Objective To analyze the expression levels of miR-27a-3p mRNA and Yes-associated protein 1(YAP1)mRNA in breast cancer,and to explore their relationships with clinicopathological features and the survival prognosis of patients.Methods A total of 130 breast cancer patients who underwent mastectomy in our hospital between January 2019 and January 2021 were enrolled.The expression levels of miR-27a-3p and YAP1 mRNA in breast tumor tissues and adjacent normal breast tissues were assessed by qRT-PCR.Furthermore,the relationships between their expression and clinicopathological features,as well as the survival prognosis of patients,were investigated.Results Compared with adjacent normal breast tissues,the expression of miR-27a-3p mRNA in breast tumor tissues was lower(P＜0.05),while that of YAP1 mRNA was higher(P＜0.05).A negative correlation was observed between the expression of miR-27a-3p mRNA and YAP1 mRNA in breast tumor tissues(r=-0.456,P＜0.05).The expression of miR-27a-3p mRNA was correlated with tumor diameter,histological grade,tumor staging by the TNM system,lymph node metastasis,and vascular invasion in patients with breast cancer(P＜0.05).The YAP1 mRNA expression was correlated with histological grade,tumor staging by the TNM system,lymph node metastasis,and vascular invasion(P＜0.05).Kaplan-Meier survival analysis revealed that the 3-year overall survival rate of the miR-27a-3p low-expression group was 71.60％(48/67),which was lower than the 91.50％(54/59)of the miR-27a-3p high-expression group(log-rank x2=8.211,P=0.004).The 3-year overall survival rate of the YAP1 high-expression group was 73.80％(45/61),lower than that of the YAP1 low-expression group(87.70％,57/65)(log-rank x2=4.429,P=0.035).Multivariate regression analysis indicated that lymph node metastasis(hazard ratio[HR]=1.409;95％CI,1.057-1.644;P=0.046),vascular invasion(HR=1.541;95％CI,1.076-1.869;P=0.045),low miR-27a-3p mRNA expression(HR=0.593;95％CI,0.388-0.925;P=0.018),and high YAP1 mRNA expression(HR=0.628;95％CI,0.405-0.912;P=0.022)were relevant factors affecting the 3-year overall survival of patients with breast cancer.Conclusion A significant downregulation of miR-27a-3p mRNA and upregulation of YAP1 mRNA are observed in breast tumor tissues.The low expression of miR-27a-3p mRNA and the high expression of YAP1 mRNA are associated with adverse clinicopathological features and poor survival prognosis,and are risk factors affecting the 3-year overall survival of patients with breast cancer.They show promise as new potential therapeutic targets for breast cancer.

Bladder perfusion is one of the main methods for the treatment of bladder cancer. In order to further improve the standardization of bladder cancer bladder perfusion operation for nursing staff, this paper, guided by evidence-based methods, formed the expert consensus on the whole process management of bladder perfusion for bladder cancer through Delphi expert consultation and expert demonstration meeting, and provided guidance for the standardization of clinical nursing practice and management institutionalization of bladder cancer bladder perfusion from seven aspects, namely, perfusion environment, operators, drug allocation, operation process, adverse reactions, health education and continuous nursing.

Purpose: Screening for hepatocellular carcinoma (HCC) as an effective instrument to reduce the burden of late diagnoses remains underutilized in China, much of the Asian countries, and in a sense all over the world. Modifiable psychosocial factors should be identified to improve screening utilization and reduce the burden of late diagnoses. However, valid psychosocial measures are unavailable. This study aimed to translate, culturally adapt, and validate the preventive health model (PHM) instrument for measuring psychosocial factors of HCC screening among patients with hepatitis B. Methods: This study was conducted from June 2020 to April 2021 in three rigorous phases: (1) committee-based translation from English to Chinese; (2) cognitive interviews (n = 33) and Delphi expert consultations (n = 7) for cultural adaptation; and (3) a cross-sectional study (n = 305) for validation. Results: In phase I, two items were reworded, and two retranslated for semantic equivalence. In phase II, issues related to comprehension, sensitive wording, wording clarity, question relevance, and cultural sensitivity were addressed by including pictures, rewording five items, and developing seven items. In phase III, exploratory and confirmatory factor analyses suggested a five-factor 20-item solution: it explained 76.9% of the variance; had adequate factor loading (.60–.91), convergent and discriminant validity; satisfactory model fit indices; and reliability (Cronbach's α, .86−.91). Known-group analysis showed that patients with optimal HCC screening behavior had significantly higher scores on each subscale than those not having such. Conclusion The Chinese PHM instrument is culturally sensitive, reliable, and valid to measure the psychosocial factors of HCC screening. It can help nurses and researchers to tailor strategies to improve clinical HCC screening practices in high-risk HCC regions.

Objective:To translate Organizational Silence Behavior Scale (OSBS) for healthcare professionals into Chinese and to test the reliability and validity.Methods:The OSBS for healthcare professionals was translated and back-translated according to the Brislin translation mode and then the Chinese version of OSBS for healthcare professionals was developed. The items on the scale were screened and optimized through expert consultation and pre-survey. Using the convenient sampling method, a total of 550 nurses from 3 Class Ⅲ Grade A hospitals in Zhengzhou were selected from April 2019 to October 2020 to conduct a survey to evaluate the reliability and validity of the scale. A total of 550 questionnaires were issued and 520 valid questionnaires were returned. The effective response rate was 94.5% (520/550) .Results:The content validity index of the Chinese version of the OSBS for healthcare professionals was 0.930, and the content validity index of the item level was from 0.860 to 1.000. Exploratory factor analysis extracted 4 common factors and the cumulative variance contribution rate was 63.61%. The confirmatory factor analysis results showed that the ratio of chi-square to degrees of freedom (χ 2/ df) was 1.087, the root mean square residual ( RMR) was 0.035, the root mean square error approximation ( RMSEA) was 0.021, adjust goodness-of-fit index ( AGFI) was 0.990, goodness-of-fit index ( GFI) was 0.887 and incremental fit index ( IFI) was 0.990. The Cronbach 's α coefficient of the total scale was 0.893, and the Cronbach 's α coefficient of each dimension was 0.839 to 0.944. The test-retest reliability coefficient of the total scale was 0.974, and the test-retest reliability coefficient of each dimension was 0.818 to 0.881. Conclusions:The Chinese version of OSBS for healthcare professionals has good reliability and validity, which can be used as a tool to evaluate the silent behavior of nursing staff in China.

Objective:To explore the effect of the Internet combined pre-rehabilitation intervention model in patients with chronic hepatitis B liver cirrhosis.Methods:From June 2019 to December 2020, convenience sampling was used to select 140 patients with chronic hepatitis liver cirrhosis in Henan Provincial People's Hospital. The patients were randomly divided into the observation group and the control group according to the order of entry, 70 cases in each group.The observation group carried out the Internet combined pre-rehabilitation model and the control group implemented conventional nursing intervention. We compared the disappearance time of clinical symptoms, mastery of rehabilitation knowledge and the quality of life of the two groups of patients.Results:The disappearance time of abdominal distension, ascites, anorexia, fatigue, and lower extremity edema of the observation group was shorter than that of the control group, and the differences were statistically significant ( P<0.05) .After the intervention, the disease knowledge such as reasonable work and rest, reasonable diet, emotional control, exercise and medical treatment of observation group was better than that of the control group, and the differences were statistically significant ( P<0.05) . After the intervention, the quality of life scores of patients in the observation group were higher than those in the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The Internet combined pre-rehabilitation intervention model can shorten the disappearance time of clinical symptoms in patients with chronic hepatitis B liver cirrhosis, improve the patient's knowledge of disease rehabilitation and the quality of life, which is worthy of clinical application.

Objective To explore the effects of nursing management based on plan-do-check-action (PDCA) on side effects and life quality after peripherally inserted central catheter (PICC) in acute leukemia patients with chemotherapy.Methods From January 2017 to January 2018, we selected 96 acute leukemia patients in Hematology Department of He'nan Provincial People's Hospital as subjects by convenience sampling. All of the patients were divided into observation group and control group with the random number table, 48 cases in each group. Control group carried out routine nursing care, while observation group received nursing management based on PDCA. And then, we recorded and compared the indwelling time of PICC, hospital stay, complications and the incidence of side effects of patients between two groups. The Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS) and Quality of Life Index (QLI) were used to evaluated patients' anxiety, depression and life quality three days before intervention and one month after intervention. The self-designed questionnaire was used to assess the quality of special nursing in two groups.Results The indwelling time of PICC and hospital stay of observation group were shorter than those of control group with statistical differences (P<0.01). Before intervention, there was no statistical difference in the scores of SAS, SDS and QLI (P>0.05). After intervention, the scores of SAS and SDS of observation group were lower than those of control group; and the scores of QLI and dimension scores of the quality of special nursing of observation group were higher than those of control group; the differences were all statistical (P<0.01). The incidences of complications and side effects of observation group were statistically lower than those of control group (P<0.05). Conclusions Nursing management based on PDCA can effectively decrease the occurrence risk of side effects and complications after PICC among acute leukemia patients with chemotherapy, improve patients' life quality and emotional state which has clinical application and promotion value.

Objective To establish a double-antibody sandwich ELISA for the rapid detection of shiga toxin typeⅡ ( StxⅡ) in shiga toxin-producing Escherichia coli ( STEC) infection. Methods A pool of murine hybridomas was used to screen out the optimal antibody pair for the establishment of double-anti-body sandwich ELISA. The established ELISA system was used to detect StxⅡin the culture supernatants of 16 clinical strains of STEC. Specificity and sensitivity of the established ELISA system were also evaluated. Results Two antibodies, S2D8 and S2C6, were successfully screened out, based on which the double-anti-body sandwich ELISA was set up. StxⅡand its variants rather than StxⅠwas detected in the culture super-natants of STEC with a lowest detection limit of 4 ng/ml. Its performance was consistent with that of commer-cial colloidal gold test kit, indicating the characteristics of good specificity and sensitivity. Conclusion The S2D8/S2C6-based ELISA laid a foundation for researches which designates the shiga toxin as a potential can-didate on the diagnosis and therapy of STEC infection.

Aim To investigate the effects of isoliquiri-tigenin(ISL)on C6 glioma cell proliferation and differ-entiation.Methods C6 glioma cells’viability and proliferation were respectively measured by SRB test. Colony formation of C6 glioma cells from different groups was assayed.After culturing the cells from each group,giemsa staining was used to observe cell mor-phology.RT-PCR was applied to detect mRNA expres-sion of GFAP.Western blot was applied to detect the expression of GFAP.Results ISL effectively inhibited the viability of C6 glioma cells when compared with the control group in a concentration-dependent manner (P<0.01).The morphological observation under light mi-croscope showed that:in the control group,most of the undifferentiated C6 cells showed long fusiform and po-lygonal shape.Compared to the control group,the C6 cells treated with ISL revealed alteration in morphology such as astrocytes with smaller smooth,round body and much finer longer,tapering processes.The cloning for-mation rate detection revealed that:the colonies in the control group semerged earlier and were larger than those experimental ones,the cloning formation rate was higher,while almost no effective cells colony emerged in ISL treated groups(P <0.01 ).Western blot and RT-PCR analysis showed that GFAP expression in the ex-perimental groups increased(P <0.01).Conclusion ISL may inhibit the proliferation of C6 glioma cells and induce their differentiation.

Objective To explore the expression of heme oxygenase-1 (HO-1) in gastric mucosa of patient with portal hypertensive gastropathy (PHG),and its role in the pathogenesis of PHG.Methods The specimens were obtained from the gastric mucosa of 22 healthy subjects,20 portal hypertension (PHT) without PHG,and 22 PHG patients.Histological changes of the gastric mucosa were detected,and portal venous flow (PVF) was measured.The expression of HO-1 protein in gastric mucosal specimens was assessed by immunohistochemistry and Western blot analysis,respectively.The relationship between HO-1 expression and PHG severity score,HO-1 expression and PVF,PHG severity score and clinical parameters were investigated.Results HO-1 protein expression in gastric mucosa of PHG and PHT was significantly higher than that of the control (P ＜ 0.05),while there was no significant difference in this variable between PHG and PHT (P ＞ 0.05).The positive correlation was detected between HO-1 expression and PHG severity score in PHG patients (r =0.459,P ＜0.05),however,PHG severity score was irrelevant to severity of esophageal varice (r =0.059,P ＞ 0.05) or to Child-Pugh classification (r =-0.001,P ＞ 0.05).Of all the patients with PHT and PHG,no significant correlation was found between HO-1 expression and PVF (r =0.071,P ＞ 0.05).Conclusion HO-1 protein is up-regulated in gastric mucosa of PHG patients,which may contribute to gastric circulation disorder of PHG.

ObjectiveTo investigate the prognostic value of fluid responsiveness in patients with acute respiratory stress syndrome (ARDS).Methods Fifty-nine mechanically ventilated patients with ARDS were enrolled.Their stroke volume variations (SVV) were detected by pulse contour continuous cardiac output (PiCCO) analysis device,and then the patients were subsequently divided into fluid responsive group (SVV≥15％) and fluid non-responsive group (SVV ＜ 15％ ).The differences in 28-day survival,length of ICU stay and duration of mechanical ventilation between the 2 groups were compared.Thereafter,the 28-day survival of patients was analyzed by Kaplain-Meier survival model and the relationship between SVV and mortality within 28 days was analyzed by logistic regression model.Results In comparison with fluid non-responsive group,the 28-day survival of fluid responsive group was significantly increased (85.3％ vs.56.0％,P =0.012),the length of ICU stay was significantly shortened [( 13.1 ±5.2 ) d vs. (21.6±9.0) d,P=0.008],and the duration of mechanical ventilation was significantly shortened as well [( 11.4 ± 5.3 ) d vs.( 18.3 ± 4.9) d,P =0.022] ; Logistic analysis revealed that SVV ＜ 15％ increased the risk of 28-day mortality ( OR =4.82 ; 95％ CI:2.67 ～ 11.71,P =0.009 ).ConclusionsSVV-based fluid responsiveness can be served as a prognostic predictor for ventilated patients with ARDS.
