Objective To explore the application value of intravoxel incoherent motion(IVIM)imaging parameters in the differential diagnosis of benign and malignant breast nodules and their correlation with the expression of Ki-67 receptor in breast cancer. Methods The data of 41 female patients who completed 3.0 T breast magnetic resonance imaging(MRI)with complete surgical pathology results in the Second Hospital of Jiaxing City from October 2022 to January 2025 were analysed and evaluated.Conventional images and diffusion-weighted images with 11 b values were collected. The IVIM imaging parameters of real diffusion coefficient(D),perfusion related diffusion coefficient(D*)and perfusion fraction(f)were measured and calculated in the region of interest of each lesion. The receiver operating characteristic curve were plotted to quantify the differential diagnostic efficacy of each parameter of IVIM imaging in benign and malignant breast nodules. The differences of parameters between benign and malignant breast nodules and between the groups with different expression levels of Ki-67 receptor in breast cancer were analysed, and the correlation between each parameter and the expression level of Ki-67 in breast cancer was counted. Results The D value of benign breast nodules group(benign groups)was significantly higher than that of malignant breast nodules group(malignant groups),and the D*value was significantly lower than that of malignant group, and the differences between benign group and malignant group were statistically significant(t=-4.773,t=2.063,P<0.05);Thefvalue of benign group was slightly lower than that of malignant group, and the differences between two groups were not statistically significant(t=0.035,P>0.05).Among the parameters of IVIM imaging,D value had the best differential diagnostic efficacy for benign and malignant breast nodules, with area under the curve(AUC)of 0.870(95%CI:0.755-0.985)and a specificity of 75.0%;D*value had the second best differential diagnostic efficacy after D value, with an AUC of 0.789(95%CI:0.658-0.920),but it had the highest sensitivity of 88.2%;And the differential diagnosis efficiency off value was the worst, much less than D and D*values. The D value in the high Ki-67 expression group of breast cancer was lower than that in the low expression group, while the D* and f values in the high expression group were higher than that in the low expression group, and the differences of each imaging parameter of IVIM were not statistically significant between two groups(t=-2.617,t=2.169,t=0.647,P>0.05).There was a significant negative correlation between the expression of the Ki-67 receptor and the D value(r=-0.615,P<0.05),and no significant correlation was seen with either D *or f value(r=0.223,r-0.031,P>0.05).Conclusion The D and D*values of IVIM imaging parameters have great clinical value in the differential diagnosis of benign and malignant breast nodules.
2.Advances in novel therapies for children with Hodgkin lymphoma
International Journal of Pediatrics 2025;52(10):676-680
Hodgkin lymphoma(HL)is one of the common pediatric malignancies(especially in adolescents). The prognosis of pediatric patients with HL is excellent with traditional first-line treatment regimens involving short-course,high-dose combination chemotherapy and low-intensity radiotherapy, resulting in an overall response rate of 90%. However,10%-25% of pediatric HL patients are estimated to have relapsed/refractory(r/r)disease,and survivors of pediatric HL are at risk of late effects caused by traditional treatment regimens. For children with r/r HL,novel therapies, such as brentuximab vedotin, immune checkpoint inhibitors and immune cell therapy, including chimeric antigen receptor T-cell therapy and EBV-specific cytotoxic T lymphocytes are an integral part of their salvage treatment and consolidation treatment after hematopoietic stem cell transplantation. Furthermore, combining novel therapies with first-line treatment regimens for pediatric HL may help enhance therapeutic efficacy and reduce treatment toxicity. This review is aimed to describe current advances in novel therapies for pediatric HL,and serve as a reference for clinical practice.

Objective:To investigate the awareness levels of doctors from various departments regarding fertility preservation to provide reference and suggestions for further improving awareness and promoting multidisciplinary development in fertility preservation. Methods:From November to December 2023, the China Alliance of Fertility Preservation initiated this survey covering 88 hospitals in 23 cities across 20 provinces in China. The survey focused on the degree of understanding of basic concepts, technological perceptions, treatment concepts, and related challenges regarding fertility preservation. Descriptive statistics and Fisher's exact test were used to analyze the difference in awareness levels across various departments. Results:There was a significant difference in the awareness of fertility preservation guidelines among physicians from different departments ( P<0.001). Reproductive center physicians had the highest level of understanding [68.3% (41/60)]. Doctors in the reproductive centers had a higher awareness rate of the five fertility preservation techniques (ovarian tissue cryopreservation, oocyte cryopreservation, sperm cryopreservation, testicular tissue cryopreservation, and embryo cryopreservation) than that in other departments (all P<0.05). Regarding treatment concepts, more reproductive center physicians believed that psychological counselling about fertility preservation before oncological treatment was necessary ( P=0.016). Hematologists had more worries and concerns about fertility preservation treatment. Additionally, 96.7% (58/60) of reproductive center physicians expressed the need to establish effective referral mechanisms within the same city, province, or across provinces. However, 55.6% (35/63) of oncologists, 64.6% (31/48) of hematologists, and 77.8% (7/9) of pediatricians considered treating primary tumors far more important than fertility preservation. Conclusion:Doctors from different departments have varying levels of understanding regarding fertility preservation. It is necessary to strengthen the dissemination and training of fertility preservation knowledge among physicians from various departments, as well as to promote a multidisciplinary treatment model to improve efficiency, to establish a comprehensive referral mechanism to improve patients' opportunities for fertility preservation.

Chinese Journal of Trauma 2024;40(10):865-881
Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
