1.Preliminary evaluation of modified anterior splint combined with anterior repositioning splint after successful mandibular manipulation in treatment of acute anterior disc displacement without reduction of temporomandibular joint.

OBJECTIVES:
Magnetic resonance imaging (MRI) was used to study the efficacy of modified anterior splint (MAS) combined with anterior repositioning splint (ARS) in maintaining the normal disk-condyle relationship.
METHODS:
We obtained clinical and imaging data of 10 patients with acute anterior disk displacement without reduction (ADDwoR) of the temporomandibular joint undergoing successful mandibular manipulation in the Affiliated Stomatological Hospital, Nanchang University from January to June 2024. The MAS and ARS were used for subsequent treatment. Maximum active mouth opening (MMO) and visual analogue score (VAS) were recorded before treatment and 2 weeks, 1 month, and 3 months after treatment. The changes in the disk-condyle relationship, disk-condyle angle, joint effusion, and double contour sign before and after treatment were analyzed through MRI.
RESULTS:
Three months after treatment, the MMO increased from 28.90 mm to 46.85 mm, VAS decreased from 4.68 to 0.61, and the disk-condyle angle decreased from 58.84° to 25.99°. MRI analysis found that 6 patients showed normal disk-condyle relationship, 2 patients exhibited displacement with reduction, and 2 patients exhibited ADDwoR. The disk-condyle angle was 58.84° before treatment and 25.99° after treatment.
CONCLUSIONS
The MAS combined with ARS could further stabilize the coordinated disk-condyle relationship and reduce the recurrence rate.

2.Study of CT dual-low technology angiography in patients with stroke

Objective This study focused on the low voltage and low concentration contrast agent technology in vascular imaging of stroke patients.Methods Totally144 patients of ischemic stroke confirmed from January 2022 to January 2023 were enrolled.They were divided into four groups according to different imaging methods:conventional group(tube voltage 120kV,iodine concentration 370mgI/ml),low voltage group(tube voltage 80kV,iodine concentration 370mgI/ml),low concentration group(tube voltage 120kV,iodine concentration 320mgI/ml)and dual-low group(tube voltage 80kV,iodine concentration 320mgI/ml),the post-processing technology adopted ASiR algorithm for 3D reconstruction.Then to compare contrast signal-to-noise ratio(CNR)and signal-to-noise ratio(SNR)of the reconstructed image,image quality score,CT values of internal carotid artery(ICA),anterior cerebral artery(AA)A1,middle cerebral artery(MA)M1,posterior cerebral artery(PA)P1,vertebral artery(VA)and basilar artery(BA),mean iodine intake and radiation dose parameters including volume CT dose index(CTDIvol)and dose length product(DLP).Results The CNR and SNR values in dual-low group were significantly less than the other three groups(P<0.05),while image quality score and CT value of each artery were no differences among the four groups(P>0.05).What's more,CTDIvol,DLP and iodine intake volume in dual-low group were significantly less than the others(P<0.05).Conclusion The dual-low technology scanning could obtain satisfactory head and neck CT angiography for stroke patients.
3.Study on influencing factors and needs of medical personnel in scientific research of a tertiary hospital in Jieyang

Objective:To understand the needs, problems and challenges of medical personnel in the hospital for scientific research, propose tailored management measurements and suggestions for the hospital.Methods:The questionnaire was prepared by " Questionnaire Star" , the convenience sampling method was used, and information was collected by WeChat online chat group. Factor analysis and multi-dimensional preference analysis were adopted to analyze information collected.Results:Four main factors that affect the scientific research of medical personnel were identified. The most significant impact is the lack of information platform, followed by the lack of environmental atmosphere, lack of personal capacity and lack of personal interest. The research needs of medical personnel are divided into two categories: one is the more basic skill needs, which are mainly targeted at female, nurses, undergraduates, 5~10 years′ working experiences, medium-grade professional title, and people with no knowledge and experience in research. The other one is the needs of skill improvement, which are mainly targeted at younger, physicians, early career, lower professional titles, and had certain research knowledge.Conclusions:So far, there is a lot of space for improvement. It is recommended to promote the capacity building by training, strengthen the research team building and construct of research communication platform, at the same time, update the scientific research management system.
4.Clinical study of the first stage suture after laparoscopic common bile duct exploration

Objective To research the clinical application value of laparoscopic bile duct exploration and the first stage suture for choledocholithiasis. Methods The clinical data of 86 patients with choledocholithiasis were analyzed retrospectively from January 2015 to January 2017 in Affiliated Suzhou Hospital of Nanjing Medical University. According to the different methods of laparoscopic bile duct exploration, they were divided into one stage suture group (observation group, 46 cases) and T tube drainage group (control group, 40 cases). Results The recovery time of gastrointestinal function, hospitalization time and hospitalization expenses in observation group were significantly better than those in control group: (22.71 ± 10.92) h vs. (35.63 ± 11.12) h, (8.4 ± 2.6) d vs. (13.5 ± 2.3) d and (12.1 ± 3.2) thousand yuan vs. (13.5 ± 4.2) thousand yuan, and there were statistical differences (P<0.05). Conclusions Compared with T tube drainage, the first stage suture after laparoscopic common bile duct exploration has shorter hospitalization time, lower treatment cost and faster recovery. It is a safe and feasible operation method, and is worthy of popularization and application.
5.Application of full-volume three-dimensional ultrasound in evaluation of left heart function of patients with coronary heart disease

Objective To evaluate the effect of full-volume three-dimensional echocardiography in evaluation of left heart function of patients with coronary heart disease. Methods A total of 50 normal controls and 50 patients with coronary heart disease were enrolled, and underwent threedimensional echocardiography. The three-dimensional ultrasound images were collected and analyzed.The left ventricle and the 17-segment volume-time curve were obtained. The local ejection fraction (REF), local stroke volume (RSV), local end-systolic volume (RESV), left ventricular 17-segment local end-diastolic volume (REDV) were compared. Results The levels of RESV and REDV in the coronary heart disease group were higher, and the REF level was lower than that in the normal group (P < 0. 05). The levels of RESV and REDV in the coronary heart disease group were significantly decreased, and the postoperative REF was significantly increased (P < 0. 05). The preoperative REF of the coronary heart disease group was significantly lower than that of the normal group (P <0. 05), and the postoperative REF level was increased to different extents to normal range (P <0. 05). Conclusion Full-volume three-dimensional ultrasonography can be used to accurately assess the cardiac volume and cardiac function of ischemic remodeling in coronary heart disease.

7.Early predictive and prognostic value of 18F-fluorodeoxyglucose positron emission tomography-CT for response assessment in non-small cell lung cancer treated with epidermal growth factor receptor tyrosine kinase inhibitor

Objective To evaluate whether an early change in 18F-fluorodeoxyglucose (18F-FDG) uptake can predict tumor response to epidermal growth factor receptor tyrosine kinase inhibitor (EGFR-TKI) and prognosis in patients with non-small cell lung cancer (NSCLC). Methods From August 2009 to April 2015, 22 patients with NSCLC who were eligible to EGFR-TKI treatment were enrolled. PET-CT scan was performed before (baseline) and 1 month after EGFR-TKI administration. Up to 5 hottest single tumor lesions (no more than 2 per organ) were considered to be target lesions. Maximum standardized uptake values (SUVmax) were measured, and post-treatment percentage changes in SUVmax (ΔSUV%) were calculated. PET responses were classified using PET response criteria in solid tumors (PERCIST). Then conventional CT scan was performed every 2 months for follow-up. Kappa statistic was used to compare agreement between the RERCIST recommendations-based therapeutic response evaluation and those based on RECIST1.1 criteria. Fisher exact test was used to compare the probability of disease progression in the early metabolic response and non-response groups. Predictive accuracy of ΔSUV% with respect to response or non-progression at CT scan was evaluated by ROC analysis. Progression-free survival (PFS) was determined by Kaplan-Meier survival analysis, and between-group comparison was performed by log-rank test. Results After 1 month of EGFR-TKI treatment, 12 patients (55%) showed partial metabolic response (PMR), 6 (27%) had stable metabolic disease (SMD), and 4 (18%) had progressive metabolic disease (PMD). There was a moderate agreement(Kappa=0.506,P<0.05) between PET response at 1 month based on PERCIST recommendations and CT response at 3 months according to RECIST 1.1. Non-progression was significantly more frequent in patients with an early PMR (χ2=11.941, P=0.005). Progression had been confirmed later during therapy in all patients with PMD . By using ROC analysis, the area under the curve for prediction of response was 0.906 (95% CI, 0.766—1.000; P=0.002), corresponding to a sensitivity of 88.9% and specificity of 84.6% at a cut-off of 40.36% in ΔSUV%. Using a cut-off value of 25.84% in ΔSUV%, highΔSUV% group (ΔSUV% ≥ 25.84%) had significantly longer PFS than low ΔSUV% group (ΔSUV%<25.84%). Conclusion Early assessment of PET-CT at 1 month of EGFR-TKI treatment could be useful to predict tumor response and clinical outcome in patients with NSCLC.
8.Simultaneous Determination of 21 Inorganic Elements in Hypericum japonicum by ICP-MS

OBJECTIVE:To establish the method for simultaneous determination of 21 inorganic elements in Hypericum japoni-cum. METHODS:ICP-MS method was adopted. The power was 1300 W;flow rate of cooling gas was 1.5 L/min;flow rate of carrier gas was 0.8 L/min;flow rate of auxiliary gas was 0.2 L/min;integration time was 10 s;delay time of 1 s;repetition times was one time;measurement mode was standard curve method. SPSS 16.0 statistical software was used for relationship analysis and main component analysis. RSDs of precision,stability and reproducibility tests were all lower than 5.0%. The recoveries were 93.9%-106.9%(RSD were 0.22%-2.94%,n=6).CONCLU-SIONS:The method is simple,precise,stable and reproducible,and can be used for simultaneous determination of 21 inorganic el-ements in H. japonicum.

OBJECTIVETo investigate the microsurgical tchniques and effects for the resection of anterior clinoid meningioma (ACM).
METHODSBetween January 2003 and March 2013, a total of 46 ACM patients were operated on via the pterion approach or lateral subfrontal approach. There were 16 male patients and 30 female patients, their mean age was 48.5 (16-69) years. Symptoms lasted from 1 month to 9 years(average 24.7 months). Preoperative CT and MRI examination showed that the tumors were located in the anterior clinoid process (25 cases on the right side, 21 cases on the left side). The average maximal tumor diameter was 4.4 cm (1.9-7.4 cm). The patients were peroidly followed-up on outpatient on 3 months to 6 months, then every year postoperativelly. The clinical data, radiological findings, surgical records and outcome of patients were retrospectively analyzed, and the prognostic factors were ananlyzed by using of the unpaired data χ(2) test with continuity correction.
RESULTSOf fourty-six patients operated on, apparently complete removal was achieved in 36 patients (78.3%), 10 patients (21.7%) had minimal residual tumors, 5 of which had gamma knife radiosurgery. Preexisting visual deficit improved in 21 of 41 patients (51.2%), unchanged in 16(35.6%), and worsened in 9(20.0%). At 3-6 months after discharge, 37(80.4%) patients had a good recovery, 7(15.6%) patients were moderately disabled, 1(2.2%) presented with severe disability, and 1 (2.2%) patient died of surgery-related causes. The tumor wrapping blood vessels (χ(2) = 4.676, P = 0.031) and violating cavernous sinus(χ(2) = 4.973, P = 0.026) were causes of unfavorable prognosis. During the mean follow-up of 35.1 months (range, 4-107 months) for 40 patients, the average Karnofsky score was 83.2. Tumor recurred in 4 cases: 2 of which required reoperation, and 2 had gamma knife radiosurgery.
CONCLUSIONSTumor wrapped vessels and invasion of the cavernous sinus are important unfavorable prognostic factors for ACM resection. By using appropriate surgical strategies and techniques, ACM can be removed safely via the pterion approach or lateral subfrontal approach with relatively low morbidity and mortality.

The technology of powder X-ray diffraction (XRD) was used for analysis Chloriti Lapis and the XRD Fourier fingerprints were established. The dates were analyzed by fuzzy cluster and fingerprint similarity evaluation software to compare the similarity of samples. XRD fingerprint with 10 common peaks of 14 batches of Chloriti Lapis were established. The average, median coefficients of crystal lattice spacing d (A), peak position 2 theta, relative intensity value I/I0 (%) were all more than 0.95. And similarity( angle cosine value) were all more than 0. 97. There were small number samples differed from others. And obvious differences between the pre-and post-processing samples. This paper shows the powder XRD Fourier fingerprint can be used for appraisal and study of the Chloriti Lapis.
