World Journal of Emergency Medicine 2026;17(1):7-14
BACKGROUND: To provide a comprehensive analysis of the landscape of artificial intelligence (AI) applications in cardiac arrest (CA).
METHODS: Comprehensive searches were conducted in PubMed, the Cochrane Library, Web of Science, and EMBASE from database inception through 10 June 2025. Studies that applied AI in both in-hospital cardiac arrest (IHCA) and out-of-hospital cardiac arrest (OHCA) populations across the following domains were included: prediction of cardiac arrest occurrence, prognostication of CA outcomes, applications of large language models (LLMs), and evaluation of cardiopulmonary resuscitation (CPR) and other AI-driven interventions related to CA.
RESULTS: The scoping review included 114 studies, encompassing data from 9,574,462 patients in total. AI was most commonly applied to the prediction of CA (overall, n=40; IHCA, n=30; OHCA, n=4; and both, n=6), CPR-related decision support during CA (n=16), and post-arrest prognosis and rehabilitation outcomes (overall, n=38; OHCA, n=21; IHCA, n=3; and both, n=14). Additional application areas included LLM-based applications (n=8), emergency call handling (n=4), wearable device-based detection (n=3), heart rhythm identification (n=2), education (n=2), and extracorporeal cardiopulmonary resuscitation (ECPR) candidate identification (n=1). Across all application scenarios, the highest area under the receiver operating characteristic curve (AUROC) value for pre-arrest CA prediction in IHCA patients was 0.998 using a multilayer perceptron (MLP) model, whereas the optimal AUROC for pre-arrest CA prediction in OHCA patients was 0.950 using extreme gradient boosting (XGBoost) or random forest (RF) models. For CPR-related decision support during CA, the highest AUROC achieved was 0.990 with a convolutional neural network (CNN) model. In prognostic prediction, the optimal AUROC for IHCA patients was 0.960 using XGBoost, while for OHCA patients it reached 0.976 using an MLP model.
CONCLUSION: This review shows that AI is most commonly used for the prediction of CA and CPR-related support, as well as post-arrest and rehabilitation outcomes. Future research directions include drug discovery, post-resuscitation management, neurorehabilitation, and clinical trial innovation. Further studies should prioritize multicenter clinical trials to evaluate AI models in real-world settings and validate their effectiveness across diverse patient populations. Overall, AI has significant potential to improve clinical practice, and its role in CA application is increasingly important.

Chronic heart failure (CHF) is a complex clinical syndrome caused by ventricular dysfunction, with mitochondrial energy metabolism disorder being a critical factor in disease progression. Heart-Yang deficiency syndrome, as the core pathogenesis of CHF, persists throughout the disease course. Insufficiency of heart-Yang leads to weakened warming and propelling functions, resulting in the accumulation of phlegm-fluid, blood stasis, and dampness. This eventually causes Qi stagnation with phlegm obstruction and blood stasis with water retention, forming a vicious cycle that exacerbates disease progression. According to the theory of reinforcing Yang, the clinical experience of the traditional Chinese medicine (TCM) master Tang Zuxuan in treating CHF with heart-Yang deficiency syndrome, and achievements from molecular biological studies, this study innovatively proposes an integrated research framework of "TCM syndrome differentiation and staging-mitochondrial metabolism mechanisms-intervention with Yang-reinforcing prescriptions" which is characterized by the integration of traditional Chinese and Western medicine. Heart-Yang deficiency syndrome is classified into mild (Stage Ⅰ-Ⅱ), severe (Stage Ⅲ), and critical (Stage Ⅳ) stages. The study elucidates the precise correlations between the pathogenesis of each stage and mitochondrial metabolism disorders from theoretical, pathophysiological, and therapeutic perspectives. The mild stage is characterized by impaired biogenesis and substrate-utilization imbalance, corresponding to heart-Yang deficiency and phlegm-fluid aggregation. Linggui Zhugantang and similar prescriptions can significantly improve the expression of peroxisome proliferator-activated receptor gamma co-activator-1α(PGC-1α)/silent information regulator 2 homolog 1 (SIRT1) and ATPase activity. The severe stage centers on oxidative stress and structural damage, reflecting Yang deficiency with water overflow and phlegm-blood stasis intermingling. At this stage, Zhenwu Tang and Qiangxin Tang can effectively mitigate oxidative stress damage, increase adenosine triphosphate (ATP) content, and repair mitochondrial structure. The critical stage arises from calcium overload and mitochondrial disintegration, leading to the collapse of Yin-Yang equilibrium. At this stage, Yang-restoring and crisis-resolving prescriptions such as Fuling Sini Tang and Qili Qiangxin capsules can inhibit abnormal opening of the mitochondrial permeability transition pore (MPTP), reduce cardiomyocyte apoptosis rate, and protect mitochondrial function. By summarizing the characteristics of mitochondrial energy metabolism disorders at different stages of CHF, this study explores the application of the theory of reinforcing Yang in treating heart-Yang deficiency syndrome and provides new insights for the clinical diagnosis and treatment of CHF.

ObjectiveTo investigate the mechanism by which Jianpi Xiao'ai prescription (JPXAP) inhibits colorectal cancer progression by regulating the inducible nitric oxide synthase-arginase 1 (iNOS-ARG1) metabolic axis and inducing mitochondrial reactive oxygen species (mito-ROS)-mediated mitochondrial structural and functional impairment. MethodsAn arginine metabolism disorder model of human colorectal cancer HCT116 cells was established by combined treatment with recombinant human interferon-γ (IFN-γ, 10 μg·L-1) and N(ω)-hydroxy-L-arginine (Nor-NOHA, 200 μmol·L-1) for 24 h, followed by intervention with 5%, 10%, or 20% JPXAP-containing serum. Cell proliferation was assessed using cell counting kit-8 (CCK-8), 5-ethynyl-2′-deoxyuridine (EdU) staining, and colony formation assays. Cell invasion and migration were evaluated using Transwell chamber and wound healing assays. Mitochondrial membrane potential (MMP) and ROS levels were assessed by JC-1 and MitoSOX staining, respectively. Mitochondrial ultrastructure was observed by transmission electron microscopy (TEM). The expression of iNOS, ARG1, and mitochondrial dynamics-related proteins, including mitofusin 2 (MFN2) and dynamin-related protein 1 (DRP1), was analyzed by Western blot and immunofluorescence. The levels of L-arginine, citrulline, and urea were determined by colorimetric methods and enzyme-linked immunosorbent assay (ELISA). ResultsCompared with the blank group, the model group exhibited significantly upregulated iNOS expression, downregulated ARG1 expression, a decreased ARG1/iNOS ratio, reduced L-arginine and urea levels, and increased citrulline levels (P<0.05). Meanwhile, mito-ROS accumulation was significantly increased, the JC-1 red/green fluorescence ratio was decreased, and mitochondria showed swelling and cristae disruption, indicating that metabolic disorder induced mitochondrial injury. Compared with the model group, all JPXAP-treated groups further decreased the ARG1/iNOS ratio, enhanced nitric oxide (NO) and reactive nitrogen species accumulation, further reduced L-arginine and urea levels, and increased citrulline levels (P<0.01). EdU-positive rate, colony formation rate, wound healing rate, and Transwell invasion number all decreased significantly with increasing serum concentration (P<0.01). Mito-ROS levels were further elevated, and the JC-1 red/green ratio further decreased. TEM revealed aggravated mitochondrial swelling and vacuolization. MFN2 expression was downregulated and DRP1 expression was upregulated (P<0.01),in a dose-dependent manner. ConclusionJPXAP further activates NO-mediated oxidative/nitrosative stress under arginine metabolism imbalance, inducing mito-ROS accumulation, MMP collapse, and mitochondrial dynamics imbalance, thereby inhibiting colorectal cancer cell proliferation and migration. These findings reveal an antitumor mechanism of JPXAP based on coordinated targeting of the "metabolism-mitochondria" axis.

Health economics evidence plays an important role in linking clinical value evidence with health resource allocation decisions in the development of clinical practice guidelines. It can not only effectively balance clinical effectiveness and economic feasibility but also avoid forming "idealized" recommendations that are detached from the affordability of the healthcare system or the burden-bearing capacity of patients. To promote guideline developers to use health economics evidence more standardizedly and fully, this paper conducts an in-depth analysis of the current application status, existing challenges, access channels, and application processes of health economics evidence in current guidelines, and on this basis, puts forward considerations and suggestions for strengthening and standardizing the application of health economics evidence in China's clinical practice guidelines.

Kv1.3,a voltage-gated potassium channel,is highly expressed in T lymphocytes, the nervous system, and vascular smooth muscle cells. It plays a critical role in membrane excitability and electrical signal transduction, serving as an important target for studying T-cell function and providing a promising direction for developing therapeutics against autoimmune and inflammatory diseases. Therefore, the de-velopment of specific inhibitors of Kv1.3 channel has emerged as a novel therapeutic strategy for these disorders. In this study,we isolated and purified a novel Kv1.3-inhibitory peptide toxin,LmKTx13,from the venom of the scorpion Lychas mucronatus using reversed-phase high-performance liquid chroma-tography(RP-HPLC).LmKTx13 consists of 38 amino acid residues, including six cysteines that form three disulfide bonds.Whole-cell patch-clamp recordings revealed that LmKTx13 potently inhibited Kv1.3 with an IC50 of 7.92±3.0 nmol/L.Selectivity analysis showed that 2 μmol/L LmKTx13 also in-hibited Kv1.2 and Kv1.7,but exhibited no significant effects on other potassium channel subtypes or voltage-gated sodium channels.Further investigation into the mechanism demonstrated that LmKTx13 acts as a pore-blocking inhibitor of Kv1.3.By analyzing the effects of LmKTx13 on Kv1.3 channel gating ki-netics and performing sequence alignment of the pore regions of Kv1.3 and Kv1.5,we constructed site-directed mutants and identified the pore region of Kv1.3 as the critical binding site for LmKTx13.Key residues involved in the interaction included T425,G427,and H451.In summary,we discovered a no-vel pore-blocking Kv1.3 inhibitor,LmKTx13,from L.mucronatus venom, which exhibits high affinity and selectivity for Kv1.3.These findings highlight its potential as a potential lead molecule for developing Kv1.3-targeted therapeutics.

Metastasis is a pivotal and intricate process in the progression of malignant tumors, strongly correlating with poor prognosis.Approximately 90%of cancer-related mortality is attributed to metastasis, with the five-year survival rate for patients with metastatic solid tumors ranging from 5%to 30%.Consequently,a comprehensive understanding of the underlying biological mechanisms driving metastasis is essential for unraveling its core processes and developing novel therapeutic strategies. The metastatic cascade involves tumor cells navigating numerous biological barriers, including detachment from the primary tumor, invasion of blood vessels or lymphatics, survival in circulation, extravasation into distant organs and subsequent adaptation to the microenvironment. To surmount these challenges, tumor cells undergo phenotypic changes, genetic mutations and dysregulating signaling pathways.Additionally, microenvironmental factors(such as angiogenesis, matrix remodeling and immune evasion)play a critical role, orchestrating the initiation and growth of metastatic lesions in an interdependent manner.Organ-specific metastasis,a distinct subset of metastasis, involves dynamic bidirectional interactions between tumor cells and the microenvironment of target organs. These interactions determine the selectivity of metastatic spread and drive the adaptive evolution of both the tumor and the organ, which encompasses multiple layers of cellular interactions, including cell-cell and cell-matrix signaling.Tumor cell mutations, the release of specific signaling molecules, the capacity to withstand circulatory pressures, and signaling exchanges with target organs collectively govern the selective nature of organ-specific metastasis.Furthermore, factors intrinsic to the target organ-such as its regenerative potential, metabolic profile, immune surveillance mechanisms and matrix stiffness-further facilitate the adaptive remodeling of metastatic cells within these environments. Thus, the bidirectional selection and adaptation between tumor cells and target organs form a dynamic, complex system that reshapes our understanding of metastatic tumor development. While current research emphasizes shared biological features in metastasis, the successful formation of metastatic tumors depends not only on these common mechanisms but also on the unique characteristics governing organ-specific metastasis. The interplay between generalizable and organ-specific mechanisms profoundly influences the metastatic outcome. This review aimed to consolidate our current knowledge of these shared and distinct processes, analyze the evolving understanding of the bidirectional selection between tumor cells and target organs, and assess the current status of metastatic risk prediction models for patients without metastasis.Furthermore, the paper discussed the challenges and opportunities in managing advanced-stage metastatic tumors, offering new insights and potential clinical strategies to improve prognosis and treatment outcomes.
