1.Causal relationship between gout and Alzheimer's disease: a two-sample Mendelian randomization analysis

BackgroundDementia seriously affects the quality of life and lifespan of elderly people, with Alzheimer's disease (AD) being the most common type of dementia. Previous studies have suggested that gout may reduce the risk of developing AD, but the causal relationship between the two still requires further research. ObjectiveTo investigate the potential causal relationship between gout and AD through a two-sample Mendelian randomization (MR) analysis, so as to provide references for the prevention and treatment of AD. MethodsData from Genome-Wide Association Studies (GWAS) extracted in 2024 were analyzed, using pooled data on gout (6 810 cases in the case group and 477 788 cases in the control group) published by UK Biobank in 2021 as the exposure variable, and data on AD (3 899 cases in the case group and 214 893 cases in the control group) published by FinnGen in the same year as the outcome variable. The inverse-variance weighted, MR-Egger regression, weighted median estimation, simple model and weighted model were used to analyze the potential causal relationship between gout and AD. Pleiotropic effects were assessed using MR-Egger regression. Heterogeneity assessment was conducted using Cochran's Q test. The leave-one-out analysis was carried out for sensitivity analysis. And a funnel plot was drawn to detect potential publication bias. ResultsThe inverse-variance weighted analysis demonstrated a negative causal relationship between gout and AD (OR=0.004, 95% CI: 0~0.700, P<0.05). The plot resembled a symmetrical inversed funnel, indicating the absence of publication bias. No heterogeneity was detected by Cochran's Q test. The MR-Egger regression indicated no significant horizontal pleiotropy. Concerning the reverse directions, no significant associations between AD and gout were noted. ConclusionThere is a negative causal relationship between gout and AD, with gout potentially reducing the risk of developing AD. 2.Progress and prospect of the antihypertensive effect from Bidens pilosa L.

Hypertension is a systemic chronic vascular disease. From the perspective of Traditional Chinese Syndromes, hypertension belongs to the category of liver fire, vertigo, liver yang, headache and so on. Chinese medicine treatment of hypertension has gradually become a hot research topic, and using Chinese herbal medicine to reduce blood pressure has also achieved good results. In recent years, researches on anti-hypotension of Bidens pilosa L. has gradually increased. The related research of Bidens pilosa L., including the ancient literature, modern research, functional components and mechanism were mainly summarized, the application of Bidens pilosa L. in lowering blood pressure were anticipated, with a view to provide reference for the further development and utilization of Bidens pilosa L. in treatment of hypertension.
3.Association between urinary metal mixtures and mild cognitive impairment among elderly residents in Guangdong compounds

Background Environmental metal exposure is closely associated with the onset and progression of mild cognitive impairment (MCI) in the elderly. Effectively identifying hazardous metal exposure and assessing their interaction effects have significant public health implications. Objective To explore the relationship between urinary single metal and metal mixture exposure and MCI in elderly compound residents. Methods This study included 391 elderly individuals aged 60 and above from residential compounds in Zhongshan City, Guangdong Province. Concentrations of iron (Fe), copper (Cu), selenium (Se), arsenic (As), cadmium (Cd), manganese (Mn), chromium (Cr), nickel (Ni), vanadium (V), cobalt (Co), antimony (Sb), thallium (Tl), zinc (Zn), calcium (Ca), and magnesium (Mg) in urine were measured using inductively coupled plasma mass spectrometry. Cognitive function in the elderly was assessed using the Chinese version of the Mini-Mental State Examination (MMSE). Logistic regression was used to explore the relationship between single metal exposure level and MCI. LASSO regression and multi-metal logistic regression models were used to identify key metal ions associated with MCI. Bayesian kernel machine regression (BKMR) was employed to analyze the relationship between key metal ion mixtures and MCI, as well as the interactions between metals. Age, gender, education level, occupation, and body mass index were adjusted as covariates. Results A total of 78 among the 391 elderly individuals surveyed (19.94%) were diagnosed with MCI (MCI group), and the other 313 individuals were controls. The levels of Se, Cd, Mn, and As in the urine of the MCI group were significantly higher than those in the control group (P < 0.05). In the single-metal model, after adjusting for covariates and using the first quartile (Q1) of each metal concentration as the reference, the OR for MCI in the elderly in the Q4 group of Se was 2.190 (95%CI: 1.017, 4.716); for Cd, the OR was 2.345 (95%CI: 1.041, 5.283) in the Q3 group and 2.371 (95%CI: 1.043, 5.393) in the Q4 group; for Mn, the OR was 2.355 (95%CI: 1.038, 5.344) in the Q2 group; for As, the OR was 3.377 (95%CI: 1.442, 7.908) in the Q3 group and 2.886 (95%CI: 1.227, 6.788) in the Q4 group; for Sb, the OR was 2.779 (95%CI: 1.234, 6.257) in the Q2 group. When urinary metal concentrations were ln-transformed and included as continuous variables in the single-metal model, Cd concentration was positively correlated with MCI (OR=1.377; 95%CI: 1.008, 1.882; P=0.044). Cd, Se, Mg, Ca, Mn, As, Cr, Co, Tl, and Sb were selected by the LASSO regression model and included in the multi-metal model. In the multi-metal model, compared with Q1, the OR for MCI in the elderly was 0.395 (95%CI: 0.164, 0.953) in the Q2 group of Co and 0.390(95%CI: 0.167,0.911) in the Q3 group of Co; for Mn, the OR in the Q2 group was 2.636 (95%CI: 1.053, 6.596); for Sb, the OR in the Q2 group was 2.640 (95%CI: 1.047, 6.658). As continuous variables, Mg (OR=0.472; 95%CI: 0.248, 0.899; P=0.022) and Co (OR=0.857; 95%CI: 0.737, 0.996; P=0.044) concentrations were negatively correlated with MCI. The BKMR mixture analysis suggested that Mg and Co exhibited a synergistic negative correlation with MCI, while Mn and Sb exhibited a synergistic positive correlation with MCI. Mg and Co attenuated the positive correlation of Mn and Sb with MCI, whereas Mn weakened the protective effects of Mg and Co. Conclusion Elevated levels of Se, Cd, As, Mn, and Sb in urine may increase the risk of MCI in the elderly, while Mg and Co have protective effects. Potential synergistic or antagonistic interactions may be found among Mn, Sb, Mg, and Co, which should not be overlooked in terms of their impact on the cognitive function of the elderly.
4.Establishment and validation of depressive symptom predictive model in middle school students

Objective:
To investigate the influencing factors of depressive symptoms and to construct and verify the prediction model of depressive symptoms in middle school students, so as to provide risk assessment tools for effectively screening depressive symptom.
Methods:
Physical examination and questionnaire survey were conducted among middle school students in one city in Guangdong Province from September to October in 2021 ( n =2 376) and from September to October in 2022 ( n =4 344) by a multistage cluster sampling method, and a nomographic prediction model of depressive symptoms in middle school student was constructed. The questionnaire survey was conducted using the student health status and influencing factors questionnaire (secondary school version) and the Center for Epidemiological Studies Depression Scale (CES-D) to measure the lifestyle and depressive symptom of middle school students.
Results:
The detection rate of depressive symptoms in 2021 was 23.3%. Multivariate Logistic regression analysis showed that irregular breakfast ( OR =2.64), school bullying ( OR =4.28), being beaten by parents ( OR =2.86), using mobile devices for a long time ( OR =1.08) and sitting for a long time ( OR =1.05) were positively related to depressive symptoms in middle school students ( P <0.05). Long sleep duration ( OR =0.78) and outdoor activity durations of 1-<2, 2-<3 and ≥3 h/d (compared with <1 h/d, OR =0.63, 0.61, 0.49) were negatively related to depressive symptoms in middle school students ( P < 0.05 ). Multivariate Logistic regression analysis showed that 7 statistically signifucant predictive factors constructed a nomogram, and the AUC of the nomogram was 0.77, which had been verified internally and externally with good differentiation and reliability.
Conclusions
The nomogram prediction model of depressive symptoms provides a convenient and effective risk assessment tool for depressive symptoms among middle school students. The life behavior, diet behavior and injury behavior of middle school students play an important role in the formation of depressive symptoms. It should pay attention to the impact of the behavioral factors on the mental health of middle school students.
5.Wireless closed-loop deep brain stimulation using microelectrode array probes

Deep brain stimulation(DBS),including optical stimulation and electrical stimulation,has been demonstrated considerable value in exploring pathological brain activity and developing treatments for neural disorders.Advances in DBS microsystems based on implantable microelectrode array(MEA)probes have opened up new opportunities for closed-loop DBS(CL-DBS)in situ.This technology can be used to detect damaged brain circuits and test the therapeutic potential for modulating the output of these circuits in a variety of diseases simultaneously.Despite the success and rapid utilization of MEA probe-based CL-DBS microsystems,key challenges,including excessive wired communication,need to be urgently resolved.In this review,we considered recent advances in MEA probe-based wireless CL-DBS microsystems and outlined the major issues and promising prospects in this field.This technology has the potential to offer novel therapeutic options for psychiatric disorders in the future.
6.Exploration and Current Status Analysis of Pharmaceutical Service Fees in China

Objective To summarize the exploration and current situation of pharmaceutical service fees in China,to analyze it in conjunction with practices in foreign countries,and to provide references for further improvement of pharmaceutical service fees in China.Methods The official websites of the National Health Commissi,National Healthcare Security Administration,and their provincial braches were retrieved to summarize the changes in policies in China and the latest pharmaceutical service fees policies in each province.At the same time,databases such as CNKI,VIP,and Pubmed were searched to summarize research on pharmaceutical service fees.Results There were some studies in China on the calculation of pharmaceutical service costs and fee standards,but they mostly focused on pharmacy dispensing services,with less discussion on the technical and labor value of specific pharmaceutical services,while international research in this area was relatively abundant.Currently,eight provinces and one municipality directly under the central government have issued medical service price items related to pharmaceutical service,with differences in fee items,fee standards,medical insurance payment ratios,and monitoring and evaluation indicators.Conclusions The inclusion of pharmaceutical services in the national medical pricing system recognizes the transformation of pharmaceutical services and the value of pharmacists.However,it is still in the early stages,and there is room for improvement and enhancement.
7.Right heart modified myocardial performance index and ductus venosus spectrum parameters in pre-eclampsia fetuses for predicting adverse pregnancy outcomes

Objective To observe the values of changes of right heart modified myocardial performance index(Mod-MPI)and ductus venosus(DV)spectrum parameters in pre-eclampsia fetuses for predicting adverse pregnancy outcomes.Methods Eighty-one pregnant women diagnosed as pre-eclampsia were prospectively enrolled and divided into severe pre-eclampsia(SPE)group(n=39)and mild pre-eclampsia(MPE)group(n=42),while 85 healthy pregnant women were taken as controls(control group).Fetal right heart function parameters,including right ventricular isovolumetric relaxation time(IRT),isovolumetric contraction time(ICT),ejection time(ET),total spent time(TST),Mod-MPI,tricuspid valve peak flow velocity ratio in early and late diastole(TV-E/A),as well as blood flow velocities in each waveform of DV spectrum(S,V,D,and A wave)were obtained,and the pulsatility index(PI)and the ratio of blood flow velocities in each waveform of the DV(S/V,S/D,S/A,V/D,V/A,D/A)were calculated.Intrauterine fetal distress,preterm delivery,neonatal asphyxia and newborn with low weight were considered as adverse pregnancy outcomes.The correlations of right heart Mod-MPI and TV-E/A with DV parameters in pre-eclampsia fetuses were assessed,and their predictive efficacies for adverse pregnancy outcomes were evaluated for right heart Mod-MPI and DV using the receiver operating characteristics(ROC)and the area under the curves(AUC).Results Compared with control group and MPE group,fetal right heart IRT,ICT and Mod-MPI increased and ET decreased in SPE group(all P<0.05).No significant differences of right heart TST and TV-E/A among 3 groups(both P>0.05).Fetal DV A-wave velocity and V/D values progressively decreased but PI progressively increased in control,MPE and SPE groups(all P<0.05).Fetal right heart Mod-MPI in pre-eclampsia was moderately positively correlated with DV PI(r=0.637,P=0.016),while TV-E/A was weakly negatively correlated with DV V/D(r=-0.355,P=0.043).Adverse pregnancy outcomes were noticed in 59 pre-eclampsia cases.The AUC of fetal right heart Mod-MPI and DV PI for predicting adverse pregnancy outcomes in pre-eclampsia cases was 0.897 and 0.848,respectively,without significant difference(Z=0.460,P=0.400).Conclusion Changes of right heart Mod-MPI and DV spectrum parameters in pre-eclampsia fetuses had high value for predicting adverse pregnancy outcomes.
8.Prenatal ultrasonic manifestations of fetal rhomboencephalosynapsis

Objective To observe prenatal ultrasonic manifestations of fetal rhomboencephalosynapsis(RES).Methods Data of 15 singletons with RES were retrospectively analyzed,and the prenatal intracranial and extracranial ultrasonic manifestations were observed.Results All 15 fetuses were found with transverse cerebellar diameter lower than the 3rd percentile of normal value in the same gestational week and bilateral cerebellar hemisphere fusion(15/15,100%).Cerebellar vermis completely loss was observed in 14 fetuses(14/15,93.33%),while partially loss was noticed in 1 fetus(1/15,6.67%).Invisible of the fourth cerebral ventricle was detected in 4 fetuses(4/15,26.67%),while the fourth cerebral ventricular index(4Vi)<1 was found in 11 fetuses(11/15,73.33%).No"split line"sign between vermis and cerebellar hemisphere was detected.A total of 14(14/15,93.33%)were found with combining intracranial or extracranial malformations,including 13(13/14,92.86%)with other intracranial malformations,10(10/14,71.43%)with extracranial malformations,whereas 1(1/15,6.67%)fetus had no other malformation.Conclusion Prenatal ultrasonic manifestations of fetal RES mainly included small cerebellum transverse diameter,complete or partial loss of vermis,invisible or dysplasia of the fourth cerebral ventricle,fusion of bilateral cerebellar hemispheres,as well as combining with other intracranial and extracranial malformations.
9.Application of fetal heart quantitation in evaluation of ventricular function of fetuses with small-for-gestational-age and growth restriction

Objective:To investigate the value of fetal heart quantification (fetal HQ) in assessing ventricular function of fetuses with small-for-gestational-age (SGA) and fetal growth restriction (FGR).Methods:A total of 152 singleton pregnancies with estimated fetal weight (EFW) or abdominal circumference less than the 10th percentile in the Third Affiliated Hospital of Zhengzhou University were prospectively selected from August 2022 to September 2023, where fetal EFW or abdominal circumference were in the 3rd to 9th percentile with normal Doppler findings were classified as the SGA group ( n=79), and the rest as the FGR group ( n=73). In the same period, 161 cases of normal single fetuses were matched as the control group.Based on the gestational week in which FGR occurred, the FGR group were categorized into the early-onset group (<32 weeks, n=46) and the late-onset group (≥32 weeks, n=27), and fetuses in the FGR group with absent end-diastolic velocity of the umbilical artery were defined as severe FGR ( n=11), and the rest as mild FGR ( n=62). Fetal ventricular fractional area change (FAC), global longitudinal strain (GLS), longitudinal fractional shortening (LFS) and 24-segment fractional shortening (FS) were obtained by fetal HQ. The cardiac systolic function between groups were compared, the correlations between each parameter and gestational week were analyzed, the inter-observer and intra-observer repeatability tests were performed. Results:Compared with the control group, ventricular FAC, LFS, and GLS were lower in the SGA and the FGR group, right ventricular FS of segments 9-24 were reduced in the SGA group, and left ventricular FS of segments 10-19, 21-24 and right ventricular FS of segments 18-24 were reduced in the FGR group, the differences were statistically significant (all P<0.05). Left ventricular GLS, LFS and right ventricular FAC, GLS, LFS, FS of segments 1-14 were lower in the severe FGR group than in the mild FGR group, the differences were statistically significant (all P<0.05). The values of left ventricular GLS and LFS were higher in the early-onset FGR group than in the late-onset FGR group, the differences were statistically significant (all P<0.05). The ROC showed that the ventricular systolic function parameters predicted adverse perinatal outcomes with an AUC>0.6 (all P<0.05). Left ventricular GLS and right ventricular partial-segments FS were no correlations with gestational week in the FGR group (-0.3< rs<0.3, all P<0.05). There was no correlation between the parameters and gestational week in the SGA group (all P>0.05). The inter-observer and intra-observer intraclass correlation coefficients (ICC) were >0.75, with good reproducibility. Conclusions:Fetal HQ can quantitatively assess the changes of ventricular function in SGA and FGR fetuses, and the ventricular overall, longitudinal and localized contractile function in SGA and FGR fetuses are reduced, and abnormal ventricular systolic function is associated with adverse perinatal outcomes.
10.Preventive effect of polyglycolic acid sheet combined with oral glucocorticoid on esophageal stenosis after endoscopic submucosal dissection of large early esophageal cancer

A retrospective study was conducted on data of 45 patients who underwent endoscopic submucosal dissection (ESD) at Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology from January 2013 to March 2024 and were pathologically confirmed as having early esophageal cancer with postoperative defect ≥3/4 of the esophageal circumference, to evaluate the preventive effect of polyglycolic acid sheet (PGA) combined with oral glucocorticoid for esophageal stenosis after ESD of large early esophageal cancer. Patients were divided into two groups according to teatment: oral glucocorticoid alone group ( n=27) and PGA combined with oral glucocorticoid group ( n=18). The results showed that the postoperative esophageal stenosis rate was significantly lower in the PGA combined with oral glucocorticoid group compared with the oral glucocorticoid alone group [44.4% (8/18) VS 74.1% (20/27), χ2=4.034, P=0.043]. For cases of non-circumferential esophageal defects, only 1 case [9.1% (1/11)] experienced postoperative esophageal stenosis in the PGA combined with oral glucocorticoid group, while 10 cases [62.5% (10/16)] in the oral glucocorticoid alone group. During follow-up, no serious complications related to glucocorticoid use or adverse events related to PGA use were observed. These findings suggest that PGA combined with oral glucocorticoid is safe and effective for preventing esophageal stenosis after ESD for large early esophageal cancer and may be more effective than using only oral glucocorticoid.
