1.Comparative study of posterior axillary edge approach and arthroscopic assisted reduction in treatment of Ideberg type Ⅰ and Ⅱglenoid fracture of the scapula.

OBJECTIVE:
To compare the effectiveness of posterior axillary edge approach and arthroscopic assisted reduction in the treatment of Ideberg type Ⅰ and Ⅱ glenoid fracture of the scapula.
METHODS:
The clinical data of 26 patients with fresh Ideberg type Ⅰ and Ⅱ scapular fractures admitted between June 2021 and September 2024 who met the selection criteria were analyzed retrospectively. The patients were divided into two groups according to different treatment methods. Ten cases in the posterior axillary edge group were fixed by open reduction plate through the posterior axillary edge approach, and 16 cases in the arthroscopy group were treated with suture anchor fixation under arthroscopy. There was no significant difference in baseline data between the two groups ( P>0.05), such as gender, age, surgical side, Ideberg type, cause of injury, time from injury to operation, rotator cuff injury, and superior labrum anterior posterior (SLAP) injury, etc. The operation time and fracture healing time were recorded and compared between the two groups, and the shoulder pain was evaluated by visual analogue scale (VAS) score at 1 week, 1 month, and 3 months after operation. At 3 and 6 months after operation, the range of motion of shoulder joint in anteflexion, abduction, external rotation, internal rotation, and backward extension was evaluated, the upper limb dysfunction was evaluated by the Disability Assessment Scale of Arm, Shoulder, and Hand (DASH), and the shoulder joint function was evaluated by the Constant-Murley score. The differences between 6 months and 3 months after operation (changes) were statistically analyzed.
RESULTS:
Patients in both groups were followed up 11-13 months, with an average of 12.5 months. The operation time and fracture healing time in the posterior axillary edge group were significantly shorter than those in the arthroscopy group ( P<0.05). There was no complication such as wound infection, vascular and nerve injury, loss of reduction, bone nonunion, or glenohumeral instability in both groups. At 1 week after operation, the VAS score in the posterior axillary edge group was significantly higher than that in the arthroscopy group ( P<0.05); there was no significant difference in the VAS score between the two groups at 1 and 3 months after operation ( P>0.05). At 6 months after operation, the changes of shoulder joint in anteflexion, internal rotation range of motion and DASH scores in the posterior axillary edge group were significantly lower than those in the arthroscopy group ( P<0.05), while the changes of abduction, external rotation, backward extension range of motion and Constant-Murley scores were not significantly different between the two groups ( P>0.05).
CONCLUSION
For Ideberg type Ⅰ and Ⅱ glenoid fracture of the scapula, the posterior axillary edge approach for internal fixation has a short operation time, fast fracture healing, and is beneficial for early functional recovery; arthroscopic assisted reduction has minimal trauma and can handle joint injuries simultaneously. Both surgical procedures are safe and effective, and individualized selection should be made based on soft tissue conditions and combined injuries.

2.Analysis and prediction of globally and Chinese epidemiological trends of inflammatory bowel disease from 1990 to 2021

Objective:To analyze the changes in the number of patients, incidence, mortality rate, disability-adjusted life year (DALY), years lived with disability (YLD), and years of life lost due to premature mortality (YLL) globally and in China from 1990 to 2021, and to predict future trends in the number of patients and DALY of inflammatory bowel disease (IBD) globally and in China.Methods:Descriptive epidemiology was applied. Data on globally and Chinese IBD burden indicators, including prevalence, incidence, mortality, DALY, YLL, and YLD were collected from the Global Burden of Disease (GBD) 2021 Database from 1990 to 2021, and the trends in the changes and distributions of age and gender were analyzed. The age-standardized rate was standardization based on the world standard population age structure estimated by GBD. Auto-regressive integrated moving average model was used to predict the number of IBD patients and DALY globally and in China from 2022 to 2030.Results:In 1990 and 2021, the global number of IBD patients was 2.170 2 and 3.830 1 million, respectively, while in China which was 62 100 and 168 100, respectively. The global crude incidence rate and age-standardized incidence rate were 3.74/100 000, 4.22/100 000, 4.75/100 000, and 4.45/100 000, respectively. The crude incidence rate and age-standardized incidence rate in China were 0.71/100 000, 0.74/100 000 and 1.75/100 000, 1.40/100 000, respectively. In 1990 and 2021, the global crude mortality rate and age-standardized mortality rate of IBD were 0.40/100 000, 0.60/100 000 and 0.54/100 000, 0.52/100 000, respectively; the crude mortality rate and age-standardized mortality rate in China were 0.37/100 000, 0.75/100 000 and 0.40/100 000, 0.33/100 000, respectively. Compared with those in 1990, the global crude DALY, YLD and YLL of IBD all increased in 2021, which were 1 510.8 thousand person-years vs. 948.9 thousand person-years, 579.2 thousand person-years vs. 330.9 thousand person-years, 931.6 thousand person-years vs. 618.0 thousand person-years; the age-standardized DALY, YLD and YLL all decreased, which were 18.07/100 000 vs. 21.54/100 000, 6.79/100 000 vs. 7.27/100 000, 11.27/100 000 vs. 14.27/100 000, respectively. Compared with those in 1990, the crude YLD and the age-standardized YLD in China both increased (26.9 thousand person-years vs. 10.1 thousand person-years, 1.47/100 000 vs. 0.91/100 000), while the crude DALY, the age-standardized DALY, crude YLL and the age-standardized YLL all decreased (136.9 thousand person-years vs. 162.2 thousand person-years, 7.68/100 000 vs. 18.38/100 000, 110 thousand person-years vs. 152 thousand person-years, 6.21/100 000 vs. 17.47/100 000).From 1990 to 2021, male and female age-standardized incidence and prevalence of IBD were all in upward trend. The difference in the incidence of IBD between males and females was relatively small, and the global age-standardized incidence of IBD in males were slightly higher than those in females, while in China the rates are similar between the two genders. The global and Chinese age-standardized prevalence in females were slightly higher than those in males. From 1990 to 2021, the estimated annual percentage change (EAPC) of age-standardized IBD incidence in global and China were 0.24 (95% confidence interval (95% CI): 0.16 to 0.31) and 1.55 (95% CI: 1.25 to 1.86), respectively; the EAPC of age-standardized DALY in global and China were -0.50 (95% CI: -0.58 to -0.41) and -2.71 (95% CI: -2.99 to -2.43), respectively. The age distribution of disease onset shifted towards middle-aged and old population. It was predicted that by 2030, the annual number of new IBD cases in global would increase to 410 100, while in China, it would decrease to 21 184. Furthermore, the global DALY of IBD would increase to 1 670 527 person-year, and in China which would be 140 563 person-year. Conclusions:From 1990 to 2021, the global and Chinese number of patients and the incidence of IBD both sustained increase. The age of IBD onset towards older population. The incidence trend of IBD was aging, with significant gender bias. The global community and China continue to face many severe challenges in IBD.

4.Intracranial seizure-onset electroencephalogram patterns in mesial temporal lobe epilepsy

Objective:To explore the seizure-onset intracranial electroencephalogram patterns in mesial temporal lobe epilepsy (MTLE) by stereo-electroencephalography (SEEG).Methods:The clinical data of 16 patients with focal epilepsy admitted to Epilepsy Center, Shanghai Deji Hospital from March 2017 to December 2019 were retrospectively analyzed. Tonic discharge was taken as sign of the initial period; transition period from the interictal period to the initial period was named as the preictal period, characterized by clonic discharge that was different from the interictal period. The initial and preictal discharge patterns were described, and their corresponding relationship were studied; at the same time, the spatial distribution and synchrony of the discharges at initial and preictal periods were observed.Results:One hundred and thirty-two seizures were recorded in these 16 patients. Two patterns at initial period were noted: low-voltage fast activity (LVFA)+slow depolarizing shifts (SPSs, n=29, 22%), and rhythmic activity (RA) of low-medium amplitude without SPSs ( n=103, 78%); spatial spread of LVFA+SPSs was much wilder than that of RA. In addition, 3 patterns at preictal period were summarized: hypersynchronous (HYP, n=93, 70.5%, most commonly found in the anterior hippocampus [87/219]), sentinel spike ( n=22, 16.7%), and pattern of smooth transition from the interictal period to the initial period ( n=17, 12.9%). No special correspondence in discharge patterns was noted between the initial period and preictal period. Conclusion:Seizure is a process of gradual evolution from interictal period, preictal period to initial period. HYP potential is the most common pattern of seizure in MTLE at preictal stage, mainly appeared in the anterior hippocampus; RA potential is the most common pattern of seizure in MTLE at initial stage, with lower frequency and more limited spatial spread.
5.Detection of Japanese encephalitis virus and Tembusu virus in mosquito specimens from Daozhen county, Guizhou province

Objective:To understand the situation of arboviruses carried by mosquito specimens in Daozhen county, Guizhou province.Methods:In July 2023, mosquito specimens were collected in Daozhen county, Guizhou province, using photocatalytic mosquito trap method. Reverse transcription polymerase chain reaction (RT-PCR) technology was used to detect the types of viruses carried by mosquitoes, and phylogenetic trees were constructed using the neighbor-joining method to analyze the phylogenetic position of the detected viruses.Results:A total of 5 313 mosquito specimens belonging to 2 genera and 2 species were collected in Daozhen county, Guizhou province, including 3 953 Culex tritaeniorhynchus, 1 355 Armigeres subalbatus, and 5 other mosquito species. The specimens were divided into 54 batches according to their species for grinding, nucleic acid extraction, and testing. Among them, 6 batches of Cx. tritaeniorhynchus specimens tested positive for flavivirus genus using universal primers, and further identification revealed that 5 batches were positive for Japanese encephalitis virus (JEV) nucleic acid, and 1 batch was positive for Tembusu virus (TMUV) nucleic acid. Phylogenetic analysis showed that the detected JEV strains were all genotype I and fell within the same evolutionary branch; the detected TMUV was located on the third branch of the evolutionary tree, with the main host source of the strains on this branch being mosquitoes. Conclusions:In the mosquito specimens collected in Daozhen county, Guizhou province in 2023, genotype I JEV and TMUV were detected, with TMUV being detected for the first time in mosquitoes in Guizhou province. It is essential to enhance the surveillance of JEV and TMUV to mitigate the potential public health risks they pose.
6.ZBH2012001,a novel serotonin and norepinephrine reuptake inhibitor,exerts antidepressant effect via dual mechanism of monoamine enhancement and inflammation suppression

OBJECTIVE To evaluate the mechanisms underlying the antidepressant effect of ZBH2012001,a novel serotonin and norepinephrine reuptake inhibitor(SNRI),in general and its ability to enhance monoaminergic transmission and suppress neuroinflammation in particular.METHODS① Male ICR mice were divided into vehicle(distilled water),duloxetine(DLX,10 or 20 mg·kg-1)and ZBH2012001(5,10 and 20 mg·kg-1)groups.One hour following ig administration,the antidepressant effect of ZBH2012001 was evaluated using the tail suspension test(TST)and forced swimming test(FST).② Radioligand binding assay was conducted to evaluate the affinity of ZBH2012001 for human serotonin transporters(hSERTs)and human norepinephrine transporters(hNETs).③ Mice were divided into vehicle(distilled water),DLX(10 or 20 mg·kg-1)and ZBH2012001(5,10 and 20 mg·kg-1)groups.One hour following drug administration,the 5-hydroxytryptophan(5-HTP)-induced head-twitch test or yohimbine-induced lethality test were performed to evaluate the effect of ZBH2012001 on the function of the 5-hydroxytryptamine(5-HT)and norepinephrine(NE)systems.④ Mice were divided into vehicle(distilled water+0.1%acetic acid),reserpine model(distilled water+reserpine 5 mg·kg-1),DLX(DLX 20 mg·kg-1+reserpine 5 mg·kg-1)and ZBH2012001(ZBH2012001 5,10 and 20 mg·kg-1+reserpine 5 mg·kg-1)groups.One hour following drug administration,reserpine was injected intraperitoneally to establish a monoamine-depletion model.The ptosis,akinesia,and hypothermia assays were performed to evaluate the effect of ZBH2012001 on the down-regulation of the reserpine-induced monoamine system.The TST in mice was used to evaluate the effect of ZBH2012001 on reserpine-induced depressive-like behavior while high-performance liquid chromatography with electrochemical detection(HPLC-ECD)was used to measure the levels of monoamines and their metabolites in the hippocampal tissue of reserpine-induced monoamine-depletion mice.ELISA was employed to detect the contents of tumor necrosis factor-alpha(TNF-α)and interleukin-6(IL-6)in the hippocampal tissue of reserpine-induced monoamine-depletion mice.Western blotting was used to assess the expressions of ionized calcium-binding adapter molecule-1(Iba-1)and nuclear factor-kappa B(NF-κB)in the hippocampal tissue of reserpine-induced monoamine-depletion mice.RESULTS ① Compared with the vehicle group,ZBH2012001(5,10 and 20 mg·kg-1)significantly reduced the immobility time both in the TST in mice(P<0.01,respectively),and ZBH2012001(20 mg·kg-1)and in the FST in mice(P<0.05).② ZBH2012001 competitively inhibited the binding of[3H]-imipramine to hSERTs and[3H]-nisoxetine to hNETs,with the half maximal inhibitory concentration(IC50)values of 84.95 and 712.90 nmol·L-1,respectively.③Com-pared with the vehicle group,ZBH2012001(10 and 20 mg·kg-1)significantly increased the head twitches induced by 5-HTP in mice(P<0.01,respectively)and increased the mortality rate in mice induced by yohimbine(P<0.05,P<0.01).④ In the reserpine-induced monoamine-depletion model in mice,compared with the vehicle group,mice in the reserpine model group exhibited ptosis,akinesia and hypothermia feature(P<0.01,respectively),significantly prolonged immobility time in the TST(P<0.01),significantly decreased the levels of NE,5-HT and dopamine(DA)(P<0.05,P<0.01),significantly increased the metabolic conversion rate of 5-HT and DA(P<0.01,respectively),significantly elevated levels of TNF-α and IL-6(P<0.05,respectively),and significantly increased expressions of Iba-1 and NF-κB(P<0.05,respectively)in the hippocampus.Compared with the model group,ZBH2012001(5,10 and 20 mg·kg-1)significantly antagonized ptosis and hypothermia behaviors induced by reserpine(P<0.01,respectively),ZBH2012001(10 and 20 mg·kg-1)significantly shortened the immobility time in reserpine-treated mice(P<0.05,P<0.01),ZBH2012001(20 mg·kg-1)significantly increased the levels of NE and 5-HT in the hippocampus of reserpine-treated mice(P<0.05,respectively),decreased the metabolic conversion rate of 5-HT(P<0.05),significantly reduced the contents of TNF-α and IL-6 in the hippocampus of reserpine-treated mice(P<0.05,respectively),ZBH2012001(5,10 and 20 mg·kg-1)significantly reduced the expression of Iba-1 protein in the hippocampus of reserpine-treated mice(P<0.01,respec-tively),and ZBH2012001(20 mg·kg-1)significantly reduced the expression of NF-κB protein in the hippocampus of reserpine-treated mice(P<0.05).CONCLUSION ZBH2012001 exerts its antidepres-sant effect through a dual mechanism involving monoamine enhancement and inflammation suppres-sion.
7.Association between triglyceride-glucose index and carotid plaque in patients with type 2 diabetic kidney disease

Objective:To explore the association between triglyceride-glucose (TyG) index and carotid artery plaque in patients with type 2 diabetic kidney disease (DKD).Methods:Clinical data of 620 DKD patients admitted in the Department of Endocrinology, the Affiliated Huai′an First People′s Hospital of Nanjing Medical University from August 2018 to August 2022 were retrospectively analyzed, including 366 cases with carotid artery plaque and 254 cases without carotid plaque. According to TyG index quartile patients were divided into Q 1,Q 2, Q 3 and Q 4 groups with TyG index<8.94,≥8.94 and<9.44,≥9.44 and<9.96, and≥9.96, respectively. The prevalence of carotid plaque in DKD patients with different TyG index levels was analyzed. The relationship between TyG index and carotid plaque occurrence in DKD patients were analyzed with Logistic regression analysis and restricted cubic lines (RCS). Results:The age, course of disease, smoking rate, SBP, HbA1c, TG, BUN, eGFR and TyG indexes in carotid plaque group were significantly higher than those in non-carotid plaque group (all P<0.05). Binary logistic regression analysis showed that age, disease course, smoking rate, SBP, HbA1c, TG, BUN, low eGFR and TyG index were independent influencing factors for carotid plaque ( OR=1.05, 1.05, 1.88, 1.01, 1.09, 1.11, 1.09, 0.99 and 1.28, all P<0.05). The risk of carotid plaque in DKD patients in Q 3 and Q 4 groups was 2.20 and 2.50 times higher than that in Q 1 group. After adjusting for age, sex, course of disease, smoking, BMI, blood pressure (SBP and DBP), blood lipids (TC, HDL and LDL) and renal function, the risk of carotid plaque in DKD patients in Q 3 and Q 4 groups was higher than that in Q 1 group ( OR=1.95 and 2.24). RCS analysis showed that the correlation between TyG index and the risk of carotid plaque in DKD patients was linear(χ 2=0.40, P=0.527), and DKD patients with TyG index>9.95 had a higher risk of carotid plaque. Conclusions:TyG index is significantly elevated in DKD patients with carotid plaque, and TyG index is an independent risk factor for the occurrence of carotid plaques in DKD patients.
8.Detection and analysis of Liaoning virus from Aedes vexans collected in Hunchun city, Jilin Province

Objective:This study conducted mosquito-borne viruses RNA screening and analysis of virus evolution characteristics on mosquito specimens collected in 2023 from Hunchun city, Jilin province, China.Methods:Firstly, morphological method were employed for mosquito specimen classification. Then, real-time fluorescence quantitative reverse transcription-polymerase chain reaction (qRT-PCR) was used to detect the RNA of six mosquito-borne viruses in the collected mosquitos, i. e., Banna virus (BAV), Kadipiro virus (KDV), Liaoning virus (LNV), Tahyna virus (TAHV), Getah virus (GETV) and Japanese encephalitis virus (JEV). And by sequencing, the viral genome sequence of positive samples was obtained.Results:A total of 5490 mosquito specimens were collected from Hunchun city, Jilin province, included 4400 Aedes vexans (80.15%), 1090 Anopheles sinensis (19.85%). A total of 41 groups were obtained by mixing samples according to the time, location, and mosquito species collected. qRT-PCR result showed that only the Aedes vexans sample with the number JLHC2321 was tested positive for LNV, while the remaining samples were tested negative for the detected viruses. According to the phylogenetic analysis of the segment 10 gene, this LNV strain had the closest genetic relationship with NE9731 and belonged to the type II branch. Meanwhile, the amino acid sequence analysis based on the coding sequence (CDS) in the segment 10 showed that JLHC2321 only had 2 amino acid differential sites with the GII reference strain NE9731.Conclusions:This study detected LNV for the first time in Aedes vexans in Hunchun city, and our result provide important basic data for the monitoring and prevention strategies of mosquito-borne viruses in the region.
9.Detection and analysis of Liaoning virus from Aedes vexans collected in Hunchun city, Jilin Province

Objective:This study conducted mosquito-borne viruses RNA screening and analysis of virus evolution characteristics on mosquito specimens collected in 2023 from Hunchun city, Jilin province, China.Methods:Firstly, morphological method were employed for mosquito specimen classification. Then, real-time fluorescence quantitative reverse transcription-polymerase chain reaction (qRT-PCR) was used to detect the RNA of six mosquito-borne viruses in the collected mosquitos, i. e., Banna virus (BAV), Kadipiro virus (KDV), Liaoning virus (LNV), Tahyna virus (TAHV), Getah virus (GETV) and Japanese encephalitis virus (JEV). And by sequencing, the viral genome sequence of positive samples was obtained.Results:A total of 5490 mosquito specimens were collected from Hunchun city, Jilin province, included 4400 Aedes vexans (80.15%), 1090 Anopheles sinensis (19.85%). 10.Detection of Japanese encephalitis virus and Tembusu virus in mosquito specimens from Daozhen county, Guizhou province

Objective:To understand the situation of arboviruses carried by mosquito specimens in Daozhen county, Guizhou province.Methods:In July 2023, mosquito specimens were collected in Daozhen county, Guizhou province, using photocatalytic mosquito trap method. Reverse transcription polymerase chain reaction (RT-PCR) technology was used to detect the types of viruses carried by mosquitoes, and phylogenetic trees were constructed using the neighbor-joining method to analyze the phylogenetic position of the detected viruses.Results:A total of 5 313 mosquito specimens belonging to 2 genera and 2 species were collected in Daozhen county, Guizhou province, including 3 953 Culex tritaeniorhynchus, 1 355 Armigeres subalbatus, and 5 other mosquito species. The specimens were divided into 54 batches according to their species for grinding, nucleic acid extraction, and testing. Among them, 6 batches of Cx. tritaeniorhynchus specimens tested positive for flavivirus genus using universal primers, and further identification revealed that 5 batches were positive for Japanese encephalitis virus (JEV) nucleic acid, and 1 batch was positive for Tembusu virus (TMUV) nucleic acid. Phylogenetic analysis showed that the detected JEV strains were all genotype I and fell within the same evolutionary branch; the detected TMUV was located on the third branch of the evolutionary tree, with the main host source of the strains on this branch being mosquitoes. Conclusions:In the mosquito specimens collected in Daozhen county, Guizhou province in 2023, genotype I JEV and TMUV were detected, with TMUV being detected for the first time in mosquitoes in Guizhou province. It is essential to enhance the surveillance of JEV and TMUV to mitigate the potential public health risks they pose.
