1.Elevated advanced glycation endproducts is a risk factor for stenosis after primary arteriovenous fistula surgery.

OBJECTIVES:
To investigate the effect of serum advanced glycation endproducts (AGEs) on stenosis after first autologous arteriovenous fistula (AVF) in patients with end-stage renal disease (ESRD).
METHODS:
Patients with ESRD undergoing standard native arteriovenous fistula (AVF) for the first time in the Department of Nephrology, Affiliated Hospital of Guilin Medical University from February to June 2022 were prospectively enrolled. The preoperative general data, clinical examination results and ultrasound data of the operated limbs were collected. The patients with and without stenosis within 2 months after the operation were compared for preoperative serum AGEs levels detected using ELISA and the clinical parameters. Logistic regression analysis was used to analyze the independent risk factors of AVF stenosis, and the sensitivity and specificity of AGEs for predicting postoperative stenosis were analyzed using receiver-operating characteristic (ROC) curve.
RESULTS:
Of the 94 patients enrolled, 34 had postoperative arteriovenous stenosis and 60 had no stenosis. The number of diabetic patients differed significantly between stenosis group and non-stenosis group (P<0.001). Serum AGEs levels, which were negatively correlated with serum phosphorus level (P<0.05), were significantly higher in stenosis group than in non-stenosis group (Z=-2.837, P=0.005). Serum AGE level was an independent risk factor for postoperative stenosis after AVF (OR=1.251, 95% CI:1.096-1.423, P<0.001). For predicting AVF stenosis, the area under the ROC curve (AUC) of AGEs was 0.677 (P=0.007, 95% CI: 0.572-0.770), with a specificity of 90.00% and a sensitivity of 52.94% at the optimal cut-off value of 8.43 µg/mL; AGEs combined with fibrinogen had an AUC of 0.763 (P<0.001, 95% CI: 0.664-0.844), with a specificity of 73.33% and a sensitivity of 70.59% at the optimal cut-off value of 0.30.
CONCLUSIONS
Elevated serum AGEs level is an independent risk factor for postoperative AVF stenosis, and its combination with fibrinogen has a better efficacy for predicting postoperative AVF stenosis.

2.Current status of cleaning and disinfection of digestive endoscopes in medi-cal institutions in Suzhou City

cleaning and disinfection.Results All 28 surveyed medical institutions had separate endoscope disinfection rooms, 89.29% of which had integrated endoscopic cleaning station,17.86% had automatic endoscope washer/disinfector;100% used multi-enzymatic detergent,chose the right disinfectant,monitored disinfectant concentration every day, and implemented standard disinfection time.But only 39.29% changed multi-enzymatic detergent for each endo-scope,cleaning and disinfection personnel in 78.57% of medical institutions wore personal protective equipment correctly.77 digestive endoscopes were detected,the qualified rate was 88.31%.Conclusion Cleaning and disin-fection management of digestive endoscope in secondary and above medical institutions in Suzhou City is generally standardized,there are still some problems in the manipulation procedures,relevant national regulations should be strictly complied with,efficacy of cleaning and disinfection of digestive endoscope should be further improved.
3.Relevant influential factors for cleaning quality of dental handpieces

Objective To understand the cleaning quality of dental handpieces in Suzhou City, analyze the relevant factors that influencing cleaning effect.Methods A cross-sectional study was performed with the proportional system sampling method, questionnaires were adopted to investigate the cleaning location, cleaning method and process of dental handpieces, the ATP fluorescence detection method was conducted to detect cleaning quality.Results In 10 administrative regions of this city, a total of 72 medical institutions were selected, 25 were public medical oral diagnosis and treatment institutions, 47 were private clinics.Cleaning effect of automatic handpiece cleaning machine was better than traditional manual cleaning (unqualified rate :3.95% vs 11.96%, P<0.05), unqualified rate of handpieces cleaned by cleaning personnel without inadequate knowledge was higher than that by personnel with adequate knowledge(14.88% vs 3.57%, P<0.05).Qualified rate of cleaning: different cleaning locations ranged from 5.00% to 11.23%, cleaning equipment was inadequate and sufficient 11.89% and 7.29% respectively, cleaning personnel were not designated and designated 12.16% and 9.83% respectively, but the difference were not statistically significant (all P>0.05).The quality of cleaning of handpieces could be improved if waiting time of cleaning ≤30 minutes, enzymes were used during cleaning, and purified water was used at the end rinse(all P<0.05);whether there was drying process and used lubricant, difference were both not significant.Conclusion Using automatic handpiece cleaning machine, cleaning personnel with adequate knowledge, cleaning waiting time ≤30 minutes, enzyme use during the cleaning process, and purified water use at the end rinse can improve the quality of cleaning of dental handpieces.
4.Cross-sectional survey and analysis of cleaning quality of dental handpieces in Suzhou City

Objective To explore the cleaning status and cleaning quality of dental handpieces in various types of medical institutions in Suzhou City.Methods On October 26-31, 2015, dental clinics in the whole city were sampled according to cross-sectional survey and proportional sampling method, the cleaning quality of dental handpieces in each clinic was detected by ATP bioluminescence assay.Results 72 medical institutions, 201 handpieces, 402 samples in 10 administrative regions of the city were sampled, 42 samples was unqualified, unqualified rate was 10.45%, unqualified rate of cleaning of dental handpiece surface was higher than waterline of dental handpiece(17.91% vs 2.99%, P<0.05).Cleaning quality of dental handpieces in different grades of medical institutions was different(P<0.05), tertiary medical institutions were all ualified, medical institutions without grade was 14.45%.According to the classification based on name of different medical institutions, cleaning quality of handpieces was statistically significant(P<0.05), cleaning efficacy of dental handpieces in department of stomatology of public hospitals was best(unqualified rate was 4.31%), while private dental clinics had the worst cleaning efficacy(unqualified rate was 13.81%).Conclusion Education and training of dental handpieces cleaning in the whole city should be strengthened, especially the management of cleaning of dental handpieces in low grade and private dental clinics.
5.Diagnostic value of echocardiography in univentricle

Objective To explore the value of transthoracic echocardiography in diagnosis of univentricle and analyze the sonogram typing. Methods The results of 66 patients with univentricle were reviewed retrospectively,and analayzed their typing connected with the reports in the literature. Results There were 3 ultrasonic types in 66 cases:①Type A(single left ventricle) 19 cases,single ventricle with left ventricular shape,residual cavity in front of it. ②Type B(single right ventricle) 38 cases, single ventricle with right ventricular form,and residual cavity in the rear.③Type C (solitary single-ventricle) 9 cases,there was only one ventricle. Thirty-one of them were treated surgically, 5 cases without operation had MRI or cardiac catheterization examination and the remaining 26 patients were only observed by echocardiography,the positive rate of diagnosis in type was 100%, the results were compared with cardiac catheterization or MRI examination and the operation: 1 cases of mixed type total anomalous pulmonary venous connection was misdiagnosed as heart-type total anomalous pulmonary venous drainage. But 1 case of descending aorta limitations narrow complicated patent ductus arteriosus(PDA), PDA was missed. The rest were completely correct diagnosis. Conclusions The transthoracic echocardiography can be used to evaluate types and all containing malformations of univentricle,and offers reliable information for operation.
6.Value of two-dimensional speckle tracking imaging in assessing myocardial injury in various degree of rats

Objective To investigate the value of left ventricular global two-dimensional strain and strain rate index measured by two-dimensional speckle tracking imaging (2D-STI) in assessing myocardial injury in various degree of rats following acute myocardial infarction. Methods Fifty-five Wistar rats were randomly divided into myocardial infarction(MI) group ( n =45) and sham-operation(SO) group ( n = 10).To establish rats acute myocardial infarction model with different infarct extent, MI group were randomly divided into MI15 group,MI30 group and ML60 group( n = 15,respectively) which underwent occlusion of left anterior descending coronary artery for 15 minutes, 30 minutes and 60 minutes respectively. Echocardiography was performed at baseline and 24 hours after reperfusion. High frame rate twodimensional images were recorded from the left ventricular short-axis views at the papillary muscle level.Left ventricular global circumferential strain(GSc) and strain rate(GSRc) were measured using EchoPAC work station. Left ventricular internal diameter at diastole (LVIDd) and systole ( LVIDs), fractional shortening(FS) and ejection fraction(EF) were measured by anatomical M-model echocardiography. Area of necrosis(AN) of each segment was measured after triphenyl tetrazolium chloride(TTC) staining. Results ① Compared with baseline and SO group, LVIDd and LVIDs of MI15, MI30 and MI60 group significantly increased respectively,whereas FS and EF significantly decreased( P <0. 05). Compared with MI15 group and MI30 group, LVIDd and LVIDs of MI60 group significantly increased, whereas FS and EF significantly decreased(P <0. 05). ② Compared with baseline and SO group,GSc and GSRc of MI15 group, MI30 group and MI60 group significantly decreased. GSc and GSRc of MI group decreased with ischemia duration ( P <0.05). ③ GSc and GSRc significantly correlated with AN respectively ( P <0. 01) while the correlation coefficient was 0. 90 and 0. 88 respectively, and GSc and GSRc were significantly predictors of AN( P <0.01) while the Beta was 0.558 and 0.491 respectively.④AN increased with ischemia duration( P <0.05). Conclusions Left ventricular global circumferential strain and strain rate index measured by 2D-STI,which decreased significantly as the area of necrosis increased, can accurately assess myocardial injury after myocardial infarction in various degree.
7.Characteristics and Drug Resistance of Staphylococcus spp Nosocomial Infection in Suzhou Area 2004-2007

OBJECTIVE To investigate the infection and drug resistance of Staphylococcus spp from hospitalized cases in Suzhou area.METHODS The data from hospitalized cases of 32 hospitals in Suzhou(from 2004 to 2007) were analyzed retrospectively.RESULTS From 2004 to 2007,17 668 cases of nosocomial infection were collected from 32 hospitals in Suzhou area.The infection rate of Staphylococcus aureus was 5.78%,7.11%,8.39% and 7.50%,respectively;the number of meticillin-resistant S.aureus(MRSA) infection cases was 66(34.74%),107(33.86%),138(37.00%) and 219(53.16%) respectively and the total number was 530(41.05%).The nosocomial infection caused by S.epidermidis accounted for 5.99%,5.47%,5.35% and 5.25%,respectively from 2004 to 2007.The number of meticillin-resistant S.epidermidis(MRSE) infection cases were 118(59.90%),128(52.67%),119(50.00%) and 134(46.53%) and the total number was 499(51.66%).The main infection site of S.aureus and S.epidermidis was respiratory tract(74.28% and 71.81%,respectively).Antibiotic resistance strains of S.aureus and S.epidermidis was on rising,and most of them were multi-drug resistance strains.All of the strains were sensitive to vancomycin.CONCLUSIONS In Suzhou area,nosocomial infection and drug resistance of Staphylococcus is on the rise.Evevy hospital must take effective measures to control nosocomial infections of Staphylococcus and drug resistance.
