Background/Aims: Dysregulated cholesterol metabolism is a hallmark of hepatocellular carcinoma (HCC) that drives tumor initiation and progression. However, clinical targeting of cholesterol metabolism has yielded limited benefits due to stringent feedback in tumor cells. Identifying a central mediator capable of restoring cholesterol homeostasis within the cell’s intrinsically fine-tuned regulatory framework is urgently needed. Methods: We integrated a proteomic dataset from patients with cholesterol-dysregulated HCC into a global cholesterol metabolic regulatory network to identify potential therapeutic targets for disrupted cholesterol homeostasis. The prognostic significance of the candidate targets was further validated in an independent cohort through immunohistochemistry. Functional and mechanistic studies were conducted in vitro using HCC cell lines and in vivo using mouse models. The pharmacological efficacy of the candidate agent was evaluated in both subcutaneous and orthotopic HCC mouse models. Results: ER lipid raft-associated 1 (ERLIN1), a pivotal regulator of cholesterol metabolism reprogramming, was identified as an independent favorable prognostic indicator in HCC. ERLIN1 constrains HCC progression both in vitro and in vivo by stabilizing the INSIG1–SCAP–SREBP2 axis and maintaining the metabolic balance of intracellular cholesterol. Under hypoxia, impaired factor-inhibiting hypoxia-1-dependent hydroxylation of ASB11 at asparagine residues 90 and 92 enhances ASB11-mediated ERLIN1 degradation. Pharmacological targeting of this axis using zoledronic acid (ZoA) attenuated HCC progression by weakening the ASB11–ERLIN1 interaction and restoring cholesterol homeostasis. Conclusions ERLIN1 represents a druggable metabolic vulnerability in cholesterol-dysregulated HCC. Targeting the ASB11–ERLIN1 axis with the clinically approved ZoA reestablishes cholesterol homeostasis and offers a promising therapeutic strategy to overcome the current limitations of cholesterol-targeted HCC therapies.

Objective To observe the effects of Hedysarum Polybotrys polysaccharide(HPS)on the glucose metabolism of small intestinal smooth muscle in rats with diabetic gastroparesis(DGP)of spleen qi deficiency syndrome;To explore its mechanism based on AMPK/GLUT4 signaling pathway.Methods Totally 72 male Wistar rats were randomly divided into 12 in the blank group and the remaining 60 rats were assigned to the model group.The DGP spleen qi deficiency syndrome model was replicated by intraperitoneal injection of streptozotocin+irregular feeding with high-fat and high sugar feed combined with swimming exhaustion method.The model rats were randomly divided into model group,metformin group and HPS high-,medium-and low-dosage groups.The metformin group was given 100 mg/kg metformin hydrochloride by gavage,while the HPS high-,medium-and low-dosage groups were given 200,100 and 50 mg/kg HPS by gavage,respectively.The blank group and model group were given purified water by gavage once a day for 8 consecutive weeks.The gastric emptying rate and small intestine propulsion rate in rats were detected,HE staining was used to observe the smooth muscle morphology of gastric antrum and ileal tissue,immunofluorescence staining was used to detect the expression of glucagon like peptide-1(GLP-1)in ileal tissue,ELISA was used to detect the contents of adiponectin(APN),glucagon(Glu)and insulin(INS)in serum,Western blot was used to detect the protein expressions of glucose transporter(GLUT)4,GLUT1,AMP-activated protein kinase(AMPK)and p-AMPK in ileal tissue.Results Compared with the blank group,the model group rats showed significantly increased random blood glucose,significantly decreased body mass,gastric emptying rate and small intestinal propulsion rate(P<0.01);the edema in the submucosal layer,loose arrangement of connective tissue,infiltration of a small number of lymphocytes,granulocytes and macrophages,reduced number of goblet cells in the ileal tissue,shedding of intestinal villous epithelial cells and widening of the gap between the submucosal layer and the lamina propria;the expression of GLP-1 in ileal tissue was significantly decreased(P<0.05),the contents of serum APN and INS were significantly decreased,and the content of Glu significantly increased(P<0.01),the expressions of GLUT4 and p-AMPK/AMPK proteins in ileal tissue were significantly decreased,while the expression of GLUT1 protein significantly increased(P<0.01).Compared with the model group,rats in metformin group and HPS high-dosage group showed a random decrease in blood glucose,an increase in body mass,gastric emptying rate,and small intestine propulsion rate(P<0.05,P<0.01);a more regular arrangement of gastric antral tissue cells,a reduction of shedding cells and edema,the smooth muscle structure of the ileal tissue was relatively intact,with evenly distributed cells and reduced infiltration of inflammatory cells;the expression of GLP-1 in ileal tissue increased,the contents of serum APN and INS increased,and the content of Glu decreased,the expressions of GLUT4 and p-AMPK/AMPK proteins in ileal tissue increased,while the expression of GLUT1 protein decreased(P<0.05,P<0.01).Conclusion HPS may up-regulate GLUT4 and down-regulate GLUT1 expression through activates AMPK,promotes glucose uptake and utilization by small intestinal smooth muscle,and improves glucose metabolism of DGP rats with spleen qi deficiency syndrome.

AIM:This study aims to investigate the mechanism by which Qingfei-Jiedu-Huatan Formula(QJHF)regulates autophagy in alveolar macrophages through mTOR in the treatment of severe pneumonia(SP)in rats.METHODS:Sixty SPF-grade male rats were randomly assigned to six groups:control,model,QJHF,moxifloxacin(MOX),rapamycin(RAPA),and QJHF+RAPA,with ten rats in each group.An SP rat model was established using Klebsiella pneumoniae.After seven days of treatment,changes in IL-33 and IFN-γ levels in bronchoalveolar lavage fluid(BALF)were measured using ELISA.Histopathological alterations in lung tissue were assessed via HE staining,and the autophagy of alveolar macrophages was detected using immunofluorescence co-localization methods.The expression levels of mTOR,beclin-1,and LC3 mRNA in lung tissue were analyzed using qPCR,while Western blot was employed to assess the protein levels of p-mTOR/mTOR,beclin-1,and LC3-II/LC3-I.RESULTS:Compared to the control group,the model group exhibited a deteriorated condition,characterized by alveolar wall rupture and thickening,significant inflammatory cell infiltration in the alveolar cavity,and extensive lung tissue damage(P<0.01).Elevated levels of IL-33 and IFN-γ in BALF were also observed(P<0.01),along with increased colocalization of CD68 and LC3 in immunofluorescence analy-sis.The mTOR mRNA expression in lung tissue decreased(P<0.01),while LC3 and beclin-1 mRNA expressions in-creased(P<0.01).Additionally,the protein expression ratio of p-mTOR/mTOR decreased(P<0.01),whereas LC3-II/LC3-I and beclin-1 protein levels increased(P<0.01).In comparison to the model group,significant improvements were noted after treatment with QJHF and MOX(P<0.01),while RAPA treatment led to a worsening of these indicators(P<0.05).A slight improvement was observed with the QJHF combined with RAPA intervention,though this was not statisti-cally significant.No significant differences were found between the MOX and QJHF groups.However,the QJHF+RAPA group displayed notable improvements in various indicators compared to the RAPA group(P<0.05).CONCLUSION:The QJHF can mitigate the inflammatory response associated with severe pneumonia,potentially by activating mTOR phos-phorylation activity,which in turn inhibits excessive autophagy in alveolar macrophages.

Objective:To explore the effect of combining transcranial magnetic stimulation (rTMS) with acupuncture in the treatment of patients with vascular cognitive impairment (VCI).Methods:A total of 40 VCI patients were randomly assigned to either the control group or the experimental group ( n=20 per group) using a random number table. Both groups received routine treatment and rTMS, while the experimental group was additionally provided with acupuncture for four weeks. The rTMS was at 10Hz at 90% of the motor threshold. Before and after the treatment, both groups′ cognition was evaluated using the Montreal Cognitive Assessment (MoCA) and the Mini-Mental State Examination (MMSE). Ability in the activities of daily living (ADL) was quantified using the modified Barthel Index (MBI). Serum levels of interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α) were measured using enzyme-linked immunosorbent assays. Results:Significant improvement was observed in both groups′ average MoCA, MMSE, and MBI scores, along with reductions in peripheral serum IL-6 and TNF-α levels. The experimental group exhibited significantly higher MoCA subdomain scores in the visuospatial and executive function, attention, and delayed recall subdomains, as well as a higher total MoCA score compared to the control group. The average MMSE and MBI scores were significantly higher in the experimental group than the control group, and that group′s serum IL-6 and TNF-α levels were significantly lower, on average. Pearson correlation analysis demonstrated a positive correlation between the improvements in MoCA and MMSE scores and the reductions in IL-6 and TNF-α levels in both groups.Conclusions:rTMS combined with acupuncture effectively improves cognition and ADL ability among VCI patients. The underlying mechanism may be associated with the reduction of neuroinflammatory factors IL-6 and TNF-α.

Thyroid-associated ophthalmopathy(TAO)is an autoimmune disease whose etiology and pathogenesis have not yet been clarified.It is generally believed that its occurrence is the result of a combination of immune,genetic,envi-ronmental,and other factors.In recent years,scholars have paid more attention to the influence of microorganisms on TAO,and they found that alterations of the ocular surface and intestinal flora can stimulate the immune system to generate a series of immune responses,which induces autoimmune responses to TAO.The"gut-eye axis"may be an important fac-tor involved in the development of TAO.The intestinal flora and its metabolites may affect ocular tissues through blood cir-culation and the lymphatic system.This interaction between the gut and eye axis may affect the occurrence and develop-ment of TAO through various pathways,including migration of immune cells,cross immune reactions,and metabolite cir-culation.This article reviews the research progress on the correlation between alterations of the ocular surface and intesti-nal flora and TAO and explores the possible mechanisms of the flora involvement in the development of TAO,providing new ideas for the diagnosis and treatment of TAO.

Objective To explore the effect of enhanced external counterpulsation(EECP)on ser-um vascular endothelial growth factor(VEGF)level and cardiopulmonary function in patients with stable angina pectoris(SAP).Methods A prospective double-blind controlled trial was con-ducted on 300 SAP patients admitted to Departments of Cardiovascular Medicine and Geriatrics of Jincheng People's Hospital from January 2021 to December 2023.They were randomly divided in-to study group(150 cases)and control group(150 cases).Coronary angiography or coronary CT angiography indicated that there were 101,101 and 98 cases,respectively,of one-,two-and three-vessel diseases.The patients of both groups were given conventional drug therapy(secondary pre-ventive drug for coronary heart disease),and those of the study group received EECP therapy ad-ditionally.The changes in serum VEGF level and related indicators of cardiopulmonary exercise test before and after one course of standardized treatment(36 h)were observed and compared be-tween the two groups.Results The VEGF level was significantly higher in the patients with three-vessel disease than those with two-and one-vessel diseases(P＜0.05).The level was obvi-ously increased in the study group and control group after intervention than before(238.41±82.57 ng/L vs 218.75±82.58 ng/L,224.47±85.08 ng/L vs 218.96±83.04 ng/L,P＜0.01),and the former group obtained better improvement of VEGF than the latter group(P＜0.01).The level in the study group and the control group with one-,two-and three-vessel lesions was notably higher than those before intervention(P＜0.05,P＜0.01).The study group with one-,two-and three-vessel lesions had statistical differences in the VEGF level after intervention when compared with that in the control group(P＜0.01).The levels of Peak VO2/kg and VO2/kg@AT were sig-nificantly increased in both groups after intervention than before(P＜0.01),with those of the study group notably higher than those of the control group[19.87±5.18 ml/(kg·min)vs 17.15±5.18 ml/(kg·min),P＜0.01;14.33±2.24 ml/(kg·min)vs 12.81±1.57 ml/(kg·min),P＜0.01].Conclusion EECP treatment is helpful to increase VEGF level and improve cardiopul-monary function in SAP patients.

Objective To explore the application of ropivacaine local infiltration anesthesia combined with patient controlled intravenous analgesia(PCIA)for postoperative analgesia and its influences on pain stress indexes in elderly patients after laparoscopic cholecystectomy(LC).Methods 518 elderly patients underwent LC under general anesthesia in the hospital were retrospectively enrolled from January 2020 to October 2023.According to different analgesia regimens,they were divided into group A(n=316,ropivacaine local infiltration anesthesia combined with PCIA)and group B(n=202,PCIA).The perioperative anesthesia indexes,scores of visual analogue scale(VAS)and Ramsay sedation after surgery,dosage of PCIA drug,hemodynamics,pain stress indexes[adrenaline(AD),norepinephrine(NE),cortisol(Cor)]and adverse events at 24 h after surgery were compared between the two groups.Results There were no differences in anesthesia time,recovery time,extubation time or intraoperative dosages of main anesthetics(dexmedetomidine,remifentanil and cisatracurium besilate)between the two groups(P＞0.05).Rest VAS and motion VAS in group A were lower than those in group B at 6,12 and 24 h after surgery,Ramsay sedation scale were higher than those in group B at 12 and 24 h after surgery,the differences were statistically significant(P＜0.05),dosage of PCIA drug and effective compression times were lower than those in group B at 24 h after surgery,the differences were statistically significant(P＜0.05).There was no difference in percutaneous arterial oxygen saturation(SpO2)level between the two groups(P＞0.05).The levels of mean arterial pressure(MAP),heart rate(HR),AD,NE and Cor in group A were lower than those in group B,the differences were statistically significant(P＜0.05).There was no difference of adverse events rate between the two groups at 24 h after surgery(P＞0.05).Conclusion Ropivacaine Calot's triangle block combined with PCIA for postoperative analgesia can effectively reduce rest VAS and motion VAS,improve sedation effect,maintain hemodynamics balance and reduce levels of pain stress factors in elderly patients after LC,with good safety.
