Angle class Ⅱ malocclusion is often characterized by mandibular retraction and lip incompetence, which affects the patient's lateral appearance and may even lead to upper airway stenosis. It can be classified into dental and skeletal types. For skeletal class Ⅱ malocclusion patients with mandibular retraction during the peak growth period, mandibular anterior guidance with a functional orthodontic appliance is generally considered as the optimal clinical treatment approach. At present, there remains a paucity of clinical reports on the clinical application of bracket-free clear aligners in mandibular anterior guidance, both domestically and internationally. This article presented a case of an adolescent patient with skeletal class Ⅱ malocclusion accompanied with deep overbite treated with bracket-free clear aligner for mandibular anterior guidance in combination with intermaxillary class Ⅱ traction.During the treatment, vertical correction involved anterior intrusion of the anterior teeth to improve the deep overbite, while horizontal correction included maxillary and mandibular expansion to coordinate the width of the dental arches, and asymmetric anterior guidance was used to correct the midline deviation. After 35 months of treatment, the patient's convex facial profile and mandibular retrusion were significantly improved. The subspinale-nasion-supramentale angle(ANB)was decreased from 6.8° to 3.9°,the overbite and overjet were normalized, and the bilateral canine and molar reached a neutral relationship. The mentolabial sulcus depth(Si-LiPg′)and the soft tissue thickness of pogonion to pogonion(Pm-Pm′)were decreased, resulting in a shallower mentolabial sulcus and a more harmonized lateral facial soft tissue profile. The mandibular incisor to mandibular plane angle(IMPA)was decreased from 116.6° to 110.7°,indicating retraction of the lower incisors during mandibular anterior guidance. In conclusion, the orthodontic strategy of mandibular advancement with clear aligners in skeletal class Ⅱ malocclusion patients can avoid excessive overcompensation of the upper and lower anterior teeth and shorten the orthodontic treatment cycle.

Objective:To investigate the feasibility of transcatheter edge-to-edge repair (TEER) using a short-clip strategy for patients with moderate-to-severe or greater degenerative mitral regurgitation caused by commissural prolapse. Methods:This retrospective study included patients with severe mitral regurgitation secondary to commissural prolapse who underwent TEER at the Second Affiliated Hospital of Zhejiang University School of Medicine between September 2022 and July 2024. Preoperative clinical and imaging data, intraoperative details, procedural outcomes, and 1-month postoperative follow-up results were collected. Results:A total of 19 patients were enrolled, aged (74.1±6.1) years, including 12 males. Among them, 10 patients had external commissural prolapse, and 9 patients had internal commissural prolapse. Preoperatively, all patients exhibited severe mitral regurgitation (4+), with an effective regurgitant orifice area of (0.55±0.17) cm2, left atrial volume of (104.77±36.57) ml, left ventricular end-diastolic volume of (102.29±32.47) ml, left ventricular end-diastolic dimension of (5.34±0.59) mm, and prolapse width of (1.18±0.34) cm. All procedures utilized short clips (NTR or NTW clips) to target the prolapsed commissural region and were completed successfully without intraoperative complications. At 1-month follow-up, no mortality, stroke, single-leaflet device attachment, myocardial infarction, or unplanned mitral reintervention occurred. Mitral regurgitation severity improved to ≤2+ in all patients, with left atrial volume of (74.49±33.83) ml, left ventricular end-diastolic volume of (85.90±18.05) ml, and left ventricular end-diastolic dimension of (4.93±0.37) mm (all P<0.05). Conclusion:The short-clip strategy, focusing on precise clip placement at the commissural interface, is feasible and effective for TEER in patients with severe mitral regurgitation due to commissural prolapse.

Objective:To comprehensively assess the nutritional literacy among family caregivers of elderly patients with chronic heart failure (CHF), so as to provide reference for developing clinical nutrition management strategies for elderly CHF patients. Methods:This study was a descriptive qualitative research. From February to March 2025, purposive sampling was used to select 17 family caregivers of elderly CHF patients at Affiliated Hospital of Yangzhou University as research subjects for semi-structured interviews. The data was analysed using a directed content analysis approach. Results:Three themes and nine subthemes were identified, including functional nutrition literacy (lack of disease-related nutrition knowledge, difficulty in knowledge transformation and utilization, and barriers to voluntary nutritional adherence), interactive functional literacy (limited channels for accessing nutrition information, broken healthcare services, insufficient motivation for proactive communication, and demand for professional dietary guidance), and critical nutrition literacy (challenges in distinguishing information authenticity, reliance on inherent cognition for evaluation) .Conclusions:Family caregivers of elderly patients with CHF commonly lack relevant nutritional knowledge. Healthcare providers should attach importance to the education of nutrition knowledge and skills for family caregivers, establish a multidimensional nutrition education system, develop personalized nutrition guidance plans, and integrate the family-community-medical collaborative support network to systematically strengthen caregivers' understanding of disease nutrition management for elderly CHF patients and enhance their compliance with nutrition care practices.

Journal of Traditional Chinese Medicine 2024;65(10):1013-1018
Based on the duality of "healthy" and "pathogenic" of the immune system in physiological and pathological states, and combined with the analogy between "immune function" and "healthy qi", the theory of "transformation of healthy qi into pathogenic qi" was proposed to guide the pattern identification and treatment of autoimmune diseases. The theory of "transformation of healthy qi into pathogenic qi" means that "healthy qi", which is originally used by the body's original ability to defend and remove harmful factors or normal functional activities, transformed into "pathogenic qi", which is factors that damage and destroy the human body or abnormal body state. In the pathogenesis of autoimmune diseases, the pathogenic factors and causes of congenital endowment, tissue damage under the influence of external pathogens, abnormal function of zang-fu (脏腑) organs and meridians, abnormal transmission and transformation of qi, blood, body fluids was proposed. Autoimmune diseases have the dynamic mechanism of latent pathogen at early stage, internal and external contraction at the onset stage, and the expansion of the pathogenic qi at the complete period, and also have the characteristics of the specificity, invisibility and contradiction of healthy qi and pathogenic qi. In terms of treatment, it advocates the ideas of treatment with both attack and supplementation throughout the disease, identifying diseases with special prescriptions and formulas for specific diseases, dynamically adjusting treatment by identifying the remission and onsets of the disease, observing the changes in the dynamics of healthy qi transforming into pathogenic qi, and treating disease before it arises with early intervention.
