1.Evaluating the effectiveness of a primary care general practitioner rotation training program based on three core competencies

Objective:To evaluate the effectiveness of a rotation training program for primary care general practitioners (GPs) in Taizhou, China, designed around three core competencies.Methods:A longitudinal study was conducted. A total of 111 primary care GPs participating in the 1st to 5th sessions of the Taizhou GP rotation training program at Taizhou Hospital (the designated training center) in Zhejiang Province from July 2022 to December 2024 were included. Based on the World Organization of Family Doctors (WONCA) competency model, combined with literature review and expert consultation, 15 potential training components were identified. A needs assessment survey among primary care GPs was then conducted, leading to the finalization of three core competencies for the training: ①Evidence-based management of common diseases, frequently-occurring diseases, and common symptoms; ②Early identification, management, and referral of critically ill patients; ③Chronic disease management and health promotion capabilities. A 4-month full-time immersion training program was structured around these competencies, comprising three phases: theoretical training (1 week), clinical comprehensive skills training (14 weeks), and primary care practice training (1 week). Assessments included a pre-and post-training theoretical knowledge test and a final clinical assessment. The final assessment utilized a three-station Objective Structured Clinical Examination (OSCE) evaluating patient consultation, clinical reasoning & decision-making, and clinical procedural skills. A self-administered satisfaction survey was also distributed post-training.Results:The 111 participants were predominantly aged 30-49 years (72.1% (80/111)), held bachelor′s degrees (87.4% (97/111)), were licensed physicians (78.4% (87/111)), and had junior or intermediate professional titles (87.4% (97/111)). Post-training theoretical scores were significantly higher than pre-training scores (76.26±7.00 vs. 69.94±6.40, t=-10.45, P<0.001). All participants 100%(111/111) passed the final OSCE assessment. The mean scores for the OSCE stations were: patient consultation 85.99±7.30, clinical reasoning & decision-making 82.72±7.61, and clinical procedural skills 89.60±5.65. Satisfaction rates were 100.0% (111/111) for the overall program, the three-phase design, the core competency training content, theoretical training, clinical skills training, and clinical rotation departments. Satisfaction was 98.2% (109/111) for the 4-month full-time duration, 99.1% (110/111) for the "2+X" clinical rotation model (2 weeks each in General Practice and Emergency Medicine+elective rotations in 4 other departments), and 97.3% (108/111) for the primary care practice base. Conclusions:The competency-based rotation training program for primary care GPs in Taizhou effectively enhanced participants′ theoretical knowledge and clinical practical skills, and achieved high levels of participant satisfaction. This model offers valuable insights for optimizing primary care GP training in similar settings.
2.Extracellular vesicles derived from synovial fluid in patients with rheumatoid arthritis promote angiogenesis of HUVEC

Objective To investigate the effects of extracellular vesicles(EVs)derived from synovial fluid of rheumatoid arthritis(RA)patients on angiogenesis of human umbilical vein endothelial cells,and to preliminarily explore the underlying mechanisms.Methods Synovial fluid samples of knee joint were collected from 20 patients with RA and 20 patients with osteoarthritis(OA)in this study.EVs were purified using ultracentrifugation.The morphology and size of EVs were observed by transmission electron microscopy and nanoparticle tracking analysis.CD9,CD63,cytochrome c(Cyt-c),vascular endothelial growth factor(VEGF),lysyl oxidase(LOX),matrix metalloproteinase 2(MMP2),tumour necrosis factor alpha(TNF-α),transforming growth factor beta1(TGF-β1)in EVs were detected using Western blot.Human umbilical vein endothelial cells(HUVEC)were treated with the EVs.The growth,migration and angiogenesis of HUVEC were observed by CCK8 assay,TranswellTM chamber assay,scratch test and matrigel angiogenesis assay,respectively.The effect of EVs on the PI3K/AKT pathway in HUVEC was assessed using Western blot.Results Both EVs from RA synovial fluid(RA-EVs)and OA synovial fluid(OA-EVs)were cup-shaped,mainly between 30-400 nm in diameter,expressing CD63 and CD9,but not Cyt-c.RA-EVs carried more VEGF,LOX,MMP2,TNF-α and TGF-β1 than OA-EVs.Compared with the OA-EVs intervention,RA-EVs significantly promoted the proliferation,migration,and angiogenesis of HUVECs,as well as upregulated PI3K/AKT phosphorylation.The inhibitor of PI3K suppressed angiogenesis induced by EVs.Conclusion EVs in synovial fluid of RA carried more cytokines and enzymes that related angiogenesis and inflammation.These EVs exert their pro-angiogenic effects by activating the PI3K/AKT pathway,then contributing to the pathological progression of RA.
3.Diagnostic Value of Serum sC5b-9,5-HT,and Peripheral Blood C1q mRNA Level Changes on Cognitive Function Impairment in Patients with Bipolar Disorder during Manic Episodes

Objective To investigate the changes in serum complement activation product(sC5b-9),serotonin(5-HT),and peripheral blood C1q mRNA levels in patients with bipolar disorder during manic episodes,and their correlation with cognitive impairment.Methods From January 2022 to December 2023,75 patients with bipolar disorder during manic episodes and 75 healthy individuals who were treated at Baoji Central Hospital were selected and divided into BD group and healthy group.Serum sC5b-9,5-HT,peripheral blood C1q mRNA levels,and Montreal Cognitive Assessment(MoCA)scores were detected and compared.Patients in the BD group were further grouped based on MoCA scores:cognitive impairment group(22~25 points)(n=40)and non-impairment group(≥26 points)(n=35).Differences in serum sC5b-9,5-HT,and peripheral blood C1q mRNA levels were compared,and multiple Logistic regression analysis was performed to identify influencing factors of cognitive impairment in bipolar disorder patients.Results Compared to the healthy group,the BD group showed significantly elevated serum sC5b-9,5-HT,and peripheral blood C1q mRNA expression levels(P<0.05).The BD group's 8 MoCA domain scores and total MoCA score were significantly lower than the healthy group(P<0.05).Compared to the non-impairment group,the cognitive impairment group demonstrated significantly higher serum sC5b-9,5-HT,and peripheral blood C1q mRNA expression levels(P<0.05).Serum sC5b-9,5-HT,and peripheral blood C1q mRNA expression levels were risk factors for cognitive impairment in patients with bipolar disorder during manic episodes(P<0.05).Conclusion Patients with bipolar disorder during manic episodes show abnormally elevated serum sC5b-9,5-HT,and peripheral blood C1q mRNA expression levels.Cognitive impairment in these patients is closely related to the expression levels of serum sC5b-9,5-HT,and peripheral blood C1q mRNA.
4.Angiotensin Ⅱ activates p53/SAT1 signaling pathway to induce ferroptosis in white adipocytes

Aim To investigate the effect and mechanism of angiotensin Ⅱ(Ang Ⅱ)on ferroptosis in white adi-pocytes.Methods The 3T3-L1 preadipocytes were differentiated into white adipocytes by inducer stimulation.The experiment was divided into control group,Ang Ⅱ group,Ang Ⅱ+Fer-1(ferroptosis inhibitor)group and Ang Ⅱ+PFT-α(p53 inhibitor)group.Ang Ⅱ was used to treat cells.RT-qPCR and Western blot were used to detect the expression levels of ferroptosis factors and adipokines.JC-1 kit was used to detect mitochondrial membrane potential(MMP)level.Iron ion kit was used to detect intracellular iron content.Glutathione(GSH)kit was used to detect GSH content.Fer-1 and Ang Ⅱ were added to treat cells to detect the the changes of ferroptosis level.The expression of p53 and spermidine/spermine N1-acetyltransferase 1(SAT1)protein was detected.Subsequently,PFT-α and Ang Ⅱ were added to co-treat cells to detect the changes of p53 and SAT1 protein expression,and to observe the effect of inhibiting p53 expression on the expression levels of ferroptosis factors and adipokines.Results 3T3-L1 cells were successfully differentiated into white adipocytes by stimulator-induced differentiation.Ang Ⅱ induced ferroptosis in white adipocytes.RT-qPCR results showed that compared with control group,the mRNA expression of anti-ferroptosis factor glutathione peroxidase 4(GPX4),solute carrier family 7 member 11(SLC7A11)and iron regulatory protein 1(IRP-1)was down-regulated in Ang Ⅱ group,and the mRNA expression of pro-ferroptosis factor acyl-CoA synthetase of long-chain family member 4(ACSL4)was up-regulated.Western blot results showed that compared with control group,the protein expression of SLC7A11 and GPX4 was down-regulated in Ang Ⅱ group,and the protein expression of ACSL4 was up-regulated.Ang Ⅱ treatment increased the content of intracellular iron ions and decreased the levels of GSH and MMP.Compared with Ang Ⅱ group,the mRNA expression of IRP-1 and SLC7A11 was up-regulated in Ang Ⅱ+Fer-1 group.Ang Ⅱ induced changes in the expression profile of adipokines in white adipocytes.Western blot results showed that compared with control group,the protein ex-pression of pro-inflammatory adipokine leptin(LEP),resistin(RETN),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)was up-regulated in Ang Ⅱ group,and the protein expression of anti-inflammatory adipokine adiponectin(AD-PN)and omentin 1(ITLN1)was down-regulated.In addition,Ang Ⅱ increased the protein expression of p53 and SAT1.Inhibition of p53 expression can improve the level of ferroptosis and adipokine expression in white adipocytes trea-ted with Ang Ⅱ.Western blot results showed that compared with Ang Ⅱ group,the protein expression of p53 and SAT1 was down-regulated in Ang Ⅱ+PFT-α group,the protein expression of SLC7A11 and GPX4 was up-regulated,and the protein expression of ACSL4 was down-regulated.The protein expression of ADPN was up-regulated in Ang Ⅱ+PFT-αgroup,and the protein expression of TNF-α,LEP and RETN was down-regulated.Conclusion Ang Ⅱ induces fer-roptosis in white adipocytes through activating the p53/SAT1 signaling pathway.
5.A Multicenter Controlled Study on the Evaluation of Aseptic Compounding Operations Ability of PIVAS Personnel by Media Fill Test

Objective To evaluate the practical application of the Media Fill Test(MFT)in assessing aseptic compounding competency of personnel in Pharmacy Intravenous Admixture Services(PIVAS).Methods A multicenter controlled study was conducted across six tertiary hospitals(center ①-⑥)in China.Participants were divided into an inexperienced group(Group A,n=118)and an experienced group(Group B,n=118),each performing five MFT operations.Positive controls validated medium efficacy.Contamination rates and pass rates were analyzed using chi-square and Fisher's exact tests.Results Valid samples included 584 for Group A and 588 for Group B.The sample pass rate was 66.78%(390/584)in Group A and 91.67%(539/588)in Group B,while personnel pass rates were 46.15%(54/117)and 80.51%(95/118),respectively,with significant intergroup differences(both P<0.01).All centers except Center ⑥ showed significantly higher pass rates in Group B(all P<0.05).Conclusion MFT effectively differentiates technical proficiency levels and is suitable for training evaluation of novice PIVAS staff.

8.Epidemiological characteristics of 7 species of pathogens causing acute respiratory infection in hospitalized children in Zhongshan City from 2019 to 2023

OBJECTIVE To analyze the 7 species of common pathogens causing acute respiratory infection(ARI)in children in Zhongshan,Guangdong,from 2019 to 2023,and to provide references for preventive measures and clinical treatment of ARI.METHODS A total of 14 261 children with ARI aged 14 years and younger,hospital-ized in the Pediatric Department of Zhongshan City People's Hospital from Jan.2019 to Oct.2022 and from Mar.to Dec.2023,were selected as the study subjects.The positive detection rates of seven common respiratory pathogens were compared across three periods:pre-pandemic period(2019),pandemic control period(from 2020 to Oct.2022)and normalization period(from Mar.to Dec.2023)of COVID-19.RESULTS The overall detection rates of pathogens were 59.72%,37.83%and 85.82%in the pre-pandemic,control and normalization periods,respectively.The pathogens with the highest detection rates were Streptococcus pneumoniae(Spn)at 19.81%,respiratory syncytial virus(RSV)at 11.33%,Mycoplasma pneumoniae(MP)at 9.53%,Haemophilus influ-enzae(Hi)at 5.90%,influenza A virus(FluA)at 3.49%,adenovirus(ADV)at 3.30%and Bordetella pertussis(BP)at 0.72%.The mixed infection rate was 8.46%in the pre-pandemic period,mainly involving"MP+Spn"and"Spn+RSV".During the pandemic control period,the mixed infection rates were 4.95%,4.65%and 3.66%,respectively.After the pandemic normalization,the mixed infection rate rose to 17.75%,mainly invol-ving"Hi+Spn"and"Spn+RSV".The detection rates of RSV were 7.82%,11.90%and 14.44%in the pre-pan-demic,control and normalization periods,respectively,with a delayed epidemic pattern observed.The detection rates of Spn were 26.47%,17.45%and 16.99%in the three periods,respectively.The detection rates of MP were 12.53%,1.83%and 24.17%,respectively,with a delayed outbreak observed.CONCLUSIONS The preven-tion and control of COVID-19 involves non-pharmaceutical interventions,which are later lifted.The transmission of these seven common respiratory pathogens is affected,necessitating continuous monitoring of the pathogens.
