1.Quality analysis and comparison of portable blood glucose meters in Zhejiang Province

Objective:We conducted a quality analysis and a survey of current situation of portable blood glucose meters from hospitals and patient sources in Zhejiang Province..Methods:The study is a retrospective cross-sectional study that included 252 portable blood glucose meters, of which 35 portable blood glucose meters (4 in provincial hospitals, 29 in municipal hospitals, and 2 in county-level hospitals) selected for the "Zhejiang Province Portable Blood Glucose Meter Flight Quality Control Inspection" project in April 2023, 126 portable blood glucose meters collected for the "Zhejiang Hospital Portable Blood Glucose Meter Whole Hospital Comparison" project in January 2024, and 91 portable blood glucose meters collected from patients in the "Zhejiang Hospital Home Portable Blood Glucose Meter Convenience Service" project from 2021 to 2023. By comparing the blood glucose results with the reference analyzer, evaluate the qualification rate of portable blood glucose meters between hospitals and patients, imported and domestic brands, and analyze the distribution of blood glucose concentration intervals for unqualified portable blood glucose meters. Conduct a survey questionnaire on the use of portable blood glucose meters by patients to analyze their trust in the results, quality control execution rate, comparison awareness rate, and comparison execution rate.Results:The overall qualification rate of portable blood glucose meters in hospitals (95.7%) was higher than that of portable blood glucose meters use by patients (70.3%), and the difference was statistically significant ( χ 2=31.94 , P<0.001). Among the unqualified blood glucose meters, the unqualified blood glucose concentration is mainly concentrated in the high concentration range (65.9%). Among imported brands, the qualification rate of blood glucose meters from hospitals(99.3%) is higher than that from patients(74.0%), and the difference is statistically significant ( χ 2=36.97, P<0.001). Meanwhile, 75% of patients fully trust the detection results of the blood glucose meter, but its instrument qualification rate is 69%. The awareness rate of patients regarding the comparison of portable blood glucose meters is 73%, but the execution rate of quality control and standard comparison of portable blood glucose meters by patients is 0. Conclusion:The quality of portable blood glucose meters in Zhejiang Province is uneven, with the highest pass rate for portable blood glucose meters from hospitals. Patients have weak awareness of quality control and comparison of portable blood glucose meters.
2.Effect of neuroendoscopy-assisted microsurgical vascular decompression under combined monitoring of abnormal muscle response and brainstem auditory evoked potential in the treatment of hemifacial spasm

Objective To investigate the effect of neuroendoscopy-assisted microsurgical vascular decompression(MVD)on hemifacial spasm(HFS)under the combined monitoring of abnormal muscle response(AMR)and brainstem auditory evoked potential(BAEP),and its impact on serum myelin basic protein(MBP)and neuron-specific enolase(NSE).Methods 110 patients with HFS from January 2019 to December 2023 were prospectively selected and divided into a control group(n=55)and study group(n=55)according to the random number table method.The control group received neuroendoscopy-assisted MVD treatment,while the study group received neuroendoscopy-assisted MVD treatment under the combined monitoring of AMR and BAEP.The therapeutic effects,the levels of serum MBP and NSE,facial nerve function grading,electrophysiological parameters of facial nerve(compound muscle action potential,blink reflex wave amplitude R1,R2,and R2'),quality of life[hemifacial spasm scale(HFS-8)score],and sleep quality[Pittsburgh sleep quality index(PSQI)score]before and after surgery,and complication rates of the two groups were compared.Results The total effective rate of the study group was 94.55%(52/55),which was higher than that of the control group(80.00%,44/55),the difference was statistically significant(P<0.05).At 1 and 3 d after operation,the levels of serum MBP and NSE in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).At 6 months after surgery,the facial nerve function grading of the study group was better than that of the control group,the blink reflex wave amplitudes R1,R2 and R2'were lower than those of the control group,and the compound muscle action potential was higher than that of the control group,the differences were statistically significant(P<0.05).At 6 months after surgery,the HFS-8 score and PSQI score of the study group were lower than those of the control group,the difference was statistically significant(P<0.05).The difference in the incidence rate of complications between the two groups was not statistically significant(P>0.05).Conclusion The combined monitoring of AMR and BAEP under neuroendoscopy-assisted MVD treatment of HFS patients has significant effects,improving patients'neurological function,improving their sleep quality and quality of life,and has high safety.

Objective To evaluate the role of preoperative serological indexes in predicting long-term survival and tumor recurrence of hepatocellular carcinoma (HCC) patients after liver transplantation, aiming to explore its significance in expanding the Milan criteria. Methods Clinical data of 669 recipients undergoing liver transplantation for HCC were retrospectively analyzed. The optimal cut-off value was calculated by the receiver operating characteristic (ROC) curve. The risk factors affecting the overall survival and recurrence-free survival rates of HCC patients after liver transplantation were identified by univariate and multivariate regression analyses. The correlation between preoperative serum liver enzymes and pathological characteristics in HCC patients was analyzed. The predictive values of alpha-fetoprotein (AFP) combined with γ -glutamyl transferase (GGT) and different liver transplant criteria for the survival and recurrence of HCC patients after liver transplantation were compared. Objective:To investigate the influence of effects of transarterial chemoembo-lization (TACE) before liver transplantation on the prognosis of hepatocellular carcinoma.Methods:The retrospective cohort study was conducted. The clinicopathological data of 311 hepatocellular carcinoma patients undergoing TACE before liver transplantation who were admitted to the Third Medical Center of Chinese PLA General Hospital from January 2005 to December 2012 were collec-ted. There were 276 males and 35 females, aged from 47 to 59 years, with a median age of 52 years. All the 311 patients underwent TACE before liver transplantation. Observation indicators: (1) effects of hepatocellular carcinoma patients undergoing TACE and its relationship with clinicopathological factors; (2) follow-up; (3) influencing factors for prognosis of hepatocellular carcinoma patients after liver transplantation. Follow-up was conducted using outpatient examination or telephone interview to detect recurrence and metastasis of tumor and survival and graft loss of patients up to December 2017. The patients were followed up every 2 to 4 weeks within 3 months after liver transplantation, and once every 1 to 3 months thereafter. Measurement data with normal distri-bution were represented as Mean± SD, and comparison between groups was analyzed using the t test. Measurement data with skewed distribution were represented as M(range) or M( Q1, Q3), and comparison between groups was analyzed using the Mann-Whitney U test. Count data were described as absolute numbers or percentages, and comparison between groups was analyzed using the chi-square test. Comparison of ordinal data was analyzed using the nonparametric rank sum test. The COX regression model was used for univariate and multivariate analyses. The Kaplan-Meier method was used to draw survival curves and calculate survival rates, and the Log-rank test was used for survival analysis. Results:(1) Effects of hepatocellular carcinoma patients undergoing TACE and its relationship with clinicopathological factors. Of the 311 patients undergoing TACE, 57 cases had pathologic complete response (pCR) and 254 cases had pathologic partial response (pPR), respectively. Cases with alpha fetoprotein (AFP) <20 μg/L,20?400 μg/L, >400 μg/L, cases with microvascular invasion, cases with tumor number as single nodule, cases with tumor distribution at right lobe of liver, cases with tumor caliber of feeding artery (CFA) >1 mm were 26, 26, 5, 51, 6, 43, 46 in patients with pCR, versus 87, 64, 103, 158, 59, 125, 159 in patients with pPR, showing significant differences in the above indicators ( Z=3.35, χ2=4.54, 15.71, 12.89, 6.79, P<0.05). (2) Follow-up. All the 311 patients were followed up for 47.0 to 59.0 months, with a median follow-up time of 44.6 months. There were 11 cases undergoing tumor recurrence and 11 cases undergoing tumor metastasis in the 57 patients with pCR, and there were 96 cases undergoing tumor recurrence and 66 cases under-going tumor metastasis in the 254 patients with pPR. The 1-, 3-, 5-year tumor recurrence free rates were 98.2%, 91.1%, 80.3% in the 311 patients, respectively. The 1-, 3-, 5-year tumor recurrence free rates were 100.0%, 91.1%, 80.3% in the 57 patients with pCR, versus 82.0%, 68.4%, 59.4% in the 254 patients with pPR, showing significant differences in the above indicators ( χ2=13.47, P<0.05). Cases with graft loss were 11 and 96 in the 57 patients with pCR and the 254 patients with pPR, respectively, showing a significant difference ( χ2=7.06, P<0.05). (3) Influen-cing factors for prognosis of hepatocellular carci-noma patients after liver transplantation. Results of univariate analysis showed that gender, basic diseases as viral hepatitis C, AFP (20?400 μg/L, >400 μg/L), Milan criteria, microvascular invasion, tumor number, tumor distribution, tumor CFA, times of TACE, effects of TACE were related factors influencing prognosis of hepatocellular carcinoma patients after liver transplantation ( hazard ratio=0.49, 3.97, 1.78, 1.84, 2.41, 1.96, 3.00, 1.76, 0.19, 2.01, 3.07, 95% confidence interval as 0.30?0.81, 2.23?7.05, 1.03?3.06, 1.18?2.85, 1.63?3.56, 1.28?3.01, 2.04?4.40, 1.20?2.59, 0.13?0.28, 1.28?3.14, 1.63?5.76, P<0.05). Results of multi-variate analysis showed that AFP >400 μg/L, exceeding Milan criteria, tumor number as multiple nodule,effects of TACE as pPR were independent risk factors influencing prognosis of hepatocellular carcinoma patients after liver transplantation ( hazard ratio=1.59, 2.06, 1.99, 2.05, 95% confidence interval as 1.22?2.07, 1.35?3.13, 1.29?3.07, 1.02?4.10, P<0.05) and tumor CFA >1 mm was an independent protective factor influencing prognosis of hepatocellular carcinoma patients after liver transplantation ( hazard ratio=0.10, 95% confidence interval as 0.05?0.19, P<0.05). Conclusions:The effects of TACE are related to AFP, microvascular invasion, tumor number, tumor distribution and tumor CFA. AFP >400 μg/L, exceeding Milan criteria, tumor number as multiple nodule,effects of TACE as pPR are independent risk factors influencing prognosis of hepatocellular carcinoma patients after liver transplantation and tumor CFA >1 mm is an independent protective factor influencing prognosis of hepatocellular carcinoma patients after liver transplantation.
7. Microcystic, elongated and fragmented invasive pattern in endometrial adenocarcinoma: a clinicopathologic analysis of 72 cases

Objective:
To investigate the clinicopathologic features of microcystic, elongated and fragmented (MELF) pattern invasion of endometrial adenocarcinoma.
Methods:
HE and immunohistochemistry staining method were used to analysis morphologic features and immunophenotype of 72 patients of endometrial adenocarcinoma with MELF pattern invasion, and chi-square test was used to analysis the clinicopathologic features.
Results:
The mean age of 72 patients was 54 years (40 to 70 years). Thirty-two patients were pre-menopausal and 40 were post-menopausal. According to the FIGO staging system (2014), 32 cases(44.4%)were at stage Ⅰ, 22 cases(30.6%)at stage Ⅱ, 17 cases(23.6%)at stage Ⅲ and 1 case(1.4%) at stage Ⅳ. Microscopically, MELF invasion showed microcystic, elongated slit-like or fragmented glands in myometrium and their lining cells usually were cube or flat, as well as the single or clusters of eosinophilic tumor cells mimicking histocytes. In addition, a fibromyxoid or inflammatory stromal response was often present.Immunohistochemical staining showed that MELF invasion was positive for p16, CA125 and CA19-9, but negative for ER, PR and p53.Compared with non-MELF pattern invasion, significant differences were noted in menopause pausimenia, FIGO stages, deep invasion into myometrium, lymph metastasis, lymphovascular space invasion (LVSL), serum CA125 and CA19-9 in patients with MELF pattern invasion (all
8.Clinical living-related segmental small bowel transplantation: a case report

Objective To investigate the effect of short bowel syndrome treated with living-related small bowel transplantation(SBT).Methods A male patient with residual intestine 20cm in length,which resulted from subtotal small bowel resection and right hemi-colectomy owing to intestinal volvulus,received a living-related SBT.The donor was the patient′s mother.Donor specific blood transfusion,50mL/per week,was carried out for 8 weeks.Cytomegalovirus infection status in both donor and recipient was negative.A 160cm segment of intestine was transplanted.The graft ileocolic artery and vein was anastomosed to the recipient′s infrarenal aorta and inferior vena cava end-to-side,respectively.A distal ileostomy was performed.(Immunosuppression),anti-infection and anticoagulation therapy,and nutritional support were given(postoperatively).Results The donor had an uneventful recovery.No technical complications were observed.The recipient was alive and well 31 weeks after operation.No graft rejection or infection was found.The(patient) was taken off TPN 8 weeks after operation,and got a low-fat meal.The result of D-xylose test was near normal.Conclusions Living-related small intestine transplantation is an effective treatment for short bowel syndrome.
9.Humoral immune responses in rabbits with an experimental SARS-CoV inactivated vaccine

Objective:To evaluate the immunogenicity of an experimental SARS-CoV inactivated vaccine.Methods:The virus suspension of F69 strain was inactivated with 0.4% formaldehyde and purified,then used as the immune antigen combined with Freund′s adjuvant.Eight adult New Zealand rabbits were immunized 4 times with this vaccine.12 sets of rabbit serum were sampled from the third day to 74th day after first immunization.Titers of specific IgG antibody and neutralizing antibody were determined by indirect ELISA and micro-cytopathic effect neutralizing test.Results:Rapid and potent humoral immune responses were induced by F69 inactivated vaccine in all eight immunized rabbits.Both specific IgG antibody and neutralizing antibody all peaked just with 2 vaccinations,with the maximum titer of 1∶81 920 and 1∶20 480 respectively about 6 weeks after first immunization.Across neutralizing reaction existed between F69 and Z2-Y3 strains.Conclusion:F69 inactivated vaccine owns strong immunogenicity.Similar antigenic epitopes exist between the F69 strain and Z2-Y3 strain,which ensured the cross neutralizing reaction.
