Tuberculosis(TB),a chronic infectious disease caused by infection with the Mycobacterium tuberculosis complex(MTBC),has re-emerged as the leading cause of death from a single infectious agent worldwide.Because of widespread use and mis-use of anti-tuberculosis drugs, the emergence of multidrug-resistant TB(MDR-TB)and extensively drug-resistant TB(XDR-TB)is increasing, thus posing a serious threat to global health. The current problem of drug resistance is a major prevention and treatment challenge;therefore, the search for new drug targets is urgently needed. In recent years, substantial progress has been made in re-search on anti-tuberculosis drug targets and novel therapeutic strategies.Herein,we summarize recent research progress in anti-tuberculosis drug targets, primarily cell wall synthesis, nucleic acid replication and transcription, and energy metabolism. We also provide an overview of research progress regarding two novel therapeutic strategies,to provide a theoretical basis and research ideas for the development of new clinical drugs.

BACKGROUND:The asymmetrical biomechanical environment of adolescent idiopathic scoliosis can lead to further wedge deformation of the vertebral body, which may affect cardiopulmonary function and compress nerves in severe cases.Adolescent idiopathic scoliosis with different degrees of scoliosis should be treated with exercise, bracing, and surgery. However, the mechanical mechanism of selecting an orthopedic approach remains unclear due to the individual variability of patients.OBJECTIVE:To investigate the biomechanical mechanism of different orthopedic modalities for the treatment of adolescent idiopathic scoliosis to provide a basis for clinical selection of treatment modalities based on the spine model of adolescent idiopathic scoliosis patients.METHODS:Based on the CT images of an adolescent idiopathic scoliosis patient,a scoliosis model(C7-L5)was reconstructed in Mimics software in three dimensions, and lateral thrust force was applied at the T8/T9 thorax and vertical distraction force was applied over the C7 vertebra with the magnitude of 20,40,60,80,100,and 120 N.The intervertebral disc stress and vertebral displacement in concave and convex sides, and Cobb angle of the spine were analyzed under two orthopedic modalities.RESULTS AND CONCLUSION:(1)With lateral thrust, there was no significant change in the C7T1-T7T8 intervertebral disc. The concave and convex stress of T7T8-L4L5 segment decreased first and then increased with the increase of lateral thrust force. The correction effect of lateral thrust on the segment near T8T9 was obvious and weakened with the extension of the segment to the cephalic and caudal ends.At 120 N of lateral thrust, the thoracic Cobb angle changed from 53.2° to 32.5° and the lumbar Cobb angle changed from 50.2° to 43.9°.(2)With the vertical distraction, the thoracic intervertebral disc stresses first decreased and then increased, and all the lumbar disc stresses decreased. The C7 displacement was the most obvious, and the correction effect gradually diminished with the segment extended to the caudal end.At a vertical distraction force of 120 N,the thoracic Cobb angle changed from 53.2° to 39.4° and the lumbar Cobb angle changed from 50.2° to 47.6°.(3)It is concluded that both orthopedic modalities provide improvement in the degree of scoliosis, with the thoracic correction being greater than the lumbar correction. Also, the asymmetric stress distribution on the concave and convex sides is improved, which contributes to normal bone growth.A vertical distraction approach is appropriate for larger Cobb angles, and a lateral thrust approach is appropriate for smaller Cobb angles. The results of this study help to understand the mechanism of spinal orthosis and provide a theoretical basis for the choice of orthopedic approach.

AIM:To explore the clinical significance of stearoyl-CoA desaturase 1(SCD1)in colorectal can-cer and to evaluate the effects and mechanisms of diosgenin glucoside on the proliferation of colorectal cancer cells.METHODS:The expression of SCD1 in colorectal cancer tissues was assessed using qPCR and Western blot, and its cor-relation with clinical features was analyzed. The impact of diosgenin glucoside on the viability of SW620 and HCT116 cells was measured using the CCK-8 assay, and the proliferation was further investigated through cell cloning experiments.Ad-ditionally,the effects of diosgenin glucoside on apoptosis-related genes and SCD1 were evaluated by qPCR and Western blot.Molecular docking was employed to analyze the binding site and binding energy between diosgenin glucoside and SCD1.RESULTS:(1)Both SCD1 mRNA and protein were highly expressed in colorectal cancer tissues, with an 88%positive rate. And SCD1 mRNA expression correlated with cancer type and clinical stage.(2)Diosgenin glucoside de-creased the viability and cloning ability of SW620 and HCT116 cells in a dose-dependent manner.(3)Diosgenin gluco-side upregulated the mRNA and protein expression of pro-apoptotic proteins caspase-3 and Bax in a dose-dependent man-ner,while it downregulated the mRNA and protein expression of the anti-apoptotic protein Bcl-2.(4)Diosgenin glucoside reduced the mRNA and protein expression of SCD1 in a dose-dependent manner and bound to SCD1 via hydrogen and con-jugated bonds with a binding energy of-7.7 kcal/mol.CONCLUSION:(1)SCD1 is closely associated with the type and clinical stage of colorectal cancer.(2)The reduction in viability and proliferation of colorectal cancer cells by diosgenin glucoside may be attributed to its binding to SCD1,which lowers its expression and promotes apoptosis.

Objective:To compare the types and contents of trace elements in compound trace element preparations used in parenteral nutrition in Beijing against the doses recommended by the guidelines, so as to facilitate optimal choice of preparations for different populations in clinical practice. Methods:The available compound trace element preparations were determined according to the drug list from the Medical Insurance Recruitment and Procurement System of Beijing and the recommended doses of trace elements in the guidelines released by Chinese domestic and foreign nutrition societies were collected and compared. Results:The compound trace element preparations used in Beijing comprised 4 varieties and 6 specifications, with the daily cost ranging from 11.32 to 471 Chinese Yuan. There were 3 varieties of trace element preparations for adults, including multi-trace elements injection (Ⅱ), multi-trace elements injection (Ⅲ), and multi-trace elements injection. Among those, multi-trace elements injection (Ⅲ) met the guideline recommendations best and the price was moderate. For pediatric populations, multi-trace elements injections (Ⅰ) satisfied better the trace element requirements for preterm infants and infants from term to 3 months, while multi-trace elements injections (Ⅲ) outperformed in meeting the trace element requirements for 3-to-12-month infants and children. However, there were still some differences in the trace element contents between the two preparations and the guideline recommendations. Conclusion:The currently available varieties and specifications of trace element preparations basically meet the recommendations of evidence-based guidelines, but there is still the need to improve the instructions for and evidence on the pediatric application of certain preparations.
