1.The effects of differential torque of maxillary posterior teeth with clear aligner of tooth extraction correc-tion on anterior teeth 3D mechanics in vitro

Objective:To investigate the effects of differential torque of maxillary posterior teeth with clear aligner on anterior teeth 3D mechanics.Methods:4 maxillary models were designed for differential torque of posterior teeth on the basis of 3D me-chanical test model with first premolar extraction.The stereolithography models were made with the same size as the standard model by 3D printing.According to the torque of the 6 keys to normal occlusion of Andrews,the standard torque of Andrews was used in group A1.The posterior teeth with lingual-crown torque of 5° were used in groups A2,and those with buccal-crown torque of 5° and 10° were respecitvely used in group A3 and A4.4 sets of corresponding clear aligners were fabricated with a 0.75 mm thick thermoplastic material.Each group had 12 pairs of the same clear aligners.The anterior teeth underwent 0.25 mm activation for en-mass retraction.The six-axis force/torque transducer measurement system was used to measure the force applied by the clear align-ers on the anterior teeth in the 3D directions.Results:In buccolingual direction:in group A1,the 4 incisors received lingual force,in groups A2-A4,the anterior teeth received labial force,and the force on the central incisors was greater than that on later-al incisors.In mesiodistal direction:in group A1-A4,4 incisors received mesial force,and the force on lateral incisors was greater than that on central incisors;in group A1-A4,the canines received distal force;the force on canine in group A4 was greater than that in A2 or A3 groups(P<0.05).The force in the vertical direction on the anterior teeth among the 4 groups(P>0.05).Conclu-sion:When en-mass retraction of the anterior teeth with clear aligner,attention should be taken to the torque of the incisors and the tip of the canines.When the posterior teeth with standard torque,the activation of labial-crown overcorrection of the anterior teeth should be increased.When the posterior teeth with lingual-crown torque or buccal-crown torque,the activation of labial-crown over-correction of the anterior teeth should be appropriately reduced.Increasing the mesial overcorrection design of canines may avoid the extrusion of anterior teeth and the deepening of o-verbite due to the distal inclination of canines.
2.Incidence of pulmonary tuberculosis and its influencing factors in Hubei Province based on the geographically weighted regression model

Objective To study the spatial distribution of the incidence of pulmonary tuberculosis in Hubei Province and its influencing factors, so as to improve the theoretical basis for scientific development of tuberculosis prevention and control measures in the future. Methods The data of reported incidence of tuberculosis and related influencing factors in various counties and districts of Hubei Province in 2020 were collected. Global Moran's I index, hotspot analysis and geographically weighted regression (GWR) model analysis were used to calculate the spatial autocorrelation of the incidence of tuberculosis, and to analyze the influencing factors affecting the incidence rate of tuberculosis. Results There were obvious regional differences in the space distribution of the incidence rate of tuberculosis. Hot spot analysis showed positive spatial correlation and obvious clustering. The GWR model (AICc=784.251) in this study had higher AICc value compared to the ordinary least squares regression (OLS) model (AICc=804.2585). The GWR model showed that the increase in the proportion of the population aged 65 and above and the proportion of the ethnic minority population had a significant promoting effect on the increase of the incidence rate of tuberculosis, and there was significant spatial heterogeneity. The effect of PM2.5 concentration on the incidence rate of pulmonary tuberculosis varied in different regions, and the degree of effect was also different. Conclusion The proportion of people aged 65 and above and the proportion of ethnic minorities may significantly influence the incidence of pulmonary tuberculosis. The effect of PM2.5 concentration varies in different regions, so targeted measures should be formulated according to the situation in different regions.
3.Development and validation of a Knowledge-Attitude-Practice Scale for Dietary Management During Hemodialysis

Objective:To develop a Knowledge-Attitude-Practice (KAP) Scale for Dietary Management During Hemodialysis and to test its reliability and validity.Methods:Based on the KAP theoretical framework, an initial version of the scale was developed through a literature review and expert consultations. A convenience sampling method was used to recruit hemodialysis patients from four hospitals in Suzhou in March 2024. Questionnaire item analysis and reliability and validity tests were conducted.Results:A total of 460 questionnaires were distributed and 438 valid responses were collected, with an effective response rate of 95.22%. The final scale included three dimensions (knowledge, attitude, and practice) with 34 items. Content validity at the scale level was 0.910, and the item level ranged from 0.800 to 1.000. Exploratory factor analysis extracted three common factors, with a cumulative variance contribution rate of 74.520%. Confirmatory factor analysis showed a good model fit. The total Cronbach's α coefficient of the scale was 0.971, and the Cronbach's αcoefficients for the three dimensions were 0.963, 0.933, and 0.934, respectively. The test-retest reliability coefficient was 0.839.Conclusions:The Knowledge-Attitude-Practice Scale for Dietary Management During Hemodialysis demonstrates good reliability and validity, making it a valuable tool for assessing the KAP level of dietary management in hemodialysis patients.

6.Clinical characteristics and management status of Turner syndrome in 1 089 children

Objective:To investigate the clinical characteristics and management status of children with Turner syndrome (TS) in China.Methods:As a cross-sectional study, 1 089 TS patients were included in the database of the National Collaborative Alliance for the Diagnosis and Treatment of Turner Syndrome from August 2019 to November 2023. Clinical characteristics (growth development, sexual development, organ anomalies, etc.), karyotypes, auxiliary examinations, and treatments were collected and analyzed.Results:Among the 1 089 TS cases, 809 were recorded karyotypes. The karyotype distribution was as follows: 45, X in 317 cases (39.2%), X chromosome structural variants (including partial deletions of p or q arm, ring chromosome, and marker chromosome) in 89 cases (11.0%), 45, X/46, XX mosaicism in 158 cases (19.5%), mosaicism with X chromosome structural variants in 209 cases (25.8%), and presence of Y chromosome material in 36 cases (4.4%). Among the 824 TS cases, the age of diagnosis was 9.7(6.4, 12.2) years, with a height standard deviation score (HtSDS) of -3.1±1.2. Five hundred and fifty three cases underwent growth hormone (GH) stimulation test, and 352 cases (63.7%) had GH peak values <10 μg/L and 75.9% (577/760) had low IGF1 levels, with IGF1 SDS ≤-2 accounting for 38.2% (290 cases). Among 471 cases aged ≥8 years, 132 cases (28.0%) showed spontaneous sexual development (mean bone age (11.0±1.7) years), 10 cases had spontaneous menarche (mean bone age (12.0±2.2) years), and 2 cases had regular menstrual cycles. Common physical features included cubitus valgus (311 cases (28.5%)), neck webbing (188 cases (17.2%)), low posterior hairline (185 cases (17.0%)), shield chest (153 cases (14.0%)), high arched palate (127 cases (11.6%)), short fourth metacarpal (43 cases (3.9%)), and spinal abnormalities (38 cases (3.5%)). Congenital cardiovascular and urogenital anomalies occurred in 91 cases (19.4%) and 66 cases (12.0%)respectively. Abdominal ultrasound in 33 cases (7.2%) indicated fatty liver, hepatomegaly, intrahepatic bile duct stones, and splenomegaly. Among 23 cases undergoing oral glucose tolerance test (OGTT) test, 2 were diagnosed with diabetes mellitus and 4 with impaired glucose tolerance. Following diagnosis, 669 cases (80.7%) received rhGH treatment at a chronological age of (9±4) years and bone age of (8.3±3.2) years. Additionally, 112 cases (19.4%) received sex hormone replacement therapy starting at the age of (14±4) years and bone age of (12.6±1.2) years.Conclusions:The karyotypes of 45, X and mosaicism were most common in Chinese children with TS. The clinical manifestations were mainly short stature and gonadal dysplasia. However, a few TS children could be in the normal range of height, and some cases among those aged of ≥8 years old had spontaneous sexual development. Some exhibited physical features, congenital cardiovascular and urogenital anomalies, and dysfunction of the hypothalamic-pituitary-IGF1 axis. Moreover, a few of them developed impaired glucose tolerance and diabetes mellitus. Following diagnosis, most of the patients received rhGH treatment, and a few of them received sex hormone replacement therapy.

Severe asthma accounts for 5%-10%of the overall asthma population,resulting in a substantial decline in patients'quality of life and a significant financial burden.Despite there are some treatment options,such as combining high-dose inhaled corticosteroids with long-acting β2 agonists or oral corticosteroids,some patients continue to struggle with symptom control;in addition,prolonged steroid use can lead to various adverse effects.Recent years have witnessed continuous advancements in the diagnosis and treatment approaches for severe asthma,offering clinicians numerous novel alternatives.This article reviews their research progress encompassing disease assessment,medication therapy,and bronchoscopic interventions.

Aim To systematically evaluate the efficacy and safety of Tongxinluo capsule combined with rosuvas-tatin in the treatment of carotid atherosclerosis.Methods A comprehensive screening of Tongxinluo capsule combined with rosuvastatin was performed by Web searching the CNKI,Wanfang Data,VIP,PubMed,Embase,The Cochrane Li-brary,and Web of Science from the construction of the database to August 31,2023,to comprehensively screen the litera-ture of randomized controlled studies of Tongxinluo capsule combined with rosuvastatin in the treatment of carotid athero-sclerosis in the literature of randomized controlled studies,and Meta-analysis was performed using RevMan 5.4 software.Results A total of 8 studies were included with 858 total cases,426 in the treatment group and 432 in the control group.Meta-analysis showed that the addition of Tongxinluo capsule to rosuvastatin treatment lowered total cholesterol(MD=-0.54,95%CI:-0.61~-0.46,P<0.00001),triglyceride(MD=-0.28,95%CI:-0.37~-0.20,P<0.00001),low density lipoprotein cholesterol(MD=-0.35,95%CI:-0.42~-0.29,P<0.00001)and increased high density lipoprotein cholesterol(MD=0.27,95%CI:0.05~0.50,P=0.02)levels,lowered carotid intima-media thickness(MD=-0.35,95%CI:-0.48~-0.22,P<0.00001)and plaque Crouse's score(MD=-1.33,95%CI:-2.11~-0.55,P=0.0009),and the differences were statistically significant(P<0.05).Adverse effects were reported in five studies,and the difference was not statistically significant(MD=1.80,95%CI:0.80~4.08,P=0.16).Conclusion Tongxinluo capsule combined with rosuvastatin in the treatment of carotid atherosclerosis can improve the blood lipid level and reduce the degree of atherosclerosis,with better efficacy and safety,which can be further promoted for clinical use.

Sepsis-induced liver injury (SILI) is an important cause of septicemia deaths. BaWeiBaiDuSan (BWBDS) was extracted from a formula of Panax ginseng C. A. Meyer, Lilium brownie F. E. Brown ex Miellez var. viridulum Baker, Polygonatum sibiricum Delar. ex Redoute, Lonicera japonica Thunb., Hippophae rhamnoides Linn., Amygdalus Communis Vas, Platycodon grandiflorus (Jacq.) A. DC., and Cortex Phelloderdri. Herein, we investigated whether the BWBDS treatment could reverse SILI by the mechanism of modulating gut microbiota. BWBDS protected mice against SILI, which was associated with promoting macrophage anti-inflammatory activity and enhancing intestinal integrity. BWBDS selectively promoted the growth of Lactobacillus johnsonii (L. johnsonii) in cecal ligation and puncture treated mice. Fecal microbiota transplantation treatment indicated that gut bacteria correlated with sepsis and was required for BWBDS anti-sepsis effects. Notably, L. johnsonii significantly reduced SILI by promoting macrophage anti-inflammatory activity, increasing interleukin-10+ M2 macrophage production and enhancing intestinal integrity. Furthermore, heat inactivation L. johnsonii (HI-L. johnsonii) treatment promoted macrophage anti-inflammatory activity and alleviated SILI. Our findings revealed BWBDS and gut microbiota L. johnsonii as novel prebiotic and probiotic that may be used to treat SILI. The potential underlying mechanism was at least in part, via L. johnsonii-dependent immune regulation and interleukin-10+ M2 macrophage production.
