1.Study on the absorption-enhancing effect of self-assembled nanoparticles from Shaoyao gancao decoction

OBJECTIVE To investigate the effect and mechanism of self-assembled nanoparticles from Shaoyao gancao decoction (SGD-SAN) on the intestinal absorption behavior of its main active components. METHODS SGD-SAN was prepared and characterized. Using an in-situ single-pass intestinal perfusion model in rats, the absorption characteristics of five active components (albiflorin, paeoniflorin, liquiritin apioside, liquiritin, glycyrrhizic acid) from SGD-SAN in the jejunum and ileum were studied, with the absorption rate constant ( K a ) and apparent permeability coefficient ( P eff ) as indicators, and compared with free drugs. In the intestinal segment with optimal absorption, the effects of drug concentration and efflux transporter inhibitors (P-glycoprotein inhibitor verapamil, multidrug resistance-associated protein 2 inhibitor indomethacin, breast cancer resistance protein inhibitor reserpine) on the intestinal absorption characteristics of these components were examined. RESULTS The obtained SGD-SAN exhibited a spherical shape with uniform sizes, an average particle diameter of (155.57±2.65) nm, a polydispersity index of 0.34±0.03, and a Zeta potential of (-9.30±1.12) mV. The average total content of five active components, including albiflorin, was 12.26%, and remained unaffected by enzymatic degradation and intestinal physical absorption. Compared with the free drug group, the five active components in the SGD-SAN group exhibited higher absorption rates in the ileal segment, with significantly elevated K a and P eff values (except for the P eff value of glycyrrhizic acid in the ileal segment) ( P <0.05 or P <0.01). Their absorption demonstrated a concentration-dependent trend. In the free drug groups, the absorption of each component was regulated by corresponding inhibitors ( P <0.05 or P <0.01); whereas in the SGD-SAN groups, except for albiflorin and paeoniflorin, the absorption of the remaining components was not affected by the inhibitors ( P >0.05). CONCLUSIONS SGD-SAN significantly enhances the intestinal absorption efficiency of active components. The above absorption-enhancing effect may be related to the avoidance of efflux transporter influence and the presence of a mixed absorption mode.
2.Clinical outcome of kidney transplantation from DBD donors complicated with acute kidney injury

Objective To evaluate the clinical outcome of kidney transplantation from donation after brain death(DBD)donors complicated with acute kidney injury(AKI).Methods Clinical data of 216 DBD donors were retrospectively analyzed,and they were divided into the AKI group(n=69)and control group(n=147)according to the Kidney Disease:Improving Global Outcomes(KDIGO)guidelines.Donors in the AKI group were further divided into the KDIGO stage 1 and stage 2-3 subgroups.One hundred and thirty-five recipients were assigned into the AKI group and 288 recipients in the control group.Postoperative recovery of renal function and clinical outcomes of the recipients were recorded.The risk factors of delayed graft function(DGF)were identified.Results The highest serum creatinine(Scr)level,Scr level before procurement,the highest blood sodium level and blood sodium level before procurement in the AKI group were higher than those in the control group.The application duration of vasopressors in the AKI group was longer than that in the control group.In the AKI group,the amount of fluid resuscitation within 48 h was higher,the HCO3-level at admission was lower,and the incidence of diabetes insipidus and hypotension was higher than those in the control group.The highest Scr level and the Scr level before procurement in KDIGO stage 2-3 donors were significantly higher than those in KDIGO stage 1 counterparts(all P<0.05).Compared with the control group,the incidence of DGF and acute rejection was higher,the proportion of continuous renal replacement therapy was higher,the Scr level within postoperative 90 d was higher,and the urine amount within postoperative 3 d was less than those of recipients in the AKI group.Compared with KDIGO stage 1 recipients,KDIGO stage 2-3 recipients had higher Scr levels at postoperative 3,4,5 and 15 d,and less urine amount at postoperative 2 d(all P<0.05).Univariate analysis showed that donor age,the highest Scr level,the highest blood sodium level and the amount of fluid resuscitation within 48 h were the risk factors for DGF in recipients after kidney transplantation.Multivariate analysis showed that donor age was the independent risk factor for DGF in recipients after kidney transplantation(all P<0.05).Conclusions For the application of DBD donors complicated with AKI,active organ maintenance should be performed to alleviate AKI.It exerts no effect upon graft function and survival rate at postoperative 6 months,which may achieve equivalent efficacy as non-AKI donors and may be used as a source of extended criteria donor kidneys.
3.Compound ANBP promotes diabetic wound healing by regulating TCA circulation-related enzymes

AIM:To investigate the effects of the compound ANBP on wound healing in diabetic rats and ex-plore its mechanism of action.METHODS:Ninety male SD rats were randomly divided into blank,model,compound ANBP,Beifuxin,and nicotinamide mononucleotide(NMN)groups,with 16 rats in each group.Wound healing in each group was observed and samples were taken on days 3,7 and 14 to analyze the wound healing rate.Local histopathological changes were observed using HE and Masson staining.The expressions of pyruvate dehydrogenase E1 subunit alpha 1(PDHA1),citrate synthase(CS),isocitrate dehydrogenase(IDH1)and oxoglutarate dehydrogenase(OGDH)were de-tected through immunofluorescence and Western blot.The number and morphology of mitochondria in the wound tissue were observed using transmission electron microscopy.RESULTS:Histomorphological changes revealed significant im-provement in diabetic wound healing in the blank and compound ANBP groups compared to that of the model group.The wound healing rates of the blank,compound ANBP,Beifuxin,and NMN groups were significantly increased on days 3,7,and 14(P<0.01).Compared to the model group,granulation tissue generation was higher in the other groups,cover-ing the wound defect and producing abundant collagen fibers.At 3,7,and 14 days after intervention,the blank,com-pound ANBP,Beifuxin,and NMN groups showed significantly enhanced fluorescence intensities of TCA cycling-related enzymes PDHA1,CS,IDH1,and OGDH indicating increased expression of these enzymes.The levels of the TCA cy-cling-related enzymes were significantly increased(P<0.01)in the compound ANBP,Beifuxin and NMN groups but were significantly decreased(P<0.01)in the model group.An increase in the number and density of mitochondria and a de-crease in the cavitation rate of mitochondria with improved morphology(P<0.05)was observed in the group treated with compound ANBP.CONCLUSION:Compound ANBP may increase the number of mitochondria,improve mitochondrial morphology and function,upregulate the expression levels of PDHA1,CS,IDH1,and OGDH proteins,and accelerate the regeneration of wound granulation tissue,thus promoting the healing of diabetic wounds in rats.
4.Global characterization of modifications to the charge isomers of IgG antibody

Posttranslational modifications of antibody products affect their stability,charge distribution,and drug activity and are thus a critical quality attribute.The comprehensive mapping of antibody modifications and different charge isomers(CIs)is of utmost importance,but is challenging.We intended to quanti-tatively characterize the posttranslational modification status of CIs of antibody drugs and explore the impact of posttranslational modifications on charge heterogeneity.The CIs of antibodies were fraction-ated by strong cation exchange chromatography and verified by capillary isoelectric focusing-whole column imaging detection,followed by stepwise structural characterization at three levels.First,the differences between CIs were explored at the intact protein level using a top-down mass spectrometry approach;this showed differences in glycoforms and deamidation status.Second,at the peptide level,common modifications of oxidation,deamidation,and glycosylation were identified.Peptide mapping showed nonuniform deamidation and glycoform distribution among CIs.In total,10 N-glycoforms were detected by peptide mapping.Finally,an in-depth analysis of glycan variants of CIs was performed through the detection of enriched glycopeptides.Qualitative and quantitative analyses demonstrated the dynamics of 24 N-glycoforms.The results revealed that sialic acid modification is a critical factor ac-counting for charge heterogeneity,which is otherwise missed in peptide mapping and intact molecular weight analyses.This study demonstrated the importance of the comprehensive analyses of antibody CIs and provides a reference method for the quality control of biopharmaceutical analysis.
5.Kinetic metrics changes of FDG in key organs after chemo-immunotherapy in patients with locally advanced non-small cell lung cancer identified by total-body PET/CT dynamic imaging

Objective:To evaluate the kinetic metrics changes of FDG in key organs after chemo-immunotherapy in patients with locally advanced non-small cell lung cancer (NSCLC) identified by total-body PET/CT dynamic imaging, and explore its potential biological significance.Methods:From August 2020 to March 2021, 16 patients (13 males, 3 females; age: 43-67 years) with locally advanced NSCLC who underwent total-body 18F-FDG PET/CT dynamic imaging in Sun Yat-sen University Cancer Center were retrospectively analyzed. ROIs of key organs were drawn at baseline and after chemo-immunotherapy to obtain the time-activity curves (TACs). The kinetic metrics, including K1, k2, k3 and metabolic rate of FDG (MR FDG), were fitted by the two-tissue compartment model. Paired t test or Wilcoxon signed rank test was used to compare the differences of FDG kinetic parameters in each organ before and after treatment. Results:Compared with baseline, SUV max of colon (3.23±1.29 vs 4.81±2.73), MR FDG ((2.77±1.96) vs 3.56(1.07, 9.89) μmol·100 g -1·min -1) of lungs, SUV max (2.16±0.27 vs 2.33±0.41), k3 ((0.008±0.002) vs (0.012±0.004) min -1) and MR FDG ((2.65±0.81) vs (3.76±1.59) μmol·100 g -1·min -1) of spleen, and SUV max (2.59±0.45 vs 4.49±2.73), k2 ((0.76±0.37) vs (1.27±0.66) min -1), k3 ((0.032±0.007) vs (0.066±0.029) min -1) and MR FDG ((5.14±1.44) vs (8.39±2.67) μmol·100 g -1·min -1) of bone marrow were increased after chemo-immunotherapy with significant differences ( t values: from -5.40 to 3.47, z=-2.02, all P<0.05). There were no significant differences of SUV max, k values and MR FDG in other organs ( t values: from -2.00 to 2.35, z values: from -1.45 to -0.05, all P>0.05). Conclusions:After chemo-immunotherapy, the activation of immune system may be manifested as the increase of FDG kinetic rate constants in spleen and bone marrow. The lung and colon may be target organs for immune-related adverse effects.
6.Meta-synthesis of qualitative research on caring experience of caregivers of patients with spinal cord injury

Objective:To analyze and synthesize the care experience of caregivers of patients with spinal cord injury.Methods:Qualitative research on the care experience of caregivers of patients with spinal cord injury was retrieved by computer in Chinese and English databases such as PubMed, Embase, Web of Science, Cochrane Library, China National Knowledge Infrastructure (CNKI) , Wanfang Database, VIP, and China Biomedical Literature Database. The retrieval time was from the establishment of the database to December 11, 2020. The quality of the articles was evaluated by the quality evaluation criteria for qualitative research of the Joanna Briggs Institute (JBI) Evidence-Based Health Care Center in Australia, and the results were pooled for Meta-synthesis.Results:A total of 8 articles were included, and 33 research results were extracted, and 8 new categories were summarized, and 3 integrated results were synthesized. Integrated result 1 was the change in caregiver's psychological experience (In the process of long-term care, the psychological process of the caregiver was constantly changing, and a positive care experience was obtained through psychological support) . Integrated result 2 was the change in personal and social relationships (Caregivers' personal roles and social relationships were affected by taking care of patients for a long time, seeking multiple social supports, and gradually adjusting and adapting) . Integrated result 3 was the change in knowledge and skills (Due to the sudden nature of spinal cord injury, caregivers lacked caring knowledge and skills and were eager to learn caring skills) .Conclusions:The care experience of caregivers of patients with spinal cord injury is multifaceted. Medical and nursing staff should pay attention to the care needs and real experience of caregivers, provide them with professional care guidance and support, help them to be well qualified as caregivers, and improve the physical and mental health and quality of life of patients with spinal cord injury and their caregivers.

8.A prospective multi-center cross-sectional study of urethral mobility in normal parous women in China

Objective:To assess the urethral mobility of normal parous women in China and explore the impacts of related risk factors on it using translabial ultrasound.Methods:Females who met the inclusion criteria in 37 tertiary hospitals from February 2017 to August 2018 were included. All women underwent standardized translabial ultrasound examination and the urethral rotation angle (URA), bladder neck position at maximum Valsalva maneuver (BNP-V) and bladder neck descent (BND) were measured. Questionnaires were used to collect basic information including age, height, weight, body mass index (BMI), past medical history, maternity history, and urinary incontinence related history. Mann-Whitney U test and multiple linear regression analysis were adopted to explore the influences of age, BMI, delivery mode and parity on normal parous women′s urethral mobility. Then, the study subjects were divided into different groups and the corresponding values of URA, BNP-V and BND were compared. Results:Compared with parous women with normal BMI and no history of vaginal delivery, those who were overweight and/or had a history of vaginal delivery were more likely to gain greater URA and BND ( P<0.05). The URA and BND were not significantly different between women with different times of cesarean sections ( P>0.05); while for women with a history of vaginal delivery, these two parameters increased with the increase of the number of transvaginal deliveries ( P<0.05). Conclusions:BMI and vaginal delivery are important risk factors for the urethral mobility of normal parous women. The urethral mobility increases with the increase of BMI and the number of vaginal deliveries.
9.A Meta integration of qualitative studies on self management experience of peritoneal dialysis patients

Objective:To systematically evaluate the experience of self-management in peritoneal dialysis patients.Methods:Databases including PubMed, Embase, Web of Science, the Cochrane Library, PsycInfo, Scopus and CNKI, Wanfang, VIP, CBM and Google Scholar were searched and relevant qualitative research on self-management experience of peritoneal dialysis patients was retrieved with a time limitation from the establishment of the databases to April 6, 2020. The quality of articles was evaluated by the quality evaluation standard of qualitative research of JBI evidence-based health care center (2008) , and the results were Meta integrated.Results:A total of 8 studies were included, and 26 complete research results were extracted. We summed up 10 new categories, and obtained 3 integrated results. Integration result 1: cognitive and behavioral management. Patients try to maintain the independence and confidence of peritoneal dialysis management through facing renal failure and adapting to dialysis treatment. Integration result 2: crisis identification and management of complications. Through continuous learning of knowledge and skills related to dialysis treatment, as well as identification and treatment of worsening symptoms, patients can improve their ability of crisis identification and management. Integration result 3: role and social relationship management. Patients constantly adjust their roles and emotions to adapt to changes in daily life and maintain social functions and strive to strengthen communication with medical care, family and society to obtain social relationship support, and increase patients' confidence in self-management.Conclusions:The self-management of peritoneal dialysis patients involves many aspects. The medical staff should give understanding, guidance, and support to the patients' self-management and self-care process in combination with family and society, to help them with better self-management and improve their quality of life.
10.Application of the sandwich teaching method in the course of Surgical Nursing

Objective To investigate the application effect of the sandwich teaching method in the course of Surgical Nursing . Methods Two classes of the students majoring in nursing were randomly divided into control group and observation group, with 36 students in each group. The students in the control group received the traditional teaching method, and those in the observation group received the sandwich teaching method . The teaching effect was compared between the two groups by theoretical assessment and questionnaire survey. SPSS 24.0 was used for analysis of variance and the t-test. Results Compared with the control group, the observation group had significantly higher scores of course assessment, learning atmosphere, and learning initiative. The observation group also had significantly higher good rates of learning atmosphere and learning initiative than the control group(P<0.05). Conclusion The sandwich teaching method can significantly improve students' abilities of communication and problem solving, arouse their enthusiasm of learning, and increase the interestingness of classroom, and therefore, it has a practical value in teaching.
