1.Prognostic correlation analysis of multiple myeloma based on HALP score of peripheral blood before chemotherapy

[Objective] To explore the predictive value of HALP score for prognosis in patients with multiple myeloma (MM). [Methods] A retrospective analysis was conducted on laboratory indicators and related clinical data of newly diagnosed multiple myeloma (NDMM) patients, treated at the Affiliated Hospital of North Sichuan Medical College from January 2016 to October 2023, prior to their first treatment. The HALP score was calculated, and the optimal cutoff value for HALP was determined using X-tile software. Survival analysis was performed using Kaplan-Meier curves for high HALP and low HALP groups. Univariate and multivariate analyses were conducted using the Cox regression model, and a forest plot was generated using Graphpad Prism to illustrate factors that may impact patient prognosis. The predictive ability of HALP score combined with β2-microglobulin and ECOG score for prognosis in MM patients was evaluated using receiver operating characteristic curve (ROC) analysis. [Results] A total of 203 MM patients were included, with the optimal cutoff value for HALP score being 29.15 (P<0.05). Among them, 101 patients were in the low HALP score group, and 102 patients were in the high HALP score group. The results of univariate and multivariate analysis using the Cox regression model showed that a HALP score <29.15 was an independent risk factor for progression-free survival (PFS) and overall survival (OS) (P<0.05). ROC curve analysis indicated that the combination of HALP score with β2-microglobulin and ECOG score had a higher predictive value for prognosis in MM patients compared to using HALP score alone. [Conclusion] The HALP score is closely related to the prognosis of patients with NDMM. A low HALP score indicates a poorer prognosis, while the combination of HALP score with β2-microglobulin and ECOG score provides a higher predictive value when assessed together.
3.A qualitative study of nurses ′ sense of pay equity in operating room of public hospitals

Objective:To discuss the pay equity of operating room nurses in public hospitals and provide reference for promoting the salary performance reform of operating rooms.Methods:Using purposive sampling method, 22 cases of operating room nurses from 8 public hospitals in Sichuan Province were selected for semi-structured in-depth interviews from December 2023 to May 2024, and the themes were summarized and distilled by Colaizzi phenomenological data analysis.Results:Twenty-two operationg room nurses were interviewed, 6 males and 16 females with 25-55 years old. The sense of pay equity of operating room nurses was refined into 3 themes and 8 subthemes: ① sense of distributional equity: different representative groups have different views on the basic salary of the position, demand attention to the difference between different subspecialty surgeries, explicit the value embodiment of professional ability, and support for learning and growth; ②sense of procedural equity: the pay policy reflects the interests of the majority of the nurses, and the right to have a say in and ask questions about the pay policy; ③sense of interactive equity: explain the rationale for the pay policy information, focusing on nurses′feelings and perceptions.Conclusions:The sense of fairness of nurses in the operating rooms of public hospitals is expressed in the results of the distribution of subspecialty surgical work, the process of salary decision-making, and the sense of fairness in the interaction between supervisors and subordinates. Nursing managers should pay attention to the reasonable distribution of the pay-earnings ratio of the salaries of nurses in the operating rooms, safeguard the rights of nurses in the process of salary distribution, and pay attention to the sense of the operating room nurses in the implementation of the process or in deciding the results in order to enhance their fairness psychology.

5.Clinical observation of venetoclax combined with homoharringtonine and cytarabine in the treatment of acute myeloid leukemia

OBJECTIVE To observe the short-term efficacy and safety of venetoclax combined with homoharringtonine and cytarabine in the treatment of acute myeloid leukemia (AML). METHODS The data of 40 newly diagnosed AML patients admitted to our hospital from October 2022 to November 2023 were retrospectively collected and divided into observation group and control group according to treatment plan, with 20 cases in each group. The patients in the control group were given Daunorubicin hydrochloride for injection+Cytarabine for injection, and the patients in the observation group were given Venetoclax tablets+ Homoharringtonine injection+Cytarabine for injection. The patients in both groups were given relevant medicine, with 28 days as one cycle. The short-term efficacy, negative rate of minimal residual disease (MRD), duration of granulocyte deficiency, duration of platelet (PLT) <20×109 L-1, transfusion volume of suspended red blood cells and platelet, and the occurrence of adverse drug reactions were evaluated in both groups after 1 cycle of induction chemotherapy. RESULTS The complete remission or complete remission with incomplete hematologic recovery (CR/CRi) rate in the observation group was significantly higher than control group (P<0.05), and the negative rate of MRD in the observation group was also significantly higher than control group (P<0.05). However, in low-, medium- and high-risk patients, there was no statistical significance in CR/CRi rates between the two groups (P>0.05). There were no significant differences in the duration of agranulocytosis, the duration of PLT <20×109 L-1, the amount of suspended red blood cell transfusion, the amount of platelet transfusion, the incidence of hematologic toxicity and the incidence of non-hematologic toxicity between 2 groups (P>0.05). CONCLUSIONS Venetoclax combined with homoharringtonine and cytarabine show good short-term efficacy and safety in the treatment of AML.
6.Influence of Positive Psychological Quality on Quality of Life of Rural Left-behind Elderly

Objective To explore the influence of positive psychological quality on the quality of life of rural left behind elderly. Meth-ods 356 rural left behind elderly in Bengbu and Huaibei cities of Anhui province were investigated with WHO Quality of Life-BREF (WHO-QOL-BREF), Chinese version of the Meaning in Life Questionnaire (C-MLQ), Purpose-in-Life Test (PIL) and Index of Well-Being (IWB). Results The scores of WHOQOL-BREF in physical, psychological, social relations and environment are significantly lower in the rural left behind elderly than in the norm in China (t>3.105, P<0.001). The scores of WHOQOL-BREF of the left behind elderly in rural areas signifi-cantly correlated with the sense of life meaning, life objective and subjective well being (P<0.05). The quality of life of left behind elderly in rural areas was analyzed by regression analysis, age, health status, education and other demographic variables and life goals, life sense, sense of autonomy, etc. entered the regression equation (P<0.05). The structure model analysis showed that the life meaning and purpose of life were the important factors that affected the quality of life of rural left behind old people, in which the subjective well-being played a meditational role. Conclusion The positive psychological quality of life meaning, life purpose and other positive psychological qualities sig-nificantly impact the quality of life of rural left behind elderly, and should carry out targeted mental health guidance.
7.Comparison of short-term efficacy and safety between amiodarone and esmolol treatment in elderly patients with acute myocardial infarction complicated with ventricular arrhythmia

Objective To compare the efficacy and safety of intravenous application of amiodarone or esmolol in elderly patients with ventricular arrhythmia following acute myocardial infarction (AMI) within 48 hours,and provide reference for safe use of medication in clinical practice.Methods Elderly hospitalized patients with severe ventricular arrhythmia following AMI in department of Cardiology in Shengjing Hospital of China Medical University from January 2009 to June 2013 were enrolled into the study.The patients were divided into amiodarone group and esmolol group.The patients in amiodarone group received intravenous injection of amiodarone 150 mg in 5% glucose solution 20 ml within 10 minutes,then an Ⅳ infusion of amiodarone 1 mg/min was given by syringe pump for 6 hours,then an Ⅳ infusion of amiodarone 0.5 mg/min was given for 24-48 hours.After finishing the intravenous application,the patients were given amiodarone orally 0.2 g thrice daily for 7 days.The patients in esmolol group received intravenous infusion of esmolol 0.5 mg/kg within 1 minute,then a venoclysis of esmolol 0.1 mg/(kg · min)was given by syringe pump for 6 hours,then a venoclysis of esmolol 0.05 mg/(kg · min) was given for 24-48 hours.After finishing the intravenous application,the patients were given metoprolol orally 25 mg,twice daily for 7 days.The patient who still had ventricular tachycardia after intravenous application of amiodarone or esmolol for 6 hours in amiodarone group or esmolol group should receive the above-mentioned esmolol or amiodarone treatment,respectively.The efficacy and safety within 48 hours between the 2 groups were compared.Results The effective rate in amiodarone group and esmolol group within 48 hours were 92.1% (35/38) and 75.0 (27/36),respectively (P =0.046).The incidence of adverse reactions in the 2 groups were 39.5% (15/38) and 19.4% (7/36),respectively (P =0.060).Two patients who had accompanied heart failure in the amiodarone group developed acute liver injury 24 hours after intravenous application.Two weeks later,their liver function returned to normal after drug withdrawal and symptomatic treatment.Conclusions The effective rate of intravenous application of amiodarone was higher than that of esmolol in treatment for elderly patients with ventricular arrhythmia following AMI.Amiodarone and esmolol are relatively safe.The liver function should be monitored in the elderly AMI patients when intravenous application of amiodarone were given.Once liver injury occurs,the drug should be discontinued immediately and the symptomatic treatment should be given.

9.Relationships between suicide attitudes and perception of life purpose and meaning of life in college students.

OBJECTIVETo investigate the suicide attitudes of college students and analyze the impact of the perception of life purpose and meaning of life on their suicide attitudes using structural equation modeling.
METHODSA total of 1050 college students were tested by Suicide Attitude Questionnaire (QSA), Purpose in Life test (PIL) and Chinese version of Meaning of Life questionnaire (C-MLQ). A theoretical model was established for confirming the influence of the purpose in life and meaning in life on suicide attitudes of the college students.
RESULTSThe college students had generally a negative attitude towards suicide. The female students tended to show more objective attitudes towards suicide and the students from rural areas had a stronger attitude against euthanasia.The meanings and purposes in life were closely correlated with the attitudes towards suicide, and the structural equation modeling well fitted the data (Χ(2)/df=1.924, GFI=0.936, AGFI=0.915, NFI=0.937, CFI=0.940, and RMSEA=0.045). The perception of meaning and purposes in life had a direct predictive value on the suicide attitude of the college students, and the presumptions derived from the theoretical model were strongly supported by structural equation modeling.
CONCLUSIONSThe perceptions in meanings in life and purposes in livelihood have important influence on the suicide attitudes of college students, and intervention with effective life education should be administered to guide the suicide attitude of the college students.
