1.Value of multiple diffusion weighted imaging models in predicting the efficacy of neoadjuvant-treated locally advanced gastric cancer

Objective To investigate the value of quantitative parameters of diffusion weighted imaging(DWI)based on mono-expo-nential model(MEM),diffusion kurtosis imaging(DKI)model,and stretched-exponential model(SEM)in predicting the efficacy of neoadjuvant therapy in locally advanced gastric cancer(LAGC).Methods Forty LAGC patients who underwent MRI examinations before neoadjuvant therapy and before radical surgery were prospectively enrolled.A radiologist delineated lesions on DWI images and acquired quantitative parameters before and after treatment,including lesion volume,apparent diffusion coefficient(ADC)of MEM,mean diffusivity(MD)and mean kurtosis(MK)of DKI model,distribution diffusion coefficient(DDC),and α of SEM.According to pathological tumor regression grade(TRG),the patients were stratified into good response group(TRG 0-1)and poor response group(TRG 2-3).The pre-treatment parameters and Δ of pre-and post-treatment parameters were compared between the two groups with Mann-Whitney U test;multivariate analysis was performed with binary logistic regression.Multiple DWI models and the combined model were established,and the prediction efficiency of each model was calculated.Results There was no significant differ-ence in each parameter before neoadjuvant therapy between the two groups(P>0.05).The delta of volume,ΔADC,ΔMD,and ΔDDC pre-and post-treatment were all statistically different between the two groups(P<0.05).The area under the curve(AUC)of ΔADC,ΔMD,and ΔDDC in predicting good response for LAGC were 0.900,0.806,and 0.762,respectively.The AUC of the combined model was 0.946.Conclusion Quantitative parameters of MEM,DKI model,and SEM can help predict the efficacy of neoadju-vant-treated LAGC patients.

3.Construction of Traditional Chinese Medicine Constitution Classification Program from a Multidimensional Perspective

By sorting out the literature related to constitution and referring to the concept and method of "element", many ancient and modern theories of constitution are deconstructed into multi-dimensional "constitution elements". These constitution elements are divided into two levels: basic and derivative. The basic level is the most basic element of constitution, including zang-fu organs/body and material. The zang-fu organs/body dimension focuses on the fixed functional units in the human body, and the material dimension focuses on the rheology functional units of human body. The derivative level is the secondary physiological or psychological state in life activities, including emotion and qi transformation. The emotional dimension focuses on the state of human psychological activities, and the qi transformation dimension focuses on the state of human physiological activities in cold, heat, dampness, and dryness. Based on this, we can further study the influence of factors such as age, gender, region, and social class, on the combination rule of "constitution element", so as to form a more comprehensive classification program of constitution.
4.Severe liver injury caused by Hehe pills (和合丸)

A 64-year-old female patient with bilateral knee joint pain received Hehe pills 10 g twice daily by herself. After 2 months of treatment, she stopped taking the medicine. On the 10th day after discontinuation of medication, yellowish skin, mucosa, and sclera appeared. Laboratory tests showed alanine aminotransferase 274 U/L, aspartate aminotransferase 274 U/L, alkaline phosphatase 713 U/L, total bilirubin 412.7 mmol/L, direct bilirubin 216.2 mmol/L. Severe liver injury caused by Hehe pills was considered. Symptomatic and supportive treatments including liver protection and jaundice reduction, and bilirubin adsorption were given. One month later, the yellowish skin, mucosa, and sclera in the patient were significantly improved, and 2 months later, liver function returned to normal. The patient′s liver injury was most likely related to the Aconiti Kusnezoffii Radix, Aconiti Radix, Olibanum, and Myrrha contained in Hehe pills.

Plague, infectious disease in modern medicine, refers to a type of disease with strong pathogenicity and infectiousness, it refers to the infectious diseases of western medicine. Due to its wide variety, the knowledge and understanding of plagues of Traditional Chinese Medicine (TCM) doctors in different stages have evolved and developed with the times. This article, via collating ancient documents, differentiatesthe classification in TCM and analyzes itsrelated theories to perfect the type of plagues in TCM, providing the theoretical basis for the research of plague in modern times.
