ObjectiveTo investigate the role and mechanism of Lck/Yes-related novel protein tyrosine kinase (Lyn) on lipopolysaccharide (LPS)-induced M1-type polarization of macrophage. MethodsThe LentiCRISPR-V2 plasmid was digested with the restriction endonuclease BSMBI-V2, and the digested DNA fragments were recovered. The digested plasmid was ligated with Lyn-sgRNA using T4 ligase to generate the Lenti-Lyn-gRNA lentivirus. THP-1 cells were infected with the Lenti-Lyn-gRNA lentivirus to obtain a stable cell line with Lyn knockout, and a monoclonal THP-1 cell line with complete Lyn knockout (Lyn⁻/⁻) was established subsequently. Wild-type Lyn (LynWT) and Lyn⁻/⁻ THP-1 cells were induced with 100 ng/mL phorbol myristate acetate (PMA) for 48 h to differentiate into M0 macrophages, which were further polarized into M1 macrophages by stimulation with 100 ng/mL LPS for 24 h. Quantitative real-time polymerase chain reaction (qPCR) was performed to detect the expression of M0 macrophage markers, including integrin αM (CD11b), macrophage antigen (CD68), and monocyte differentiation antigen (CD14). The expression of Lyn in M1 macrophages differentiated from wild-type THP-1 cells (LynWT-M1) was measured by qPCR, and the ratio of phosphorylated Lyn to total Lyn (P-Lyn/Lyn) in LynWT-M1 cells was determined by Western blot. In M1 macrophages differentiated from Lyn-knockout THP-1 cells (Lyn⁻/⁻-M1), qPCR was used to detect the mRNA expression of inducible nitric oxide synthase (iNOS), interleukin-6 (IL-6), and chemokine (C-X-C motif) ligand 10 (CXCL-10). Western blot was conducted to assess the protein expression of iNOS, as well as the protein levels of molecules related to the Janus kinase 1 (JAK1)-signal transducer and activator of transcription 1 (STAT1)signaling pathway, including JAK1, phosphorylated JAK1 (P-JAK1), STAT1, and phosphorylated STAT1 (P-STAT1). Additionally, the expression of the M1 macrophage marker cluster of differentiation 80 (CD80) was analyzed by flow cytometry. ResultsThe Lyn-/- monoclonal cell line was successfully constructed. The expression of CD11b was significantly elevated in Lyn-/- M0 macrophages, and the differentiation of M1 macrophages was successful. Knockdown of Lyn inhibited mRNA expression of iNOS, IL⁃6, CXCL⁃10, protein expression of iNOS and CD80 expression in M1 macrophages (P<0.05). Western blot assay showed that Lyn knockdown inhibited protein expression of JAK1 and P-STAT1 (P<0.01). ConclusionAfter CRISPR/Cas9-mediated Lyn knockout, the expression levels of JAK1 and P-STAT1, the key molecules in the JAK/STAT signaling pathway of M1 macrophages, are significantly downregulated; concomitantly, the expression of M1 macrophage-specific secretory factors (iNOS, IL⁃6, CXCL⁃10 mRNA) and CD80 is also downregulated, which may be achieved via targeted regulation of the JAK1/P-STAT1-mediated JAK/STAT signaling pathway.

Numerous techniques for C2 screw fixation have been recently reported. However, concerns remain regarding the risk of spinal cord or vertebral artery injury and inadequate biomechanical stability. To our knowledge, the specific transforaminal “in-out-in” screw fixation technique has not been previously reported. This study aimed to investigate the feasibility and preliminary clinical outcomes of a transforaminal “in-out-in” multi-cortical purchase screw for posterior C2 screw fixation. Between October 2022 and March 2023, 10 patients underwent posterior atlantoaxial internal fixation. All patients had severe hypoplasia of the C2 pedicle on at least one side, precluding the use of standard C2 pedicle screws. A transforaminal “in-out-in” screw was used as an alternative. No spinal cord injury, vascular injury, or other major complications were observed. No implant failure was noted at the final follow-up. In conclusion, the transforaminal “in-out-in” screw may achieve rigid three-column fixation with multiple cortical purchases. It represents a safe and effective alternative for posterior C2 fixation in patients with severely narrow C2 pedicles where traditional pedicle screw placement is not feasible.

With the accelerating trend of population aging, the number of elderly patients with lung cancer continues to rise, and the disease burden is becoming increasingly heavy. The clinical management of these patients faces severe challenges due to their decreased physiological reserve, complex comorbidities, and significant individual heterogeneity. Consequently, under traditional diagnosis and treatment models, doctors often struggle to identify the individualized risks of elderly patients in a timely and comprehensive manner, which can easily lead to decision biases such as undertreatment or overtreatment. In view of this, this study advocates for the establishment of an umbrella decision-making model specifically tailored for elderly lung cancer patients. Grounded in a multidisciplinary team (MDT) platform, this model deeply integrates oncological indicators with the comprehensive geriatric assessment (CGA) system. By holistically considering multidimensional variables including tumor burden, organ function, frailty index, cognitive status, and social support, the model establishes an operational mechanism characterized by "single entry, precise stratification, and targeted selection". Accordingly, patients can be scientifically triaged into distinct intervention tiers, such as active surveillance, minimally invasive surgery, drug therapy, radiotherapy, and best supportive care, thereby achieving real-time alignment between treatment intensity and patient fitness. This article elaborates on the construction logic and key operational procedures of this novel decision-making framework, aiming to guide clinical practice beyond the limitations of a tumor-centric perspective toward a holistic, dynamic, whole-course management strategy. This transition seeks to ensure optimal quality of life and clinical net benefit for elderly patients alongside survival prolongation.

OBJECTIVE To construct and validate a prognostic prediction model for first-line immune checkpoint inhibitors (ICIs) therapy for advanced non-small cell lung cancer (NSCLC).METHODS The clinicopathological data and peripheral blood parameters from 211 patients with advanced NSCLC who received first-line ICIs therapy at our hospital from January 2020 to December 2023 were retrospective collected. The patients were randomly divided into a training set (151 cases) and a validation set(60 cases) at a ratio of 7∶3. Using progression-free survival (PFS) as the outcome measure, LASSO regression was used to preliminarily screen predictive variables in the training set,and the preliminarily screened predictive variables were entered into a multivariable Cox regression model to construct Model 1. Meanwhile, restricted cubic splinec analysis was performed on the continuous variables preliminarily screened by LASSO regression. The variables with nonlinear relationships ( P <0.1) were analyzed by multivariable Cox regression in RCS form to construct Model 2. The concordance index, receiver operating characteristic curves, calibration curves, and decision curve analysis were used to compare the predictive performance of the two models; the optimal model was selected to construct a nomogram.Sensitivity analysis was performed by reconstructing the multivariable Cox regression model after excluding cases with unknown programmed death-ligand 1(PD-L1)status. Kaplan-Meier method was used to plot survival curves to analyze the survival differences among patients with different risk strata.RESULTS The variables used to construct Model 1 were sex, smoking history, PD-L1 expression status, bone metastasis, and carcinoembryonic antigen (CEA);the variables used to construct Model 2 were sex, smoking history, PD-L1 expression status, bone metastasis, CEA, neutrophil(Neu), and albumin(ALB). The concordance indices of Model 2 were 0.710 in the training set and 0.672 in the validation set, both superior to those of Model 1 (0.649,0.634). The areas under the curve (AUC) of Model 2 for 12, 18, and 24 months PFS were 0.737,0.782 and 0.788 in the training set,and 0.709,0.630 and 0.663 in the validation set, respectively, all outperforming Model 1 (0.653, 0.704, 0.697 in the training set;0.688, 0.605, 0.651 in the validation set). The calibration curves of both models showed good consistency between the predicted probability and actual probability, and the clinical net benefit of Model 2 was higher than that of Model 1. Sensitivity analysis indicated that the prediction model was robust. The median PFS of patients in the low-risk group was significantly longer than that in the high-risk group( P <0.05).CONCLUSIONS Sex, smoking history, PD-L1 expression status, bone metastasis, Neu, ALB, and CEA were independent influencing factors for PFS in advanced NSCLC patients receiving first-line ICIs. The prediction model constructed based on the above factors has good predictive performance and can be used to predict the prognosis of advanced NSCLC patients.

ObjectiveTo investigate the therapeutic effect of paeoniflorin on metabolic dysfunction-associated fatty liver disease (MASLD) combined with insulin resistance (IR), and to elucidate its regulatory mechanisms on the Toll-like receptor 4 (TLR4)/nuclear transcription factor (NF)-κB-insulin receptor substrate 1 (IRS1) inflammation-insulin signaling axis. MethodsMASLD mouse model was established using a 60% high-fat diet. Mice were randomly assigned to a normal control group, a MASLD model group, and paeoniflorin groups (low dose: 25 mg·kg-1·d-1, medium dose: 50 mg·kg-1·d-1, high dose: 100 mg·kg-1·d-1), receiving intragastric administration for consecutive 12 weeks. Concurrently, a palmitic acid/oleic acid (PA/OA)-induced lipid deposition model was established in HepG2 cells. Cell counting kit-8 (CCK-8) assay was performed to determine the optimal concentrations of PA/OA and paeoniflorin for intervention. Mice body weight, liver index, and serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), triglycerides (TG), and total cholesterol (TC) were measured. Fasting blood glucose (FBG) and fasting insulin (FINS) levels were measured, and the homeostatic model assessment of insulin resistance (HOMA-IR) was calculated, while an oral glucose tolerance test (OGTT) was performed to evaluate glucose metabolism. Hematoxylin and eosin (HE) staining was used to observe pathological changes and inflammatory cell infiltration in liver tissue, and oil red O staining was used to evaluate lipid droplet deposition in liver tissue and HepG2 cells. Potential targets were screened using network pharmacology and molecular docking. Western blot analysis was performed to detect the expression of HSP90AA1, TLR4, p-NF-κB p65, p-IRS1, and p-Akt proteins in liver tissue and HepG2 cells. ResultsCompared with the normal control group, mice in the MASLD model group exhibited significantly increased body weight, liver index, and serum levels of ALT, AST, TG, and TC, as well as markedly elevated FBG, FINS, and HOMA-IR, and impaired glucose tolerance (P<0.01). HE staining revealed marked hepatic steatosis and inflammatory cell infiltration, while oil red O staining showed a significant increase in lipid droplet deposition. Compared with the MASLD model group, after intervention with various doses of paeoniflorin, the increase in body weight and liver index was reduced, serum ALT, AST, TG, and TC levels decreased, and FBG, FINS, and HOMA-IR were significantly lowered, with glucose tolerance markedly improved (P<0.05, P<0.01), showing a certain dose-dependent trend. HE and oil red O staining results showed a marked reduction in hepatic steatosis and lipid deposition. In vitro experiments demonstrated that, compared with the control group, PA/OA treatment significantly induced increased lipid deposition in HepG2 cells. Compared with the model group, paeoniflorin intervention significantly reduced intracellular lipid droplets, and lipid deposition showed a dose-dependent decreasing trend. Mechanism studies indicated that, compared with the normal control group, the MASLD model group exhibited significantly elevated expression of TLR4, HSP90AA1, and p-NF-κB p65, while p-IRS1 (Ser307) expression was elevated and p-Akt (Ser473) expression was reduced (P<0.01). Compared with the model group, the expression of the aforementioned proteins was significantly reversed in all treatment groups following intervention (P<0.05, P<0.01). ConclusionPaeoniflorin significantly alleviates lipid deposition and insulin resistance in MASLD mice. Its mechanism of action may involve targeting HSP90AA1, TLR4, and NF-κB1 to regulate the TLR4/NF-κB-IRS1 inflammation-insulin signaling axis. This regulation suppresses inflammatory responses and restores insulin signaling, thereby ameliorating abnormalities in glucose and lipid metabolism.
