1.AHP Combined with Response Surface Method to Optimize the Simmering Process of Rhei Radix et Rhizoma and Correlation Analysis between Composition and Color

OBJECTIVE To explore the optimal parameters of simmered Rhei Radix et Rhizoma and the correlation between the chroma values and the intrinsic composition of simmered Rhei Radix et Rhizoma decoction pieces powder.METHODS The single-factor-response surface method was used to investigate the simmering temperature,simmering time,paper dosage and plant ash dos-age,the response surface experiment was carried out on the basis of the single factor experiment,the appearance traits,total anthraqui-nones,free anthraquinones,leachables,sennoside A and B contents were taken as indicators,the analytic hierarchy process(AHP)was used to give weights to each index,and the process was optimized.The chroma values of raw and simmered products were deter-mined by electronic eye,the correlation and regression analysis were carried out by SPSS22.0 software,and the chroma-component re-gression equation was constructed.RESULTS The optimal process of simmering Rhei Radix et Rhizoma was 140 ℃,5 times of plant ash,2 layers of wet paper wrapped and being simmered for 2.5 h.CONCLUSION The simmering process of Rhei Radix et Rhizoma optimized by AHP combined with response surface method is reasonable and feasible,the color of decoction pieces has a significant correlation with the component content,and the regression equation constructed is reliable,which can predict the intrinsic component content of decoction pieces through chroma values.
2.Preoperative discrimination of colorectal mucinous adenocarcinoma using enhanced CT-based radiomics and deep learning fusion model

Objective:To develop a preoperative differentiation model for colorectal mucinous adenocarcinoma and non-mucinous adenocarcinoma using a combination of contrast-enhanced CT radiomics and deep learning methods.Methods:This is a retrospective cohort study. Clinical data of colorectal cancer patients confirmed by postoperative pathological examination were retrospectively collected from January 2016 to December 2023 at Shanghai General Hospital Affiliated to Shanghai Jiao Tong University School of Medicine (Center 1, n=220) and the First Affiliated Hospital of Bengbu Medical University (Center 2, n=51). Among them, there were 108 patients diagnosed with mucinous adenocarcinoma, including 55 males and 53 females, with an age of (68.4±12.2) years (range: 38 to 96 years); and 163 patients diagnosed with non-mucinous adenocarcinoma, including 96 males and 67 females, with an age of (67.9±11.0) years (range: 43 to 94 years). The cases from Center 1 were divided into a training set ( n=156) and an internal validation set ( n=64) using stratified random sampling in a 7∶3 ratio, and the cases from Center 2 were used as an independent external validation set ( n=51). Three-dimensional tumor volume of interest was manually segmented on venous-phase contrast-enhanced CT images. Radiomics features were extracted using PyRadiomics, and deep learning features were extracted using the ResNet-18 network. The two sets of features were then combined to form a joint feature set. The consistency of manual segmentation was assessed using the intraclass correlation coefficient. Feature dimensionality reduction was performed using the Mann-Whitney U test and the least absolute shrinkage and selection operator regression. Six machine learning algorithms were used to construct models based on radiomics features, deep learning features, and combined features, including support vector machine, logistic regression, random forest, extreme gradient boosting, k-nearest neighbors, and decision tree. The discriminative performance of each model was evaluated using receiver operating characteristic curves, the area under the curve (AUC), DeLong test, and decision curve analysis. Results:After feature selection, 22 features with the most discriminative value were finally retained, among which 12 were traditional radiomics features and 10 were deep learning features. In the internal validation set, the Random Forest algorithm based on the combined features model achieved the best performance (AUC=0.938, 95% CI: 0.875 to 0.984), which was superior to the single-modality radiomics feature model (AUC=0.817, 95% CI: 0.702 to 0.913, P=0.048) and the deep learning feature model (AUC=0.832, 95% CI: 0.727 to 0.926, P=0.087); in the independent external validation set, the Random Forest algorithm with the combined features model maintained the highest discriminative performance (AUC=0.891, 95% CI: 0.791 to 0.969), which was superior to the single-modality radiomics feature model (AUC=0.770, 95% CI: 0.636 to 0.890, P=0.045) and the deep learning feature model (AUC=0.799, 95% CI: 0.652 to 0.911, P=0.169). Conclusion:The combined model based on radiomics and deep learning features from venous-phase enhanced CT demonstrates good performance in the preoperative differentiation of colorectal mucinous from non-mucinous adenocarcinoma.
3.Suture tape and headless compression screws in treatment of Lisfranc injury with comminuted fractures of the first and second proximal metatarsal bones

BACKGROUND:Lisfranc ligament is an important structure to maintain the transverse and longitudinal arch of the foot.This injury is a serious middle-foot injury.Lisfranc ligamentous injuries are complex,and their treatment,along with the preferred method of fixation,is controversial. OBJECTIVE:To compare the short-term efficacy of plate combined with Suture tape versus plate combined with headless compression screw in the treatment of Lisfranc injury with comminuted fractures of the 1st and 2nd proximal metatarsal bones. METHODS:A retrospective analysis was performed on 48 patients with Lisfranc injury due to comminuted fractures of the 1st and 2nd proximal metatarsal bones in Seventh Department of Orthopedics,Dongguan Hospital of Traditional Chinese Medicine from January 2019 to June 2022.Among them,25 were fixed with plate combined with Suture tape(observation group)and 23 were fixed with plate combined with headless compression screw(control group).Preoperative classification was performed according to Myerson classification system based on preoperative imaging data.Postoperative follow-up was performed according to fracture healing time,visual analog scale,and American Orthopaedic Foot and Ankle Society(AOFAS)criteria to assess the recovery of foot functions.Postoperative complications were compared and analyzed between the two groups. RESULTS AND CONCLUSION:(1)All cases completed the operation successfully and obtained follow-up in the two groups.The postoperative follow-up time of the two groups was 12-36 months,with a mean of(18.0±5.42)months.(2)There were no significant differences in operation time and intraoperative blood loss between the two groups(P>0.05).(3)The fracture healing time of observation group was slightly longer than that of control group(P<0.05).(4)After 3,6,and 12 months of follow-up,the visual analog scale score of the observation group was significantly lower than that of the control group(P<0.05).(5)At 6 and 12 months after operation,AOFAS score of foot function in the observation group was significantly improved compared with the screw group at various time points after operation(P<0.05),and was significantly higher than that before operation(P<0.05).(6)The postoperative complications were 1 case of traumatic arthritis in the observation group and 1 case of incision infection,1 case of screw fracture,and 2 cases of traumatic arthritis in the control group.There was no significant difference between the two groups(P>0.05),considering the correlation with a small sample size.(7)It is indicated that as for the surgical method of Lisfranc injury with comminuted fractures of the 1st and 2nd proximal metatarsal bones,the application of plate combined with Suture tape internal fixation has a reliable effect in the treatment of Lisfranc joint injury,which can improve the function of the foot joint of patients,and has the advantages of less surgical trauma,fewer postoperative complications,and lower risk of long-term iatrogenic traumatic arthritis.Compared with headless compression screw,it is more beneficial to the recovery of foot function.
4.Preparation, optimization, and in vitro evaluation of Pediococcus acidilactici HRQ-1 microcapsules.

We have isolated an intestinal probiotic strain, Pediococcus acidilactici HRQ-1. To improve its gastrointestinal fluid tolerance, transportation and storage stability, and slow-release properties, we employed the extrusion method to prepare the microcapsules with P. acidilactici HRQ-1 as the core material and sodium alginate and chitosan as the wall material. The optimal conditions for preparing the microcapsules were determined by single factor and orthogonal tests, and the optimal ratio was determined by taking the embedding rate, survival rate, storage stability, gastrointestinal fluid tolerance, and release rate as the evaluation indexes. The results showed that under the optimal embedding conditions, the embedding rate reached (89.60±0.02)%. Under the optimal formula of freeze-drying protective agent, the freeze-drying survival rate reached (76.42±0.13)%, and the average size of the microcapsules produced was (1.16±0.03) mm. The continuous gastrointestinal fluid simulation experiments confirmed that the microcapsules ensured the viable bacterial count and can slowly release bacteria in the intestinal fluid. The curve of the viable bacterial count during storage at 4 ℃ and room temperature indicated that the prepared microcapsules achieved strains' live number protection. The formula and preparation process of P. acidilactici microcapsules may provide a technological reserve for the preparation of more live bacterial drugs in the future.

6.Relationship of lipoprotein(a)with inflammatory biomarkers and cardiac damage in patients with essential hypertension

Objective To explore the effect of lipoprotein a[Lp(a)]on the inflammatory state and cardiovascular system of patients with essential hypertension.Methods The clinical data of 234 patients with essential hypertension admitted from January 2022 to November 2023 were retrospectively collected.According to the serum Lp(a)concentration,they were divided into the normal Lp(a)group(n=185)and the high Lp(a)group(n=49).The differences in serum inflammatory markers C reactive protein(CRP),neutrophil-to-lymphocyte ratio(NLR),and fibrinogen between the two groups were compared.Pearson correlation analysis was used to analyze the correlation between Lp(a)and inflammatory markers,and the incidence of cardiac target organ damage between the two groups of patients was compared.Results Compared with the normal Lp(a)group,the CRP concentration,NLR and fibrinogen concentration of hypertensive patients in the high Lp(a)group increased(P<0.05,P<0.01),and the concentration of Lp(a)was positively correlated with CRP,NLR and fibrinogen(r=0.168,0.165,0.321,P<0.05).The incidence of myocardial infarction was higher in the high Lp(a)group[22.45%(11/49)]than in the normal Lp(a)group[7.57%(14/185)](P<0.01).Conclusion In patients with essential hypertension,high Lp(a)concentration is associated with a stronger inflammatory response,and elevated Lp(a)concentration is related to an increased incidence of myocardial infarction,suggesting that Lp(a)may affect target organ damage in patients with essential hypertension by promoting inflammation.

Lead is one of the most important occupational hazards in China, and occupational exposure is the leading cause of lead poisoning. Lead can be absorbed by the body through air, food, drinking water and skin, and accumulate in multiple organs in the body, posing health risks to humans, especially to lead workers. Many previous studies have shown that lead can affect the function of glial cells such as microglia, astrocytes and oligodendrocytes, resulting in irreversible neurological damage. This article provides an overview of the neurotoxic mechanism induced by lead through glial cells, elucidates that lead can induce neurodegenerative diseases such as Alzheimer′s disease, Parkinson′s disease, and amyotrophic lateral sclerosis, and reviews the relationship between lead and glial cells, in order to provide reference for further research on the neurotoxic mechanism of lead on glial cells.
