1.Research Progress in the Mechanisms of Acupuncture in Regulating DNA Methylation

DNA methylation is the first epigenetic modification found in humans.Abnormal changes in DNA methylation are closely associated with the development and progression of diseases.Acupuncture,an important component of traditional Chinese medicine,has been shown to have significant therapeutic efficacy.The mechanisms underlying acupuncture are complex,involving physiological and pathological processes of integrated interactions across multiple targets.However,current research mostly focuses on a single target,highlighting the need for a more upstream approach to the investigation of the mechanisms.Herein,we reviewed studies on the direct or indirect regulation of DNA methylation via acupuncture.We also discussed its mechanisms of action in pain,obesity,depression,and Alzheimer disease,in order to provide a new perspective on the therapeutic mechanisms of acupuncture and the role of DNA methylation in the field of acupuncture research.Future research should concentrate on the effect of acupuncture on the DNA methylation of specific genes,the quantification of changes in DNA methylation at different acupoints,the development of individualized acupuncture prescriptions,further investigation of the specificity of the effects at different acupoint,and the expansion of the research to integrate epigenetics and genomics.This will provide a theoretical basis for the internationalization and the promotion of clinical application of acupuncture.
2.Application of shear wave elastography in the diagnosis of diabetes with peripheral neuropathy

Objective To evaluate the application of shear wave elastography(SWE)in the diagnosis of peripheral neuropathy in patients with diabetes.Methods Totally 85 patients with type 2 diabetes(T2DM)were selected from the Chengdu Office Hospital of People's Government of Tibetan Autonomous Region,including 46 patients with peripheral neuropathy(DPN)and 39 patients without peripheral neuropathy(NDPN).Compared for clinical data(gender,age,disease duration),cross-sectional area of the median nerve measured by high-frequency ultra-sound(CSA)and shear wave elastography(SWE)parameters(mean Young's modulus value,Emean)and shear wave velocity(SWV)between two groups of patients.Multifactor Logistic regression analysis was carried out on the indicators between the above groups to screen independent predictors in the diagnosis of peripheral neuropathy in diabetes patients,and a combined model was constructed.The area under the operating characteristic curve(AUC),sensitivity and specificity of the subjects were used to evaluate the diagnostic efficacy of the single model and com?bined model of the quantitative parameters(CSA,Emean,SWV)measured by clinical data,high?frequency ultra?sound and SWE in the diagnosis of peripheral neuropathy in diabetes patients.Results Age,course of disease,Emean,SWV and CSA were statistically significant in the diagnosis of peripheral neuropathy in diabetes patients(all P<0.05).AUC was 0.658,0.754,0.839,0.822 and 0.736,respectively.The combination model based on disease course,CSA and SWV showed the highest diagnostic efficiency,with AUC,sensitivity,and specificity of 0.887(0.800-0.946),80.43%,and 84.62%,respectively.Conclusions The combined model based on the course of disease,CSA and SWV have a high diagnostic efficiency in peripheral neuropathy of diabetes patients,and has good clinical application value.
3.Application of Proton Density Fat Fraction of Magnetic Resonance Imaging in Evaluation of Thigh Skeletal Muscle in Healthy People

Purpose To explore the ability of proton density fat fraction(PDFF)and decay constant T2* values in MRI to reflect skeletal muscle aging.Materials and Methods 3T MRI data of skeletal muscle in the middle thigh of 211 healthy adults from the Third Affiliated Hospital of Southern Medical University from August to December 2023 were prospectively collected.Gender,age,height,weight and body mass index(BMI)were recorded.PDFF value and T2* value of thigh skeletal muscle were measured at post-processing workstation,and statistical differences among different age,gender and BMI groups were analyzed.The correlation between PDFF value and T2* value of thigh skeletal muscle and age and BMI was analyzed.Results There were statistically significant differences in PDFF values of thigh skeletal muscle among different age groups(H=18.476-85.619,all P<0.01).There were significantly differences in T2*values of the left and right quadriceps muscles,hamstrings and adductors among different age groups(H=13.342-47.566,all P<0.05).There were statistically significant differences in the PDFF values of right quadriceps,left and right hamstring,adductor and sartor muscles between male and female groups(Z=-4.929--1.626,all P<0.05),while there were statistically significant differences in T2* values of left sartor muscle(Z=-2.971,P=0.003).There was no statistical significance in PDFF value of skeletal muscle of thigh in different BMI groups(P>0.05),but there were statistically significant differences in T2* value of left and right quadriceps muscle,hamstring muscle and adductor muscle(H=9.542-24.495,all P<0.05).There was a moderate positive correlation between age and PDFF value of thigh skeletal muscle(r=0.635,P<0.01),but a slight negative correlation with T2* value of left and right quadriceps,hamstring and sarcoleus(r=-0.451--0.189,all P<0.01).There was a slight positive correlation between BMI and T2* values of thigh skeletal muscle(r=0.317,P<0.01).There was a moderate negative correlation between the PDFF value and T2* value of all thigh skeletal muscles(r=-0.749--0.624,P<0.01).The PDFF and T2* values of the front and back thigh muscles(quadriceps,hamstring)were most significantly correlated with age and BMI.Conclusion PDFF based on MRI can reflect the age-related changes in the microenvironment of thigh skeletal muscle,and is a potential imaging biological marker for accurate and non-invasive quantitative evaluation of thigh skeletal muscle aging.
4.Heterogeneous analysis on association between exposure and outcome in cohort studies: based on propensity score matching and subpopulation treatment effect pattern plot

This paper briefly introduces the basic principle, application scenarios and main steps of subpopulation treatment effect pattern plot (STEPP), and summarizes the theoretical basis of its combined application with propensity score matching (PSM) in cohort study data for the heterogeneity analysis on the association between exposure and outcome risk. In this paper, the combined application of PSM and STEPP in cohort study data is summarized by simulation of "cohort study of the risk of venous thromboembolism (VTE) in cancer patients with different infusion catheter types". The absolute risk value and relative risk value of VTE in the peripherally inserted central catheter group and the central venous catheter group in subpopulations are introduced, as well as the results of heterogeneity analysis among subpopulations. The limitations and research prospects of the combined application of PSM and STEPP are discussed to provide reference for researchers in practical application.
5.Neck Seven-Line Method Combined with Periauricular Acupuncture for 33 Patients with Sudden Hearing Loss in Non-Acute Stage: A Randomized Controlled Trial

ObjectiveTo evaluate the effectiveness and safety of neck seven-line method combined with periauricular acupuncture as salvage treatment for sudden hearing loss in non-acute stage. MethodsSixty-six patients with non-acute stage of sudden hearing loss with a disease duration of 15-90 days were randomly divided into 33 cases each in treatment group and control group. The treatment group was given neck seven-line method combined with acupuncture at periauricular points; the control group used sham acupuncture and sham electroacupuncture at the same points. Both groups were treated 3 times a week for 6 weeks. The pure tone average hearing threshold of impaired frequencies were examined before treatment, after treatment finish and at follow-up (week 10), and the difference between pure tone average hearing threshold of impaired frequencies before and after treatment was calculated; calculate the proportion of patients with ≥10 dB improvement in pure tone average hearing threshold of impaired frequency after treatment and at follow-up; compare the patients' Tinnitus Evaluation Scale (TEQ) scores, Chinese Medicine Quality of Life Assessment Scale (CQ-11D) health utility values and the difference between before and after treatment, and record the occurrence of adverse events. ResultsThe pure tone average of impaired frequency in the treatment group were (50.57±18.07) dB and (47.70±17.42) dB at post-treatment and follow-up respectively, and (54.38±21.77) dB and (53.36±20.99) dB in the control group at post-treatment and follow-up. Compared with the pre-treatment period, the pure tone average hearing threshold of impaired frequency in the two groups significantly decreased (P<0.05) at post-treatment and follow-up. The difference of pure tone average hearing threshold of impaired frequency in the treatment group after treatment and at the follow-up visit compared to that before treatment was lower than those in the control group (P<0.05). After treatment, 13 patients (39.39%) in the treatment group and 10 patients (30.30%) in the control group showed improvement of ≥10 dB in pure tone average hearing threshold of impaired frequency; at the follow-up visit, 18 patients (54.55%) in the treatment group and 10 patients (30.30%) in the control group showed improvement of ≥10 dB in the pure tone average hearing threshold of impaired frequency, and there was no statistical significance for comparison between groups at the time of post-treatment and follow-up (P>0.05). The TEQ score of the treatment group significantly lower than that before treatment (P<0.05); the TEQ score of the treatment group and the difference between before and after treatment significantly lower than that of the control group (P<0.05). The difference in CQ-11D health utility values and the difference between before and after treatment were not statistically significant between the two groups (P>0.05). Eight cases of acupuncture-related adverse events occurred among 66 patients, including subcutaneous haematomas after needling, severe pain during needling and needle fainting, which disappeared after symptomatic treatment and did not affect the following treatment. ConclusionNeck seven-line method combined with periauricular acupuncture could be used as salvage treatment for sudden hearing loss in non-acute stage to improve the pure tone average hearing threshold of impaired frequency, alleviate tinnitus, with safety.

7.Pharmaceutical care of voriconazole combined with micafungin in the treatment of aspergillus endocarditis

OBJECTIV E To provide ideas for diagnosis and treatment of fungal endocarditis. METHODS The diagnosis and treatment of 1 case of aspergillus endocarditis participated by clinical pharmacists were analyzed. Clinical pharmacists suggested that blood macrogenomic next-generation sequencing (mNGS)detection and blood microbial culture should be performed to assist in the diagnosis of fungal endocarditis ;considering the mechanism ,target and safety of the drug ,it was suggested to use voriconazole combined with micafungin for antifungal treatment ;through combining with the patient ’s condition and weighing up the pros and cons ,the dose of micafungin was adjusted to 300 mg per day ;clinical pharmacists prevented ,evaluated and dealt with adverse drug reactions during treatment ,and also provided medication guidance and long-term follow-up for the patient after discharge. RESULTS The clinician adopted the advice of the clinical pharmacists. Aspergillus fumigatus was detected in blood mNGS,which bought time for early diagnosis and individualized treatment of the patient. After 1 month treatment of voriconazole combined with micafungin ,the patient ’s condition was well controlled. The alkaline phosphatase and γ-glutamyl transferase of the patient returned to normal after treatment with adenosylmethionine succinate. With the help of medication guidance ,education and discharge follow-up of clinical pharmacists ,the medication compliance of the patient was good and the condition was stable. CONCLUSIONS The clinical pharmacists participate in the diagnosis and treatment process of the patient ,formulate an individualized anti-infective treatment plan for the patient and achieve good results ,which reflect the professional ability and service level of the clinical pharmacist ,and provide ideas for the clinical treatment of fungal endocarditis.
8.Preventive and therapeutic effects of Bateri-7 on radiation-induced intestinal injury in mice

Objective:To study the protective effect of Mongolian medicine Bateri-7 on radiation-induced intestinal injury in mice.Methods:C57BL/6J male mice were randomly divided into control group, irradiation group and irradiation plus drug administration group, with 10 or 15 mice in each group. For irradiation group, the mice were given a single dose of 12 Gy 60Co γ-rays with total body irradiation. For drug treatment, the mice were gavaged with Bateri-7 (530 mg/kg) 7 d before irradiation until 3 d after IR. At 6 h and 24 h after irradiation, the Tunel positive cells in intestine were detected immunohistochemically. At 3.5 d after irradiation, the structure of intestinal villi was observed by HE staining, and the BrdU and Ki67 positive cells were detected immunohistochemically. The expression levels of IL-6, TNF-α and Cxcl-5 were detected by qPCR. The FITC-dextran in peripheral blood was also determined. Results:The survival of irradiated mice was significantly increased by Bateri-7 ( χ2= 5.84, P < 0.05), but there was no significant difference in weight between two groups ( P > 0.05). The villi length of small intestine in the irradiation plus drug group was significantly longer than that in the irradiation group ( t = 20.24, P < 0.05), and there was no significant difference in the depth of intestinal crypt between two groups ( P > 0.05). At 6 and 24 h after irradiation, the number of Tunel positive cells in intestinal crypts in the irradiation plus drug group was significantly reduced in comparison with the irradiation group ( t = 3.52, 2.90, P < 0.05). At 3.5 d after irradiation, the level of FITC-dextran in serum and the expressions of IL-6, TNF-α and Cxcl-5 in small intestine of mice in the irradiation plus drug group were significantly lower than those in the irradiation group, respectively( t = 6.92, 7.01, 7.18, 13.16, P < 0.05). The number of BrdU and Ki67 positive cells in the crypt of mice in the irradiation plus drug group was higher than that of the irradiation group ( t = 3.91, 2.57, P < 0.05). Conclusions:Mongolian medicine Bateri-7 can effectively alleviate irradiation-induced intestinal injury of mice, which may have a good preventive and therapeutic effect on radiation enteritis.
9.Interventional effect of metformin on pulmonary inflammation and pulmonary fibrosis in silicotic rats

Objective:To investigate the interventional effect of metformin on pulmonary inflammation and pulmonary fibrosis in silicotic rats.Methods:In April 2019, 48 Wistar male rats of SPF grade were randomly divided into negative control group, metformin control group, silicon dioxide (SiO 2) model group, low, medium and high dose metformin intervention group according to the random number table method, 8 rats in each group. The SiO 2 model group and the low, medium and high dose metformin intervention groups were given 1 ml 50 mg/ml of SiO 2 by intratracheal instillation, the negative control group and the metformin control group were given 1 ml normal saline by intratracheal instillation. 24 hours later, the low, medium and high dose metformin intervention groups and the metformin control group were treated with 100, 200, 400 and 400 mg/kg metformin daily, the control and SiO 2 model groups received normal saline daily. Then the rats were sacrificed at the 28th day after SiO 2 exposure. The changes of rat body weight and pathological examination of rat lung tissue were observed, and the lung organ coefficient, the content of hydroxyproline (HYP) , the expression levels of inflammatory factors transforming growth factor beta1 (TGF-β1) , tumor necrosis factor-alpha (TNF-α) , interleukin-1beta (IL-1β) and the protein expression of E-cadherin (E-Cad) , Vimentin, α-SMA were detected. Results:Compared with the negative control group, SiO 2 model group had a significant decrease in the body weight of rats ( P<0.05) , lung organ coefficient, alveolitis and fibrosis scores, HYP content and the levels of TGF-β1, TNF-α, IL-1β were all significantly increased ( P<0.05) . Compared with the SiO 2 model group, the weights of the rats in the medium and high dose intervention group of metformin increased significantly ( P<0.05) . And after intervention with different doses of metformin, the lung organ coefficient, alveolitis and fibrosis scores, HYP content and the levels of TGF-β1, TNF-α and IL-1β were significantly decreased ( P<0.05) . Immunohistochemistry and Western blotting results showed that compared with the negative control group, the expression of E-Cad of the SiO 2 model group was decreased, and the expression levels of Vimentin and α-SMA were significantly increased ( P<0.05) . After metformin intervention, the expression of E-Cad was significantly increased, the expression levels of Vimentin and α-SMA were significantly decreased ( P<0.05) . Conclusion:Metformin can reduce lung tissue inflammation and fibrosis in rats exposed to SiO 2 dust, which may be related to reducing the expression of inflammatory factors in lung tissue and inhibiting the EMT process.
