1.Validation of the Chinese version of the DSM-5 Social Anxiety Disorder Severity Scale in adults

Objective:To examine the validity and reliability of the DSM-5 Social Anxiety Disorder Severity Scale(SAD-D)in a Chinese adult population.Methods:The Chinese version of the DSM-5 Social Anxiety Disor-der Severity Scale was administered via online data collection platform Credamo to 300 adults(Sample 1,for item analysis,exploratory factor analysis and item selection of brief version of SAD-D)and 528 adults(Sample 2,for confirmatory factor analysis,criterion validity test,measurement invariance analysis and internal consistency reliabil-ity analysis for both SAD-D and its brief version).Criterion validity was tested with the Social Phobia Scale(SPIN)and Personal Report of Confidence as a Speaker(PRCS).A brief version of the scale was developed by u-sing the Ant Colony Optimization(ACO).A retest was conducted with 152 participants from Sample 2 after three weeks.Results:Exploratory factor analysis indicated that the SAD-D was a unidimensional scale with factor load-ings ranging from 0.49 to 0.82,and the results of the confirmatory factor analysis also supported the unidimension-al structure(x2/df=3.49,RMSEA=0.069,CFI=0.971,TLI=0.962,SRMR=0.028).The scores of Chinese version of the SAD-D were positively correlated with the SPIN scores(ICC=0.70,P<0.001)and PRCS scores(ICC=0.73,P<0.001).The Cronbach'α of the scale was 0.92,and the retest reliability was 0.85.The scale dem-onstrated cross-gender measurement invariance(△CFI<0.01,△RMSEA<0.01).The brief version of the SAD-D was selected as items 2,5,and 6,and its Cronbach'α coefficient was 0.86.Conclusion:The Chinese version of the SAD-D has satisfactoryvalidity andreliability,making it suitable for the assessment of social anxiety symptoms with Chinese adults.

3.Barriers and facilitators to implementing evidence based mental health practice in primary schools

Objective:
To explore the facilitators and barriers of the implementation of evidence based mental health practice, in order to provide practical experience for promoting the development of evidence based mental health services in primary schools in China.
Methods:
Semi structured interviews were conducted with 4 education bureau managers, 8 school administrators, 7 classroom teachers, and 7 treatment providers after providing evidence based practice in 10 primary schools in Henan Province, China. Data was analyzed using thematic analysis.
Results:
Evidence based practice in primary schools faced multiple factors at the macro level, school level, and individual level. A total of 8 facilitators and 9 barriers were extracted. Among these factors, some factors were particularly striking. These included the "exclusion" of teacher title evaluation system, time conflict between practice and school schedule, stigmatization of mental health and mismatch between perceived effectiveness of services and expectations.
Conclusion
Evidence based mental health practice is feasible in Chinese schools. The implementation process needs to take full account of macro, school and individual multi level factors to move evidence based mental health services from theory and data to practice in China.
4.Evaluation of characteristics of carotid plaques and immediate outcomes after carotid artery stenting in diabetic and non-diabetic patients by optical coherence tomography

Objective:To evaluate the characteristics of carotid plaque and the immediate outcomes after carotid artery stenting (CAS) in diabetic and non-diabetic patients by optical coherence tomography (OCT).Methods:Patients underwent CAS and OCT before and after operation in the Department of Neurology, Jinling Hospital from January 2014 to March 2019 were enrolled retrospectively. The clinical features, the characteristics of carotid plaque on OCT and the immediate outcomes after CAS were compared between diabetic group and non-diabetic group. The risk factors of stent malapposition were analyzed.Results:A total of 46 patients were enrolled. Their age was 64.02±8.32 years and 41 were males (89.1%). There were 20 patients (43.5%) in the diabetes group and 26 (56.5%) in the non-diabetes group. The proportions of atherosclerotic plaque with thin fibrous cap (40.0% vs. 7.7%; χ2=5.166, P=0.023), plaque rupture (55.0% vs. 23.1%; χ2=4.945, P=0.026) and macrophage infiltration (60.0% vs. 30.8%; χ2=3.930, P=0.047) in the diabetic group were significantly higher than those in the non-diabetic group. Multivariate logistic regression analysis showed that older age (odds ratio [ OR] 1.208, 95% confidence interval [ CI] 1.033-1.413; P=0.018), coronary heart disease ( OR 15.953, 95% CI 1.142-222.952; P=0.040), alcohol consumption ( OR 6.192, 95% CI 1.098-34.923; P=0.039) and lower systolic blood pressure ( OR 0.944, 95% CI 0.894-0.997; P=0.037) were independently associated with stent malaposition. Conclusion:Compared with the non-diabetic patients, carotid plaque in diabetic patients may be more unstable. Older age, coronary heart disease, alcohol consumption and lower systolic blood pressure were associated with stent malaposition after carotid stenting. OCT can reveal the characteristics of carotid plaque and the immediate outcomes after CAS, which can provide strong evidence for treatment decision.
5.Effectiveness and safety of holmium laser therapy via flexible bronchoscopy in 40 children with tracheobronchial tuberculosis

Objective:To explore the effectiveness and safety of holmium laser treatment via flexible bronchoscopy in children with tracheobronchial tuberculosis (TBTB).Methods:The clinical data of 40 children with TBTB admitted in Qilu Children′s Hospital of Shandong University from February 2016 to June 2019 were retrospectively analyzed. According to the treatment approach, they were divided into conventional treatment group (20 cases) and holmium laser treatment group (20 cases). The sex, age, course of disease and lesion location before treatment, and the time till relief of atelectasis and airway obstruction after treatment were compared between the two groups with t test and χ 2 test. Results:Among the 40 cases, 24 were males and 16 females, and 35 had lymph node fistula (87.5%) and 5 had granulation proliferation (12.5%). There was no statistically significamt difference between the conventional group and laser group in sex (male ratio: 50%(10/20) vs. 75%(15/20), χ 2=2.66, P=0.10), age ((3.2±2.2) years vs. (2.2±1.8) years, t=1.41, P=0.16), course of disease (<1 month ratio: 30%(6/20) vs. 35%(7/20), χ 2=0.11, P=0.73), lesion location (single lesion ratio: 45%(9/20) vs. 60%(12/20), χ 2=0.90, P=0.34). The effectiveness was evaluated after 3 months, and the total effective rate were 100% in laser group and 40% in conventional group, the difference was significant (χ2=25.34, P<0.01). No serious adverse events occurred in the two groups during the observational period. Conclusions:Holmium laser treatment via bronchoscopy is an effective way to remove the tuberculosis foci in the trachea, which can reduce the incidence of further related complications, and could be applied in clinical approach.
6.The application of three-dimensional computed tomography angiography during thoracoscopic complex pulmonary segmentectomy

Objective Identification of anatomic structures are essential for totally thoracoscopic anatomic pulmonary segmentectomies, however sometimes the procedure are difficulty.This study was to assess whether three-dimensional computed tomography angiography(3D-CTA) could contribute to the preoperative arrangement of thoracoscopic complex segmentectomy.Methods Between September 2012 and August 2014, 29 patients were performed thoracoscopic complex segmentectomies under the guidance of preoperative 3D-CTA.The segmentectomies pattern were based on the nodules' diameter, location,and pathology.The targeted vessels and bronchus were marked in preoperative simulated segmentectomies.Results Of the 29 cases, 9 right upper lobe segmentectomies, 13 left upper segmentectomies, and 7 bibasilar segmentectomies were achieved, among which 6 subsegmentectomies were also inclued.The mean lesion diameter, operative time and intraoperative blood loss were(1.35 ± 0.80) cm, (190.53 ± 50.83) min, and (26.90 ± 32.24) ml respectively.Under the guidance of preoperative 3D-CTA , 8(27.5%) nodules were detected accurately, moreover 2(6.9%) aberrant arteries and 1 (3.4%) aberrant bronchus were observed.According to the marked vessels and bronchus preoperatively, 27 (93.1%) arteries, 25 (86.2%)veins,and 29(100%) bronchus were identified and dissected in the operation.Three cases converted to unplanned segmentectomies.No serious complications or death occurred.Conclusion 3D-CTA is an effective tool to enhance security and efficiency in thoracoscopic complex anatomical segmentectomy.
7.Relationship between reconstruction of the clivo-axial angle and curative effect in posterior approach surgery for the treatment of upper cervical spine anomaly instability

Objective The aim of this study was to describe the clinical outcomes and improvement of clivo-axial angle (CAA) during the posterior approach surgery of upper cervical anomaly instability patients.Methods All of 32 patients with symptomatic upper cervical anomaly instability were followed up from 2004 Apr.to 2014 Aug.in which 1 patient was excluded because of reoperation.There were 10 patients with OS odontiodeum,5 with odontoid deformity,4 with Klipple-Feil syndrome,4 with atlantoaxial dislocation,4 with atlantoaxial instability,1 with rheumatoid arthritis,1 with old odontoid fracture,1 with OS odontoideum and foramen magnum stenosis,1 with basilarinvagination and Klipple-Feil syndrome.They were all performed posterior reduction internal fixation and fusion,and then released spinal cord compression with the promotion of CAA.The patients were divided into two groups on the basis of decompression or not:non-decompression group 17 cases (male 8 cases,female 9 cases;age 13-65 years old,average 50.1 years old) and decompression group 14 cases (male 7 cases,female 7 cases;age 19-68 years old,average 49.6 years old).The CAA numbers were measured and compared.The clinic date was compared between preoperatively and postoperatively,which included Neck Disability Index and Nurick Classification System for Myelopathy.Results All patients were followed up,the follow-up time of non-decompression group was 0.7-10.7 years,average 2.9 years;the follow-up time of compression group was 0.3-5.5 years,average 2.9 years.Analysis of CAA,NDI and Nurick score were performed with t-test.The postoperative CAA,NDI and Nurick score improved significantly compared to preoperative ones.There was no difference between non-decompression group and decompression group.One patient felt nausea and vomited,one got soleal vein thrombosis,one had a swelling face and blurring vision and one had decompression syndrome after operation,all of these were improved using respectively therapy.No such serious complications as injury to vertebral artery,infection or internal fixation loosening occurred.Conclusion Neck ability and spinal cord function was significantly improved through increasing CAA for the upper cervical anomaly instability patients underwent posterior reduction internal fixation and fusion.
8.Effect of Early Rehabilitation Combined with Electrotherapeutics on Motor Function of Patients with Acute Stroke

ObjectiveTo investigate the effect of early rehabilitation combined with electrotherapeutics on motor function of patients with acute stroke.Methods90 patients with acute stroke were randomly divided into the treatment group and control group with 45 cases in each group. All patients of two groups were treated by routine medication, the cases of the treatment group were added with physical therapy and electrotherapeutics. The motor function of all patients was assessed before and one month after treatment.ResultsAfter treatment, the scores of Fugl-Meyer Assessment, Modified Barthel Index and NFI of all patients in the two groups improved obviously, but the effect of the treatment group was superior to that of the control group ( P<0.05).ConclusionEarly rehabilitation combined with electrotherapeutics can improve the motor function of patients with acute stroke.
9.Active components in the extracts of Radix Salvia miltiorrhizae by supercritical carbon dioxide fluid

Objective To investigate the ex tr action technique for seperating the active components in the root of Salvia mi ltiorrhizae bunge by supercritical fluid, and to analyze the extracted product s by HPLC-MS n . Methods The extraction condition s were established as follows: 950ml?L -1ethanol as the first entrainer, t he pressure of 20.0 MPa, temperature at 45 ℃, and extracting time 1 h; then 100 mL?L -1 ethanol was selected as the second entrainer, pressur e was 30.0 MPa, temperature was 65 ℃, and extracting time was 3 h. Results Compared with traditional refluxing extraction and ultrasonic extraction, supercritical fluid extraction was better and more effect ive. Conclusion Supercritical extraction is simple, highly selec tive and efficient in extracting the active components in Salvia miltiorrhizae bunge.
10.Evaluation of Studies on Shuang Huanglian Injection for Acute Respiratory Infections

[Objective] Systematic review was carried out in quality analysis of literature of Shuang Huanglian Injection (SHI) for acute respiratoiy infections ( ARI) . [Methods] Database was set up according to the principles of clinical epidemiology and evidence-based medicine and the evaluation scale for ARI. The data was managed by descriptive analysis and uniformity test. [Results] In 49 clinical reports, 42.16% adopted the randomized and controlled method but the quality was not so good; the criteria of inclusion and exclusion was absent or not standardized in most of the reports; blinded method was adopted scarcely; drop-out and missed cases were rarely mentioned while adverse effects were reported usually. [Conclusion] The design of clinical study of SHI for ARI has been improved in recent years but there is still a lot of inadequany.
