1.Construction and validation of an osteoporosis risk prediction model for middle-aged and elderly healthy physical examination population

Objective:To construct and validate the risk prediction model of osteoporosis (OP) in the middle-aged and elderly healthy physical examination population.Methods:In this cross-sectional study, 18 030 middle-aged and elderly people with bone mineral density tested in Health Management Center of Guangxi Zhuang Autonomous Region Hospital from January 2020 to December 2022 were selected. The general data, physical examination index and biochemical blood index were collected. The subjects were divided into training set (12 621 cases) and validation set (5 409 cases) in a ratio of 7∶3 with the simple random sampling method. The variables were screened with minimum LASSO regression and logistic regression and the corresponding nomogram prediction model for the risk of osteoporosis in the middle-aged and elderly health examination population was established. The performance of the nomogram model was evaluated with the area under the receiver operating characteristic curve (ROC AUC), specificity, sensitivity, calibration curve (CAL), and decision curve (DCA).Results:The results of LASSO regression and multivariate logistic regression in training set showed that gender, age, body mass index, hip circumference, waist circumference, systolic blood pressure, total cholesterol, glutamyl transpeptidase and albumin/globulin ratio were the independent best predictors of OP risk in the middle-aged and elderly health examination population (all P<0.05). The ROC AUC-value of the training set was 0.895 (95% CI: 0.886-0.904), with a sensitivity of 87.25% and a specificity of 85.01%. The ROC AUC value of the validation set was 0.892 (95% CI: 0.886-0.898), with a sensitivity of 83.74% and a specificity of 82.46%. The CAL showed a C-index value of 0.790 in the training set and a C-index value of 0.784 in validation set. The CALs all showed deviation correction and obvious curves similar to the ideal line. DCA showed that when the OP risk threshold probability of the training set was 45%-93%, and the OP risk threshold probability of the validation set was 45%-92%, the nomogram model had better efficacy in predicting OP risk in the middle-aged and elderly physical examination population, and the two results were still relatively consistent. Both CAL and DCA showed good performance. Conclusion:This study establishes a practical prediction model for osteoporosis risk in the middle-aged and elderly population, it can provide an early warning for the timely detection of OP risk for the middle-aged and elderly people.
2.Application of IOE in stroke patients with swallowing disorders under continuity of care

Objective To explore application of intermittent oral to esophageal tube feeding(IOE)supported by continuity of care in stroke patients with swallowing disorders.Methods A total of 100 stroke patients with swallowing disorders admitted to the Second Affiliated Hospital of Wenzhou Medical University from January to September 2023 were selected as subjects,and they were divide into control group(n=50)and observation group(n=50)by using random number table method.Control group received nutritional support through a nasogastric tube,while observation group received IOE.Swallowing function of patients before and after intervention,nutritional indicators,item short from health survey(SF-36)scores,and incidence of complications were compared between two groups.Results After the intervention,the swallowing function of observation group improved compared to control group(P<0.05).Nutritional indicators of both groups increased,and observation group was higher than control group(P<0.05).The scores of each item on the SF-36 in observation group were higher than those in control group(P<0.05).The incidence of complications in observation group was reduced compared to control group(P<0.05).Conclusion Implementing IOE with continuity of care support can improve swallowing function of stroke patients with swallowing disorders,reduce occurrence of complications.

Reducing the level of emotional expression conflict of patients is of great significance to promote the functional recovery of breast cancer patients,improve their prognosis and improve their quality of life.We introduce the research progress of ambivalence over emotional expression in breast cancer patients in terms of the connotation,measurement tools,influencing factors,and intervention methods.There is still a lack of research on measuring the emotional expression conflict of breast cancer patients in China,and it is still necessary to develop a measurement tool with cultural characteristics to obtain the true level of emotional expression conflict of patients.In clinical practice,complementary and alternative medicine,cognitive behavioral intervention,mindfulness training,expressive writing intervention,and other intervention strategies have achieved certain results in improving patients'negative emotions,but personalized intervention strategies still need further exploration.
6.HIV-, HBV- and HCV- NAT yield in individual donations: a multi-center analysis in Henan province from 2017 to 2019

【Objective】 To analyze the HIV-, HCV- and HBV- NAT yield rate in different areas of Henan province, so as to provide the basis for disease prevention and control as well as the establishment of a unified quality control standard for nucleic acid testing(NAT) in the Henan province. 【Methods】 The number and prevalence of NAT yielding samples with isolated infectious virus, namely HIV, HCV and HBV, in 18 blood stations in Henan province from 2017~2019, as well as the trends were analyzed. The NAT quality of each laboratory and each testing system was analyzed according to the ratio of reactive individual donation(ID) results to reactive minipools(MP). 【Results】 The HBV, HCV and HIV ID-NAT yield numbers in 3 501 251 blood donations were HBV 2 606(74/100 000), HCV 21 (0.63/100 000), and HIV 34(1.00/100 000). The HBV ID-NAT yield rate showed an upward trend in the whole province from 2017 to 2019, while the prevalence of HIV and HCV ID-NAT yield didn′t differ significantly during three years. 5 kinds of NAT detection systems were applied in 18 blood centers. among which Ⅰ, Ⅱ, Ⅳ and Ⅴ were triplex detection systems. 2661 ID-reactive samples were implicated in 5 595 MP-reactive samples, with a resolution rate of 47.56%. The resolution rate of triplex NAT system Ⅰ, Ⅱ, Ⅳ and Ⅳ was 39.63%~47.95%, 40.43%~54.36%, 51.61% and 70.00%~45.45%, respectively. An upward trend in triplex NAT resolution rate was observed in 8 laboratories, i. e.B, D, E, F, I, K, L and Q, and an descending trend in A and C. The NAT system Ⅲ, a ID-NAT system, was used only by laboratory C, presenting a NAT-yield rate of 0.19% (282/145 474) and resolution rate of 46.45% (131/282). 【Conclusion】 The majority of NAT-yield of one infectious virus in Henan province is HBV, presenting annual increasing trend. The quality management of NAT laboratories should be strengthened as the divergence was seen in the performance of different NAT laboratories.
7.Single-ELISA reactive rate of four blood screening items in 18 blood station laboratories: Henan, 2019

【Objective】 To analyze the cause of single-ELISA reactive of four blood screening items in 18 blood stations in Henan, so as to provide the basis for improving the quality of blood screening. 【Methods】 The single-ELISA reactive rate of HBsAg, anti-HCV, HIV Ag/Ab and anti-TP of 18 blood station laboratories in Henan throughout 2019 was calculated, and the causes were analyzed according to different ELISA reagent combinations and gray area settings in each laboratory. 【Results】 The overall single-ELISA reactive rates of HBsAg, anti-HCV, HIV Ag/Ab and anti-TP were 1.740(2 154/1 237 789), 0.564‰(698/1 237 789), 1.421‰(1 759/1 237 789) and 1.561‰(1 932/1 237 789), respectively, showing significant differences by detection items (P <0.05). Person correlation analysis showed that the single-ELISA reactive rate was independent of the gray area settings.but dependent on laboratories and reagent combinations. The single-ELISA reactive rate of HBsAg, anti-HCV, HIV Ag/Ab and anti-TP in D laboratory was the highest and higher than that in other labs using the same reagent.The laboratories with high HBsAg single-ELISA reactive rate were mostly those using a combination of imported reagents and domestic reagents, including the top 6 laboratories. The laboratories with high anti-HCV single-ELISA reactive rate were mostly those using certain domestic reagents. No obvious rules was noticed by single-ELISA reactive for anti-HIV. Laboratories with high anti-TP single-ELISA reactive rate were mostly those using combination 4. 【Conclusion】 The HBsAg single-ELISA reactive rate was the highest in the four blood screening items of blood station laboratories in Henan. The single-ELISA reactive rate is related to the laboratory itself and the reagent manufacturer, suggesting that laboratory quality control should be strengthened and proper reagent combination should be selected to reduce the waste of blood.
8.The clinical curative effect of locking plate and anatomical plate for the treatment of complex tibial plateau fractures

Objective To observe the clinical curative effect of locking plate and anatomical plate for the treatment of complex tibial plateau fractures. Methods Retrospective analyzed clinical data of 80 cases of complex tibial plateau treated in our hospital from January 2013 to January 2014, according to the different internal fixation methods and di-vided into observation group (n=40, locking plate fixation) and control group (n=40, normal anatomical steel plate treatment), through the review and analysis home visits, telephone follow-up, and review records of two groups were compared, the operation time, blood loss, operation group and control group in the amount, full weight bearing time, HSS score, complications, and the healing time of fracture by X-ray and clinical examination judgment were compared. Results The average operation time of observation group was(112.3±16.1)min, intraoperative blood loss(285.9±21.8)mL, the average operation time of control group was(130.8±22.3)min, blood loss(321.6±37.9)mL. All fractures were healed, the healing time of observation group was(13.0±1.2) weeks, full weight bearing time (14.1±1.7) weeks, the control group of fracture healing time (15.4±2.7)weeks, full weight bearing time was(16.5±2.9) weeks; HSS score of the ob-servation group was (88.3±7.2)points, the control group was (83.1±6.3)points. Comparison of two groups of patients with the above indexes, with significant difference (P<0.05 or P<0.01), the excellent rate 1 year after the operation of observation group was 90.0%, significantly higher than that in control group, the excellent and good rate was 80.0%, excellent rate of two groups with knee joint function, the difference was significant (P<0.05); the complication rate of observation group was 7.5%, the control group was 22.5%, with significant difference (P<0.05). Conclusion Compared with normal anatomical plate, the complex tibial plateau fracture locking has less bleeding, fast fracture healing, less complications and other advantages of steel plate, and can significantly improve the knee joint function of patients, is worthy of popularization and application.
9.Effect of polymeric scaffolds on attachment and growth of bone marrow mesenchymal stem cells.

To investigate the effect of three kinds of polymeric scaffolds on attachment, proliferation and differentiation of bone marrow mesenchymal stem cells, the cells were different polymeric scaffolds of PLA-PEG, PLA, PLGA, respectively. The proliferation of cell was evaluated by cell count; the attachment and morphology of BMSCs were observed by SEM; and differentiation was detected by alkaline phosphatase activity, fluorescence, and RT-PCR methods. Results showed that the cells in PLGA group spread better among BMSCs adhered to the three polymeric scaffolds. The activity of ALP was detected after 3 days culture in these three groups. There were no significant differences between PLA-PEG and PLGA groups, but the activity of ALP was higher than PLA group. The gene expressions of osteocalicin and collagen I were also observed in the early culture time. Calcium nodes formation in these polymeric scaffolds were detected. BMSC spreading first, then overlapping growth and secretion of matrix around the bottom and surface of scaffolds were observed through SEM. In summary, PLA-PEG and PLGA are better polymeric scaffolds for the bone tissue engineering, compared with PLA.

10.Polymerase chain reaction analysis of the clonality of Porphyromonas gingivalis and collagenase gene.

OBJECTIVETo investigate the genotypic characterization of Porphyromonas gingivalis (Pg) and the heterogeneity of a potential virulence factor-PrtC.
METHODSArbitrarily primed polymerase chain reaction (AP-PCR) was applied to 80 Pg strains isolated from 24 unrelated Chinese periodontitis patients. PCR reaction was used to detect a fragment of the collagenase gene (PrtC gene). To evaluate the sequence heterogeneity of the Pg PrtC genes, sequence analysis of four PrtC gene of clinical isolates was performed.
RESULTSRandom primer OPA-05 and OPA-17 distinguished 7 AP-PCR profiles (I through VII). The majority of the strains belonged to type VII which accounted for 25.8%. A 548bp fragment of PrtC gene was detected from 24 clinical strains. The PCR products were verified by the restriction endonucleases PstI and PvuI. Sequence analysis showed 4 PrtC genes were heterogeneous in their nucleotide composition and differed from reference strain Pg 53977.
CONCLUSIONSThe results demonstrated genetic diversity existed among these clinical strains isolated from Chinese periodontitis patients and the PrtC genes are heterogeneous in their nucleotide sequence.
