OBJECTIVE To evaluate the cost-effectiveness of lecanemab for early Alzheimer disease (AD) from a whole-society perspective. METHODS Based on data from the Clarity AD study, a global multicenter, randomized, double-blind, placebo-controlled study of lecanemab for early AD, a Markov model was developed according to the disease progression of AD, with a monthly cycle and a time horizon extending to 99% patient mortality. Quality-adjusted life year (QALY) and incremental cost-effectiveness ratio (ICER) were used as outcome indicators, with a willingness-to-pay (WTP) threshold set at twice China’s per capita gross domestic product (GDP) for 2025 (199 330.00 yuan/QALY). The model simulated the cost-effectiveness of lecanemab plus standard of care (SoC) versus placebo plus SoC for early AD. One-way sensitivity analysis, probabilistic sensitivity analysis, and scenario analysis were employed to test the robustness of the results. RESULTS Compared with placebo plus SoC, the ICER for lecanemab plus SoC was 4 364 711.54 yuan/QALY. One-way sensitivity analysis showed that the hazard ratio for disease progression, the price of lecanemab, body weight, the monthly progression probability of mild cognitive impairment, and the discount rate were key parameters affecting model robustness. Results of the probabilistic sensitivity analysis showed that the probability of lecanemab being cost-effective was 0 at the current WTP threshold. Scenario analysis indicated that a price reduction of at least 94.88% would be required for lecanemab to fall below the WTP threshold. CONCLUSIONS At the current WTP threshold, lecanemab plus SoC is not cost-effective compared with placebo plus SoC for early AD.

Objective To summarize and analyze the clinicopathological characteristics, imaging manifestations, treatment and prognosis of anaplastic lymphoma kinase(ALK)-positive histiocytosis(APH)involving the central nervous system(CNS),so as to enhance understanding of this rare disease. Methods A retrospective analysis was conducted on 5 cases of CNS-involved APH diagnosed in Huashan Hospital,Fudan University between 2019 and 2023.Clinical,imaging and pathological data were collected, and supplemented by immunohistochemical staining(IHC),fluorescence in situ hybridization(FISH)and high-throughput sequencing for auxiliary diagnosis and molecular characterization. The findings were summarized and integrated with previous literature for a comprehensive analysis. Results All five patients were male, with a mean age of 27.6 years,in which 2 cases exhibited multi-system involvement, while 3 cases exhibited single system involvement.Imaging revealed multiple intracranial lesions in multi-system cases and solitary lesions in single system cases, with well-defined boundaries and homogeneous enhancement.Histologically, tumor cells were intermingled with lymphocytes, displaying an alternating"light and dark"pattern in 1 case.Granuloma-like structures were observed in 4 cases, along with frequent nuclear grooves, indentations and convolutions.Tumor cells usually infiltrated surrounding tissues.IHC demonstrated that tumors expressed histiocytic markers(CD68/CD163)and ALK predominantly expressed in the cytoplasm.FISH confirmed ALK gene rearrangements in all patients, while high-throughput sequencing identified KIF5B-ALK fusions in 2 cases. All single system CNS cases achieved complete remission after surgical resection with or without adjuvant chemotherapy/ALK inhibitors.Among multi-system cases, one achieved partial remission, and one experienced relapse and progression. Conclusion CNS APH is prone to preoperative misdiagnosis. Its histopathological features,ALK immunohistochemical expression, and ALK gene fusions are critical for diagnosis.Patients with single system involvement demonstrate overall superior outcomes compared to multi-system cases.Total resection is effective for localized disease, while multi-system cases require systemic therapy.Multidisciplinary collaboration is crucial for precise diagnosis and treatment.

Objective To investigate the role and molecular mechanism of ginsenoside Rb1(Rb1)in regulating neutrophil extracellular trapping networks(NETs)to intervene in atherosclerosis(AS).Methods In vivo:an AS model was constructed with ApoE knockout mice superimposed on a high-fat diet. The pathological and morphological changes of aortic root plaques were observed by HE staining and oil red O staining;Immunofluorescence labelling of neutrophils citrullinated histones(Cit-H3)and macrophages as well as IL-1β at the aortic root plaque site were used to assess the inflammatory infiltration. In vitro:NETs induced by PMA and cholesterol crystals were taken as models respectively.Direct effect of Rb1 against NETs formation assessed by Sytox staining and immunofluorescence staining with Cit-H3 and myeloperoxidase.Rb1 on ROS levels was assessed by DCFH-DA.Rb1 on histone H3 citrulline modification was assessed by Western blotting. Results In vivo:Rb1 significantly inhibited plaque formation, lipid deposition(P<0.05)and intra-plaque inflammatory infiltration(P<0.05).In vitro:Rb1 significantly inhibited NETs formation(P<0.05),neutrophil ROS levels(P<0.05),and Cit-H3 levels(P<0.05).Conclusions Rb1 significantly inhibited AS progression by inhibiting plaque NETs formation, which may be partly through the inhibition of histone H3 citrullination resulting from activation of the neutrophil oxidative stress pathway.

Objective To investigate the relationship between sleep disorders and coronary heart disease through big data combined with Mendelian randomization analysis. Methods Data from 2005 to 2018 National Health and Nutrition Examination Survey in the United States were utilized.Logistic regression analysis was employed to evaluate the association between sleep disorders and coronary heart disease, while analyzing relevant influencing factors.A two-sample Mendelian randomization approach was implemented using Genome-Wide Association Studies to establish causal relationships. Results Logistic regression analysis demonstrated a significant association between sleep disorders and coronary heart disease(P<0.001),with the neutrophil-to-lymphocyte ratio serving as a mediating factor in this relationship(P<0.001).Mendelian randomization analysis revealed a positive correlation between sleep disorders and coronary heart disease(OR=1.030,95%CI:1.01-1.04).Conclusion Sleep disorders can increase the risk of coronary heart disease by activating inflammatory factors.
