Objective:To analyze the impact of lung-protective ventilation strategy on postoperative pulmonary function and prognosis in abdominal surgery patients with a history of coronavirus disease 2019 (COVID-19) infection. Methods:A total of 80 patients with a history of COVID-19 who underwent emergency or elective abdominal surgery at the Central Hospital of Yongzhou from December 2023 to April 2025 were selected. They were divided into the lung-protective group ( n=40) and the traditional ventilation group ( n=40) using a random number table method. The lung-protective group was set with a tidal volume (VT) of 6 ml/kg, positive end-expiratory pressure (PEEP) of 5 cmH 2O, and lung recruitment was performed every 30 minutes; the traditional ventilation group was only set with a VT of 8-10 ml/kg. Blood gas analysis indicators [partial pressure of arterial carbon dioxide (PaCO 2) and partial pressure of arterial oxygen (PaO 2)] and pulmonary function indicators [forced expiratory volume in one second (FEV 1), forced vital capacity (FVC), and FEV 1/FVC] were compared between the two groups before surgery and 3 days after surgery. The modified clinical pulmonary infection score (MCPIS) was calculated, and the incidence of postoperative complications was statistically analyzed in both groups. Results:There were no statistically significant differences in blood gas analysis and pulmonary function indicators between the two groups before surgery (all P>0.05). Compared with preoperatively, PaCO 2 decreased and PaO 2 increased in both groups 3 days after surgery (all P<0.05); 3 days after surgery, PaCO 2 in the lung-protective group was lower than that in the traditional ventilation group, and PaO 2 was higher than that in the traditional ventilation group (all P<0.05). Compared with preoperatively, FEV 1 and FVC decreased, and FEV 1/FVC increased in both groups 3 days after surgery (all P<0.05); 3 days after surgery, FEV 1, FVC, and FEV 1/FVC in the lung-protective group were all higher than those in the traditional ventilation group (all P<0.05). The MCPIS of the lung-protective group 3 days after surgery was lower than that of the traditional ventilation group, with a statistically significant difference [(2.75±0.45) vs (4.23±0.68), t=11.479, P<0.05]. There was no statistically significant difference in the total incidence of complications between the lung-protective group and the traditional ventilation group ( P>0.05). Conclusions:Lung-protective ventilation strategy can improve postoperative blood gas analysis indicators and pulmonary function in abdominal surgery patients with a history of COVID-19, thereby improving prognosis, with good safety.

Journal of Biomedical Engineering 2024;41(6):1185-1194
Cerebral hemorrhage is a serious cerebrovascular disease with high morbidity and high mortality, for which timely diagnosis and treatment are crucial. Electrical impedance tomography (EIT) is a functional imaging technique which is able to detect abnormal changes of electrical property of the brain tissue at the early stage of the disease. However, irregular multi-layer structure and different conductivity properties of each layer affect image reconstruction of the brain EIT, resulting in low reconstruction quality. To solve this problem, an image reconstruction method based on an improved densely-connected fully convolutional neural network is proposed in this paper. On the basis of constructing a three-layer cerebral model that approximates the real structure of the human head, the nonlinear mapping between the boundary voltage and the conductivity change is determined by network training, which avoids the error caused by the traditional sensitivity matrix method used for solving inverse problem. The proposed method is also evaluated under the conditions with or without noise, as well as with brain model change. The numerical simulation and phantom experimental results show that conductivity distribution of cerebral hemorrhage can be accurately reconstructed with the proposed method, providing a reliable basis for the diagnosis and treatment of cerebral hemorrhage. Also, it promotes the application of EIT in the diagnosis of brain diseases.

ObjectiveTo analyze the long-term trends of the disease burden of diabetes attributed to metabolic factors in China from 1990 to 2019, and provide scientific recommendations for diabetes prevention and control in China. MethodsDescriptive analysis was conducted on the disease burden data of diabetes attributed to metabolic factors in China from 1990 to 2019, obtained from GBD 2019, encompassing death form diabetes, disability-adjusted life years (DALY), years of life lost (YLL), and years lived with disability (YLD). Joinpoint regression models were employed to analyze the long-term trends in mortality and DALY rates. Furthermore, the study examined the impact of two metabolic risk factors, high fasting plasma glucose (FPG) levels and high body mass index (BMI) levels, on the disease burden of diabetes. ResultsFrom 1990 to 2019, the overall standardized mortality and DALY rates attributed to metabolic factors for diabetes in the general population in China showed an upward trend, with both average annual percent changes (AAPCs) of 0.1% in the total population. The trend was upward in males with AAPCs of 0.9% and 0.6%, while it was downward in females with AAPCs of -0.4% and -0.3%. As age increased, the disease burden of diabetes attributed to metabolic factors showed an upward trend, with high FPG and high BMI ranking as the top two attributing risk factors. The disease burden of diabetes attributed to metabolic factors was higher in Chinese males than females. ConclusionThe disease burden of diabetes attributed to metabolic factors is increasing among the overall population and particularly among males, while the burden for female is declining. There is a need to increase intervention efforts for males aged 65 and above, provide scientific guidance on residents’ diet and lifestyle habits, and control blood glucose and body weight.
