1.Electroacupuncture Modulates Alveolar Macrophage Polarization via the Vagus Nerve to Alleviate Pulmonary Inflammation in a Mouse Model of Chronic Obstructive Pulmonary Disease

OBJECTIVE To investigate the anti-inflammatory effect of electroacupuncture in mice with chronic obstructive pul-monary disease(COPD)and its underlying mechanisms.METHODS Forty C57BL/6 mice were randomly divided into normal group,model group,electroacupuncture(EA)group,vagotomy group,and vagotomy+EA group,with 8 mice in each group.Except for the normal group,all groups were exposed to cigarette smoke for 12 weeks to establish a COPD model.After model establishment,the vagotomy group and the vagotomy+EA group underwent left cervical vagotomy before EA.EA treatment was performed at the Feishu(BL13)and Zusanli(ST36)acupoints once daily for 20 minutes for a total of 14 days.After EA,the pulmonary ventilation function of the mice was detected by a pulmonary function analysis system;lung tissue pathology was observed by HE staining;the levels of inter-leukin(IL)-6,tumor necrosis factor-α(TNF-α),IL-10,and transforming growth factor-β(TGF-β)in lung tissue were detected by ELISA;the expression of CD86 and CD206 proteins in lung tissue was detected by Western blot;the distribution of F4/80+CD86+(M1 type)and F4/80+CD206+(M2 type)cells in lung tissue was determined by flow cytometry;the expression of CD86 and CD206 in lung tissue was observed by immunofluorescence.RESULTS Compared with the normal group,the model group showed significantly decreased lung function(P<0.01),obvious lung pathological damage,increased M1 proportion,IL-6,TNF-α,CD86 content and expression(P<0.05,P<0.01),and decreased M2 proportion,IL-10,TGF-β,CD206 content and expression(P<0.01).Compared with the model group,the EA group showed varying degrees of improvement in lung function and pathology;the M1 proportion,IL-6,TNF-α,and CD86 were reduced(P<0.05,P<0.01),while the M2 proportion,IL-10,TGF-β,and CD206 were increased(P<0.05,P<0.01).The vagotomy group showed worsened lung function and pathology,increased IL-6,TNF-α,and CD86 content and expression(P<0.05,P<0.01),and decreased IL-10,TGF-β,and CD206 content and expression(P<0.05,P<0.01).Compared with the EA group,the vagotomy+EA group showed increased M1 proportion,IL-6,TNF-α,and CD86 content and expression(P<0.01),and decreased M2 proportion,IL-10,TGF-β,and CD206 content and expression(P<0.05,P<0.01).CONCLUSION EA at Feishu(BL13)and Zusanli(ST36)acupoints can improve lung function and pulmonary inflammation in COPD mice,promoting the polarization of alveolar macrophages from M1 to M2,which is mediated by the vagus nerve.

3.Impact of centrifugal therapeutic plasma exchange on platelet counts in patients with neurological autoimmune diseases

[Abstract] [Objective] To assess the effect of a centrifugal haemocyte separator on platelet counts in patients with neurological immune-mediated disorders during/after therapeutic plasma exchange (TPE). [Methods] This study included 189 patients (108 females and 81 males) who were treated in the department of neurology at Huashan Hospital, Fudan University, from March 2021 to March 2022. A total of 820 TPE treatments were carried out. Each patient received 2 to 5 TPEs, with each TPE amounting to the patient's plasma volume. The peripheral blood cell counts of the patients were evaluated before TPE and after 2 to 5 TPEs. [Results] The duration of a single TPE in this study was 94(84,107) minutes, and the actual volume of a single TPE replacement was 2 456(2 142, 2 785) mL. The number of patients who underwent TPE for 2, 3, 4, and 5 sessions was 17, 28, 18, and 126, respectively. The platelet (PLT) counts of the patients before and after the TPE were 195×109/L (range:150 to 245) and 220×109/L (range:170 to 270), respectively (P<0.05). Consequently, the overall PLT counts exhibited a significant decrease from baseline following TPE, yet the PLT counts remained within the normal range after TPE. Spearman's correlation analysis indicated that platelet loss did not correlate with the duration of TPE (ρ=0.037), the age of the patient (ρ=0.015), or the volume of the single replacement fluid (ρ=0.034), P>0.05, weakly correlated with the number of TPE sessions (ρ=0.017), and moderately correlated with the PLT counts before TPEs (ρ=0.446). [Conclusion] The TPE procedure exhibited a measurable impact on the patients' platelet levels, but the platelet counts remained within the normal range, therefore did not interfere with the patients' subsequent treatment protocols. The decrease in platelet level was correlated with the baseline platelet level before treatment and the number of TPE sessions.
4.Influencing factors of secondary diabetes mellitus after chemotherapy for hematologic tumors

Objective To investigate the influencing factors of secondary diabetes mellitus after chemotherapy for hematologic tumors. Methods The medical records of 396 patients with hematologic tumors admitted to Jinhua People's Hospital from January 2018 to January 2024 were retrospectively analyzed. According to whether patients developed secondary diabetes mellitus during chemotherapy,they were divided into secondary diabetes mellitus group (n=127) and non-diabetes mellitus group (n=269). Transcriptome sequencing was used to explore the different signaling pathways between two groups. Multivariate Logistic regression analysis was used to screen the influencing factors of secondary diabetes mellitus after chemotherapy,and the prediction model of secondary diabetes mellitus after hematologic tumor chemotherapy was constructed. Results The common differential genes in bone marrow samples of two groups were mainly concentrated in vascular endothelial function regulation and vascular homeostasis pathway. There were significant differences in age,body mass index (BMI) and family history of diabetes mellitus,hypertension and hyperlipidemia,dexamethasone on conversion cumulative quantity,endothelin-1 (ET-1),vascular cell adhesion molecule 1 (VCAM-1),endothelial nitric oxide synthase (eNOS) between two groups (P<0.05). Multivariate Logistic regression analysis showed that age,BMI,history of hyperlipidemia,dexamethasone on conversion cumulative quantity,ET-1,VCAM-1 and eNOS were independent influencing factors for secondary diabetes mellitus in patients with hematologic tumors after chemotherapy (P<0.05). The area under the curve of predictive model was 0.844 (95%CI:0.806-0.883,P<0.001),indicating that the predictive ability of the model was good. Conclusion Age,BMI,combined hyperlipidemia,dexamethasone on conversion cumulative quantity,ET-1,VCAM-1 and eNOS are all independent influencing factors for secondary diabetes mellitus in patients with hematologic tumors after chemotherapy,and the prediction model can be used as a tool for early clinical screening.

6.An in vitro study of hepatotoxicity induced by valproic acid and its metabolites

Objective To confirm the hepatotoxicity of valproic acid (VPA ) and its metabolites (2-propyl-4-pentenoic acid ,3-hydroxy valproic acid ,5-hydroxy valproic acid) on human liver cells .Methods Cells were divided into control group and VPA-treated group .The control group was conventionally cultured while the VPA-treated group was treated with valproic acid and its metabolites . The rate of cell proliferation was assayed by CCK 8 protocol . The mRNA levels of CYP1A1 , CYP1A2 ,PCNA ,Bax and Bcl-2 were measured by real time PCR .The correlated protein levels were measured by Western Blotting .The activity of LDH ,AST and ALT were also detected .Results Compared to the control group ,with the increases of concentrations and reaction time of VPA and its metabolites ,the proliferation rate of L02-cell was reduced ,the mRNA and protein levels of CYP1A1 ,CYP1A2 ,and Bax was increased ,the mRNA and protein level of PCNA and Bcl-2 was decreased , AST ,ALT ,and LDH were also elevated in the treated group .Conclusion Valproic acid and its metabolites were positively re-lated to hepatotoxicity .
7.An in situ gel system for nasal delivery of menthol: preparation and safety evaluation

Objective To prepare an in situ gel system for nasal delivery of menthol and to evaluate the safety of this formulation.Methods Menthol in situ gel was prepared with deacetylatedgellan gum.The nasal mucocilia toxicities of this formulation was evaluated using in situ toad palate model.Guinea pig skin sensitization test and the rabbit skin irritation test were conducted.Skin allergy and irritation reaction were monitored and scored.Results No significant effect on nasal mucosa ciliary movement and the morphology of rat nasal mucosa were observed.The formulation did not induce any dermal irritation in rabbits.Skin allergic reaction was not found in guinea pigs.Conclusion The preparation of menthol in situ nasal gel with low ciliary toxicity was easily achieved.This gel has good physiological flexibility.The further investigation was warranted for this formulation as an intranasal drug delivery system.
8.Influence of gamma globulin to serum B cell activating factor in therapy of neonatal thrombocytopenic purpura

Objective To study the influence of gamma globulin to serum B cell activating factor (BAFF) in therapy of neonatal thrombocytopenic purpura.Methods Fifteen cases with neonatal thrombocytopenic purpura(treatment group) were treated with gamma globulin.Serum BAFF levels of treatment group before and after treatment and 20 healthy neonates(control group) were tested.Results Serum BAFF level of the treatment group before treatment was(0.96±0.48) μg/L,which was statistically significant compared with that of the control group[(0.48 ±0.35) μg/L](P <0.05).Serum BAFF level of treatment group after treatment was (0.45 ± 0.37) μg/L,which was statistically significant compared with that of before treatment (P < 0.05),but was not statistically significant compared with that of the control group.Conclusion Gamma globulin can play a role in the therapy of neonatal thrombocytopenic purpura by reducing the BAFF.
9.Estimation of Shunt Severity of Patent Ductus Arteriosus by Color Doppler Flow Convergence Region

The left to right shunt rate(F)and the transverse seetional area of ductus arteriosus(A)were calculated by color Doppler flow convergence method in 20 patients with patent ductus arteriosus.The results indicated that the shunt rate(F)was correlated fairly with Qp-Qs,Qp/Qs(measured by pulsed wave Doppler),the pressure gradient(△P)and the diameter of left ventricle(DLV)(r=0.81,0.62,0.69 and 0.53;P<0.001,0.005,0.001 and 0.01,respectively).The transverse sectional area of ductus arteriosus(A)was also correlated closely with the diameter of ductus arteriosus(Dd)measured directly in two-dimensional view(r=0.81,P<0.001).The F value of group Qp/Qs≥2:1 was apparently higher than that of group Qp/Qs<2:1(P<0.005).The color Doppler flow convergence method can rapidly and exactly evaluate the left to right shunt severity of patent ductus arteriosus.
