Sustained inflammatory responses are closely related to various severe diseases,and inhibiting the excessive activation of inflammasomes and pyroptosis has significant implications for clinical treatment.Natural products have garnered considerable concern for the treatment of inflammation.Huanglian-Wumei decoction(HLWMD)is a classic prescription used for treating inflammatory diseases,but the necessity of their combination and the exact underlying anti-inflammatory mechanism have not yet been elucidated.Inspired by the supramolecular self-assembly strategy and natural drug compatibility theory,we successfully obtained berberine(BBR)-chlorogenic acid(CGA)supramolecular(BCS),which is an herbal pair from HLWMD.Using a series of characterization methods,we confirmed the self-assembly mechanism of BCS.BBR and CGA were self-assembled and stacked into amphiphilic spherical supra-molecules in a 2:1 molar ratio,driven by electrostatic interactions,hydrophobic interactions,and π-πstacking;the hydrophilic fragments of CGA were outside,and the hydrophobic fragments of BBR were inside.This stacking pattern significantly improved the anti-inflammatory performance of BCS compared with that of single free molecules.Compared with free molecules,BCS significantly attenuated the release of multiple inflammatory mediators and lipopolysaccharide(LPS)-induced pyroptosis.Its anti-inflammatory mechanism is closely related to the inhibition of intracellular nuclear factor-kappaB(NF-κB)p65 phosphorylation and the noncanonical pyroptosis signalling pathway mediated by caspase-11.

3.Value of bedside real-time ultrasound measurement of optic nerve sheath diameter combined with serum matrix metalloproteinase-9 and neutrophil-to-lymphocyte ratio for increased intracranial pressure after surgery in patients with severe craniocerebral injury

Objective To evaluate the clinical value of bedside ultrasonographic measurement of optic nerve sheath diameter(ONSD)combined with serum matrix metalloproteinase-9(MMP-9)and neutrophil-to-lymphocyte ratio(NLR)in assessing postoperative intracranial hypertension in patients with severe traumatic brain injury.Methods A total of 100 patients with severe traumatic brain injury were enrolled as study subjects.According to postoperative intracranial pressure status,the patients were divided into normal group(n=33)and elevated group(n=67).ONSD,MMP-9 and NLR lev-els were compared between the two groups.The correlations among ONSD,serum MMP-9 and NLR in the elevated group were analyzed.The influencing factors of postoperative intracranial hypertension in patients with severe traumatic brain injury were evaluated.Receiver operating characteristic(ROC)curve analysis was performed to assess the diagnostic value of ONSD combined with serum MMP-9 and NLR for postoperative intracranial hypertension.Results ONSD,serum MMP-9 and NLR levels were significantly higher in the elevated group than those in the normal group(P<0.05).ONSD was positively correlated with MMP-9(r=0.367,P=0.002),ONSD was positively correlated with NLR(r=0.419,P<0.01),and MMP-9 was positively correlated with NLR(r=0.324,P=0.007).Elevated ONSD,serum MMP-9 and NLR were risk factors for increased in-tracranial pressure after severe craniocerebral injury surgery(P<0.05).The area under the curve for evaluation of postoperative intracranial pressure increase in patients with severe craniocerebral in-jury by combination of ONSD,serum MMP-9 and NLR was 0.976(95%CI,0.952 to 1.000),the sensitivity was 91.04%,and the specificity was 93.94%.The combined evaluation value of ONSD,serum MMP-9 and NLR for increased intracranial pressure after surgery in patients with severe craniocerebral injury was higher(Zcombined with-ONSD=3.453,P=0.001,Zcombined with-MMP-9=3.637,P<0.001,Zcombined with-NLR=2.654,P=0.008).Conclusion In patients with increased intracra-nial pressure after severe craniocerebral injury surgery,their levels of ONSD,serum MMP-9 and NLR increase,and the increase of the three indicators will increase the risk of increased intracranial pressure after surgery.The combined detection of the three indicators has certain value in evaluating the increase of intracranial pressure in patients after surgery.
4.Exploration on the Effects of Ditan Yizhi Decoction Regulating Glucose Metabolism on Cognitive Impairment in Vascular Dementia Rats Based on ROS/Drp1 Axis

Objective To observe the effects of Ditan Yizhi Decoction on mitochondrial dynamics-mediated glucose metabolism in vascular dementia(VaD)rats based on the ROS/Drp1 axis;To explore its mechanism in treating VaD.Methods Ten male SD rats were randomly selected from 70 as the sham-operation group,and VaD models were prepared using the modified bilateral common carotid artery permanent ligation method for the remaining rats.The successfully modeled rats were randomly divided into model group,positive drug group(donepezil hydrochloride),inhibitor group(Mdivi-1)and Ditan Yizhi Decoction low-,medium-and high-dosage groups(12.86,25.725,51.45 g/kg),and intervened with corresponding method for 4 consecutive weeks.Morris water maze experiment was used to assess the learning memory ability of rats,HE and Nissl staining were used to observe the morphology of hippocampal tissue,transmission electron microscopy was used to observe the mitochondrial ultrastructure of hippocampal neurons,DHE fluorescent probe was used to detect the content of ROS in hippocampal neurons,Western blot was used to detect the expressions of Drp1,p-Drp1,Mfn2,Opa1,HK1,PKM2,GLUT1 and LDHA,the contents of serum IL-1β,IL-6 and TNF-α were detected by ELISA.Results Compared with the sham-operation group,rats in the model group had a prolonged escape latency(P<0.01)and a reduced number of crossing platforms(P<0.01);neuronal gaps in the CA1 region of the hippocampus were enlarged,with irregular cell morphology and blurred borders,neuronal consolidation,lysis and fragmentation of Nissl bodies and reduced number of Nissl bodies,swelling and deformation of mitochondria,disorganization of the cristae,and disruption of the bilayer membrane structure;the content of ROS in CA1 region of the hippocampus was elevated,the protein expressions of Mfn2 and Opa1 significantly decreased(P<0.01),the expressions of p-Drp1,HK1,PKM2,GLUT1,LDHA proteins significantly increased(P<0.01),and serum contents of IL-1β,IL-6 and TNF-α significantly increased(P<0.01).Compared with the model group,the escape latency was significantly shortened in Ditan Yizhi Decoction groups,positive drug group and inhibitor group(P<0.01),and the number of crossing platforms increased(P<0.05,P<0.01);the number of neurons in the hippocampal CA1 region increased,with normal morphology,orderly arrangement,abundant Nissl bodies,recovered mitochondrial morphology,and decreased rupture;the ROS content in hippocampal CA1 region decreased(P<0.01),while the expressions of Mfn2 and Opa1 proteins increased(P<0.01),the expressions of p-Drp1,HK1,PKM2,GLUT1 and LDHA proteins decreased(P<0.01),and the serum contents of IL-1β,IL-6 and TNF-α decreased(P<0.05).Conclusion Ditan Yizhi Decoction can improve cognitive impairment and neuronal morphology in VaD rats,and the mechanism maybe related to regulation of mitochondrial dynamics through the ROS/Drp1 axis,attenuating glycometabolic disorders,and reducing inflammatory response.

6.Role of platelets in the malignant progression of hepatocellular carcinoma

Hepatocellular carcinoma (HCC) is the most common primary hepatic malign-ancy in clinic. The prognosis of patients remains extremely poor because of the high malignancy and easy recurrence and metastasis of HCC. In recent years, the roles of platelets in promoting the malignant progression of HCC have increasingly attracted much attention. It is known that platelets could promote HCC cells proliferation and invasion through tumor microenvironment. On the other hand, platelets are capable to promote HCC cells distant metastasis by facilitating tumor cells evasion of immune surveillance. Besides, the platelet-derived growth factors and proangiogenic factors are also involved in the proliferation, invasiveness, and neovascularization of HCC. In addition, patients with HCC normally have a background of cirrhosis, and it is still controversial that whether the thrombocytopenia by portal hypertension and hypersplenism can promote the malignant progress of HCC. In view of this, the diagnostic and prognostic value of platelet levels, as well as platelet-associated scores in HCC have increasingly become research focus. The authors elaborate the detailed mechanisms of platelets in malignant progression of HCC, and discuss the recent research progress of platelets as effective diagnostic or prognostic tools for the assessment of HCC, which is of great importance to optimize the current treatment regimen and explore novel therapeutic strategies against HCC.
7.Effect and Mechanism of Lily Polysaccharide Combined with Astragalus Polysaccharide on Depressive Behavior in Chronic Stress Mice

ObjectiveTo explore the mechanism of antidepressant effect of lily polysaccharide (LLP)and astragalus polysaccharide(APS). MethodSixty KM mice were randomly divided into blank group, model group, fluoxetine hydrochloride (8 mg·kg-1)group, LLP (0.2 g·kg-1)group, APS (0.2 g·kg-1)group and polysaccharide combination (LLP+APS,0.1 g·kg-1+0.1 g·kg-1)group, with 10 mice in each group. Except the blank group, the other groups were given chronic unpredictable mild stress (CUMS) induced mouse depression model. On the 29th day of modeling,fluoxetine hydrochloride group was given corresponding dose of fluoxetine hydrochloride, and polysaccharide groups were given corresponding drug. The depressive behavior of mice was evaluated by behavioral indexes such as body mass change, open field test. The morphological changes of hippocampal CA1 neurons were observed by Nissl staining. The contents of 5-hydroxytryptamine (5-HT), adrenocorticotropic hormone (ACTH), and corticosterone (CORT), in brain tissue and plasma were measured by enzyme-linked immunosorbent assay (ELISA). Western blot was used to detect the expression levels of related proteins in adenylate cyclase/cyclic adenylate phosphate/protein kinase A (AC/cAMP/PKA) signal pathway. ResultCompared with the blank group, mice in the model group gained weight slowly, the total distance, central distance and sugar water preference rate decreased significantly (P<0.01), the depressive behavior was significant, the hippocampal neurons were seriously damaged, the content of 5-HT decreased (P<0.01), the contents of ACTH and CORT increased significantly (P<0.01), adenylate cyclase 6(ADCY6), PKA and cAMP response element binding protein-1 (CREB-1) and brain-derived neurotrophic factor (BDNF) protein expression decreased significantly (P<0.01). Compared with the model group, depressive behavior of mice in LLP group, APS group and LLP+APS group was significantly improved (P<0.01). The antidepressant effect of LLP+APS was better than that of LLP and APS. Each administration group could alleviate the damage of hippocampal neurons in varying degrees, significantly increase the content of 5-HT in brain tissue (P<0.01), and reduce the levels of ACTH and CORT in plasma (P<0.05). The protein levels of ADCY6, PKA, CREB-1 and BDNF were significantly increased (P<0.05). ConclusionThe antidepressant effect of LLP+APS is significantly enhanced and has a synergistic effect. The mechanism may be closely related to affecting the content of neurotransmitters, inhibiting HPA axis activity and activating AC/cAMP/PKA signal transduction pathway.
8.Predictors of efficacy of corticosteroid switching from abiraterone plus prednisone to dexamethasone in patients with metastatic castration-resistant prostate cancer.

Corticosteroid switching can reverse abiraterone resistance in some patients with metastatic castration-resistant prostate cancer (mCRPC). Here, we investigated the potential biomarkers for predicting the efficacy of corticosteroid switching during treatment with abiraterone acetate (AA). We retrospectively analyzed 101 mCRPC patients receiving corticosteroid switching from West China Hospital and Sun Yat-Sen University Cancer Center between January 2016 and December 2018. All cases received AA plus prednisone as first-line therapy during mCRPC. Primary end points were biochemical progression-free survival (bPFS) and overall survival (OS). The risk groups were defined based on multivariate analysis. A total of 42 (41.6%) and 25 (24.8%) patients achieved 30% and 50% decline in prostate-specific antigen (PSA), respectively, after corticosteroid switching. The median bPFS and median OS on AA plus dexamethasone were 4.9 (95% confidence interval [CI]: 3.7-6.0) months and 18.8 (95% CI: 16.2-30.2) months, respectively. Aldo-keto reductase family 1 member C3 (AKR1C3) expression (hazard ratio [HR]: 2.15, 95% Cl: 1.22-3.80, P = 0.008) and baseline serum alkaline phosphatase (ALP; HR: 4.95, 95% Cl: 2.40-10.19, P < 0.001) were independent predictors of efficacy before corticosteroid switching in the multivariate analysis of bPFS. Only baseline serum ALP >160 IU l-1 (HR: 3.41, 95% Cl: 1.57-7.38, P = 0.002) together with PSA level at switch ≥50 ng ml-1 (HR: 2.59, 95% Cl: 1.22-5.47, P = 0.013) independently predicted poorer OS. Based on the predictive factors in multivariate analysis, we developed two risk stratification tools to select candidates for corticosteroid switching. Detection of serum ALP level, PSA level, and tissue AKR1C3 expression in mCRPC patients could help make clinical decisions for corticosteroid switching.

9.Effects of Short-term High-salt Diet on Airway Inflammation in Mice with Allergic Asthma

ObjectiveTo explore the effects of high-salt diet on airway inflammation in mice with allergic asthma. MethodsTwenty 4-week-old female BALB/c mice were randomly divided into four groups, including control group, allergic asthma model group, 4% high-salt diet group and 8% high-salt diet group. Ovalbumin(OVA)was injected intraperitoneally and inhaled to make an asthmatic model. The control group and the model group were fed ordinary diet and drinking water, and the high-salt groups were fed 4% or 8% high-salt diet and 1% saline respectively. After 25 days, the nasal symptoms of each group of mice were observed at the last challenge. The mice were sacrificed 24 hours later. The inflammatory cells of different subtype in the blood were measured by five classification of blood cell analysis. Hematoxylin-eosin (HE) staining was used to observe the infiltration of eosinophils, and periodic acid-schiff (PAS) staining was used to observe the goblet cell proliferation in the lung tissue. Flow cytometry was used to detect the proportion of helper T cells 1(Th1 cells), Th2, Th17 cells and regulatory T cells (Treg cells) in the lung tissue, and the enzyme-linked immunosorbent assay was used to determine the levels of cytokines interleukin-4 (IL-4), IL-5, IL-13, and IL-17 of bronchoalveolar lavage fluid (BALF). ResultsCompared with the allergic asthma model group, 4% and 8% high-salt diet significantly reduced the proportion of eosinophils in blood, the proportion of Th17 cells in lung tissue, and IL-13 and IL-17 levels in BALF, but he effect of 8% high-salt diet on IL-13 was more significant than that of 4% high-salt diet (P<0.05). However, only 8% high-salt diet significantly relieved nasal symptom, improved eosinophil infiltration and goblet cell proliferation, reduced the proportion of Th2 cells and Th2/ Th1 ratio in lung tissue and the levels of IL-4 and IL-5 in BALF (P<0.05). ConclusionsShort-term high-salt diet can alleviate airway inflammation in mice with allergic asthma, and the effects of 8% high-salt diet are better than that of 4% high-salt diet.

However, only 8% high-salt diet significantly relieved nasal symptom, improved eosinophil infiltration and goblet cell proliferation, reduced the proportion of Th2 cells and Th2/ Th1 ratio in lung tissue and the levels of IL-4 and IL-5 in BALF (P<0.05). ConclusionsShort-term high-salt diet can alleviate airway inflammation in mice with allergic asthma, and the effects of 8% high-salt diet are better than that of 4% high-salt diet.
