22.7,respectively; P ＜ 0.01 ).Two times of packing of doubledeck cotton was compared with medical crepe paper,two times of packing of medical nonwoven fabrics,medical three-dimensional paper bag and paper plastic package for the cost,respectively,the differences were obvious (t=24.5,28.2,26.4,respectively;P＜0.01).Conclusions Disposable medical crepe paper type,medical non-woven type,three-dimensional medical bag type,paper and plastic packaging has good resistance bacteria sterilization effect,the packaging costs are significantly lower than the cotton packing,it has an absolute advantage as re-sterilization packaging of medical devices.

Objective To explore pefioperative nursing for patients with benign prostatic hyperplasia who received greenlight photoselective vaporization.Methods Totals of 160 cases patients with BPH underwent PVP treatment were recruited.Before operation,the nursing plan and mental nursing were finished.During and after the operation,the condition changes of patients were carefully observed,and complications prevention and nursing were well done.Results The patients' preoperative emotion was stable,and all operations went smoothly and successfully.And no obvious complications was found and all patients were cured.Conclusions The good perioperative nursing care can ensure the success of operation and promote the rehabilitation of patients.
