OBJECTIVE To form an expert consensus addressing clinical issues regarding the use of parenteral direct thrombin inhibitors (DTIs) in special populations. METHODS Led by the Sichuan Academy of Medical Sciences & Sichuan Provincial People’s Hospital(the Affiliated Hospital of UESTC), a multidisciplinary working group was formed comprising experts from multiple fields, including clinical pharmacy, cardiac surgery, obstetrics, pediatrics and evidence-based medicine. Through literature review and the Delphi method, clinical questions regarding the efficacy and safety of parenteral DTIs used in special populations were identified. A structured design was adopted using the “Population-Intervention-Comparison-Outcome” (PICO) framework;systematic searches were conducted in CJFD, PubMed, Embase and other databases. Relevant evidence from randomized controlled trials, cohort studies and systematic reviews were included and synthesized. Evidence quality was assessed using the Grading of Recommendations Assessment,Development and Evaluation (GRADE) approach, and recommendations were formulated through three rounds of Delphi surveys and expert consensus meetings. RESULTS &CONCLUSIONS Seven clinical questions were ultimately selected (with a consensus rate exceeding 90%), resulting in the formulation of seven recommendations on the use of parenteral DTIs in special populations, including children, pregnant women, patients with hepatic or renal impairment, patients with mesenteric venous thrombosis, and individuals with thrombophilia. These recommendations clarify the preferred agents, dosing ranges, monitoring parameters, and safety management strategies for parenteral DTIs in these special populations. This expert consensus, which is formulated based on the best available evidence, provides evidence-based guidance for standardized and individualized use of parenteral DTIs in special populations.

Objective To investigate the effects of Ras-related protein Rab-2B(RAB2B)on the biological behaviors of pancreatic cancer cells and elucidate its underlying mechanism. Methods PANC-1 cells, which exhibit relatively high RAB2B expression, and BXPC-3 cells, which display relatively low RAB2B expression, were selected from five pancreatic cancer cell lines.RAB2B-siRNA and pcDNA3.1-RAB2B plasmids were transfected into PANC-1 and BXPC-3 cells using a cell transfection technique. The CCK-8 assay was employed to evaluate the prolif-erative capacity of pancreatic cancer cells following RAB2B intervention.Wound healing and Transwell chamber assays were utilized to assess the migratory and invasive capabilities of pancreatic cancer cells.Additionally, the mRNA and protein expression levels of RAB2B,NF-κB,and Fibronectin 1(FN1)were analyzed by qRT-PCR and Western blot(WB),respectively. Results RAB2B mRNA and protein expression levels were significantly down-regulated in PANC-1 cells following transfection(P<0.05).CCK-8 assay results demonstrated that the proliferative capacity of PANC-1 cells was markedly reduced(P<0.05),and the wound-healing ability was substantially impaired(P<0.01)upon RAB2B knockdown.Transwell assays revealed a significant decrease in cell migration(P<0.01),while Western blot analysis indicated that the expression levels of phosphorylated p65 and FN1 were notably diminished(P<0.01).Conversely,overexpression of RAB2B reversed these aforementioned alterations. Conclusions Knockdown of RAB2B in PANC-1 cells significantly suppresses cell proliferation and migration, whereas overexpression of RAB2B in BXPC-3 cells markedly promotes these processes. This effect is likely mediated through the activation of the NF-κB signaling pathway and the subsequent regulation of FN1 expression.

A mounting body of research suggests that circRNAs significantly contribute to the develop-ment of myocardial fibrosis. The microarray results of human circular RNA expression profile indicated that circHERC4_041 expression increased in the myocardium of patients with heart failure,RT-qPCR a-nalysis confirmed that the myocardial expression level of circHERC4_041 in individuals with heart failure were considerably elevated compared to that in healthy organ donors.Fluorescence in situ hybridization(FISH)confirmed that circHERC4_041 was abundant in the cytoplasm of human cardiomyocyte AC16.Overexpression of circHERC4_041 in mouse myocardial fibroblasts(mCFs)mediated by adenovirus in-hibited the expression of fibrosis-related proteins in mCFs.Experiments involving cell proliferation, wound healing, and Transwell assays demonstrated that overexpression of circHERC4_041 suppressed the growth and mobility of mCFs(P<0.001).Sequence analysis results suggested that circHERC4_041 con-tains potential ribosome entry sequence(IRES)and open reading frame(ORF).Western blot confirmed that circHERC4_041 could translate the 516 amino acid HERC4-516aa protein, which was mainly located in the cytoplasm of the cell.Cell functional experiments confirmed that circHERC4_041 inhibited the fi-brotic phenotype of mCFs by specifically translating HERC4-516aa(P<0.05).The specific interaction between HERC4-516aa and transglutaminase 2(TGM2)was confirmed by IP-MS screening and Co-IP i-dentification.Further results found that the degradation of TGM2 was promoted through proteasome path-way.The overexpression of TGM2 in mCFs facilitated by adenoviral vectors could counteract the suppres-sive effects of HERC4-516aa on the fibrotic phenotype of mCFs.Therefore, this study confirmed that the HERC4-516aa protein translated by circHERC4_041 can specifically bind to TGM2 to inhibit the fibrotic phenotype of myocardial fibroblasts.

Programmed cell death protein 1(PD-1)and its ligand, programmed cell death 1 ligand 1(PD-L1),represent a pair of prototypical immune checkpoint that plays a critical role in tumor immune evasion. However, the development of targeted therapeutics against these two proteins is limited by their low levels and high glycosylation modifications in eukaryotic cells. In this study,we designed two func-tional but truncated variants of PD-1(PD-133-150)and PD-L1(PD-L1 19-239);and subcloned them into eukaryotic expression vectors using golden gate assembly technology.Using these vectors,we achieved high level yields of these two proteins in transiently-transfected HEK-293T cells. After one-step affinity purification, the yields of PD-133-150 and PD-L119 239 proteins reached 5 mg and 3 mg per liter of cell cul-ture medium, with over 95%purity.Using biolayer interferometry and flow cytometry analysis,we deter-mined the binding kinetics, equilibrium constants, and the cellular binding activities of these proteins.Compared with the PD-1 extracellular domain expressed in insect or E.coli cells, the PD-133-150 purified from HEK-293T cells has a 24-fold and a 50-fold increase in its binding affinity to PD-L1.In addition, the dissociation rate of the binding decreased to less than l/400th of the original rate. Thus,we speculate that N-glycosylation could modulate the PD-1/PD-L1 interactions.Together,we established an effective eukaryotic expression and purification platform for functional characterization of PD-133150/PD-L119239 in-teractions,thereby providing high-quality molecular tools for PD-1/PD-L1 antibody screening and im-mune checkpoint research.
