1.Three-dimensional videonystagmography characteristics in patients with benign paroxysmal positional vertigo

Objective To analyze the characteristics of nystagmus during the Dix-Hallpike and Roll tests in patients with benign paroxysmal positional vertigo (BPPV) using three-dimensional videonystagmography (3D-VNG), in order to to optimize diagnostic and therapeutic strategies of BPPV. Methods A retrospective analysis was conducted on 68 patients with posterior semicircular canal (PSC)-BPPV and 26 patients with horizontal semicircular canal (HSC)-BPPV. Nystagmus data obtained from 3D-VNG were reviewed for all patients, with a focus on the eye movement components during the Dix-Hallpike test in PSC-BPPV patients and the Roll test in HSC-BPPV patients. The direction and reversal rates of the vertical, horizontal, and torsional components were recorded and analyzed. Results All PSC-BPPV patients exhibited highly consistent three-dimensional nystagmus characteristics during the Dix-Hallpike test: vertical nystagmus was uniformly upward, torsional nystagmus was predominantly clockwise in left-side BPPV patients (17/23) and counterclockwise in right-side BPPV patients (44/45), while the horizontal component was mostly directed contralaterally (50/68); upon transitioning from the head-hanging to the sit-up position, vertical nystagmus components in all patients reversed, and torsional and horizontal nystagmus components reversed in approximately 50.0% or more patients. Among HSC-BPPV patients, right-side BPPV patients all showed right-beating (geotropic) horizontal nystagmus with predominantly upward vertical component (16/19), while most left-side BPPV patients showed left-beating horizontal nystagmus (6/7) with predominantly downward vertical component (6/7). During head rotation toward the healthy side, most (25/26) HSC-BPPV patients exhibited a reversal in the horizontal nystagmus direction, reduced intensity compared to the affected side, with a reversal in vertical components in 3 patients, and atypical torsional components. Conclusions 3D-VNG could precisely quantitative analyze three-dimensional features of nystagmus in BPPV patients, improve diagnostic accuracy in canal and side localization, particularly in PSC-BPPV patients.
2.Analyzing the mediating effect of sleep quality on the impact of occupational stress on depressive symptom among secondary industry workers

Objective To explore the mediating role of sleep quality on the impact of occupational stress on depressive symptom among secondary industry occupational population. Methods A total of 895 secondary industry workers in Wuxi City were selected as the study subjects using the stratified random sampling method. The Core Occupational Stress Scale (COSS), Patient Health Questionnaire-9 items (PHQ-9) and Self-Filled Sleep Questionnaire (SSQ) were used to investigate their occupational stress, depressive symptom and sleep quality. Depressive symptom, occupational stress and sleep quality were respectively regarded as dependent variable, independent variable and mediating variable. Process program was used to construct a mediation model, and Bootstrap method was used to test its mediating effect. Results The median score and 25th, 75th percentile for occupational stress, depressive symptom and sleep quality was 43.0 (43.0, 48.0), 6.0 (3.0, 8.0) and 3.0 (1.0, 3.0) points. The detection rates of occupational stress, depressive symptom, sleep disorder were 28.7%, 63.0% and 38.8%, respectively. Spearman correlation analysis result showed that occupational stress was positively correlated with both depressive symptom and sleep quality (Spearman correlation coefficients of 0.46 and 0.49, all P<0.01). Bootstrap test result showed that occupational stress had positive effect on both depressive symptom and sleep quality (partial regression coefficients of 0.25 and 0.09, all P<0.01). Sleep quality partially mediated the impact of occupational stress on depressive symptom with the mediation effect value of 0.05 (95% confidence interval of 0.03-0.06), accounting for 20.0% of the total effect. Conclusion Both depression symptom and sleep quality are positively correlated with occupational stress in secondary industry occupational population. Sleep quality of this population partially mediate the impact of occupational stress on depressive symptom.
3.Research on patient motion monitoring with domestic innovative integrated radiotherapy CybeRay ? real-time imaging for frameless stereotactic radiosurgery

Objective:To determine the motion detection uncertainty of the real-time CybeRay ? imaging system and patient intrafractional motion with thermoplastic mask-based immobilization. Methods:Real-time CybeRay ? imaging system was used for irradiation and treatment for head phantom and patients with brain tumors. All patients were immobilized with thermoplastic masks. Real-time imaging was delivered using kilovoltage projection images during radiotherapy. The detected patient motion data was collected from 5 head phantom measurements and 27 treatment fractions of 9 brain tumor patients admitted to Kaifeng Cancer Hospital. The accuracy and uncertainty of the motion monitoring system were determined. Results:The mean and standard deviation (SD) of the detected motion in the X, Y, and Z directions for phantom were (-0.02±0.41) mm, (-0.05±0.22) mm and (0.01±0.35) mm, respectively. The detected motion in the X, Y and Z directions for patents were (-0.13±0.48) mm, (-0.05±0.48) mm and (0.11±0.36) mm, respectively. After removing the motion detection uncertainty, the actual intrafractional motion of patients were (-0.11±0.25) mm, (0±0.43) mm and (0.10±0.08) mm in three directions, respectively. Conclusions:The uncertainty of real-time imaging-based motion monitoring system of CybeRay ? is less than 0.5 mm. It is feasible to apply thermoplastic masks for brain tumor patients in clinical practice, which can provide steady immobilization and limit the SD of patient intrafractional motion within 0.5 mm. Real-time imaging-based motion monitoring system of CybeRay ? is accurate for patient motion monitoring during frameless stereotactic radiosurgery/radiotherapy.
4.Value of preoperative serum vitamin A level in the prediction of benign and malignant pulmonary nodules

Objective:To explore the application value of preoperative serum vitamin A level in the prediction of benign or malignant pulmonary nodules.Methods:It was a retrospective cohort study. A total of 1 224 patients who underwent surgery for pulmonary nodules at the General Hospital of the People′s Liberation Army from January 2016 to December 2018 were consecutively included. The demographic information, postoperative pathological results, pulmonary CT findings and preoperative serum vitamin A test results were collected. The preoperative serum vitamin A levels of patients with lung cancer and benign pulmonary nodules were compared pairwise using the χ2 test. Logistic regression analysis was used to analyze the relevant factors for the occurrence of lung cancer and a stratified analysis was performed too. Prediction models for the benignity or malignancy of pulmonary nodules were constructed based on the results of multivariate logistic regression analysis. The efficacy of the models was evaluated, and the optimal preoperative prediction model was determined. The application value of preoperative serum vitamin A levels in predicting the benignity or malignancy of pulmonary nodules was then analyzed. Results:Of the 1 224 patients, postoperative pathology confirmed 1 044 cases with lung cancer and 180 cases with benign pulmonary nodules. The mean preoperative serum vitamin A level of patients with lung cancer was significantly lower than that in patients with benign pulmonary nodules (0.90 vs 1.06 μmol/L) ( Z=-3.493; P<0.001). Preoperative serum vitamin A level was a negative related factor for the occurrence of lung cancer ( OR=0.663, 95% CI: 0.484-0.914) ( P=0.011). In patients aged<60 years ( OR=0.623, 95% CI: 0.428-0.912), male ( OR=0.649, 95% CI: 0.438-0.976), with a body mass index≥24 kg/m 2 ( OR=0.634, 95% CI: 0.420-0.974), no family history of tumors ( OR=0.634, 95% CI: 0.440-0.923), no smoking history ( OR=0.619, 95% CI: 0.412-0.941), no drinking history ( OR=0.625, 95% CI: 0.424-0.933), with pulmonary nodules measuring 1-3 cm in diameter ( OR=0.643, 95% CI: 0.455-0.920), and with solid pulmonary nodules ( OR=0.681, 95% CI: 0.466-1.001), the preoperative serum vitamin A levels were significantly negatively correlated with the occurrence of lung cancer (all P<0.05). The prediction model incorporating preoperative serum vitamin A, CT characteristics of pulmonary nodules (nodule diameter, density), and clinical characteristics (age, gender) showed the best predictive efficacy for the benignity or malignancy of pulmonary nodules (the area under the curve was 0.792). Conclusions:Among patients receiving surgical treatment for pulmonary nodules, the preoperative serum vitamin A level of patients with lung cancer is lower than that of patients with benign pulmonary nodules. The preoperative serum vitamin A level is a negative associated factor for the occurrence of lung cancer. A combined model incorporating the preoperative serum vitamin A level provides a good prediction of benign or malignant pulmonary nodules.
5.Effect of electroconvulsive therapy on brain functional network in major depressive disorder.

Electroconvulsive therapy (ECT) is an interventional technique capable of highly effective neuromodulation in major depressive disorder (MDD), but its antidepressant mechanism remains unclear. By recording the resting-state electroencephalogram (RS-EEG) of 19 MDD patients before and after ECT, we analyzed the modulation effect of ECT on the resting-state brain functional network of MDD patients from multiple perspectives: estimating spontaneous EEG activity power spectral density (PSD) using Welch algorithm; constructing brain functional network based on imaginary part coherence (iCoh) and calculate functional connectivity; using minimum spanning tree theory to explore the topological characteristics of brain functional network. The results show that PSD, functional connectivity, and topology in multiple frequency bands were significantly changed after ECT in MDD patients. The results of this study reveal that ECT changes the brain activity of MDD patients, which provides an important reference in the clinical treatment and mechanism analysis of MDD.

6.Vibration Characteristics of Hollow Screws and Locking Plates for Treating Femoral Neck Fractures

Objective To investigate the influence from natural and constrained modal of the hip joint on internal fixation after implantation of hollow screws and locking plates for treating femoral neck fractures. Methods CT image data of a patient with femoral neck fracture were analyzed, boundary of the hip joint was extracted to generate a three-dimensional (3D) model of the hip joint, and the assembly of common internal fixation models and hip joint models was established. Finite element simulation was then conducted, with focus on vibration characteristics. Results Vibration had a certain effect on internal fixation stability of hollow screws and locking plates. The occurrence of torsion would destroy the fixation environment of hollow screws and locking plates, resulting in a small displacement at the fracture end. In a constrained state, the modal frequency range was lower. Deformation of the vibration shape mostly occurred at proximal end of the femoral head, leading to the loosening phenomenon of internal fixation and prosthesis. A method for judging the stability of internal fixation was proposed from the perspective of vibration characteristics, and it was found that the stability of internal fixation with hollow screws was better than that of proximal locking plates. Conclusions For choosing internal fixation, influences from natural frequency of the screw and plate should be considered, so as to avoid natural frequency of the femur, which may cause resonance. The results can be used as a guidance for the selection of internal fixation materials and the design of structure and configuration.
7.Trends in clinicopathological characteristics of surgically-treated lung cancer: sex-based heterogeneity

Objective:To explore the sex-based heterogeneity in demographic and pathological trends of lung cancer during the past 30 years.Methods:Patients with primary lung cancer who received surgical treatment in the Department of thoracic surgery, Shanghai Pulmonary Hospital Tongji University from 1989 to 2018 were retrospectively analyzed. The differences between male and female patients in age, smoking history, pathological stage and type were compared. Mann- Kendall trend test was performed for trend analysis. Results:A total of 58 433 patients were included in this study, encompassing 30 729(52.6%) men and 27 , 704(47.4%) women. Compared with male patients, female patients were younger(56.0 years old vs. 59.7 years old), and had a higher proportion of non-smokers(98.3% vs. 52.3%), stage Ⅰ lung cancers(60.6% vs. 49.3%), and adenocarcinoma(93.7% vs. 56.1%, all P-values <0.001). Trend analyses revealed that the proportion of female patients increased year by year, and surpassed males in 2015, with the current ratio of male to female being 1∶1.5. After 2013, the age of onset in females was getting younger, and the average age decreased from 58.7 years old to 54.7 years old( P=0.02). The decrease in the proportion of smoking patients was mainly reflected by male patients(from 68.5% to 31.1%, P<0.01). Stage Ⅰ lung cancers in male and females outnumbered advanced stage in 2012 and 2010, respectively, with a much higher proportion in female patients. Among male patients, adenocarcinoma has replaced squamous cell carcinoma as the most common pathological type since 2012, while in female patients adenocarcinoma remained the most common pathological type of lung cancer, and its proportion continued to increase reaching over 98%. Conclusion:A dramatic change in gender distribution was noticed during the past 30 years. Female patients became the primary population in surgically-treated lung cancers, with a trend of getting younger. The proportion of smokers and squamous cell carcinoma decreased significantly in male patients, and adenocarcinoma has become the most common pathological type of lung cancer. The proportion of stage Ⅰ lung cancers was on a dramatic rise, with the popularization of CT screening for lung cancer.
8.Research Progress of Anti-Reflux Biliary Stent

Biliary stent is an effective method for treating malignant biliary obstruction. However, duodenobiliary reflux is commonly found after conventional biliary stent, which causes complications such as biliary retrograde infection and stent blockage, and seriously reduces the quality of patients’ life. At present, anti-reflux biliary stent is considered as a promising palliative treatment method taking drainage and preventing duodenobiliary reflux into account. In this paper, research status of anti-reflux biliary stent was reviewed, and key problems such as design and evaluation of anti-reflux valve, valve and stent connection design, biomechanical analysis of anti-reflux biliary stent were mainly summarized. The current research progress and trend of anti-reflux biliary stent were summarized and analyzed.
9.Evaluation Deviation of Round-Window Stimulation Based on Basilar Membrane Response

Objective To study the accuracy of traditional basilar membrane displacement evaluation criteria for evaluating hearing compensation performance of round window-stimulated middle ear implant, so as to provide the theoretical basis for performance evaluation of round window-stimulated middle ear implant. Methods An acoustic microscopic finite element model of cochlea was constructed based on experimental data of the cochlea geometry. Reliability of this model was verified by comparison with experimental measurement values of inner hair cell, outer hair cell, tectorial membrane displacement. Based on this model, the displacement of basilar membrane and the stereocilia shear displacement of inner hair cells under forward stimulation and round-window stimulation were comparatively analyzed. Using the stereocilia shear displacement of inner hair cells as the criterion for sense of sound, the equivalent sound pressure level (SPL) deviation under round-window stimulation was studied when using traditional basilar membrane displacement as evaluation criterion. Results At 5 kHz characteristic frequency of the studied slice of cochlea, under sound pressures with the same amplitude, the displacement of basilar membrane and the stereocilia shear displacement of inner hair cells under round-window stimulation were lower than that under forward stimulation. Conclusions Under forward stimulation, the inner hair cells were more excited and the performance for sense of sound was better than that under round-window stimulation. Concurrently, using the displacement of basilar membrane under forward stimulation as the criterion of hearing compensation performance would overestimate hearing compensation performance of middle ear implant under round-window stimulation; but the deviation was relatively small, which was a relatively reliable evaluation method.
10.Expression of CDC45 in hepatocellular carcinoma and its correlation with prognosis

Objective:To analyze the expression of cell differentiation cyclin 45 (CDC45) in hepatocellular carcinoma (HCC) tissues, to explore the relationship between CDC45 and the prognosis of HCC, and to explore the possibility of CDC45 as a potential biomarker and therapeutic target for HCC.Methods:Bioinformatics methods were used to analyze the mRNA levels of CDC45 in HCC tissues and normal tissues, and to analyze the relationship between its expression and the survival rate of HCC patients. The clinicopathological data of 90 patients with HCC undergoing surgical treatment were retrospectively analyzed. Immunohistochemical method was used to detect the expression level of CDC45 protein in HCC tissue and normal tissues, and to analyze its relationship with clinicopathological characteristics of patients with HCC.Results:CDC45 is highly expressed in colorectal cancer, breast cancer, prostate cancer, lung cancer, liver cancer, etc. The expression of CDC45 in HCC tissues was higher than that in normal tissues adjacent to cancer ( P<0.05). The expression of CDC45 is significantly related to the clinical prognosis of HCC patients ( P<0.001), and the higher the expression of CDC45, the lower the disease-free survival rate and overall survival rate of HCC patients. CDC45 level is significantly correlated with the number of tumor nodules ( P=0.003) and tumor size ( P=0.003) in HCC patients, but the correlation between CDC45 level and age, gender and alpha-fetoprotein (AFP) levels is not statistically significant (all P>0.05). Conclusions:CDC45 plays an important role in the progression of HCC and is expected to become a therapeutic target and potential biomarker for HCC.
