1.Value of Serum D-Lactic Acid,I-FABP,and E-Selectin in Early Diagnosis of Infection After Gastrointestinal Surgery and Their Relationship with Curative Effect and TCM Syndromes

ObjectiveTo investigate the value of serum D-lactic acid,intestinal fatty acid binding protein (I-FABP),and E-selectin (Es) in early diagnosis of infection after gastrointestinal surgery and their relationship with curative effect and traditional Chinese medicine (TCM) syndromes. MethodsForty-nine patients with abdominal infection after gastrointestinal surgery treated in Xianju County Traditional Chinese Medicine Hospital from June 2020 to August 2023 were selected as the infection group,and another 53 non-infected patients after gastrointestinal surgery were selected as the non-infected group. The serum D-lactic acid,I-FABP,and Es levels at the first,third,fifth, and seventh days after surgery were compared between the two groups,and the Pearson correlation coefficient was used to analyze the correlation of serum indexes. The receiver operating (ROC) curve and the area under the curve (AUC) of the patients were analyzed to analyze the value of each serum index in diagnosing infection after gastrointestinal surgery. At the same time,the serum indexes of patients with different therapeutic effects were monitored dynamically on the first,third,fifth, and seventh days of treatment. Logistic regression was used to analyze the relationship between serum indexes and curative effect. The relationship between TCM syndromes and serum indexes was analyzed. ResultsThe levels of serum D-lactic acid,I-FABP,and Es in the two groups decreased on the first,third,fifth, and seventh days. There were significant differences among groups, time, and interaction (P<0.01). On the third day,serum D-lactic acid was positively correlated with I-FABP and Es,and I-FABP was positively correlated with Es (P<0.01). On the third day after surgery,the AUC of D-lactic acid + I-FABP + Es in the diagnosis of infection after gastrointestinal surgery was 0.861,higher than 0.727 of D-lactic acid,0.745 of I-FABP, and 0.797 of Es (P<0.05). The levels of serum D-lactic acid,I-FABP,and Es in patients with remission infection were decreased continuously on the first,third,fifth,and seventh days. Patients with exacerbation of infection showed a continuous upward trend,and there were significant differences among groups, time, and interaction (P<0.01). The TCM syndromes of the patients were heat toxicity syndrome,acute deficiency syndrome,blood stasis syndrome, and fu-Qi obstruction syndrome,accounting for 61.22%,20.41%,12.24%, 6.12% respectively. There were no significant differences in different syndromes,time, and interaction. ConclusionSerum D-lactic acid,I-FABP,and Es can be used for early diagnosis of abdominal infection after gastrointestinal surgery,and the change of indicators can reflect the therapeutic effect and prognosis of patients with infection,but there is no significant correlation between the change of indicators and TCM syndromes.
2.Comparison of decompression effects between spine endoscopy hybrid technique and uniportal endoscopic surgery in treatment of lumbar spinal stenosis with bilateral symptom

BACKGROUND:Spinal canal decompression using uniportal endoscopic surgery is a new minimally invasive surgery in the treatment of lumbar spinal stenosis.However,this technique needs a steep learning curve and high requirements for surgical equipment and instruments,which limits its clinical application.We previously use the spinal endoscopy as a monitoring endoscopy and combined with unilateral biportal endoscopy to propose a hybrid technique of spinal endoscopy to achieve coaxial endoscopic operation and hands-separate operation. OBJECTIVE:To compare the clinical outcome of hybrid technique and uniportal endoscopic surgery in treatment of lumbar spinal stenosis with bilateral lower limb pain symptoms. METHODS:Ninety patients diagnosed of lumbar spinal stenosis with bilateral symptoms were included and retrospectively analyzed at First People's Hospital,Shanghai Jiao Tong University from August 2020 to August 2022.44 cases were included in group A(hybrid technique group),while 46 cases were included in group B(uniportal endoscopic surgery).The nerve decompression was observed during the surgery.Operation time,hospital stay time,and expenses were recorded in both groups.The visual analog scale scores of lower back pain and both lower extremities pain,Oswestry disability index scores of quality of life and excellent and good rate of modified Macnab criteria were recorded and compared at preoperative,postoperative 3 days,and postoperative 3 and 6 months. RESULTS AND CONCLUSION:(1)The operation time of group A was significantly shorter than that of group B(P<0.05).(2)The lower back pain and lower extremity pain of the severe side at postoperative 3 days,and 3 and 6 months were significantly relieved in both groups(P<0.05).The visual analog scale score of lower extremity pain on the mild side was significantly decreased at postoperative 3 days,3 and 6 months than preoperative score in the group A(P<0.05).The visual analog scale score of lower extremity pain on the mild side was significantly decreased at postoperative 3 days than preoperative score in the group B(P<0.05).The visual analog scale scores of lower extremity pain on the mild side at postoperative 3 and 6 months did not show significant difference than preoperative score in the group B.The comparison between the two groups showed that there was no significant difference in the visual analog scale scores of postoperative lower back pain and lower extremity pain of the severe side(P>0.05).The visual analog scale scores of lower extremity pain on the mild side in the group A were significantly lower than those of group B at postoperative 3 and 6 months(P<0.05).(3)The Oswestry disability index scores of both groups at postoperative 3 day were significantly lower than preoperative score(P<0.05),and there was no significant difference between the two groups 3 days after operation.Oswestry disability index scores of group A at postoperative 3 and 6 months were significantly decreased than preoperative score(P<0.05).The Oswestry disability index scores of group B at postoperative 3 and 6 months did not show significant differences than preoperative score(P>0.05).The comparison between the two groups showed the Oswestry disability index scores of group A were significantly lower than group B at postoperative 3 and 6 months(P<0.05).(4)The results of modified Macnab showed that the excellent and good rate of group A was significantly higher than that of group B(95%,78%,P<0.05).(5)It is indicated that the hybrid technique is a new spinal endoscopy technique,which has the advantages of less trauma and faster recovery as a minimally invasive surgery.The clinical outcome of hybrid technique is superior to that of uniportal endoscopic surgery in the treatment of lumbar spinal stenosis with bilateral symptoms.Additionally,it also has the advantages of good operational flexibility and high decompression efficiency as an open surgery.
3.A humanistic perspective on the doctor-patient destiny community under the healthy China strategy

The healthy China strategy emphasizes humanistic care in the grand narrative. The doctor-patient destiny community has become not only an important aspect of promoting the construction of a healthy China, but also a core demand for building a harmonious doctor-patient relationship. The healthy China strategy provides the context for the emergence of the doctor-patient destiny community, while the doctor-patient destiny community is a reasonable regulation for doctor-patient interactions and interprets the construction mode of the doctor-patient relationship under the healthy China strategy. From the multi-dimensional humanistic perspective of the doctor-patient destiny community, a doctor-patient interest community based on shared interests, a doctor-patient risk community based on risk prevention, a doctor-patient care community based on proactive health, and a smart community based on intelligent medicine were formed, to re-evaluate the underlying logic of doctor-patient interactions, compose a medical civilization based on health narratives, and promote the development of social civilization.
4.Cartilage Protection and Anti-Inflammatory Effects of Fraxetin on Monosodium Iodoacetate-Induced Rat Model of Osteoarthritis

ObjectiveTo establish a rat model of osteoarthritis and study the anti-inflammatory effects and mechanisms of fraxetin. MethodsEighteen 8-week-old male SPF-grade SD rats were randomly divided into three groups: Rats in the blank group received a right articular cavity injection of 50 μL of normal saline for 1 week; the model and intervention groups were injected with monosodium iodoacetate (MIA) into the right joint cavity to induce osteoarthritis, while the intervention group subsequently received fraxetin (5 mg·kg-1·d-1) for 1 week. Four weeks after drug intervention, abdominal aortic blood was collected. The animals were then euthanized, and knee joint cartilage were collected. The cartilage samples were stained with hematoxylin-eosin, safranin O-fast green, and toluidine blue for histopathological examination and scoring using the Mankin and OARSI scoring systems. The trabecular bone volume/total volume (Tb.BV/TV), trabecular bone surface density/total volume (Tb.BS/TV), and trabecular number (Tb.N) of each group were compared and analyzed using a micro-CT scanning system. The expression levels of various inflammatory factors [tumor necrosis factor α (TNF-α), interleukin-1β (IL-1β), interleukin-6 (IL-6)], and cartilage oligomeric matrix protein (COMP) were measured using enzyme-linked immunosorbent assay (ELISA). The expression levels of mitogen-activated protein kinase p38 (p38 MAPK), phosphorylation-p38 MAPK (p-p38 MAPK), c-Jun N-terminal kinase (JNK), and phosphorylation-JNK (p-JNK) were measured by western blotting. ResultsThe staining of cartilage sections of rat knee joints showed that the articular surface defects in the model group were severe, while the cartilage destruction in the intervention group was relatively reduced. Micro-CT results showed that Tb.BV/TV, Tb.BS/TV and Tb.N in the intervention group were significantly higher than those in the model group (P < 0.05); the Mankin score in the model group was significantly higher than that in the blank group (P < 0.05), the Mankin score in the intervention group was significantly lower than that in the model group (P < 0.05); while the OARSI score in the intervention group was significantly lower than that in the model group (P < 0.05). The results of the enzyme-linked immunosorbent assay showed that the serum levels of TNF-α, IL-1β, IL-6, and COMP in the model group were significantly higher than those in the blank group (all P < 0.05), while those in the intervention group were significantly lower than in the model group (P < 0.05). Western blot results showed that the expression levels of p-p38 MAPK and p-JNK in the knee cartilage tissue were significantly lower in the intervention group than in the model group (both P < 0.05), and significantly higher in the model group than in the blank group (both P < 0.05). ConclusionFraxetin may play a therapeutic role in a monosodium iodoacetate-induced rat model of osteoarthritis through the p38 MAPK pathway.

6.Impacts of pre-pregnancy exposure to per- and polyfluoroalkyl substances on menstrual characteristics among women undergoing assisted reproductive technology

Background Per- and polyfluoroalkyl substances (PFAS) are a class of persistent organic pollutants widely used in various products, leading to population exposure and long-term accumulation. At present, there is a lack of research on the relationships between pre-pregnancy PFAS and menstrual characteristics among women undergoing assisted reproductive technology (ART) in China. Objective To explore the relationships between pre-pregnancy PFAS exposure among women undergoing ART and menstrual characteristics prior to assisted reproductive treatment. Methods This study employed a cross-sectional research design, recruiting women undergoing ART treatment at the Reproductive Clinic of the International Peace Maternity & Child Health Hospital, School of Medicine, Shanghai Jiao Tong University, from 2017 to 2020 as study participants. Ultra-high-performance liquid chromatography-tandem mass spectrometry was used to detect 42 types of PFAS in pre-pregnancy serum samples. Questionnaires were administered to collect information on demographic characteristics, lifestyle habits, and menstrual characteristics (average menstrual cycle length, average menstrual period length, menstrual irregularities, and menstrual bleeding volume) of women undergoing ART. Multiple linear regression, binary logistic regression, and multinomial logistic regression analyses were performed to investigate the relationships between individual PFAS exposure before pregnancy and menstrual characteristics among ART women. Additionally, weighted quantile sum (WQS) model was applied to analyze the association between PFAS mixtures and menstrual characteristics. Results In the pre-pregnancy serum samples of the study population, 15 PFAS were detected in more than 60% of the samples, including perfluoroheptanoic acid (PFHpA), perfluorooctanoic acid (PFOA), perfluorononanoic acid (PFNA), perfluorodecanoic acid (PFDA), perfluoroundecanoic acid (PFUnDA), perfluorododecanoic acid (PFDoDA), perfluorobutanesulfonic acid (PFBS), perfluorohexanesulfonic acid (PFHxS), perfluoroheptanesulfonic acid (PFHpS), perfluorooctanesulfonic acid (PFOS), 6:2 chlorinated polyfluorinated ether sulfonate (6:2 Cl-PFESA), 8:2 chlorinated polyfluorinated ether sulfonate (8:2 Cl-PFESA), perfluoro-2-propoxypropanoic acid (HFPO-DA), perfluoro-2-methoxyacetic acid (PFMOAA), and perfluoro-(3,5,7,9,11-pentaoxadodecanoic) acid (PFO5DoDA). Among them, PFOA had the highest median concentration of 9.160 ng·mL−1. The single PFAS exposure analysis revealed a positive correlation between PFAS and irregular menstrual cycles. Specifically, for every natural-log unit (e) increase in PFOA, PFBS, or PFHxS level, the incidence of irregular menstrual cycles increased by 57%, 42%, or 39%, respectively. Most PFAS were positively correlated with the average number of menstrual cycle days, such as PFHpA (b=1.08, 95%CI: 0.11, 2.05), PFOA (b=1.69, 95%CI: 0.39, 3.00), PFBS (b=1.23, 95%CI: 0.25, 2.22), PFHxS (b=1.47, 95%CI: 0.61, 2.32), PFHpS (b=1.48, 95%CI: 0.35, 2.61), and 6:2 Cl-PFESA (b=0.90, 95%CI: 0.08, 1.72). Furthermore, levels of PFHpA (OR=1.39, 95%CI: 1.06, 1.82), PFOA (OR=1.58, 95%CI: 1.09, 2.30), PFBS (OR=1.37, 95%CI: 1.04, 1.80), PFHxS (OR=1.34, 95%CI: 1.05, 1.71), PFHpS (OR=1.53, 95%CI: 1.10, 2.14), and 6:2 Cl-PFESA (OR=1.34, 95%CI: 1.06, 1.70) were positively correlated with low menstrual blood volume, while PFOA (OR=0.40, 95%CI: 0.23, 0.71), PFHpS (OR=0.45, 95%CI: 0.29, 0.71), and HFPO-DA (OR=0.68, 95%CI: 0.48, 0.97) were negatively correlated with high menstrual blood volume. The mixed exposure model showed that PFAS mixtures were positively correlated with the average number of menstrual cycle days (b=1.60, 95%CI: 0.49, 2.71), irregular menstrual cycles (OR=1.77, 95%CI: 1.19, 2.63), and low menstrual blood volume (OR=1.59, 95%CI: 1.08, 2.35), but negatively correlated with high menstrual blood volume (OR=0.40, 95%CI: 0.22, 0.73). Conclusion Women undergoing ART in Shanghai are widely exposed to PFAS prior to conception. Exposure to PFAS before pregnancy may be related to menstrual characteristics among women seeking ART before undergoing fertility treatments, but additional data from larger populations are required to validate the findings of this study.
7.Primary splenic lymphoma misdiagnosed as Sjögren’s syndrome with liver cirrhosis: A case report

Primary splenic lymphoma is a rare malignant neoplasm, with similar clinical manifestations to Sjogren’s syndrome and liver cirrhosis, which often leads to misdiagnosis. This article reports a case of primary splenic lymphoma misdiagnosed as Sjogren’s syndrome with liver cirrhosis, in order to improve the understanding of primary splenic lymphoma, Sjogren’s syndrome, and liver cirrhosis and avoid misdiagnosis and treatment delay.
8.Smoking attitudes and behaviors among middle school students in Shijingshan District

Objective:
To investigate smoking attitudes and behaviors among middle school students in Shijingshan District, Beijing Municipality, so as to provide the evidence for implementing tobacco control interventions for students.
Methods:
In 2019 and 2023, students from three junior high schools, two senior high schools and one vocational high school in Shijingshan District were selected using a multi-stage stratified cluster sampling method. Data of basic information, smoking attitudes, cigarette use, e-cigarette use and secondhand smoke exposure were collected using questionnaire surveys. The changes in smoking attitudes and behaviors among middle school students between the two surveys were analyzed.
Results:
A total of 671 students were surveyed in 2019, including 343 males (51.12%) and 328 females (48.88%). A total of 759 students were surveyed in 2023, including 352 males (46.38%) and 407 females (53.62%). The proportion of students who thought that smoking made young people more attractive in 2023 was 3.29%, lower than 6.56% in 2019 (P<0.05). There were no statistically significant differences in the proportions of those who believed that secondhand smoke was definitely harmful, those who thought that quitting smoking was definitely difficult, and those who felt that smoking in social situations made them more comfortable between 2019 and 2023 (all P>0.05). In 2023, the proportions of current smoking rate and the attempted smoking rate were 0.66% and 5.53%, respectively, which were lower than 2.24% and 8.94% in 2019 (both P<0.05). The awareness rate of electronic cigarettes was 96.18% in 2023, higher than 88.97% in 2019 (P<0.05). The usage rate of electronic cigarettes was 5.01% in 2023, lower than 10.28% in 2019 (P<0.05). The proportion of exposure to secondhand smoke in family and public places was 73.25% in 2023, higher than 66.77% in 2019 (P<0.05). The proportion of those who saw someone smoking on campus within the past 30 days was 15.81% in 2023, lower than 29.96% in 2019 (P<0.05).
Conclusions
Compared with 2019, the smoking behaviors of middle school students in Shijingshan District decreased in 2023, and the awareness rate of electronic cigarettes and the proportion of secondhand smoke exposure in family and public places increased. Health education should be strengthened.

10.Clinical Value of a Novel Prognostic Prediction Model in Diffuse Large B-Cell Lymphoma.

OBJECTIVE:
To explore a predictive model that can better predict the prognosis of patients with diffuse large B-cell lymphoma (DLBCL), and validate its clinical value.
METHODS:
Clinical data of 134 newly treated DLBCL patients were collected from Xinhua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2015 to January 2020. Several risk factors of the patients were screened and analyzed, a novel prognostic model were then established based on this, and its clinical application potential was validated.
RESULTS:
In the novel model, predicting progression-free survival (PFS) based on the age at initial treatment, albumin level, Hans classification, Ann Arbor stage, and BCL2 expression showed better predictive performance than International Prognostic Index (IPI) score (AUC: 0.788 vs 0.620,P <0.001). Predicting overall survival (OS) based on the age at initial treatment, albumin level, lactate dehydrogenase (LDH) level, and expressions of BCL2 and MUM1 proteins also showed better predictive performance for mortality risk than IPI score (AUC: 0.817 vs 0.624,P <0.001).
CONCLUSION
This novel prognostic model can better predict the survival prognosis of DLBCL patients compared to the IPI scoring system.
