1.Application Value and Influencing Factors of Endoscopic Ultrasonography in Staging Early Upper Gastro-intestinal Tumors
Chinese Journal of Gastroenterology 2025;30(1):52-55
In recent years, the incidence and mortality rates of gastrointestinal tumors have been continuously increasing worldwide. The treatment and prognosis of gastrointestinal tumors are closely related to the depth of tumor invasion and lymph node metastasis. Therefore, early and accurate tumor staging is of critical importance. However, the diagnostic accuracy of endoscopic ultrasonography(EUS)in assessing the depth of tumor invasion remains controversial. This article reviewed the application value and influencing factors of EUS in staging early upper gastrointestinal tumors, aiming to provide useful references for clinical practice.

Objective To explore the impact of early discharge guidance, based on an intelligent education system,on postoperative pain management, functional recovery, and discharge readiness in lung cancer patients. Methods A total of 170 lung cancer patients were selected from Jinhua Municipal Central Hospital from July 2022 to June 2023.According to the time of admission, patients admitted between July to November 2022 were assigned to control group(n=82),while patients admitted between December 2022 to June 2023 were assigned to observation group(n=88).Control group received routine perioperative health education, while observation group received additional intelligent education system.Following outcomes were compared between two groups:Time to first ambulation after surgery, pain numeric rating scale(NRS)scores on postoperative 1 to 3 days and on day of discharge, performance on 2-minute step test on day of discharge, and readiness for hospital discharge scale(RHDS).Results Time to first ambulation post operative in observation group was shorter than that in control group.NRS scores at 72 hours post operative and on day of discharge were lower in observation group compared to control group.Number of steps completed in 2-minute step test in observation group was higher than that in control group.Additionally,RHDS scores in observation group was higher than that in control group, and all differences were statistically significant(P<0.05).Conclusion The discharge planning advancement driven by the intelligent education system significantly improved early postoperative ambulation, reduced postoperative pain, enhanced physical function, and increased discharge readiness in lung cancer patients, thereby effectively promoting postoperative recovery.

Journal of Practical Radiology 2024;40(3):394-397
Objective To investigate the value of multiparametric CT features for predicting the risk classification of gastric stro-mal tumor(GST).Methods The clinical data from 139 patients with GST were retrospectively collected.According to the patho-logical risk results, the patients were divided into two groups:a low-risk GST group(including very low-and low-risk)with 75 patients and a high-risk GST group(including medium and high-risk)with 64 patients. The CT features between low-risk GST group and high-risk GST group were compared using chi-squared test or t-test.The risk factors of high-risk GST were identified by univariate analysis. The prediction models were built by multivariate logistic regression. The performance of models were evaluated by receiver oper-ating characteristic(ROC)curve. Results There were significant differences in the maximum tumor diameter, minimum tumor diameter, arterial phase enhancement degree, venous phase enhancement rate, arterial phase enhancement degree rate, venous phase enhance-ment degree rate, cystic, and necrosis between low-risk GST group and high-risk GST group, which were associated with the risk classification of GST.The area under the curve(AUC)of the quantitative features-based model that combined maximum tumor diam-eter,minimum tumor diameter, arterial phase enhancement degree, venous phase enhancement rate, arterial phase enhancement degree rate and venous phase enhancement degree rate, showed a significantly higher performance than the qualitative features-based model that incorporated cystic and necrosis(0.981 vs 0.850,P<0.001).Conclusion Maximum tumor diameter, minimum tumor diameter, arterial phase enhancement degree, venous phase enhancement rate, arterial phase enhancement degree rate, venous phase enhance-ment degree rate,as well as cystic and necrosis, are associated with the risk classification of GST and can predict the high-risk GST.

The concept of'harmony'is the soul of traditional Chinese culture, which has a profound impact on the formation and development of traditional Chinese medicine(TCM).TCM is rooted in traditional Chinese culture, and the mode of thinking in TCM is in line with traditional Chinese culture.Based on the harmony culture,TCM has developed a unique view of health, disease and therapeutics. From the view of the harmony culture and by combining with years of clinical experience in treating dermatosis,Chinese medical master XUAN Guo-Wei has applied the concept of'harmony'in the TCM syndrome differentiation and treatment system in clinic, and has developed the academic thoughts of harmonizing therapy for removing toxins for the diagnosis and treatment of dermatosis. The thoughts of harmonizing therapy for removing toxins includes four aspects, namely harmonizing yin and yang, harmonizing healthy qi and pathogenic qi,harmonizing water and fire(i.e.,clod and hot),and harmonizing the administration of formula and drugs, aiming to remove toxins and expel pathogens and value the harmony. The thoughts of harmonizing therapy for removing toxins will beneficial to the comprehensive understanding of the unique health-disease-therapeutics concept in TCM,and will be helpful for managing the doctor-patient relationship, which is of enlightening significance to the modern clinical practice with TCM.

Objective:This study aims to explore the synergistic effects of DIP and other medical insurance supply-side policies.Method:City A that has piloted DIP reform was set as the treatment group, and City B without reform was set as the control group.A total of 1 120 public medical institution samples from 2019 to 2022 were collected. The total medical expenses during hospitalization and some structural expenses were analyzed using DID method.Result:DIP had a significant inhibitory effect on the medical expenses, and the expenses of checkups and examinations during hospitalization in city A,but had no impact on the drug and the material expenses during hospitalization. Conclusion:DIP played a significant cost control role and effectively controlled the total medical expenses during hospitalization. The synergistic effects of price adjustment of medical services policy and national centralized drug/material procurement policy on cost control were insufficient.DIP synergized with other supply-side policies to promote rational medical cost structure. It is suggested that medical insurance departments should focus on the synergistic effects of medical insurance supply-side policies to jointly improve the efficiency of medical insurance fund utilization.
