1.Identification of Qianghuo ( Notopterygii Rhizoma et Radix) dispensing granules with specific PCR

Objective:To establish a method for the identification of Qianghuo (Notopterygii Rhizoma et Radix) dispensing granules with PCR for the identification of dispensing granules.Methods:The collected samples were identified by DNA bar code, and the methods of extracting genomic DNA from Notopterygii Rhizoma et Radix, Qianghuo standard decoction and dispensing granules were established. Specific differential primers were designed based on ITS2 sequence, and the PCR amplification system and reaction conditions were optimized.Results:The PCR amplification system and reaction conditions were determined. The target bands of about 216 bp were obtained by amplification of Notopterygii Rhizoma et Radix, Qianghuo standard decoction and dispensing granules, and there was no interference from counterfeit products and blank.Conclusions:The specific PCR identification method of Qianghuo (Notopterygii Rhizoma et Radix) dispensing granules is established, which has good specificity. The detection limit of Notopterygii Rhizoma et Radix is 0.91 ng, and the detection limit of dispensing granules is 7.36 ng, which also provides a reference for the identification of other kinds of TCM dispensing granules.
2.The study on the relationship between transgingival characteristics of implant restorations and peri-implant health

Objective :
To investigate the relationship between the transgingival characteristics of implant restorations and peri-implant soft tissue with peri-implant mucositis and peri-implantitis using three-dimensional measurement techniques.
Methods :
A total of 121 bone-level implant restorations in posterior tooth positions from 73 patients were evaluated using three-dimensional data analysis software, based on clinical and radiographic examination data. The following characteristics were assessed: emergence angles, including the mucosal emergence angle(MEA), deep angle(DA), and total angle(TA); emergence profile; and peri-implant soft tissue. Generalized estimating equations were used to analyze the relationship between these characteristics and the occurrence of peri-implant mucositis and peri-implantitis.
Results:
TA and MEA showed significant statistical relevance to peri-implant mucositis. Implant restorations with a TA greater than 30° had a 1.7 times higher risk of developing peri-implantitis compared to those with a TA of less than 30°.
Conclusion
MEA and TA of bone-level implant restorations in posterior tooth positions are significantly correlated with peri-implant mucositis. An excessive TA increases the likelihood of peri-implantitis. It is recommended that both the coronal and overall emergence angles of implant restorations should not exceed 30°.
3.Study on the improvement mechanism of Huatan tongmai decoction on rats with polycystic ovary syndrome

OBJECTIVE To investigate the improvement mechanism of Huatan tongmai decoction on rats with polycystic ovary syndrome (PCOS) by regulating autophagy through phosphatidylinositol-3-kinase(PI3K)/protein kinase B(AKT)/mammalian target of rapamycin (mTOR) pathway. METHODS A total of 40 rats were randomly divided into blank group (purified water), model group (purified water), traditional Chinese medicine group [Huatan tongmai decoction, 24 g/(kg·d)] and chemical drug group [metformin, 0.16 g/(kg·d)], with 10 rats in each group. Except for blank group, other groups were given a combination of high-fat diet and intragastric administration of 1 mg/kg letrozole suspension to establish PCOS rat model. After modeling, they were given relevant medicine or water intragastrically, once a day, for 42 consecutive days. After the last administration, the pathological and ultrastructural changes of ovarian tissue were observed. The levels of follicle stimulating hormone (FSH), testosterone (T), estradiol (E2) ,luteinizing hormone (LH) in serum were detected,and the LH/FSH ratio was calculated. mRNA expressions of Beclin-1, p62 and microtubule-associated protein 1 light chain 3 (LC3) in ovarian tissue were detected. The expressions of related proteins of PI3K/AKT/mTOR pathway and autophagy in rat ovarian tissues were also detected. RESULTS Compared with blank group, the pathological damage and ultrastructural changes of the ovarian tissue in the model group rats were obvious, and a large number of autophagosomes could be seen in cells. The levels of T and LH and the LH/FSH ratio in serum, as well as mRNA and protein expressions of Beclin-1 and LC3, were increased significantly (P<0.05), while the levels of E2 and FSH in serum, as well as mRNA and protein expressions of p62 and the phosphorylation levels of PI3K, AKT and mTOR proteins in ovarian tissue, were significantly decreased (P<0.05). Compared with model group, the pathological damage of ovarian tissue in the administration groups was significantly reduced, the number of autophagosomes was smaller, and the expression levels of the above indicators were significantly reversed (P<0.05). CONCLUSIONS Huatan tongmai decoction can inhibit autophagy in ovarian granular cells by activating the PI3K/AKT/mTOR pathway, regulate the secretion of sex hormones, alleviate pathological damage in ovarian tissues, and promote normal follicular development, thereby exerting an ameliorative effect on PCOS rats.
4.Survey of influenza awareness of community residents in nanchang during routine epidemic prevention and control

Objective To investigate influenza awareness of community residents in Nanchang under routine epidemic management using the Knowledge-Attitude-Practice(KAP)framework.Methods A structured questionnaire,including four do-mains(disease overview,prevention,diagnosis,and treatment),was designed and administered via street and online surveys from December 2023 to February 2024.Statistical analyses,including chi-square tests and logistic regression,were performed to identify determinant factors.Results A total of 303 valid questionnaires were retrieved,with an effective rate of approximately 94.7%.Community residents in Nanchang demonstrated a certain degree of understanding of the overview,prevention,diagno-sis,and treatment of influenza,although some gaps were identified.Univariate analysis revealed statistically significant differ-ences in influenza cognition scores among urban and rural residents(P<0.05),residents with different educational levels(P<0.05)and exposure levels to influenza education(P<0.05).Multivariate regression analysis identified these as influencing fac-tors for influenza awareness.Conclusion Collaborative efforts from the government,individuals,and various societal sectors are essential to enhance the public's overall ability to prevent and control influenza.

6.Effect of miR-22-3p regulation of NF-κB signaling pathway on palmitic acid-induced inflammation injury and apoptosis of hepatocytes

Objective:To investigate the role and potential regulation mechanism of miR-22-3p in lipotoxic hepatocyte inflam-matory injury and apoptosis caused by palmitic acid(PA).Methods:Human normal immortalized hepatocytes(LO2 cells)were treated with different concentrations of PA for 24 h.CCK-8 and qRT-PCR were used to detect cell proliferation and miR-22-3p expression.miR-22-3p mimics or inhibitors were transfected into LO2 cells and then treated with 0.32 mmol/L PA for 24 h,the expression level of miR-22-3p was determined by qRT-PCR;cell viability was determined by CCK-8;biochemical kits to determine the ALT and AST contents;the mRNA and protein expression levels of inflammatory cytokines TNF-α,IL-1β and IL-6 in intracellular and culture super-natants were determined by qRT-PCR and ELISA;the cell apoptosis rate in each group was determined by flow cytometry;the expres-sion levels of NF-κB signaling pathway-related proteins were detected by Western blot and immunofluorescence.Results:With the increaseing of PA concentration,the cell survival rate and the expression of miR-22-3p decreased in a dose-dependent manner.After PA treatment,the cell proliferation activity decreased significantly,the activities of ALT and AST enzymes were increased,the expressions of inflammatory cytokines TNF-α,IL-1β and IL-6 were increased,cell apoptosis was increased,and NF-κB signaling pathway was activated.Transfection of miR-22-3p mimics significantly increased the proliferation activity of LO2 cells,and decreased the levels of ATL,AST,TNF-α,IL-1β,IL-6 and apoptosis,and inhibited the activation of NF-κB signaling pathway.Transfection of miR-22-3p inhibitor further activated NF-κB signaling pathway,and promoted cell inflammatory injury and apoptosis(all P<0.05).Conclusion:Overexpression of miR-22-3p can alleviate PA-induced apoptosis and inflammation,and the mechanism is related to inhibit the activation of NF-κB signaling pathway.
7.Genetic diversity analysis of oxacillinase in 241 clinical isolates of Pseudomonas aeruginosa

Objective:To analyze the carriage status, subtype distribution and flanking gene sequence characteristics of oxacillinases (OXA enzyme) in 241 clinical strains of Pseudomonas aeruginosa, and assess their roles in the drug resistance of Pseudomonas aeruginosa and ability to horizontally transfer across species. Methods:Clinical P. aeruginosa isolates were collected from four hospitals in Sanya, Tangshan, Zhangjiakou, and Beijing. The prevalence of oxacillinases and their flanking gene sequences was analyzed by whole-genome sequencing (NGS) and bioinformatic approaches. Results:A total of 241 isolates of P. aeruginosa were gathered, and 35 blaOXA subtypes were identified through screening of 252 blaOXA genes. These genes were classified into three subfamilies: blaOXA-50-like (241, 95.6%), blaOXA-1-like (9, 3.6%) and blaOXA-10-like (2, 0.8%). Among these, 11 subtypes (11, 31.4%) were novel blaOXA subtypes. Nine of these belonged to the blaOXA-50-like subfamily and were designated as blaOXA-1244, blaOXA-1245, blaOXA-1246, blaOXA-1250, blaOXA-1252, blaOXA-1253, blaOXA-1254, blaOXA-1255, and blaOXA-1256. The remaining two belonged to the blaOXA-10-like subfamily and were named blaOXA-1247 and blaOXA-1248. Compared to the amino acid sequence of OXA-10, the newly identified subtype OXA-1247 exhibited a mutation at position 117, where a valine was replaced by a leucine. This change was thought to improve the enzyme′s ability to hydrolyze carbapenems. In the analysis of the flanking sequences of the blaOXA genes, Class I integrons were identified in four bacterial strains. The variable regions of these integrons carried three distinct patterns of resistance gene cassettes: aac( 6′) -Ib-blaOXA-1247-ant( 3′′) -Ia, aac( 6′) -Ib-blaOXA-1248 and aac( 6′) -Ib- blaIMP-45-blaOXA-1-catB3. Among these, the strain BJ2326 carried a class I integron that was connected to the downstream IS CR1 element to form a composite class I integron structure, additionally carrying the resistance gene blaPER-1. Out of the 223 non-wild-type P. aeruginosa strains, 127 strains exhibited non-wild-type profiles to the four beta-lactam antibiotics MEM, CAZ, FEP, and TZP, with the combination of MEM+CAZ+FEP being the most prevalent, representing 57.0% of the total. Conclusions:The blaOXA genes in 241 clinical P. aeruginosa strains showed diversity. Some blaOXA genes had a co-transfer risk with the metallo-β-lactamase resistance gene blaIMP-45. Among the 11 newly discovered blaOXA subtypes, the new subtype OXA-1247 may have carbapenemase activity and potential for horizontal transfer.

9.Application of the win ratio method to clinical trials in cerebrovascular disease

Objective To introduce the concepts and principles of the win ratio method and to analyze it in the context of a case study of a clinical trial in cerebrovascular disease.Methods Based on the study of clopidogrel with aspirin in high risk patients with acute non disabling cerebrovascular events 2,and key prognostic factors,the outcome events were defined sequentially as ① time to death within 90 d,② time to recurrence of ischemic stroke within 90 d,③ time to moderate-to-severe hemorrhage within 90 d.Using clopidogrel combined with aspirin as the reference group,the winning ratio(Rw)of ticagrelor combined with aspirin was analyzed by the win ratio method,and the 95% confidence interval(CI)of Rw was estimated by the Bootstrap method and compared with the hazard ratio(HR)calculated by the competing risk model.Results When only fatal events were considered,the win ratio method suggested that the ticagrelor group was significantly better than the clopidogrel group,Rw=2.00(95% CI:1.52-2.47),and after stepwise inclusion of ischemic stroke and moderate-to-severe hemorrhage recurrence,the win ratio method yielded a value of 1.29(95% CI:1.25-1.57),and the HR value from Fine and Gray competing risk regression was 0.78(95% CI:0.65-0.95),both of which indicated that the efficacy of the ticagrelor group was superior to that of the clopidogrel group.Conclusion The win ratio method can be used to analyze clinical trials with composite endpoints after prioritizing multiple outcome variables,showing the advantages of win ratio and its promising application in cerebrovascular disease research.
10.Impact of early invasive blood pressure monitoring on outcomes in out-of-hospital cardiac arrest patients undergoing extracorporeal cardiopulmonary resuscitation

Objective:To investigate the impact of early invasive arterial blood pressure (IBP) monitoring on survival and neurological outcomes in out-of-hospital cardiac arrest (OHCA) patients undergoing extracorporeal cardiopulmonary resuscitation (ECPR).Methods:This retrospective cohort study analyzed 44 OHCA patients receiving ECPR between January 2021 and January 2023. Patients were divided into: Early intervention group : IBP established within 3 min of ECMO initiation; Late intervention group : IBP established after ICU admission. Baseline characteristics, ECMO parameters, and clinical outcomes were compared. Multivariable logistic regression (adjusted for age, initial rhythm, etc.) and Spearman's correlation were used.Results:This study included a total of 44 patients treated with OHCA and ECPR, divided into an early intervention group of 23 cases and a late intervention group of 21 cases. The early intervention group showed significantly higher: Survival to discharge (43.5% vs. 9.5%, P<0.05), Good neurological recovery (CPC 1-2: 34.8% vs. 9.5%, P<0.05).Early intervention independently predicted survival (adjusted OR=18.84, 95% CI:1.97-179.98, P=0.01). Stratified analysis by pH (cutoff 7.0) demonstrated consistent benefits in both pH>7.0 ( aOR=0.392, 95% CI:0.106-0.678) and pH≤7.0 subgroups ( aOR=0.385, 95% CI: 0.075-0.695; interaction P=0.183). Early IBP positively correlated with CPC scores ( ρ=0.40, P=0.007). Conclusions:Early IBP monitoring significantly improves survival and neurological outcomes in OHCA-ECPR patients, supporting its integration into standardized protocols.
