1.Prevalence and associated risk factors of carotid plaque and artery stenosis in China: a population-based study.

Stroke is a critical health issue in China, and carotid artery stenosis and plaque play key roles in its prevalence. Despite the acknowledged significance of this condition, detailed information regarding the prevalence of carotid artery stenosis and plaque across the Chinese population has been scarce. This study analyzed data from the China Stroke High-risk Population Screening and Intervention Program for 2020-2021, focusing on 194 878 Chinese adults aged 40 years and above. It assessed the prevalence of carotid artery stenosis and plaque and identified their associated risk factors. Results revealed a standardized prevalence of 0.40% for carotid artery stenosis and 36.27% for carotid plaque. Notably, the highest rates of stenosis were observed in north and south China at 0.61%, while southwestern China exhibited the highest plaque prevalence at 43.17%. Key risk factors included older age, male gender, hypertension, diabetes, stroke, smoking, and atrial fibrillation. This study highlights significant geographical and demographic disparities in the prevalence of these conditions, underlining the urgent need for targeted interventions and policy reforms. These measures are essential for reducing the incidence of stroke and improving patient outcomes, addressing this significant health challenge in China.

2.Current situation and influencing factors of blood glucose management in diabetic patients aged 15 and above in Gansu

Objective To analyze the relationship between follow-up service, personal characteristics, lifestyle and blood sugar management of diabetic patients in Gansu Province, and put forward scientific suggestions on influencing blood sugar management of diabetic patients. Methods Based on the data of the 6th National Health Service Questionnaire, 273 subjects were included. Chi-square test and unconditional logistic regression were used to analyze the blood glucose control and its influencing factors of diabetic patients. Results A total of 39.56% patients' blood sugar status was well controlled. 76.92% patients signed up for family doctor service, and 77.66% patients took hypoglycemic drugs according to the law of doctor's advice. There were significant differences in blood sugar control among patients in different regions, educational levels and occupational types (P <0.05) , logistic regression analysis showed that Hui patients (OR=0.21), doctors without family contract (OR=2.86) and patients taking hypoglycemic drugs intermittently (OR = 6.58). Conclusion The blood sugar control rate of diabetic patients in Gansu is low, and the nationality, contracted family doctors and medication plan affect the blood sugar control level of patients. In order to ensure the high efficiency of follow-up treatment, the related follow-up services provided by primary medical institutions, it is necessary to provide self-management programs that meet the individual characteristics and meet the needs of the disease.
4.Effectiveness of the special-grade antimicrobial stewardship based on information technology

Objective:To evaluate the effectiveness of the information management system on the clinical application of special-grade antimicrobial.Methods:Using the established knowledge database, a computer program was designed and developed, which was embedded in the electronic medical record to intervene the clinical use of the special-grade antimicrobial since 2015. The basic information of all discharged patients from the Second Affiliated Hospital of Zhejiang University School of Medicine from January 2013 to December 2020 were extracted from the HIS system, including the medical orders for antibiotics and the drug storehouse dispensing data.The trend analysis was carried out on the changes of the use rates and antibiotic use density (AUD) of the special-grade antimicrobials in the whole hospital and intensive care units (ICU). The data were processed and analyzed using SPSS 24.0.Results:From 2013 to 2015, except for meropenem and amphotericin B, the usage rate of all special-grade antimicrobials in the whole hospital showed an upward trend ( P<0.05). The proportion of special-grade antimicrobials used in the hospital increased year by year ( χ2=7 804.081, P<0.01). The total usage rate of special-grade antimicrobials in ICU showed an upward trend year by year ( χ2=67.028, P<0.01). Since the implementation of the special-grade antimicrobial information management system in 2015, the total use rate of special-grade antimicrobials in the hospital, the use rate of various antibiotics except linezolid, amphotericin B and posaconazole, and the proportion of special-grade antimicrobials used in the hospital have all shown a downward trend year by year ( P<0.01). The total usage rate and total AUD of special-grade antimicrobials in ICU showed a decreasing trend year by year ( χ2=343.514, P<0.01, β=-0.963, P=0.002). Conclusion:The information management system for special-grade antimicrobial can effectively reduce the utilization rate and AUD of most special-grade antibiotics in hospitalized patients including ICU, and has a good clinical application value in antimicrobial stewardship.
5.Health-related quality of life of diabetes patients and related factors before and after healthcare reform in Gansu province

Objective:The study investigated and analyzed the health-related quality of life of diabetes patients in Gansu province before and after the healthcare reform and its influencing factors, so as to provide scientific evidence to improve the health-related quality of life of diabetes patients.Methods:The study used data of the fourth and sixth national health service household survey in Gansu province before (2008) and after (2018) medical reform separately, and EQ-5D health utility index was calculated on the basis of Chinese time frade-off values for EuroQol Five-Dimensions Questionnaire Utility Value scoring system.Results:Compared with the period before the healthcare reform, the five dimensions of EQ-5D for residents, aged 15 and above in Gansu, changed significantly after the healthcare reform: action capability improved by 8.08% (27.43% vs. 19.35%), self-care improved by 16.16% (26.55% vs. 10.39%), normal activity improved by 8.97% (28.32% vs. 19.35%), pain/discomfort worsened by 1.38% (38.05% vs. 39.43%), anxiety/depression worsened by 1.83% (16.81% vs. 18.64%), and the EQ-VAS score increased by 3.36 (60.53 vs. 63.89). The health utility index increased by 0.04 (0.83 vs. 0.87). Multivariate regression analysis results showed that the dimension influence factors were not completely consistent, on the whole, the health-related quality of life of diabetes patients with older age, lower education level, no physical exercise and multi chronic diseases was worse ( P<0.05), and multiple chronic disease had the most serious effect on the health of patients with diabetes. The health-related quality of life of diabetes patients with the medical insurance in urban area and after the new medical reform was higher ( P<0.05). Conclusion:The new healthcare reform has active influence on the health-related quality of life of diabetes patients in Gansu province, health care providers and policy makers should pay attention to the impacts of multi prechronic diseases on the health-related quality of life of diabetes patients and the mental health of patients with diabetes.
6.Study on “Qi-invigorating”Effect and Its Mechanism of Total Saponins of Astragalus membranaceus on Rats with Qi-deficiency

OBJECTIVE:To study “Qi-invigorating”effect and its possible mechanism of total saponins of Astragalus membranaceus on rats with Qi-deficiency ,and to provide reference for elucidating the material basis of “Qi-invigorating”effect of A. membranaceus . METHODS :Forty male Wistar rats were randomly divided into normal group ,model group ,positive control group [Buzhong yiqi pills ,4.5 g/(kg·d)],A. membranaceus total saponins high-dose and low-dose groups [ 252,28 g/(kg·d),by the amount of total saponins] according to body weight ,with 8 rats in each group. Except for normal group ,the model of Qi-deficiency was made in other groups by the method of “diet disorder+fatigue ”. At the same time ,administration groups were given relevant medicine intragastrically ,and normal group and model group were given constant volume of water,once a day ,for consecutive 21 days. After last administration ,the general situation of rats was observed ;the body weight ,spleen index and thymus index of rats were detected ;weight-bearing swimming time was recorded ;the levels of spleen T lymphocyte subsets CD 3 and CD 4,the levels of ATP and ADP in liver tissue ,serum levels of ALB ,RBC and HBG in blood as well as the serum levels of SOD,MDA,lactate,LDH,CK,IL-2,IL-12 and TNF-α were all detected. RESULTS:Compared with normal group ,body weight,thymus index ,spleen index ,weight-bearing swimming time ,the level of spleen T lymphocyte subsets CD 3,ATP,ADP, ALB,IL-2 and IL- 12 were decreased or shortened significantly in model group (P<0.05 or P<0.01). The levels of MDA , lactate,CK and TNF-α were increased significantly (P< 0.05). Compared with model group ,body weight ,spleen index,weight-bearing swimming time ,the level of spleen T lymphocyte subsets CD 3 and the levels of ATP ,ADP,ALB, RBC and IL- 2 were increased significantly or prolonged(P<0.05);while the levels of MDA ,lactate,CK and TNF-α were decreased significantly in A. membranaceus total saponins high-dose group(P<0.05 or P<0.01).Weight-bearing swimming time ,the levels of ATP ,ADP and IL- 2 in A. membranaceus total saponins low-dose group were increased significantly or prolonged (P<0.05 or P<0.01),while the levels of MDA ,lactate,CK and TNF-α were decreased significantly (P<0.05 or P<0.01). Compared with positive control group ,spleen index ,spleen T lymphocyte subsets CD 3,weight-bearing swimming time and ATP level of A. membranaceus total saponins high-dose group were increased significantly or prolonged (P<0.05 or P<0.01),while MDA levels of A. membranaceus total saponins high-dose and low-dose groups were decreased significantly (P<0.05 or P<0.01). CONCLUSIONS :A. membranaceus total saponins can reduce the body ’s accumulation of blood lactic acid ,the activity of CK ,the level of lipid peroxide and regulate immunity to tonify Qi ,delay fatigue and improve exercise ability.
7. Changes of retinal microstructure in lamellar macular hole after vitrectomy

Objective:
To observe the changes of retinal microstructure in lamellar macular hole (LMH) after vitrectomy.
Methods:
A retrospective clinical observational study. Forty patients (41 eyes) with LMH and received vitrectomy in Ophthalmology Department of Peking University People’s Hospital from January 2014 to September 2018 were included in this study. Among them, 14 patients (15 eyes) were males and 26 patients (26 eyes) were females, with an average age of 67.8±8.6 years. There were 37 eyes with a lens and 4 eyes with an IOL. There were 29 eyes with LMH of tractional type, 7 eyes of degenerative type, and 5 eyes of mixed type. All patients underwent BCVA and OCT examinations. The BCVA examination was performed using the international standard visual acuity chart, which was converted into logMAR visual acuity. The average logMAR BCVA was 0.57±0.27; the mean macular retinal thickness (CRT) was 192.3±108.9 μm, the mean macular thickness (MRT) was 427.5±110.2 μm. Among the 29 eyes of tractional type, there were 17 eyes with retinal cavity, 8 eyes with macular retinoschisis, and 3 eyes with incomplete ellipsoid zone. Among the 7 eyes of degenerative type, there were 5 eyes with lamellar hole-associated epiretinal proliferation (LHEP), 5 eyes with retinal cavity, and 5 eyes with incomplete ellipsoid zone. Among the 5 eyes of mixed type, 2 eyes with LHEP, 1 eye with macular epiretinal membrane, and 4 eyes with incomplete ellipsoid zone. The average follow-up time after surgery was 12.8±5.2 months. Among them, 10 eyes were followed up for equal or greater than 24 months. After the surgery, the same equipment and method before the surgery were used for relevant examination. 8.Effects of Fatigue on Biomechanics of Landing and Sudden Cutting Tasks

Objective To determine the effect of fatigue on the risk of anterior cruciate ligament (ACL) injuries using a controlled experiment.Method Thirty-two young male adults were selected.Half of them were asked to complete landing and sudden cutting tasks when feeling fatigue,while the rest were asked to conduct the same tasks without fatigue.Results The results indicated that fatigue led to increased knee flexion and knee rotation of the support leg during the initial landing,It also resulted in significant increase in the peak knee flexion,knee internal rotation angle and knee internal rotation moment during the double support phase.Moreover,the peak value of the knee flexion angle and knee internal rotation angle increased significantly due to fatigue during the single leg support period.Conclusion Being fatigue,people might take the adaptive posture control to decrease the ground shock,which in turn decreases ACL injury risks.However,it can be neutralized by the increase in the rotational angle.
9.Polymorphism of Five X-STRs Loci with a New Pentaplex PCR

[Objective] To learn about the genetic diversity,we studied the five X-chromosomal STR (X-STR) loci in Guangdong Han Nationality Groups.[Methods] The five Loci (DXS6803,DXS981,DXS6809,DXS6789,and DXS7132) were amplified in a pentaplex PCR reaction.PCR products were analyzed using capillary electrophoresis and ABI prism 3100 Genetic Analyzer,with GeneMapper ID 3.1 Analysis Software.[Results] A total of 363 individuals (181 unrelated male and 182 unrelated female) from Guangdong Han population were tested,54 alleles were observed for these loci.Polymorphism information content is 0.6935 ~ 0.8177.Power of discrimination in females was 0.8976 ~ 0.9562.Mean exclusion chance for X-STR in standard trios with daughters was 0.7805 ~ 0.8467.[Conclusion] The five loci in the multiplex system provide high polymorphism information for forensic identification and paternity testing,particularly for difficult paternity deficiency cases.
