1.Preliminary application of histological evaluation of donor pancreas biopsy tissue in simultaneous pancreas-kidney transplantation

Objective To preliminarily investigate the safety and efficacy of donor pancreas needle biopsy in simultaneous pancreas-kidney transplantation. Methods Clinical data of 7 cases undergoing donor pancreas biopsy were collected retrospectively. All cases underwent donor pancreas biopsy before or during simultaneous pancreas-kidney transplantation. Frozen section or paraffin sectioning techniques were used for tissue preparation, and hematoxylin-eosin and Masson staining were performed to histologically evaluate the donor pancreas. The quality of donor pancreas was comprehensively assessed by combining histological findings with the donor's clinical data. Postoperative follow-up data of 5 simultaneous pancreas-kidney transplant recipients were collected to summarize the safety of donor pancreas biopsy and the prognosis of transplant recipients. Results The 7 pancreas donors were aged 28 to 62 years, with a body mass index ranging from 20.76 to 27.68 kg/m2. Liver ultrasound indicated fatty liver in 3 cases, while pancreatic ultrasound did not reveal any significant abnormalities. Among them, biopsy was performed on 2 donors after completion of pancreatic procurement and processing, and the frozen section histology showed moderate acute pancreatitis changes (edema of acinar cells, necrosis and inflammatory cell infiltration). Combined with a serum amylase level elevated more than 3 times the upper limit of normal value, these two donor pancreases were finally discarded. The remaining 5 cases underwent biopsy immediately after pancreatic vascular anastomosis during simultaneous pancreas-kidney transplantation, and histological evaluation was performed on paraffin-embedded sections. No biopsy-related complications (such as bleeding, pancreatic fistula, etc.) occurred after transplantation. One recipient died of severe infection 2 months after transplantation, while the other 4 recipients were followed up for more than 5 years, with well-functioning transplant kidneys and pancreases. Conclusions Donor pancreas biopsy is relatively safe, and the risk of biopsy-related complications after transplantation is controllable. Comprehensive assessment of donor pancreas quality by combining histological evaluation with the donor's clinical indicators is conducive to improving the accuracy of donor pancreas selection and organ utilization.
2.Integrating Transcriptomics and 3D Organoids to Investigate Mechanism of Periplaneta americana Extract Against Lung Adenocarcinoma

ObjectiveTo evaluate the antitumor activity of Periplaneta americana extract(PAE) against human-derived lung adenocarcinoma organoids(LUAD-PDOs) and to elucidate its potential mechanism based on transcriptomics. MethodsFresh tumor and adjacent normal tissues from patients with LUAD were collected to construct LUAD-PDOs and normal lung organoid(Nor-PDOs) models using 3D organoid culture technology. The effective intervention concentration of PAE was determined using the cell counting kit-8(CCK-8) assay. Experimental groups included the model group(LUAD-PDOs), normal group, model administration group(LUAD-PDOs+PAE), and normal administration group(Nor-PDOs+PAE). Hematoxylin-eosin(HE) staining was used to observe the pathological structures of PDOs, immunohistochemistry(IHC) was performed to detect the expressions of the proliferation marker Ki-67 and lung adenocarcinoma differentiation markers cytokeratin-7(CK-7) and Napsin A, TUNEL staining was applied to detect cell apoptosis. RNA sequencing(RNA-Seq) was conducted to identify differentially expressed genes(DEGs), followed by Gene Ontology(GO), Kyoto Encyclopedia of Genes and Genomes(KEGG), and Gene Set Enrichment Analysis(GSEA), alongside protein-protein interaction(PPI) network analysis to screen core mechanisms. Finally, key targets were validated by integrating external database analysis with immunofluorescence(IF). ResultsNor-PDOs and LUAD-PDOs that highly recapitulated the pathological characteristics of the primary tissues were successfully established. The CCK-8 assay determined that the effective intervention concentration of PAE was 16 g·L-1. Morphological observation showed that Nor-PDOs exhibited lumen-forming structures, whereas LUAD-PDOs displayed dense, solid structures. CCK-8 and TUNEL assays revealed that, compared with the model group, PAE intervention inhibited the proliferation of LUAD-PDOs and promoted apoptosis in LUAD cells, while showing no significant effect on the viability of Nor-PDOs. Transcriptomic analysis identified 719 DEGs that were significantly reversed after PAE intervention(347 up-regulated and 372 down-regulated)(P<0.05). GO enrichment analysis indicated that DEGs in the model administration group were significantly enriched in biological processes related to cell cycle regulation compared to the model group. KEGG pathway analysis revealed that PAE affected pathways related to proliferation and metabolism, including pathways in cancer and the p53 signaling pathway. GSEA further confirmed that PAE significantly enhanced the activity of the p53 signaling pathway(P<0.05). PPI network analysis indicated that breast cancer type 1 susceptibility protein(BRCA1) and checkpoint kinase 1(CHEK1) were the core down-regulated targets in the p53 pathway. IF verified the high expression of BRCA1 and CHEK1 in LUAD-PDOs and their significant downregulation after PAE intervention(P<0.05). Furthermore, survival analysis based on The Cancer Genome Atlas(TCGA) database indicated that low expression of BRCA1 and CHEK1 was significantly associated with prolonged overall survival in patients with LUAD(P<0.05). ConclusionPAE effectively inhibits proliferation of LUAD-PDOs and promotes their apoptosis, its anti-tumor mechanism is potentially associated with the activation of the p53 signaling pathway, with BRCA1 and CHEK1 genes likely serving as key downstream targets for the effects of PAE.
3.Meta-analysis of the efficacy and safety of CD19 CAR-T cell therapy versus standard treatment for relapsed/refractory diffuse large B-cell lymphoma

[摘 要] 目的:系统评价CD19 CAR-T细胞免疫疗法对比标准治疗在复发/难治性弥漫性大B细胞淋巴瘤(R/R DLBCL)患者中疗效与安全性,通过探索性分析探索不同CAR-T细胞产品对疗效的潜在影响,为临床治疗决策提供循证参考。方法:计算机检索中国知网、万方数据库、维普期刊数据库、中国生物医学文献数据库、PubMed、Embase和Cochrane Library数据库,搜集CD19 CAR-T细胞疗法对比标准治疗用于R/R DLBCL的随机对照试验(RCT),检索时限均为建库至2025年10月25日。由2位研究者独立进行文献筛选、数据提取和质量评价,采用R4.2.2软件进行Meta分析。结果:共纳入2项Ⅲ期RCT研究(ZUMA-7、TRANSFORM),各结局指标均根据异质性检验结果选择固定效应模型合并数据。疗效方面,CD19 CAR-T细胞疗法可显著改善患者无事件生存期(HR=0.455,95%CI:0.363~0.570,P < 0.001)和降低死亡风险(HR = 0.738,95%CI:0.575~0.947,P = 0.017);同时可显著提高完全缓解率(RR = 1.879,95%CI:1.574~2.242,P < 0.001)。按产品类型的探索性分析显示,liso-cel和axi-cel均优于标准治疗(liso-cel:HR = 0.380,95%CI:0.260~0.540,P < 0.001;axi-cel:HR = 0.510,95%CI:0.380~0.680,P < 0.001),但该分析为不同试验间的对比,证据等级有限。安全性结局显示:CAR-T细胞疗法的免疫效应细胞相关神经毒性综合征(ICANS)发生率显著升高(RR = 22.387,95%CI:4.353~115.132,P < 0.001);≥3级细胞因子释放综合征(CRS)发生率数值升高(RR = 8.181,95%CI:0.935~71.574,P = 0.058),差异无统计学意义。纳入研究的偏倚风险整体为低;敏感性分析证实结果稳健。结论:基于2项RCT的Meta分析结果,CD19 CAR-T细胞(liso-cel/axi-cel)可作为R/R DLBCL二线治疗的选择之一,其疗效优于标准治疗,且特征性不良反应(CRS/ICANS)经规范管理后可控,可根据患者基线状态个体化选择CAR-T细胞产品。本研究证据基础薄弱,上述结论尚需更多高质量、大样本RCT验证。
4.Hypotension prediction index in the prediction of better outcomes: a systematic review and meta-analysis

Background:
The hypotension prediction index (HPI) is an algorithm designed to predict hypotension. Some studies have reported that HPI-guided hemodynamic management strategies decrease intraoperative hypotension and complications; however, the effect of HPI on reducing perioperative complications is controversial. This meta-analysis aimed to assess the efficacy of the HPI in reducing major complications and intraoperative hypotension.
Methods:
We conducted this meta-analysis according to the PRISMA statement and Cochrane Handbook guidelines. A comprehensive literature review was conducted to identify studies focusing on the efficacy of HPI-guided management in reducing intraoperative hypotension and postoperative complications. The PubMed, Embase, Scopus, and Web of Science databases were searched, and the resulting data were combined to calculate the pooled mean differences or risk ratios (RRs) with 95% CIs of both randomized controlled trials (RCTs) and retrospective studies, as appropriate. Heterogeneity and potential publication bias were also assessed.
Results:
Nineteen articles (12 RCTs and 7 retrospective studies) with 2570 recruited patients were included in this meta-analysis. The critical evaluation of the study quality revealed a low risk of bias in the included RCTs. Among the non-randomized trials, one was rated 7, two were rated 8, and the remaining four were rated 9 on the Newcastle-Ottawa Scale, indicating high quality and a low risk of bias. HPI-guided management significantly reduced intraoperative hypotension and associated major complications (RR = 0.79, 95% CI [0.69–0.90], I2 = 0, P < 0.001). Blood loss and length of hospital stay were comparable between the groups.
Conclusions
HPI-guided management significantly reduced intraoperative hypotension and major complications.
5.Comparative study on analgesic effect of bupivacaine liposome and ropivacaine on quadratus lumborum block in patients undergoing laparoscopic total hysterectomy

Objective To clarify the differences in the effects of bupivacaine liposome and ropivacaine on quadratus lumborum block in patients undergoing laparoscopic total hysterectomy.Methods A total of 70 patients who underwent laparoscopic total hysterectomy at Suqian First People's Hospital Affiliated to Nanjing Medical University from January 2023 to October 2024 were selected as the research subjects.The patients were randomly divided into the control group and the observation group,with 35 cases in each group.Before general anesthesia,patients of the control group were given ropivacaine quadratus lumborum block,while patients of the observation group were given bupivacaine liposome quadratus lumborum block.The general conditions before and after operation,visual analogue scale(VAS)scores at different time points after operation,rescue analgesia,mini-mental state examination(MMSE)scores at different time points after operation,postoperative rehabilitation and analgesic adverse reactions were compared between the two groups.Results There was no statistically significant difference in the general conditions of patients in the two groups before and after the operation(P>0.05).At 24 hours,48 hours,and 72 hours after surgery,the abdominal resting VAS scores of patients in the observation group were significantly lower than those in the control group(P<0.05).The time of the first self-controlled use of the intravenous analgesic pump after surgery in the observation group was significantly later than that in the control group(P<0.05).The number of analgesic pump compressions and the rate of oxycodone rescue analgesia in the observation group were significantly fewer/lower than those in the control group within 48 hours after surgery(P<0.05).At 24 hours,48 hours,and 72 hours after surgery,the MMSE scores of patients in the observation group were significantly higher than those in the control group(P<0.05).The time for patients to get out of bed and move around after surgery and the recovery time of anal exhaust in the observation group were significantly earlier than those in the control group(P<0.05).There was no statistically significant difference in the incidence of postoperative analgesic adverse reactions between the two groups of patients(P>0.05).Conclusion Compared with the use of ropivacaine for quadratus lumborum block in patients undergoing laparoscopic total hysterectomy,bupivacaine liposome can provide a longer-lasting analgesic effect,reduce patients'dependence on intravenous analgesia,be conducive to protecting patients'cognitive function,accelerate the recovery of patients without increasing the incidence of adverse reactions.
6.The prevalence of hyperhomocysteinemia and its influencing factors in the Tibetan population at ultra-high altitude area in Nagqu City,Xizang

Objective To investigate the prevalence of hyperhomocysteincmia(HHcy)in a Tibetan population in Nagqu City and analyze the related influencing factors.Methods From May to June 2020 we recruited 758 hereditary Tibetan residents from Xinji Township,Jiaqiong Township and Seshong Township of Nagqu City as study subjects who met the inclusion criteria using the multi-stage whole cluster random sampling method.Structured questionnaires as well as clinical examinations were conducted.The prevalence of HHcy was analyzed by descriptive statistics,and the effects of different variables on the prevalence of HHcy were assessed using univariate analysis.Multifactorial Logistic regression analysis was made to explore the factors associated with the prevalence of HHcy.The expected value of each influencing factor in predicting the prevalence of HHcy was analyzed using the subject's receiver operating characteristic(ROC)curve.Results The number of HHcy among the 758 Tibetan population included amounted to 515 cases with a prevalence of 67.9%(95%CI:64.6%-71.2%).Gender,hypertension,hyperuricemia,hemoglobin abnormality,LDL-C abnormality,BMI,type of residence and duration of exercise were found to be significantly different between the two groups by univariate analysis(P<0.05).In the multifactorial Logistic regression analysis,age,hemoglobin abnormality,diastolic blood pressure,hemoglobin volume,heart rate,HDL-C and BMI showed a positive correlation with the prevalence of HHcy,whereas gender,blood glucose abnormality,FBG,ALT,type of residence(rural)and length of exercise showed a negative correlation with the prevalence of HHcy,relative to the non-HHcy patients(P<0.05).The area under the curve(AUC)of the ROC predictive model for HHcy prevalence was constructed as 0.785(sensitivity 0.701,specificity 0.737),and the risk of HHcy prevalence was predicted by age,sex,DBP,Hb,heart rate,FBG,HDL-C,BMI,type of residence and duration of exercise,with the critical values of age,DBP,Hb,heart rate,FBG,HDL-C being 46.5 years,77.5 mmHg,169.5 g/L,72.5 beats/min,3.65 mmol/L,and 1.66 mmol/L,respectively.For comparison with the ROC curve of the included traditional risk factor HHcy prediction model(AUC of 0.790,sensitivity 0.732,specificity 0.712),the results showed that the predictive ability of the two models was similar(P=0.075).Conclusion The Tibetan population in the three areas of Nagqu City has a high prevalence of HHcy,and a series of targeted measures should be taken to reduce the risk of related diseases.
7.Expert consensus on adrenal vein sampling for primary aldosteronism

Primary aldosteronism(PA)is an important cause of secondary hypertension,and adrenal vein sampling(AVS),as the gold standard for identifying the dominant side in PA,plays a critical role in guiding subsequent therapeutic strategies.AVS is an invasive procedure that is not yet widely available in China.To promote the popularization and standardization of AVS,the Chinese Society of Hypertension(Chinese Research Hospital Association)and the Group of Interventional Diagnosis and Treatment of Hypertension(China International Exchange and Promotive Association for Medical and Health Care)have summarized domestic and foreign evidence and the operational experience of many experts.This consensus has summarized and updated the significance,indications,pre-procedural preparation,procedural steps and key points,prevention and management of complications,and result interpretation of AVS.This consensus provides comprehensive and clinically relevant guidance for the clinical practice and development of AVS,effectively promoting its popularization and standardization.
8.Application progress of artificial intelligence in the evaluation of endometrial receptivity

Successful embryo implantation requires high-quality embryos and endometrium with high receptivity. However, there is no universally recognized authoritative technique for evaluating endometrial receptivity (ER) at present. In recent years, the application of artificial intelligence (AI) technology in the assessment of ER has achieved remarkable progress. In ultrasound image analysis, AI has improved the accuracy of endometrial segmentation and measurement through deep learning technology, and can combine clinical data for pregnancy outcome prediction. In histological analysis, AI algorithms can quickly and accurately identify histological features of the endometrium and analyze the ratio of epithelial and stromal cells. In genomic diagnosis, AI analyzes endometrial gene expression through machine learning methods to provide scientific basis for personalized embryo transfer. Although AI shows great potential in the assessment of ER, its application still faces challenges such as data standardization and algorithm generalization ability. In the future, through multi-center data sharing and algorithm optimization, AI is expected to play a greater role in assisted reproductive technology. This article reviews the application of AI in ER assessment, summarizes the latest research progress, and provides references for further research.
9.Development and application research of a lung rehabilitation education game software for patients with chronic obstructive pulmonary disease based on health belief model

Objective Taking into account the physical and mental characteristics of patients with COPD,we develop and implement a lung rehabilitation education game software,aiming to explore more effective health education practices for COPD.Methods Drawing on the Health Belief Model theory,the game content was developed by semi-structured interviews,brainstorming sessions,and expert consultations,and it was implemented via a WeChat mini program.Patients with COPD from the respiratory department of a tertiary general hospital of Luzhou city between March and October 2023 were conveniently selected as the study subjects.Among them,40 patients from July to October 2023 were designated as an experimental group,and another 40 from March to June 2023 constituted a control group.The experimental group was provided with routine health education combined with pulmonary rehabilitation games,whereas the control group received standard health education.After the intervention,the researchers compared the COPD knowledge questionnaire scores and pulmonary rehabilitation adherence between the 2 groups,and assessed the satisfaction of both patients and nurses with the software.Results No sample detachment.The score of the COPD knowledge questionnaire in the experimental group was higher than that in the control group,and the difference was statistically significant(Z=5.361,P<0.001).The proportion of patients in the experimental group with good adherence to pulmonary rehabilitation(85%)was significantly higher than it in the control group(25%),(x2=29.091,P<0.001).The patients'overall satisfaction rate with the game was 95%,with operational effectiveness receiving the highest satisfaction rating at 97%.Conclusion The pulmonary rehabilitation education game for patients with COPD can improve their knowledge of COPD,enhance their compliance with pulmonary rehabilitation.Patients have high satisfaction with the software.The software enriches the clinical health education methods,which can be used in clinical practice.
10.Clinical observation of thunder-fire moxibustion combined with fire-dragon cupping in the treatment of acute-phase peripheral facial palsy with co-morbid anxiety and depression

Objective:To evaluate the effect of thunder-fire moxibustion combined with fire-dragon cupping on patients with acute peripheral facial paralysis with co-morbid anxiety and depression.Methods:A total of 80 patients with acute peripheral facial paralysis with co-morbid anxiety and depression were selected and divided into two groups according to the random number table method,with 40 patients in each group.In the control group,acupuncture therapy combined with routine care was given;in the observation group,thunder-fire moxibustion and fire-dragon cupping therapy were added.Both groups were treated once a day,6 times a week,with 1 d of rest,for 4 consecutive weeks(a total of 24 treatments).The House-Brackmann(H-B)grading,facial clinimetric evaluation(FaCE),self-rating anxiety scale(SAS),and self-rating depression scale(SDS)were used before and after the intervention to evaluate the facial nerve function,quality of life,mental health,and clinical efficacy of the two groups.Results:After treatment,patients in both groups showed improvement in H-B grading,scores of all dimensions of the FaCE scale,and SAS and SDS scores compared to those before treatment,and the differences within the groups were statistically significant(P<0.05).After treatment,the total effective rate was 95.0%in the observation group and 75.0%in the control group,and the difference between the two groups was statistically significant(P<0.05).After treatment,the H-B grading of the observation group was better than that of the control group(P<0.05);the scores of all dimensions of the FaCE scale and SAS and SDS scores of the observation group were better than those of the control group(P<0.05).Conclusion:Adding thunder-fire moxibustion and fire-dragon cupping therapy to acupuncture and routine care can promote the recovery of facial nerve function,reduce anxiety and depression,and improve the patients'living standard,and thus is suitable for popularization and application.
