Objective To construct a lung cancer surgery-oriented disease-specific database covering the entire perioperative care pathway, thereby improving the quality and usability of key surgical data elements. Methods Real-world clinical data were extracted from a single-center thoracic surgery department. A standardized data model was established based on the open electronic health record (openEHR) standard. Large language model (LLM), optical character recognition (OCR), and artificial intelligence (AI)-driven techniques were employed to extract, structure, and perform quality control on unstructured clinical narratives, imaging reports, and radiological data, with a focus on capturing surgically relevant perioperative indicator. Results A multimodal database comprising 19 917 patients was established, including 7 930 males and 11 987 females, with ages ranging from 15 to 97 (61.7±9.7) years. The database includes 582 structured data variables, textual report data corresponding to 69 clinical indicators, 13 000 pulmonary function test PDF reports, and chest CT imaging data from 16 884 patients. This database comprehensively covers major information relevant to surgical diagnosis and treatment of lung cancer, significantly improving the completeness and granularity of surgical detail data. Large language models (LLMs) and optical character recognition (OCR) technologies enhanced the efficiency of converting unstructured data into structured formats, while a multi-level manual verification process ensured data accuracy and traceability. The database supports real-world research including comparisons of surgical procedures, prediction of postoperative complications, prognosis assessment, and multimodal data association analyses.

Electroencephalogram (EEG) is a non-invasive, high temporal-resolution technique for monitoring brain activity. However, affected by the volume conduction effect, EEG has a low spatial resolution and is difficult to locate brain neuronal activity precisely. The surface Laplacian (SL) technique obtains the Laplacian EEG (LEEG) by estimating the second-order spatial derivative of the scalp potential. LEEG can reflect the radial current activity under the scalp, with positive values indicating current flow from the brain to the scalp (“source”) and negative values indicating current flow from the scalp to the brain (“sink”). It attenuates signals from volume conduction, effectively improving the spatial resolution of EEG, and is expected to contribute to breakthroughs in neural engineering. This paper provides a systematic overview of the principles and development of SL technology. Currently, there are two implementation paths for SL technology: current source density algorithms (CSD) and concentric ring electrodes (CRE). CSD performs the Laplace transform of the EEG signals acquired by conventional disc electrodes to indirectly estimate the LEEG. It can be mainly classified into local methods, global methods, and realistic Laplacian methods. The global method is the most commonly used approach in CSD, which can achieve more accurate estimation compared with the local method, and it does not require additional imaging equipment compared with the realistic Laplacian method. CRE employs new concentric ring electrodes instead of the traditional disc electrodes, and measures the LEEG directly by differential acquisition of the multi-ring signals. Depending on the structure, it can be divided into bipolar CRE, quasi-bipolar CRE, tripolar CRE, and multi-pole CRE. The tripolar CRE is widely used due to its optimal detection performance. While ensuring the quality of signal acquisition, the complexity of its preamplifier is relatively acceptable. Here, this paper introduces the study of the SL technique in resting rhythms, visual-related potentials, movement-related potentials, and sensorimotor rhythms. These studies demonstrate that SL technology can improve signal quality and enhance signal characteristics, confirming its potential applications in neuroscientific research, disease diagnosis, visual pathway detection, and brain-computer interfaces. CSD is frequently utilized in applications such as neuroscientific research and disease detection, where high-precision estimation of LEEG is required. And CRE tends to be used in brain-computer interfaces, that have stringent requirements for real-time data processing. Finally, this paper summarizes the strengths and weaknesses of SL technology and envisages its future development. SL technology boasts advantages such as reference independence, high spatial resolution, high temporal resolution, enhanced source connectivity analysis, and noise suppression. However, it also has shortcomings that can be further improved. Theoretically, simulation experiments should be conducted to investigate the theoretical characteristics of SL technology. For CSD methods, the algorithm needs to be optimized to improve the precision of LEEG estimation, reduce dependence on the number of channels, and decrease computational complexity and time consumption. For CRE methods, the electrodes need to be designed with appropriate structures and sizes, and the low-noise, high common-mode rejection ratio preamplifier should be developed. We hope that this paper can promote the in-depth research and wide application of SL technology.

OBJECTIVE:
To explore the genetic basis for a boy affected with Cohen syndrome.
METHODS:
A boy admitted to Children's Hospital of Nanjing Medical University in January 2021 was selected as the study subject. Genome DNA was extracted from peripheral blood samples from the child and his parents. Whole exome sequencing (WES) was carried out. And candidate variants were verified by Sanger sequencing. This study was approved by the Medical Ethics Committee of the hospital (Ethics No.: 202106060-1).
RESULTS:
WES revealed that the child has harbored compound heterozygous variants of the VPS13B gene, namely c.1563+1G>A and c.3007insC (p.A1003Afs*13), which were inherited from his mother and father, respectively. Based on the guidelines from the American College of Medical Genetics and Genomics (ACMG), both variants were rates as pathogenic. The c.3007insC (p.A1003Afs*13) was unreported previously.
CONCLUSION
The compound heterozygous variants c.1563+1G>A and c.3007insC (p.A1003Afs*13) of the VPS13B gene probably underlay the pathogenesis of Cohen syndrome in this child. Above finding has enriched the mutational spectrum of VPS13B gene.

This study examines the effects of zearalenone(ZEA)on the survival and function of lu-teinized granulosa cells, and studies the role of activating transcription factor 6(ATF6)in regula-ting apoptosis and functional abnormalities of luteinized granulosa cells induced by ZEA.An in vitro model of luteinized granulosa cells was utilized to examine the effects of ZEA treatment on apoptosis, hormone secretion, and the expression of relevant proteins.Furthermore, the expression of ATF6 was manipulated using siRNA to elucidate its regulatory function in the ZEA-induced damage of luteinized granulosa cells in mice. Our findings revealed that ZEA inhibited the activity of luteinized granulosa cells and reduced the secretion of estradiol(E2)and progesterone(P4)in a dose-dependent manner. The expression levels of p-IRE1,ATF6 and StAR in both low(20 pmol/L)and high(40 μmol/L)ZEA groups were significantly increased after 24 h(P<0.05).GRP78 had no significant change at low concentration treatment(P>0.05),but significantly increased at high concentration treatment(P<0.05).Similarly,ATF4 and p-EIF2α had no significant change at low concentration treatment(P>0.05),but significantly decreased at high concentration treat-ment(P<0.05).HSD3B2 and CYP19A1 were significantly decreased in both low and high concentration treatments(P<0.05).After 48 h of treatment,ATF6 and GRP78 were significantly increased in both low and high concentration treatments(P<0.05).p-IRE1 was significantly de-creased at low concentration treatment(P<0.05),but remained unchanged at high concentration treatment(P>0.05).ATF4,p-EIF2α,HSD3B2 and CYP19A1 were significantly decreased in both low and high concentration treatments(P<0.05).St AR was significantly increased in both low and high concentration treatments(P<0.05).Interference with the expression of ATF6 could sig-nificantly reduce the apoptosis induced by low concentration group(P<0.05),and enhanced the hormone secretion in both high and low concentration groups(P<0.05).In conclusion,ZEA can cause damage to luteinized granulosa cells and activate ATF6 signaling pathway.Interference with ATF6 can alleviate apoptosis and hormone secretion disturbance induced by low concentration ZEA,but has limited effect on damage caused by high concentration ZEA.

Objective:To investigate the mechanism by which lycopene prevents and alleviates ionizing radiation-induced intestinal injury. Methods:Forty specific pathogen-free male C57BL/6 mice were randomly divided into 4 groups: control group (gavage with an equal volume of corn oil), radiation group (gavage with an equal volume of corn oil and exposed to a single dose of 12 Gy X-ray irradiation on day 3), lycopene group (gavage with lycopene), and lycopene + radiation group (gavage with lycopene and irradiated with 12 Gy X-ray on day 3). All gavage treatments continued for 7 days after irradiation. On day 7 post-irradiation, intestinal tissues were collected. Hematoxylin and eosin (HE), Masson's trichrome, and Sirius red staining were used to assess intestinal injury and fibrosis. Immunohistochemistry was used to detect the expression of α-smooth muscle actin (α-SMA). Immunofluorescence staining was performed to assess the expression of transforming growth factor-β (TGF-β) and connective tissue growth factor (CTGF) in intestinal tissues. Serum levels of malondialdehyde (MDA), total superoxide dismutase (T-SOD), total antioxidant capacity (T-AOC), and glutathione peroxidase (GSH-PX) were measured to evaluate oxidative stress. mRNA expression of tumor necrosis factor-α (TNF-α), interleukin (IL)-6, IL-1β, and CD68 in intestinal tissue was quantified using real-time quantitative PCR (RT-qPCR). Western blot was used to detect the expression of apoptosis-related proteins. One-way ANOVA was used to compare differences among groups. Results:Compared with the radiation group, the lycopene + radiation group showed significantly reduced radiation-induced intestinal injury index, fibrosis, and expression of α-SMA, TGF-β, and CTGF (all P<0.05). Oxidative stress assays showed that serum MDA levels were decreased, while T-SOD, T-AOC, and GSH-PX activities were significantly increased in the lycopene + radiation group (all P<0.05). The mRNA expression of inflammatory cytokines such as TNF-α and IL-6 was significantly downregulated (all P<0.05), while pro-apoptotic protein expression was reduced and anti-apoptotic protein expression was upregulated (all P < 0.05) in the lycopene + radiation group. Conclusions:Lycopene significantly alleviates ionizing radiation-induced intestinal injury through its antioxidant, anti-inflammatory, and anti-apoptotic effects, making it a promising agent for the prevention and treatment of radiation-induced intestinal injury.
