1.Value of combined predictive model based on dual-layer detector spectral CT multiparametric radiomic features and quantitative parameters in preoperative diagnosis of gastric cancer serosal invasion

Objective:To construct a combined prediction model based on dual-layer detector spectral CT radiomics features and quantitative parameters, and to evaluate its value in preoperative prediction of serosal invasion in gastric cancer.Methods:This case-control study retrospectively analyzed data from 253 gastric cancer patients confirmed by postoperative pathology at the First Affiliated Hospital of Soochow University (Center 1) and Changshu No.2 People′s Hospital (Center 2) from January 2022 to December 2023. Patients from Center 1 ( n=157) were randomly divided into training set ( n=110) and test set ( n=47) in a 7∶3 ratio, while patients from Center 2 ( n=96) served as an external validation set. Based on postoperative pathological serosal invasion status, patients were classified into serosal invasion group ( n=164) and non-serosal invasion group ( n=89), with distributions of 70/40, 30/17, and 64/32 in the training, test, and external validation sets, respectively. Spectral CT quantitative parameters, including arterial and venous phase iodine concentration (IC), normalized iodine concentration (NIC), arterial-venous IC differences, arterial-venous NIC differences (NIC pa), arterial enhancement fraction (AEF), and effective atomic number (Z eff), were measured. Radiomics features were extracted from venous-phase 40 keV monochromatic images. The least absolute shrinkage and selection operator (LASSO) algorithm was used for feature selection. The logistic regression classifier (LR-LASSO) was applied to construct the radiomics model. Univariate and multivariate logistic regression analyses identified independent risk factors for serosal invasion, including the radiomics signature (RadScore) and quantitative parameters. A clinical model was built using significant quantitative parameters, and a combined model integrated RadScore. An artificial model was based on cT4 staging assessed by two radiologists using venous-phase CT. The diagnostic performance of each model was evaluated using the area under the receiver operating characteristic curve (AUC), calibration curves, and decision curve analysis (DCA). Results:A total of six radiomics features were selected to establish the radiomics model. RadScore ( OR=7.598, 95% CI 2.259-25.562, P=0.001) and NIC pa ( OR=4.598, 95% CI 1.404-15.050, P=0.012) served as independent risk factors. The NIC pa served as the clinical model. The AUCs (95% CI) of the combined model in the training, test, and external validation sets were 0.984 (0.969-1.000), 0.855 (0.728-0.982), and 0.773 (0.665-0.882), respectively. The AUCs of the artificial model were 0.741, 0.670, 0.644; of the clinical model were 0.709, 0.633, 0.626. The AUCs of the radiomics model were 0.963, 0.824, 0.741, respectively. Calibration curves showed good agreement between predicted probability and observed probability. The DCA confirmed higher clinical net benefits for the combined model. Conclusion:The combined model integrating dual-layer detector spectral CT radiomics features and quantitative parameters exhibits high efficacy for preoperative prediction of gastric cancer serosal invasion.
2.Current status of the " Six-Good" Party branches for retired personnel in medical institutions and scientific research institutes

Objective:To analyze the current status, identify existing issues, and explore countermeasures for the development of " Six-Good" retirement Party branches in medical institutions and scientific research institutes.Methods:In August 2024, ten medical and research institutions were selected. Focusing on the current status and existing issues in Party branch building for retired personnel, questionnaire surveys were conducted among staff members of the Party office and the retired cadres department, as well as retired Party members. Experts in the field of Party building were selected for semi-structured interviews. Core themes were then extracted through content analysis.Results:The questionnaire survey included a total of 399 survey respondents, comprising 112 staff members from the Party office and the retired cadres department, as well as 287 retired Party members. 212 (53.1%) of the respondents belonged to Party branches with over 50 members, 280 (70.2%) of the respondents expressed a preference for establishing Party branches based on similar business functions or former departments prior to retirement. 373 (93.5%) of the respondents believed Party branch committee members were adequately appointed. 33.7% (127/377) of the respondents perceived ideological fluctuations among retirement Party members. 175 (43.9%) of the respondents indicated that learning content was insufficiently diverse. Through interviews with 15 experts, it was found that the construction of retired party branches was faced with issues such as low participation in offline organizational life, insufficient effectiveness in the education and management of party members, limited role play of party members, and prominent service and support pressures. In response, the experts proposed specific suggestions such as innovating organizational forms, strengthening the effectiveness of education and management, and building platforms for role play.Conclusions:The " Six-Good" Party branch building for retired personnel of the medical and scientific research institutions had achieved certain results, but there were also some problems. It is necessary to further optimize organizational settings, strengthen the branch leadership, innovate learning forms, build a platform for role play, strictly implement various systems, and strengthen assistance to difficult party members.
3.Research on Internal and External Linkage Mechanism to Promote the High-quality Development of Clinical Specialties in Public Hospitals

At present,the development of clinical specialties in public hospitals is faced with internal and external superposition problems such as high internal and external operating pressure,lagging connotation construction,insufficient driving force for sustainable development,and inadequate implementation of the autonomy of specialty development.In this regard,from the perspective of building an internal and external linkage mechanism,external governance policies and internal management measures of public hospitals should be effectively linked and coordinated to promote the transformation of clinical specialties from high-speed development to high-quality development.Among them,improving the technical ability of diagnosis and treatment of special diseases and ensuring the health of regional people is the fundamental goal,the dual-wheel drive of talent and technology is the key,the personnel and salary management oriented by business development and value output is the driving force,the reform of external governance mechanism and the effective allocation of resources is the guarantee,the performance evaluation and supervision and management is the means,and the continuous medical service system is the support.To provide a fundamental guarantee for the high-quality development of clinical specialties in public hospitals.

7.Treatment of ulnar coronoid process fractures with mini-plate internal fixation via anterior elbow approach

Objective To evaluate the clinical effect of mini-plate internal fixation via anterior elbow approach in treating coronoid process fractures.Methods A retrospective case series study was conducted on the clinical data of 43 cases of ulnar coronoid process fractures treated from December 2014 to December 2016.There were 29 males and 14 females,with an average age of 32.4 years (range,24-64 years).Twenty-four cases were injured on the right,and 19 on the left.There were 18 cases of simple coronoid process fractures,23 combined with ipsilateral capitulum radius fractures,and two combined with ipsilateral ulna olecranon fractures.According to the O'Driscoll classification,there were four cases of type Ⅰ,3 type Ⅱa,9 type Ⅱb,17 type Ⅱc,6 type Ⅲa,and 4 type Ⅲb.The elbow flexion and extension range was 40°-90°[(64 ± 18)°],and the rotation range 60°-130°[(83 ± 15)°].All the patients underwent mini-plate internal fixation via anterior elbow approach.The operation time,intraoperative blood loss,wound healing,fracture healing,and postoperative complications were recorded.Function of elbow joint was evaluated by Mayo elbow performance score (MEPS).Results All patients were followed up for 12-24 months (mean,15.7 months).The average operation time was 52 minutes (range,36-86 minutes).The average blood loss was 20 ml (range,10-50 ml).At the last follow-up,all were seen incision healing by first intention and clinical fracture healing.The index finger,and middle finger palmaris numbness occurred in one patient after surgery,and the patient recovered at 2 months of follow up.The elbow flexion and extension range was 86°-145° [(117 ± 114) °],and the rotation range of 114°-155° [(132 ± 17) °],showing significant difference in comparison with the preoperative measure (P < 0.05).According to the MEPS one year after operation,14 cases were evaluated excellent,27 good,and four fair,with an excellent and good rate of 91%.Conclusion The mini-plate internal fixation via the anterior elbow approach is effective in treating ulnar coronoid process fractures,for the operation is simple,allows full exposure,reduction,or reconstruction under direct vision,and effectively restores the function of the elbow joint.
8.Simultaneous Determination of P-coumaric Acid and Ferulic Acid in Different Compatibility Proportions of the Couple of Sparganium stoloni and Curcuma aeruginosa by HPLC

OBJECTIVE:To establish a method for simultaneous determination of P-coumaric acid and ferulic acid in the couple of Sparganium stoloni and Curcuma aeruginosa,and to investigate rational proportion of the couple.METHODS:HPLC method was adopted.The determination was performed on Agilent 20RBAX XDB-C18 column with mobile phase consisted of acetonitrile-0.1% phosphoric acid (gradient elution) at the flow rate of 1.0 mL/min.The detection wavelength was set at 266 nm,and the column temperature was 30 ℃.The sample size was 10 μL RESULTS:The linear ranges of P-coumaric acid and ferulic acid were 4.218 6-21.093 μg/mL(r=0.999 8),1.836 0-9.180 μg/mL(r=0.999 9),respectively.RSDs of precision,stability and reproducibility tests were all lower than 2.0%.The recoveries were 98.72%-100.30% (RSD=0.18%,n=9),99.11%-100.45% (RSD=0.46 %,n =9).Among 4 compatibility proportions of the couple of S.stoloni and C.aeruginosa (1 ∶ 1,2 ∶ 1,1 ∶ 2,1 ∶ 0,m/m),the dis-solution rate of P-coumaric acid and ferulic acid were the highest when the ratio of S.stoloni-C.aeruginosa (m/m)was 2 ∶ 1.CON-CLUSIONS:The method is simple,accurate,stable and reproducible,and can be used for content determination of P-coumaric acid and ferulic acid in the couple of S.stoloni and C.aeruginosa.when the proportion of the couple of S.stoloni and C.aeruginosa is 2∶1 and the contents of P-coumaric and ferulic acid are the highest,the contents of main etfective components are the highest.
9.Accumulation of Gentamvcin in Perilymph of Guinea Pigs

According to the pharmacokinetic theory, the authors studied the accumulation of gentamycin in perilymph of guinea pigs by searching the drug concentration at various time with Fluorescene Polarization Immunoassay (FPIA). The results indicated that: (1) The drug concentration in perilymph increased with the augment of total doses administered and there was almost a linear correlation between them. (2) In animals of 7-day injectious group, the drug could be still detected during 72 hours after the last dose administration. The concentration was 1 37?0.95 ?g/ml which was close to the level of serum minimal inhibitory concentration (MIC). It suggested that the elimination of the drug from inner ear was too slow and evident accumulation of gentamycin was indeed. (3) The fact mentioned above implied that the drug ototoxities could still damage the ear even if the drug administered had ceased before. It is the reason by which we could explain the problem encountered in clinic, that is why in some patients the ototoxic syndrom may still presented or enhanced even though the treatment has been stopped a few days before.
