1.Physical examination population lifestyle pattern mining and association analysis with metabolic associated fatty liver disease

Objective:To explore the lifestyle pattern of the physical examination population and analyze its association with metabolic associated fatty liver disease (MAFLD).Methods:This was a cross-sectional study. Based on the data of 196 515 physical examination individuals from the Health Management Center of the Third Xiangya Hospital of Central South University from January 2016 to December 2020, the subjects were grouped and characterized by principal component analysis and cluster analysis. Among them, 137 277 cases with MAFLD diagnosis information were included in the association analysis between lifestyle pattern and MAFLD. The differences in lifestyle pattern choice among different age, sex, education level, marital status, occupational category and medical insurance type and their differences with the risk of MAFLD were analyzed. The generalized linear mixed model was used to control confounding factors and then association analysis was conducted.Results:There were 6 types of lifestyle patterns in the physical examination population, which were respectively: indulgent type-both physical and mental damage, remedial type-excessive diet, giving type-unique intensity, comfortable type-natural health, heavy smoking type-sedentary injury, heavy drinking type-attempting to make up, accounting for 7.29%, 9.62%, 7.43%, 52.16%, 9.77%, 13.73% in the population. Among them, the male lifestyle pattern was mainly the indulgent type, the remedial type, the heavy smoking type and the heavy drinking type, showing the characteristics of unhealthy lifestyle pattern; Women tended to have healthier lifestyle patterns. After association analysis with MAFLD, it was found that the prevalence of MAFLD was more than 50% in the people who belonged to the indulgent type, remedial type, the heavy smoking type and the heavy drinking type (53.62%, 57.06%, 51.25% and 50.50%, respectively), and the prevalence of MAFLD in the giving type group was 40.17%. The risk of MAFLD in comfortable group was relatively low (28.25%), and the difference in risk of MAFLD among all modes was statistically significant after controlling for confounding factors ( P<0.001). Conclusion:According to cluster mining, there are 6 types of lifestyle patterns in the physical examination population, and the healthier lifestyle pattern has a lower risk of MAFLD.
2.Discovery of orally active and serine-targeting covalent inhibitors against hCES2A for ameliorating irinotecan-triggered gut toxicity.

Human carboxylesterase 2A (hCES2A) plays pivotal roles in prodrug activation and hydrolytic metabolism of ester-bearing chemicals. Targeted inhibition of intestinal hCES2A represents a feasible strategy to mitigate irinotecan-triggered gut toxicity (ITGT), but the orally active, selective, and efficacious hCES2A inhibitors are rarely reported. Here, a novel drug-like hCES2A inhibitor was developed via three rounds of structure-based drug design (SBDD) and structural optimization. Initially, donepezil was identified as a moderate hCES2A inhibitor from 2000 US Food and Drug Administration (FDA)-approved drugs. Following two rounds of SBDD and structural optimization, a donepezil derivative (B7) was identified as a strong reversible hCES2A inhibitor. Subsequently, nine B7 carbamates were rationally designed, synthesized and biologically assayed. Among all synthesized carbamates, C3 showed the most potent time-dependent inhibition on hCES2A (IC50 = 0.56 nmol/L), excellent specificity and favorable drug-like properties. C3 could covalently modify the catalytic serine of hCES2A with high selectivity, while this agent also showed favorable safety profiles, high intestinal exposure, and impressive effects for ameliorating ITGT in both human intestinal organoids and tumor-bearing mice. Collectively, this study showcases a rational strategy for developing drug-like and serine-targeting covalent inhibitors against target serine hydrolase(s), while C3 emerges as a promising orally active drug candidate for ameliorating ITGT.

4.Investigation of 61 562 specialty nurses needs for the retraining

Objective To investigate the demand of specialty nurses in China for retraining,and to provide references for the continuous development of specialty nurses.Methods A convenient sampling method was used to survey specialty nurses from all 31 provinces in May-June 2023.A self-designed questionnaire was used as an electronic questionnaire.Results A total of 61,562 valid questionnaires were collected,with 70.0%of specialty nurses believing that it is"very necessary"or"necessary"to carry out retraining for specialty nurses,and 68.4%being"very willing"or"willing"to participate in further training;64.6%chose a five-year cycle for retraining;and 66.5%chose online learning as their preferred mode of learning.In terms of content and evaluation methods for retraining,the top 3 in the retraining content were specialized knowledge(88.9%),clinical practice(74.3%),and nursing research(59.5%).The top 3 in retraining evaluation were specialized knowledge(87.5%),clinical practice ability(72.2%),and teaching guidance ability(44.3%).Conclusion It is feasible and necessary to carry out the retraining for specialty nurses.It is suggested that the training institutions should carry out the retraining in time according to the characteristics of each specialty and the wishes of specialty nurses,so as to promote the knowledge update and ability maintenance of specialty nurses.
5.Research on the training needs of advanced practice nurses in 1 287 medical institutions

Objective To investigate the training needs and suggestions of advanced practice nurses in medical institutions in China,and to provide references for the formulation of training programs for advanced practice nurses.Methods By a convenient sampling method,from May to July in 2023,the heads of nursing nurses in medical institutions in 29 provinces,autonomous regions and municipalities directly under the Central Government were investigated,and the self-designed questionnaires were distributed in the form of electronic questionnaire.Results 1 287 medical institutions participated in the survey,with tertiary hospitals accounting for 65.50%.93.16%of medical institutions had the use needs of advanced practice nurses,and the top 3 professional fields hoping to prioritize in the APN training were in the order of critical illness,emergency treatment and intravenous treatment.Among the APN core competence,the most important one was direct clinical practice(92.77%),followed by professional development(89.43%)and scientific research and evidence-based practice(86.40%).71.72%of medical institutions suggested that the theoretical training time is 4 weeks,combining online and offline training(50.35%),and the clinical practice time should not be less than 12 weeks(97.66%).Theoretical learning course assignment(92.93%),case report(90.83%)and clinical practice reflection diary(88.19%)were the main process evaluation methods,while 1 case study with the completion of the report and defense(87.65%)and 1 evidence-based nursing practice research plan with the completion of a report and defense(77.93%)were the main final evaluation methods.Conclusion Advanced practice nurses are important roles in the process of nursing professional development,and medical institutions in our country have a high demand for the training of advanced practice nurses.It is imperative to carry out the training of advanced practice nurses.The suggestions of medical institutions on the training content and methods focus more on the training of core competence.On this basis,the admission conditions should be clarified,and the training program of advanced practice nurses for China's national conditions should be explored.

7.Analysis of postoperative adverse reactions in 27 cases of acne scars treated with punch elevation

Objective:To analyze postoperative adverse reactions in patients with acne scars treated with punch elevation.Methods:Totally, 27 patients with moderate or severe acne scars, who were treated with punch elevation, were collected from the Department of Dermatology, Xijing Hospital, Air Force Medical University from June 2020 to March 2021. The patients received local infiltration anesthesia during the operation, and were cared for with tensile adhesive plaster bandaging, cold compresses, and waterproofing after the operation; telephone and face-to-face follow-up visits were conducted immediately, 3, 7, 14, 21, 30, and 60 days after the operation, and numerical rating scale (NRS) of intraoperative pain, postoperative swelling duration, postoperative erythema duration, and the incidence of post-inflammatory hyperpigmentation were recorded.Results:Among the 27 patients, the severity score of acne scars was 77.76 ± 43.76 points. The intraoperative pain NRS score was 3.75 ± 2.95 points after local infiltration anesthesia, the postoperative swelling lasted 3.42 ± 2.43 days, postoperative erythema lasted 4.74 ± 3.25 weeks, and the duration of postoperative erythema was not significantly correlated with the severity score of acne scars ( r = -0.13, P = 0.449). After the operation, post-inflammatory hyperpigmentation occurred in 2 patients, and no local infections or fulminant acne was observed. The patient satisfaction rate was 100% (27/27) . Conclusion:The main adverse reactions to punch elevation for acne scars were postoperative swelling and erythema, which subsided spontaneously about 1 month after the operation.
8.Identification of Down syndrome fetal encephalopathy related genes and signaling pathways via bioinformatics analysis

Objective:To identify Down syndrome (DS) fetal encephalopathy related genes and signaling pathways via bioinformatics analysis, and to explore their potential mechanisms underlying the occurrence and development of DS neuropathology.Methods:Retrospective study.In December 2021, dataset GSE59630 was downloaded from the gene expression omnibus (GEO), and differentially expressed genes (DEGs) between DS and normal fetal brain tissue were identified by R software.Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis and gene set enrichment analysis (GSEA) were performed on the genes identified.The protein-protein interaction (PPI) network was constructed based on search tool for the retrieval of interacting genes online database and Cytoscape software, and key modules and hub DEGs were identified.Real-time quantitative polymerase chain reaction technique was used to verify the expression of hub genes related to neurodegeneration in brain tissue of 3 pairs of DS and normal fetuses at the gestational age of 22-33 weeks.Results:A total of 225 DEGs were screened out from DS and normal fetal brain tissue, including 18 up-regulated genes and 207 down-regulated genes.GO functional enrichment analysis showed that DEGs were mainly enriched in neurogenesis, neuronal apoptosis, transcriptional regulation, mitochondrial energy metabolism, etc.KEGG pathway enrichment analysis revealed that DEGs were associated with a variety of neurodegenerative diseases.GSEA suggested that apoptosis and inflammatory responses play a vital part in the occurrence of DS neuropathology.Ten hub genes were identified by the PPI network established, and they were mainly related to histone acetylation and transcriptional regulation.According to the tissue verification result, the variations of RAB8A, TBP and TAF6 expression conformed to the microarray data. Conclusions:The key genes and signaling pathways identified by transcriptome analysis of fetal brain tissue facilitate the comprehensive understanding of the molecular mechanism of DS neuropathology.This study provides a novel insight into the clinical diagnosis and treatment of neurodevelopmental abnormalities and mental retardation in DS.
9.Practical Guidance for Ultrasonic Evaluation of Hemophilic Arthropathy

Hemophilic arthropathy is the most common complication of hemophilia and is also the main factor of disability in hemophilia patients. Using high-resolution musculoskeletal ultrasound examination technology, the joints of hemophilia patients can be rapidly evaluated and injury grade can be established, which can provide the basis and guidance for clinical evaluation, treatment and prognosis. Standardized ultrasound operation and scoring are very important for objective evaluation of joint condition. This paper introduces the ultrasonic examination and scoring method of hemophilic arthropathy which is suitable for the clinical needs of China in detail in order to improve the consistency of ultrasonic evaluation of hemophilia centers.
10.Practice experience of head nurses on standardized training of new nurses: a qualitative study

Objective:To explore the real experience of the head nurses of the department in the practice of standardized training for new nurses, so as to provide a reference for further forming a scientific standardized training program for new nurses.Methods:From July to August 2020, convenience sampling was used to select 18 head nurses from different clinical departments of 9 large ClassⅢ Grade A hospitals were selected as the research subject for a semi-structured interview. Data were analyzed using the Colaizzi 7-step method.Results:A total of 3 themes were extracted, namely scientific teaching practice, active management practice, and problems and prospects.Conclusions:The head nurses of the department has carried out scientific teaching practice and active management practice in clinical work to ensure the effect of standardized training for new nurses. It is still necessary to further optimize the rotation program with specialties, improve the comprehensive ability of clinical teaching nurses, and strengthen the training of new nurses' critical thinking ability, so as to improve the training effect.
