1.Traumatic ectopic testis with torsion:A case report and literature review

Objective:To investigate the clinical diagnosis,treatment and prognosis of traumatic ectopic testis with torsion.Methods:We retrospectively analyzed the clinical data on a case of traumatic ectopic testis with torsion and reviewed relevant litera-ture.Results:After diagnosed with traumatic ectopic testis with torsion,the patient underwent exploratory operation for confirmation of orchiocatabasis,followed by testicular reduction and fixation.Follow-up visit at 1 month after surgery showed good blood supply and no obvious testicular atrophy.Conclusion:Traumatic ectopic testis with torsion is an extremely rare emergency condition,for which color Doppler ultrasonography is an effective means of examination.Once suspected of or confirmed with the problem,the patient should receive exploratory surgery,testicular reduction and fixation within 6 hours,and close postoperative observation.
2.Targeted surveillance results of healthcare-associated infection in the liver transplantation intensive care unit from 2018 to 2022

Objective To analyze the characteristics of healthcare-associated infection(HAI)in patients in liver transplantation intensive care unit(ICU),and provide basis for the effective prevention and control of liver post-transplantation infection.Methods Targeted surveillance data of HAI in liver transplantation ICU from 2018 to 2022 were analyzed retrospectively.Incidence,incidence trend,infection site,pathogens and drug resistance were analyzed.Results A total of 3 762 liver transplantation patients were surveilled,106 patients developed 133 cases of HAI,with an incidence of 2.82%and a case incidence of 3.54%.There was no significant difference among the years(P=0.473).Infection mainly occurred within 2 weeks after admission to ICU,accounting for 85.85%.The main infection sites included blood system(26.32%),respiratory system(22.56%),and surgical site(19.55%).The average utilization rates of central veinous catheterization,urethral catheterization,and ventilator were 85.77%,70.58%,and 40.83%,respectively.The incidences of central line-associated bloodstream infection(CLABSI),catheter-associated urinary tract infection(CAUTI),and ventilator-associated pneumonia(VAP)were 0.54‰,0.33‰,and 1.84‰,respectively.A total of 131 strains of pathogens were detected,of which Gram-negative bac-teria accounted for 38.17%and Gram-positive bacteria accounted for 29.77%.The top three pathogens were Kleb-siella pneumoniae(15.27%),Enterococcus faecium(11.45%),and Acinetobacter baumannii(9.16%).Conclusion Effective prevention and control measures should be taken based on the characteristics of HAI in the liver transplan-tation ICU,so as to curb bacterial resistance and reduce liver post-transplantation HAI.

Antibiotic-associated diarrhea(AAD)in children is a type of diarrhea that occurs after the use of antibiotics in children,and its pathogenesis is closely related to the intestinal flora.The medication of antibiotics can affect the metabolic function of the intestinal flora and the immune function of the body,and then leads to the occurrence of AAD.In the view of Chinese medicine,AAD in children is mainly involved the spleen,and the etiology of the disease is due to the weakness of the spleen and stomach of the body constitution together with the attack of the pestilential pathogen and the accumulation of drug toxin.The pathogenesis of ADD in children is characterized by spleen deficiency with predominant dampness,deficiency of spleen qi,and insufficiency of spleen yang.Spleen deficiency is the root cause of pediatric AAD,and spleen and intestinal flora have commonality,so the treatment of pediatric AAD can be performed from the perspective of the spleen.The treatment of pediatric ADD from the spleen follows the principle of strengthening and activating the spleen,and the regulation of the spleen for achieving the purpose of treating the disease from the root can be achieved by the methods of strengthening spleen and draining dampness,strengthening spleen and replenishing qi,and strengthening spleen and warming yang separately with the fundamental prescriptions of Shenlin Baizhu Powder,Sijunzi Decoction,and Fuzi Lizhong Pills.

Objective:To analyze the effect of ultra-early enteral nutrition intervention on the prognosis of patients after severe craniocerebral injury surgery.Methods:A prospective study was conducted with 154 patients who underwent surgery for severe craniocerebral injury at the Neurosurgery Department of Qinhuangdao First Hospital.Patients were randomly divided into an experimental group and a control group using a random number table.Both groups received postoperative care in the Intensive Care Unit(ICU)with tracheal intubation retained within the postoperative 6 hours,and the head of the bed was elevated after 6 hours.The experimental group started ultra-early enteral nutritional support 6 hours after surgery,while the control group began early enteral nutritional support 24 hours after surgery.Total protein(TP),hemoglobin(Hb),albumin(ALB),prealbumin(PA),interleukin-6(IL-6),and C-reactive protein(CRP)were detected before intervention and on the 7th day post-intervention.The incidence of gastrointestinal complications,hospital-acquired infections,and prognostic indicators were recorded.Results:On the 7 days after intervention,TP,Hb,ALB and PA levels in the experimental group were significantly higher than those in the control group(P<0.05),and IL-6 and CRP levels were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of abdominal distension,diarrhea,vomiting,and aspiration between the two groups(P>0.05).However,the incidence of hospital-acquired infections,ICU stay duration,total hospital stays,and mortality rate were all lower in the experimental group compared to the control group(P<0.05).Conclusion:Ultra-early enteral nutrition intervention can effectively improve the nutritional status and prognosis of patients after severe craniocerebral injury surgery.
7.Vascularization of intravitreal injection of Conbercept in the treatment of retinopathy of prematurity

AIM: To evaluate retinal vascularization caused by the intravitreal injection of Conbercept in the treatment of a series of retinopathy of prematurity(ROP)cases in Type Ⅰ(threshold and pre-threshold period)and aggressive ROP(A-ROP).METHODS: The data of 34 ROP cases(67 eyes)treated by intravitreal injection of Conbercept(IVC)in the ophthalmology department of the Xiamen Children's Hospital from July 2017 to March 2020 were retrospectively analyzed. Reactivation, which refers to recurrence of acute phase features, occurred at any stage of the disease in the presence or absence of other diseases. RESULT: The average gestational age of the 34 children was 28.82±2.32wk. The average birth weight was 1155.18±398.22g. The lesion zone of 19 cases(37 eyes)was Zone Ⅰ. In 10 cases(20 eyes), the lesion was in Zone Ⅱ, and in 5 cases(10 eyes), the lesion was in the posterior Zone Ⅱ. The total effective rate of disease control in ROP children treated with once IVC was 73.1%(49/67), and the vascularization of Zone Ⅱ was completed. The patients showed variable changes in the vascularization in Zone Ⅲ. For the patients who received one treatment and did not reactivate, the average rate of Type Ⅰ vascularization of ROP was 9.11±2.49wk, and the A-ROP was 13.40±4.04wk. The rate of A-ROP vascularization in Zone Ⅱ was significantly longer compared to Type Ⅰ.CONCLUSION: IVC effectively completes vascularization in Zone Ⅱ.
8.Clinical trial of labetalol combined with low molecular weight heparin in the treatment of patients with hypertensive disorders in pregnancy

Objective To explore the curative effect of labetalol combined with low molecular weight heparin on hypertensive disorders in pregnancy(HDP).Methods HDP patients were divided into control group and treatment group.The control group was treated with labetalol 100 mg orally,q12 h,and the treatment group was subcutaneously injected with low molecular weight heparin 4 000 U,qd,on the basis of the control group.Both groups were treated until 1 d before delivery.The systolic blood pressure(SBP),diastolic blood pressure(DBP),plasma prothrombin time(PT),thrombin time(TT),fibrinogen(Fib),D-dimer(D-D),peak systolic velocity/end diastolic velocity(S/D),24 h urinary protein(24 h-PRO),serum placental growth factor(PLGF),soluble vascular endothelial growth factor receptor-1(sFlt-1),safety evaluation and pregnancy outcomes were compared between the two groups before and after 2 weeks of treatment.Results The treatment group and the control group included 29 and 33 cases,respectively.After treatment,the total effective rates in treatment group and control group were 93.10%(27 cases/29 cases),72.73%(24 cases/33 cases),with statistical difference(P<0.05).After treatment,the S/D of treatment group and control group were 1.61±0.18 and 1.73±0.19;SBP were(102.35±8.64)and(119.47±9.90)mmHg,DBP were(80.34±4.67)and(71.24±4.29)mmHg;Fib were(3.11±0.36)and(3.87±0.39)g·L-1;D-D levels were(1.32±0.40)and(1.75±0.53)mg·L-1;PT were(13.05±0.71)and(11.92±0.89)s;TT were(18.95±1.80)and(16.83±1.94)s;24h-PROwere(1.19±0.05)and(2.35±0.07)g;serum sFlt-1 levels were(1 528.05±102.33)and(1 901.92±131.47)pg·mL-1;the serum PLGF observation indexes were(149.33±12.19)and(125.68±9.17)pg·mL-1,respectively,and there were statistically significant differences between the treatment group and the control group(all P<0.05).The incidence of fetal distress,placental abruption,premature delivery and neonatal asphyxia in treatment group and control group were 9.09%,3.03%,12.12%,6.06%and 24.14%,20.69%,31.03%,27.59%,respectively,with statistical significance(all P<0.05).The adverse drug reactions in the treatment group were headache and dizziness in 3 cases,nausea and vomiting in 1 case,insomnia in 2 cases,and the adverse drug reactions in the control group were headache and dizziness in 2 cases,nausea and vomiting in 1 case,insomnia in 1 case.The total incidence of adverse drug reactions in treatment group and control group was 20.69%and 12.12%,respectively,with no statistical significance(P>0.05).Conclusion Labetalol combined with low molecular weight heparin can effectively improve curative effect,regulate sFLt-1/PLGF signaling pathways and improve pregnancy outcomes in HDP patients.
9.Effects of propiconazole on physiological and biochemical properties of Panax notoginseng and dietary risk assessment.

To study the residue and dietary risk of propiconazole in Panax notoginseng and the effects on physiological and bioche-mical properties of P. notoginseng, we conducted foliar spraying of propiconazole on P. notoginseng in pot experiments. The physiolo-gical and biochemical properties studied included leaf damage, osmoregulatory substance content, antioxidant enzyme system, non-enzymatic system, and saponin content in the main root. The results showed that at the same application concentration, the residual amount of propiconazole in each part of P. notoginseng increased with the increase in the times of application and decreased with the extension of harvest interval. After one-time application of propiconazole according to the recommended dose(132 g·hm~(-2)) for P. ginseng, the half-life was 11.37-13.67 days. After 1-2 times of application in P. notoginseng, propiconazole had a low risk of dietary intake and safety threat to the population. The propiconazole treatment at the recommended concentration and above significantly increased the malondialdehyde(MDA) content, relative conductivity, and osmoregulatory substances and caused the accumulation of reactive oxygen species in P. notoginseng leaves. The propiconazole treatment at half(66 g·hm~(-2)) of the recommended dose for P. ginseng significantly increased the activities of superoxide dismutase(SOD), peroxidase(POD), and catalase(CAT) in P. notoginseng leaves. The propiconazole treatment at 132 g·hm~(-2) above inhibited the activities of glutathione reductase(GR) and glutathione S-transferase(GST), thereby reducing glutathione(GSH) content. Proconazole treatment changed the proportion of 5 main saponins in the main root of P. notoginseng. The treatment with 66 g·hm~(-2) propiconazole promoted the accumulation of saponins, while that with 132 g·hm~(-2) and above propiconazole significantly inhibited the accumulation of saponins. In summary, using propiconazole at 132 g·hm~(-2) to prevent and treat P. notoginseng diseases will cause stress on P. notoginseng, while propiconazole treatment at 66 g·hm~(-2) will not cause stress on P. notoginseng but promote the accumulation of saponins. The effect of propiconazole on P. notoginseng diseases remains to be studied.

10.Early changes within the lymphocyte population are associated with the long term prognosis in severely injured patients.

OBJECTIVE:
To investigate the relationship between early lymphocyte responses and the prognosis in severely injured patients.
METHODS:
Consecutive patients with severe trauma who were treated in Peking University People's Hospital Trauma Medical Center between June 2017 and June 2020 were enrolled in this restropective chart-review study. According to the responses of lymphocyte after severe injury, the patients were divided into three groups, group 1: lymphopenia-returned to normal; group 2: persistent lymphopenia; group 3: never lymphopenic, and the outcome of 28 d were recorded. Clinical data such as gender, age, base excess, mechanism of injury, Glasgow coma scale (GCS), injury severity score (ISS) and massive blood transfusion were collected. Perform statistical analysis on the collected clinical data to understand the trend of lymphocyte changes in early trauma and the relationship with prognosis. In order to eliminate the interference of age, stratification was carried out according to whether the age was ≥ 65 years old, in different age groups, they were grouped according to whether the length of stay was ≥ 28 d, and the relationship between lymphocyte trend and length of stay was discussed.
RESULTS:
A total of 83 patients were included, 66 males and 17 females. The main injury mechanisms were traffic accident injuries and high-altitude fall injuries. The average ISS was (30±11) points. 65 patients had lymphopenia on the day of injury, 32 of them returned to normal on the 5th day, and the rest did not recover; the other 18 patients had normal lymphocyte levels after injury. Patients which are failure to normalize lymphopenia within the first 5 days following admission was related with the long hospitalization time and higher 28 d mortality rate. After further stratification by age, failure to normalize lymphopenia within the first 5 days following admission in the elderly group (age ≥65 years) was a risk factor for prolonged hospital stay (≥28 d), P=0.04. While in younger group, a high level of neutrophils within the first 5 d following admission was a risk factor for bad outcome.
CONCLUSION
A failure to normalize lymphopenia in severely injured patients is associated with significantly higher mortality and longer hospital stay. This study reveals lymphocytes can be used as a reliable indicator for the prognostic evaluation.
