Objective: To examine the clinical characteristics and prognostic factors of elderly patients with mantle cell lymphoma (MCL) and the impact of nutrition and underlying diseases on the prognosis of elderly patients with MCL. Methods: retrospectively analyzed 255 elderly patients with MCL from 11 medical centers, including Peking University Third Hospital between January 2000 and February 2021. We analyzed clinical data, such as age, gender, Mantle Cell Lymphoma International Prognostic Index score, and treatment options, and performed univariate and multivariate prognostic analysis. We performed a comprehensive geriatric assessment on elderly MCL patients with medical records that included retraceable underlying disease and albumin levels, and we investigated the impact of basic nutrition and underlying disorders on MCL prognosis in the elderly. Results: There were 255 senior individuals among the 795 MCL patients. Elderly MCL was more common in males (78.4%), with a median age of 69 yr (ages 65-88), and the majority (88.6%) were identified at a late stage. The 3-yr overall survival (OS) rate was 42.0%, with a 21.2% progression-free survival (PFS) rate. The overall response rate (ORR) was 77.3%, with a 33.3% total remission rate. Elderly patients were more likely than younger patients to have persistent underlying illnesses, such as hypertension. Multivariate analysis revealed that variables related with poor PFS included age of ≥80 (P=0.021), Ann Arbor stage Ⅲ-Ⅳ (P=0.003), high LDH level (P=0.003), involvement of bone marrow (P=0.014). Age of ≥80 (P=0.001) and a high LDH level (P=0.003) were risk factors for OS. The complete geriatric assessment revealed that renal deficiency was associated with poorer OS (P=0.047) . Conclusions: Elderly MCL patients had greater comorbidities. Age, LDH, renal function, bone marrow involvement, and Ann Arbor stage are all independent risk factors for MCL in the elderly.

Objective: To understand the prevalence of chronic kidney disease (CKD) and related factors in adults in Anhui province based on the data of Chinese Chronic Diseases and Nutrition Surveillance program (2018) in Anhui. Methods: Multi-stage stratified cluster random sampling was used to select participants aged ≥18 years. Moreover, questionnaire survey, body measurements and laboratory tests were conducted. The complex weighting method was used to estimate the prevalence of CKD in residents with different characteristics, and complex sampling data logistic regression model was used for multivariate analysis to identify related risk factors. Results: A total of 7 181 participants were included. The overall prevalence of CKD was 11.06% in adults in Anhui, and the prevalence was 12.49% in women and 9.59% in men (P<0.05). The moderate, high and very high risk for CKD progression were 8.66%, 2.02% and 0.38%, respectively. Multivariate analysis showed that age (OR=1.03, 95%CI: 1.00-1.05), BMI (OR=1.05, 95%CI: 1.01-1.09), being woman (OR=1.38,95%CI: 1.22-1.55), hypertension (OR=2.50, 95%CI: 1.76-3.56), diabetes (OR=2.28, 95%CI: 1.51-3.43), dyslipidemia (OR=1.26, 95%CI: 1.11-1.43) and hyperuricemia (OR=2.16, 95%CI: 1.68-2.78) were risk factors for CKD. Conclusion: The prevalence of CKD in adults in Anhui was relatively high and age, gender, BMI, hypertension, diabetes, dyslipidemia and hyperuricemia were found to be associated with the prevalence of CKD. To prevent CKD and its complications, attention should be paid to the management of related risk factors, including overweight and obesity, hypertension, diabetes, dyslipidemia and hyperuricemia.

AIM: To assess the astigmatism-correcting efficiency and clinical effects of cataract phacoemulsification combined with bifocal Toric intraocular lens(IOL)implantation. METHODS: Retrospective analysis. The clinical data of 46 patients(46 eyes)with cataract complicated with regular corneal astigmatism by the treatment of cataract phacoemulsification and bifocal Toric IOL implantation in our hospital from August 2020 to September 2021 were included. The patients were followed up for 3mo after operation, and the changes of uncorrected distant visual acuity(UDVA), uncorrected near visual acuity(UNVA), best corrected distant visual acuity(BCDVA), best corrected near visual acuity(BCNVA)and astigmatism before and 1, 3mo after operation were evaluated. The IOL axial rotation was measured and calculated, and a questionnaire was conducted to investigate the necessity of using glasses at different distances and overall satisfaction.RESULTS:After operation at 1 and 3mo, there were significant differences in UDVA, BCDVA, UNVA and BCNVA compared with those before operation(all P<0.001), and there was no significant difference in UDVA, BCDVA, UNVA and BCNVA at 1mo after operation and 3mo after operation(all P>0.0167). At 3mo after operation, 46 eyes(100%)of UDVA reached 0.20(LogMAR), 40 eyes(87.0%)of UNVA reached 0.20(LogMAR). Astigmatism vector analysis showed that the mean preoperative corneal astigmatism in this group was 1.88±0.70D, and the centroid value was 0.61D@177°±1.93D, the mean residual astigmatism at 3mo postoperatively was 0.33±0.30D, and the centroid value was 0.03D@34°±0.45D. After operation at 3mo, the axial rotation of IOL was 3°(0°, 5°). Only 5 eyes(11%)required some degree of refractive correction for near or intermediate distances. 83%(38 eyes)were satisfied or very satisfied with the surgical results CONCLUSION: The implantation of bifocal Toric IOL during cataract surgery could effectively correct corneal regular astigmatism, improve uncorrected distance and near vision, and had high patient satisfaction.
9. The Long Chain Acyl-coenzyme A Synthetase Family and Malignant Tumors

Chinese Journal of Biochemistry and Molecular Biology 2022;38(7):875-884
Disorders of the fatty acid metabolism can lead to cancer. The long chain acyl-coenzyme A synthetase (ACSL) family is important in fatty acid metabolism and is responsible for activating long chain fatty acids. In cancer cells, the regulatory effect of ACSLs is often disrupted, and the distribution, type, and quantity of fatty acids are altered. These alterations can lead to cancer development and other metabolic diseases. ACSLs include five subtypes in mammals, namely ACSL1, 3, 4, 5, and 6. ACSL1 is important in the synthesis and distribution of triglycerides. ACSL3 contributes to the formation of lipid droplets, which are important for maintaining lipid homeostasis. The expression of ACSL4 is related to steroid hormones and plays an important role in ferroptosis. ACSL5 can catalyze the metabolism of exogenous fatty acids but not the metabolism of de novo fatty acids. ACSL6 is important in fatty acid metabolism in the brain, spermatogenesis, and ovary. The regulatory factors of ACSLs include transcription factors, coactivators, hormone receptors, protein kinases, and small non-coding RNAs. These factors regulate mitochondria-mediated energy metabolism, endoplasmic reticulum stress, and the tumor inflammatory microenvironment through fatty acid metabolism. In addition, ACSLs serve as independent prognostic factors, biomarkers for clinical diagnosis, and therapeutic targets for various cancers. In recent years, accumulating evidence has demonstrated the important roles of ACSLs in the occurrence and development of cancer. This article focuses on the ACSL family, the relationship between ACSL and malignant tumors, and tumor therapies based on lipid metabolism by ACSLs. The information provides a theoretical basis for the further study of the ACSL family as molecular candidates for the targeted therapy of tumors.
