To investigate the effects of plasma exosomes from sheep on animal hair growth,exo-somes were extracted from two sheep breeds with significant differences in wool fineness(Xinji Fine-wool sheep and Small-tailed Han sheep)using biphasic precipitation. The exosomes were characterized by transmission electron microscopy(TEM)and nanoparticle tracking analysis(NTA).A murine alopecia areata model was established, followed by subcutaneous injection of ex-osomes for 7 consecutive days.Dorsal skin samples were collected on day 10 post-injection for his-tological analysis via hematoxylin-eosin(HE)staining.miRNA and mRNA libraries were constructed through qPCR,while TargetScan and g:Profiler were employed for target gene predic-tion and KEGG enrichment analysis,respectively.Protein-protein interaction(PPI)networks were analyzed using STRING to identify key miRNA-mRNA targets, which were preliminarily validated by qPCR.Results demonstrated that on day 5 post-injection,the Small-tailed Han sheep plasma ex-osome group(SPE)exhibited more melanin deposition spots compared to the Xinji Fine-wool sheep plasma exosome group(XPE),with XPE showing greater melanin spots than the control group(NC).Histological analysis revealed that XPE group displayed significantly fewer hair folli-cles compared to SPE and NC groups(P<0.01),while SPE group exhibited significantly larger hair follicle diameters than both XPE and NC groups(P<0.01).Bioinformatics analysis predicted 357,711,477,346,and 3 178 target genes for miR-150,miR-133b,miR-31-5p,miR-433-3p,and miR-218,respectively,with 508 genes being co-regulated by two or more miRNAs.PPI analysis i-dentified 217 genes involved in positive regulation of cellular processes,109 in developmental regu-lation,133 in cellular development,132 in cell differentiation, and 69 in regulation of cell differenti-ation.Target genes were found to regulate hair follicle growth through Wnt signaling via Frizzled receptor binding and downstream pathways involving calmodulin(CaM)and cyclic-AMP response binding protein(CREB).qPCR validation showed significantly higher relative expression levels of miR-218,miR-150,miR-31-5p,miR-133b,and miR-433-3p in SPE and XPE groups compared to NC,with SPE exhibiting greater expression than XPE.Additionally, significant differences in FZD4,WNT4,CREB1,and FZD3 expression were observed between treatment groups and NC.These findings demonstrate that plasma exosomes from Xinji Fine-wool and Small-tailed Han sheep modulate hair growth by altering miRNA-mRNA expression profiles in skin tissues.

Objective:To investigate the effects of the Epley and Semont procedures with varying lateral angles of the head on posterior semicircular canal benign paroxysmal positional vertigo (PC-BPPV). Methods:A total of 115 patients with unilateral PC-BPPV were randomly divided into five groups: Epley group, Semont group, Semont+10° group, Semont+20° group, and Semont+30° group, with 23 patients in each group. Corresponding reduction treatments were performed. Results:The total effective rates for the Epley group, Semont group, Semont+10° group, Semont+20° group, and Semont+30° group were 95.7% (22/23), 4.3% (1/23), 30.4% (7/23), 52.2% (12/23), and 87.0% (20/23) respectively. The inefficiencies were 4.3% (1/23), 95.7% (22/23), 69.6% (16/23), 47.8% (11/23), and 13.0% (3/23). Statistically significant differences were observed in the total effective rates among the five groups (χ²=54.11, P<0.01). The total effective rates in the Semont group, Semont+10° group, and Semont+20° group were significantly different from that of the Epley group (P<0.01), while no statistically significant difference was found between the Semont+30° group and the Epley group (P= 0.608>0.012 5). Conclusion:Among the four Semont methods with different lateral lying angles, the total effective rate of reduction treatment increased with the elevation of the lateral lying angle on the affected side. The efficacy of the Semont+30° group in treating PC-BPPV was not significantly different from the Epley group's reduction effect, which was markedly superior to that of the other four Semont methods at different angles. Therefore, the Semont+30° reduction technique is recommended for the treatment of PC-BPPV.

In recent decades, the prevalence of hyperuricemia and gout has increased dramatically due to lifestyle changes. The drugs currently recommended for hyperuricemia are associated with adverse reactions that limit their clinical use. In this study, we report that berberine (BBR) is an effective drug candidate for the treatment of hyperuricemia, with its mechanism potentially involving the modulation of gut microbiota and its metabolite, succinic acid. BBR has demonstrated good therapeutic effects in both acute and chronic animal models of hyperuricemia. In a clinical trial, oral administration of BBR for 6 months reduced blood uric acid levels in 22 participants by modulating the gut microbiota, which led to an increase in the abundance of Bacteroides and a decrease in Clostridium sensu stricto_1. Furthermore, Bacteroides fragilis was transplanted into ICR mice, and the results showed that Bacteroides fragilis exerted a therapeutic effect on uric acid similar to that of BBR. Notably, succinic acid, a metabolite of Bacteroides, significantly reduced uric acid levels. Subsequent cell and animal experiments revealed that the intestinal metabolite, succinic acid, regulated the upstream uric acid synthesis pathway in the liver by inhibiting adenosine monophosphate deaminase 2 (AMPD2), an enzyme responsible for converting adenosine monophosphate (AMP) to inosine monophosphate (IMP). This inhibition resulted in a decrease in IMP levels and an increase in phosphate levels. The reduction in IMP led to a decreased downstream production of hypoxanthine, xanthine, and uric acid. BBR also demonstrated excellent renoprotective effects, improving nephropathy associated with hyperuricemia. In summary, BBR has the potential to be an effective treatment for hyperuricemia through the gut-liver axis.

OBJECTIVES:
This study aims to analyze the clinical symptoms and imaging manifestations in patients with temporomandibular disorder syndrome (TMD), who are sensitive to sudden temperature drop.
METHODS:
One hundred and nineteen patients with TMD who attended the Department of Stomatology of the First Medical Center of Chinese People's Liberation Army General Hospital from December 2022 to December 2023 were included, including 44 males and 75 females, with a mean age of 32.4±13.7 years. The questionnaire was used to determine whether they were sensitive to temperature drop, and the TMD patients were divided into a temperature plunge-sensitive group and a temperature drop insensitive group. The clinical symptoms and imaging manifestations of patients in the two groups were observed. SPSS 25.0 was used for statistical analysis.
RESULTS:
There was no statistically significant difference between the gender and age of patients in the temperature plunge-sensitive group (50 patients) and the insensitivity group (69 patients) (P>0.05). The percentage of patients with pain was slightly higher in the temperature plunge-sensitive group [86.0% (43/50)] than in the insensitive group [68.1% (47/69)], and the difference was statistically significant (χ2=5.031, P=0.025), while the differences in joint murmur and mouth opening limitation between the two groups were not statistically significant. A total of 238 lateral joints were detected in both groups, the percentage of osteoarthropathic imaging changes was significantly higher in the temperature plunge-sensitive group [82.0% (82/100)] than in the insensitive group [53.6% (74/138)] (χ2=20.675, P<0.001). Magnetic imaging showed that the percentage of joint effusion was higher in patients in the temperature plunge-sensitive group [66.0% (33/50)] than in the insensitive group [42.0% (29/69)], and the difference was statistically significant (χ2=5.602, P=0.018).
CONCLUSIONS
TMD patients with maxillofacial pain symptoms, joint effusions, and abnormal imaging of osteoarticular structures are more likely to be sensitive to sudden temperature drops.

Objective:To analyze risk factors for immune checkpoint inhibitor-related pneumonitis (CIP) in non-small cell lung cancer (NSCLC) patients based on clinical and radiological characteristics, and to develop and validate a nomogram model for predicting the risk of CIP.Methods:A retrospective case-controlled study was conducted. The clinical data of 159 patients diagnosed with NSCLC in Shanxi Province Cancer Hospital between January 2020 and December 2023 who received immune checkpoint inhibitor (ICI) therapy were retrospectively analyzed. Based on the development of CIP after immunotherapy, the patients were divided into the CIP group (30 cases) and the control group (129 cases). The clinical data of NSCLC patients, hematological indicators and the data of imaging characteristics before their first ICI treatment were collected. Quantitative assessments were performed on pretreatment chest CT images, including lung total tumor volume, number of involved lung segments, and pulmonary infection index. Logistic regression analysis was used to screen out the factors influencing the development of CIP. R 4.3.0 statistical software was used to construct a nomogram model for predicting CIP based on the statistically significant risk factors identified in the multivariate logistic regression analysis. The predictive performance of the model was evaluated by using receiver operating characteristic (ROC) curves and the area under the curve (AUC). Calibration curves and decision curve analysis (DCA) were employed to assess the model's consistency and clinical benefit. Results:There were statistically significant differences in the proportions of patients with a history of chest radiotherapy and those receiving different immunotherapy regimens between the control group and the CIP group (both P < 0.001). The difference in the lactate dehydrogenase (LDH) [ M ( IQR)] between the both groups was statistically significant [211.00 U/L (57.00 U/L) vs. 276.00 U/L (136.00 U/L), Z = -3.41, P < 0.001]; additionally, the difference in lung status score between the 2 groups was statistically significant ( P < 0.001). Multivariate logistic regression analysis revealed that a history of chest radiotherapy (with vs. without: OR = 4.200, 95% CI: 1.466-12.036), the combination of immunotherapy (monotherapy vs. the combined therapy: OR = 0.106, 95% CI: 0.022-0.509), LDH ≥ 255.5 U/L (< 255.5 U/L vs. ≥ 255.5 U/L: OR = 0.988, 95% CI: 0.981-0.995), and severe lung status score(mild vs. moderate vs. severe: OR = 0.187, 95% CI: 0.059-0.593) were independent risk factors for CIP development in NSCLC patients after immunotherapy (all P < 0.05). A nomogram model for predicting CIP occurrence was constructed based on chest radiotherapy history, immunotherapy regimen, LDH, and lung status score. ROC curve analysis showed the AUC was 0.878 (95% CI: 0.813-0.942). The calibration curve demonstrated the good consistency between the predicted risk probability of CIP and the observed outcomes; DCA indicated that the model had favorable clinical benefits. Conclusions:The constructed nomogram prediction model shows a good predictive performance.
