1.Air Pollution and Cardiac Biomarkers in Heart Failure: A Scoping Review.

Ambient air pollution is increasingly being recognized as a risk factor for heart failure; however, its effects on cardiac biomarkers remain unclear. This scoping review assessed the existing evidence on the association between air pollution and cardiac biomarkers in heart failure, described the key concepts, synthesized data, and identified research gaps. Following the PRISMA-ScR guidelines, PubMed, Embase, Web of Science, and CNKI databases were searched for studies on air pollution, heart failure, and biomarkers. A total of 765 records were screened, and 81 full texts were assessed for eligibility, resulting in 15 studies. The results showed that the exposure to particulate matter was associated with elevated N-terminal pro-B-type natriuretic peptide and troponin levels. Several studies have linked particulate matter exposure to a higher cardiovascular risk and heart failure biomarkers. Inflammatory and oxidative stress markers were consistently elevated across studies, supporting the biological relevance of these associations. However, few studies have focused specifically on populations with heart failure or clinically relevant biomarkers, and the evidence for gaseous pollutants remains inconclusive. These findings highlight the need to integrate environmental risk assessment into heart failure care and inform policy efforts to reduce the pollution-related cardiovascular burden. Further research should address these gaps through improved exposure assessments and the integration of mechanistic evidence.

2.Transcatheter electrosurgery:an emerging interventional technology in cardiac disease

Transcatheter electrosurgery has emerged as an innovative interventional technique for treating structural heart disease in recent years.This technology utilizes radiofrequency energy delivered through catheter systems to perform puncture,cutting,or ablation of cardiovascular tissues,providing new treatment options for patients with complex structural heart disease.The technique has evolved from simple valve perforation to sophisticated valve modifications,developing various innovative procedures to address key clinical challenges in interventional treatment.The basic principle involves using radiofrequency energy to heat and vaporize target tissues while minimizing damage to surrounding structures.This review comprehensively discusses the fundamental principles,technical characteristics,clinical applications,and future prospects of transcatheter electrosurgery.Current applications include recanalization of occlusive lesions,intracardiac chamber access,and tissue modification for structural interventions.While showing promising results in various complex cases,the technology still faces challenges such as the learning curve and the need for specialized equipment.As the field continues to develop,transcatheter electrosurgery is expected to play an increasingly important role in structural heart interventions,though more long-term data is needed to fully evaluate its safety and efficacy.
3.Effect of Wechat reminder combined with 4R crisis management on rehabilitation of patients with chronic valvular heart disease

Objective:To explore the effect of Wechat reminder combined with 4R crisis management on cardiac function,psychological state,quality of life,inflammatory factors,compliance,adverse events and mortality in pa-tients with chronic valvular heart disease.Methods:This randomized controlled study enrolled 127 patients with chronic valvular heart disease admitted to Qingdao Municipal Hospital between May 2021 and April 2023.Patients were divided into control group(n=62)and intervention group(n=65).Patients in the control group was given routine management mode intervention comparing to those in the intervention group receiving Wechat reminder combined with 4R crisis management intervention.Both groups were treated for 3 months and followed up for 1 month.Cardiac function indexes,Symptom Checklist 90(SCL-90),China Questionnaire of Quality of life in pa-tients with Cardiovascular diseases(CQQC)scores,levels of brain natriuretic peptide(BNP),cardiac troponin I(cTnI)and inflammatory factors,intervention compliance rate,incidence rate of adverse events and mortality were compared between two groups.Results:Compared to patients in the control group,those in the intervention group had significant higher stroke volume(SV)[(66.48±2.74)ml vs.(58.02±3.32)ml],left ventricular ejection fraction(LVEF)[(56.25±2.66)%vs.(48.11±3.05)%],CQQC score[(135.37±2.26)points vs.(103.08±3.56)points](P<0.001 all),and significant lower SCL-90 score[(0.97±0.06)points vs.(2.06±0.07)points],BNP[(267.25±28.14)pg/ml vs.(317.63±28.63)pg/ml],cTnI[(69.15±5.81)ng/ml vs.(81.32±6.28)ng/ml],tumor necrosis factor α(TNF-α)[(13.38±0.87)pg/mlvs.(21.48±1.03)pg/ml],high sensitive C-reactive protein(hsCRP)[(15.73±2.19)mg/L vs.(36.76±3.08)mg/L]and platelet activating factor(PAF)[(7.51±0.73)μg/L vs.(10.53±0.91)μg/L](P<0.001 all).Compared to patients in the control group,those in the intervention group had significant higher total compliance rate(93.33%vs.80.00%,P=0.032),and significant lower incidence of adverse events(10.00%vs.25.00%,P=0.031).Conclusion:Wechat reminder combined with 4R crisis management could significantly improve cardiac function,compliance,mental state and quality of life,and reduce levels of inflammatory factors,BNP and cTnI with high safety in patients with chronic valvular heart disease.

6.A single-center analysis of pathogenic bacteria distribution and drug resistance in bacterial bloodstream infections among patients with hematological diseases

Objective:To analyze the distribution and drug resistance of pathogens of bacterial bloodstream infection in patients with hematological diseases in the Department of Hematology of Tianjin Medical University General Hospital, and to provide etiological data for clinical empirical anti-infection treatment.Methods:A retrospective analysis was conducted on the general clinical information, pathogenic bacteria and drug susceptibility test results of patients with hematological diseases diagnosed with bacterial bloodstream infection by menstrual blood culture in our center from January 2016 to December 2022.Results:Patients included 498 inpatients, with a total of 639 bacterial strains. Among the patients, 86.9% patients had malignancies, and 76.7% had agranulocytosis. Symptoms of concurrent infections, including those of the respiratory tract, oral mucosa, skin and soft tissues, and abdominal sources were observed in 68.3% patients. Gram-negative bacteria (G -) accounted for 79.0% of the isolated bacteria, and gram-positive bacteria (G +) accounted for 21.0%. The top five isolated pathogens were Klebsiella pneumoniae (22.5%), Escherichia coli (20.8%), Pseudomonas aeruginosa (15.0%), Enterococcus faecium (5.5%), and Stenotrophomonas maltophilum (5.0%). Escherichia coli exhibited a decreasing trend of resistance to quinolones, cephalosporins, and carbapenems. Klebsiella pneumoniae exhibited increasing rates of resistance to quinolones and cephalosporins between 2016 and 2018, but the rated decreased after 2019. The resistance rate to carbapenems exhibited by Pseudomonas aeruginosa was approximately 20%. Carbapenem-resistant strains of Pseudomonas aeruginosa strains were first detected in 2017, with a peak resistance rate of 35.7%, detected in 2019. A 60.0% resistance rate to methicillin was observed in methicillin-resistant coagulase-negative staphylococci (MRCNS), and one case of linezolid-resistant MRCNS was detected. Conclusions:Pathogenic bacteria of bacterial bloodstream infections were widely distributed in our center, and precautions are warranted against carbapenem resistant P. aeruginosa and Klebsiella pneumoniae.
7.Interpretation of a clinical practice guideline on the management of chronic pain associated with temporomandibular joint disorders

Temporomandibular disorders (TMD) are a heterogeneous group of diseases that affect the temporomandibular joint, chewing muscle system, dental occlusion, and even various structures throughout the body, with significant characteristics of biological-psychological-social pattern. TMD related chronic pain, as the most important clinical symptom, can result in negative emotions seriously affecting patients′ quality of life and physical and mental health. Although a variety of therapies have been previously reported to treat TMD related chronic pain, there is a lack of widely recognized therapies. Professor Jason W Busse (from Michael G DeGroote National Pain Centre, McMaster University, Hamilton ON, Canada) took the lead and collaborated with multiple internationally renowned schools/hospitals of stomatology to develop an international consensus on the management of chronic pain associated with TMD, a clinical practice guideline, which took two years and was published in December 15th, 2023 in a global top journal of clinical research The British Medical Journal. This clinical practice guideline explored the comparative effectiveness of available therapies for chronic pain associated with TMD, conditionally recommended the specific intervention for different treatment or pain relief, proposed a comprehensive, agreed, and standardized clinical practice guideline. This present article describes the methodology and key elements of the clinical practice guideline to help clinicians fully understand and appropriately apply this guidance, which could provide the references for clinical practice of TMD associated chronic pain in China.

Objective:This study aimed to investigate the role and clinical significance of MUC4 gene mutations in thrombotic events in patients with classic paroxysmal nocturnal hemoglobinuria (PNH) patients.Methods:A retrospective analysis was conducted on the clinical data and gene sequencing results of 45 patients with classic PNH admitted to the Department of Hematology, Tianjin Medical University General Hospital, from June 2018 to February 2022. MUC4 gene mutations in patients with classic PNH were summarized, and the risk factors for thrombotic events in these patients were analyzed. Additionally, the effects of MUC4 gene mutations on the cumulative incidence and survival of thrombotic events in patients with classic PNH were determined.Results:The detection rate of MUC4 gene mutations in patients with classic PNH who experienced thrombotic events (thrombotic group) was 68.8% (11/16), which was significantly higher than that in the non-thrombotic group [10.3% (3/29) ] ( P<0.001). All mutations occurred in exon 2. MUC4 mutation ( OR=20.815, P=0.010) was identified as an independent risk factor for thrombotic events in patients with classic PNH. The cumulative incidence of thrombotic events was 78.6% (11/14) in the MUC4 gene mutation group (mutation group) and 16.1% (5/31) in the non-mutation group, showing a statistically significant difference between the two groups ( P<0.001). Survival analysis showed a lower overall survival (OS) rate in the thrombotic group compared with that in the non-thrombotic group [ (34.4±25.2) % vs. (62.7±19.3) % ] ( P=0.045). The OS rate of patients was (41.7±29.9) % in the mutation group and (59.1±18.3) % in the non-mutation group ( P=0.487) . Conclusion:MUC4 gene mutations are associated with an increased incidence of thrombotic events in classic PNH patients, highlighting their role as independent risk factors for thrombosis in this population. These mutations can be considered a novel predictive factor that aids in evaluating the risk of thrombosis in patients with classic PNH.
10.Primary construction and application of National bank for HIV-1 standard strains

Objective:To construct standard strains representing the main epidemic clades of HIV-1 in China, amplify the virus strains, and establish a seed lot.Methods:Six isolates of HIV-1 virus were identified and analyzed in genotype and phenotype, according to " interpretation for the social organization of the Standard strains of pathogenic microorganism- technical specifications for establishment of HIV strains". The isolates were amplified and cultivated to generate the secondary generation primary seed lot and the third generation working seed lot as frozen storage in liquid nitrogen. Results:Six HIV-1 standard strains were obtained, of which 3 strains are CRF_ 07BC (NRPC2.4.9003, NRPC2.4.9005, NRPC2.4.9006), 1 strain is CRF_ 01AE (NRPC2.4.9001), 1 strain is CRF_ 08BC (NRPC2.4.9002), and 1 strain is URF (NRPC2.4.9004). Phenotypic detection showed that all six strains are CCR5 tropics and Non syncytia inducing virus. TCID 50 were all greater than 1 × 10 5/ml, and concentrations of p24 antigen were all higher than 2 ng/ml. A primary seed lot with no less than 20 vials per strain and a working seed lot with no less than 50 vials per strain were constructed. The standard virus strains were used in evaluating antiviral drugs PEG2kC34, PEG5kC34, LP-19, and neutralizing antibody LSEVh LS-F. Conclusions:Six standard strains of HIV-1 virus covering the three main epidemic subtypes of HIV-1 in China have been obtained, and a storage of HIV-1 standard strain was constructed. It meets the need of the preservation of HIV-1 standard strains in China and provides support for drug and vaccine evaluation.
