1.A qualitative research on psychological experience of operating room nurses participating in organ transplant surgery

Objective To explore the psychological experiences of operating room nurses participating in organ transplant surgeries, providing a basis for medical institutions to develop targeted psychological support programs. Methods Using a phenomenological research method, semi-structured, in-depth, face-to-face interviews were conducted from January to March 2026 with 17 operating room nurses who had participated in organ transplant surgeries at a tertiary hospital in Shanghai. Interviews were recorded, and observations were made. The interview data were analyzed, summarized, and themes were refined using Colaizzi’s seven-step analysis method. Results The psychological experiences of operating room nurses participating in organ transplant surgeries were identified as 5 main themes and 14 sub-themes: emotional impact and empathy load (shock and grief confronting life and death, deep empathy for patients and families, awe and gratitude towards organ donors); pressure experience and physical-mental exhaustion (urgency under time pressure, load of operational precision requirements, excessive consumption of physical and mental resources); cognitive appraisal and meaning construction (reappraisal of professional value, deep reflection on the meaning of life, positioning and identification of one’s role); coping strategies and adjustment process (active learning and experiential reflection, self-emotional regulation and social support); growth experience and psychological transformation (enhancement of psychological resilience, establishment of professional confidence, deepening of professional mission). Conclusions Operating room nurses undergo complex and profound psychological experiences while participating in organ transplant surgeries, enduring strong emotional impact and physical-mental stress while also achieving psychological growth and deepening professional identity through practice. Nursing managers should pay attention to the mental health of this group and construct a multi-level support system to promote their physical and psychological well-being.
2.TACE combined with CBCT-guided MWA simultaneous treatment for small hepatocellular carcinoma:analysis of clinical efficacy and safety

Objective To investigate the clinical efficacy and safety of simultaneous treatment of small hepatocellular carcinoma(HCC)with transarterial chemoembolization(TACE)combined with cone-beam computed tomography-guided(CBCT-guided)microwave ablation(MWA).Methods The clinical data of 69 patients with small HCC(72 lesions in total),who underwent TACE combined with CBCT-guided MWA simultaneous treatment from March 2018 to December 2022 at First Affiliated Hospital of Soochow University hospital,were retrospectively analyzed.Follow-up check was performed at 1,3,6,and 12 months after treatment.The mRECIST criteria was used to evaluate the tumor response.The objective response rate(ORR),disease control rate(DCR),progression-free survival(PFS),local tumor progression(LTP),and adverse reactions were analyzed.Results The initial complete remission(CR)rate,ORR and DCR of TACE combined with CBCT-guided MWA simultaneous treatment for small HCC was 94.2%(65/69),100%and 100%respectively.44.9%(31/69)of patients experienced tumor progression,and 20.3%(14/69)of patients experienced local tumor progression.Univariate and multivariate analyses showed that the maximum tumor diameter(≥2 cm and<3 cm)was the main risk factor for PFS(HR=4.498,P<0.001).No serious adverse events occurred during the study.Conclusion TACE combined with CBCT-guided MWA simultaneous treatment for small HCC is clinically effect and safe,and this therapy is particularly suitable for the treatment of lesions where the use of traditional image-guided methods is limited.
3.Application of pneumatic hand rehabilitation equipment in rehabilitation of hand function in children with spastic hemiplegia cerebral palsy

ObjectiveTo apply pneumatic hand rehabilitation equipment in comprehensive rehabilitation training to improve hand function in children with spastic hemiplegic cerebral palsy. MethodsFrom Januray, 2020 to September, 2021, 58 children with spastic hemiplegia cerebral palsy in Maternity and Child Health Care Hospital of Nantong University were randomly divided into control group (n = 29) and experimental group (n = 29). Both groups accepted comprehensive rehabilitation training, while the experimental group accepted pneumatic hand rehabilitation equipment training in addition, for six months. They were assessed with Peabody Developmental Motor Scales-Fine Motor (PDMS-FM), Fine Motor Function Measurement (FMFM) and Wee-Function Independence Measure (Wee-FIM) before and after training. ResultsThe scores of PDMS-FM, FMFM and Wee-FIM improved in both groups after training (|t| > 16.310, P < 0.001), and improved more in the experimental group than in the control group (t > 2.795, P < 0.01). ConclusionApplication of pneumatic hand rehabilitation equipment in comprehensive rehabilitation training can effectively promote the recovery of hand function for children with spastic hemiplegic cerebral palsy.
4.Effect of hepatocyte fatty degeneration induced by free fatty acid on macrophage polarization

ObjectiveTo investigate the effect of hepatocyte fatty degeneration induced by free fatty acid on macrophage polarization and the possible mechanism. MethodsPrimary hepatocytes of C57BL/6 mice were isolated by in situ collagenase perfusion, and then the hepatocytes were divided into control (NC) group and mixed free fatty acid (FFA) treatment group. A conditioned medium (CM) was prepared for hepatocytes and was used for the intervention of RAW264.7 macrophages. Oil red O staining was used to observe lipid deposition in hepatocytes; real-time PCR was used to measure the mRNA expression of lipid metabolism genes and macrophage M1/M2 polarization markers; ELISA was used to measure the levels of cytokines in supernatant; Western blot was used to measure the expression of proteins involved in the Toll-like receptor 4 (TLR4)/nuclear factor-kappa B (NF-κB) pathway in macrophages. The independent samples t-test was used for comparison between two groups; a one-way analysis of variance was used for comparison between multiple groups, and the Tukey test was used for further comparison between two groups. ResultsCompared with the NC group, the FFA treatment group had the deposition of massive lipid droplets in hepatocytes and significant increases in triglyceride and total cholesterol (t=15.65 and 3.49, both P<005). Besides, FFA significantly increased the mRNA expression of the lipid synthesis genes SREBP1C and FASN (t=2.89 and 2.82, both P<0.05) and reduced the mRNA expression of the lipid decomposition genes ACOX1 and CPT1A (t=14.30 and 3.36, both P<005) in hepatocytes. FFA also induced significant increases in the levels of the inflammatory cytokines interleukin-6 (IL-6), interleukin-1β, and tumor necrosis factor-α (TNF-α) in supernatant (all P<0.05). Compared with the CM-NC group, the CM-FFA group had significant increases in the mRNA expression of the M1 phenotype markers iNOS2, TNF-α, and IL-6 (all P<0.05) and a significant reduction in the mRNA expression of the M2 phenotype marker interleukin-10 (P<0.05). Moreover, Western blot showed that CM-FFA significantly upregulated the protein expression of TLR4, p-NF-κBp65, and p-IκBα in macrophages (t=2.88, 3.69, and 3.54, all P<0.05). ConclusionFFA-induced hepatocyte fatty degeneration and inflammation can promote M1 macrophage polarization, thereby initiating and triggering the development and progression of nonalcoholic fatty liver disease.
5. PPARγ Agonists Alleviate the High Fat-induced Oxidative Stress and Inflammatory Response in vitro and in vivo Through Nrf2-mediated Antioxidant Pathway

Background: Oxidative stress is important for the development and progress of non-alcoholic fatty liver disease. Recent studies have demonstrated the anti-inflammatory and antioxidant activities of peroxisome proliferator-activated receptor γ (PPARγ) agonists. Aims: To investigate the protective effect of PPARγ agonists (rosiglitazone and GW1929) on high fat-induced hepatocyte oxidative stress injury in vitro and in vivo and the underlying mechanism. Methods: C57BL/6J mice were fed with high-fat diet and administered intragastrically with rosiglitazone 30 mg/kg per day for 4 weeks. The primary mouse hepatocytes were isolated and incubated with mixed free fatty acids (FFA) after GW1929 pretreatment. HE staining and oil red staining were used to evaluate the histopathological changes and lipid deposition in the liver. Serum contents of malondialdehyde (MDA), superoxide dismutase (SOD) and glutathione (GSH) were measured. Reactive oxygen species (ROS) in hepatocytes was detected by DCFH-DA fluorescence probe. Real-time PCR and Western blotting were used to detect the expressions of genes related to oxidative stress and inflammation. Results: High-fat diet induced significant hepatic steatosis and up-regulated gene expressions of inflammatory cytokines. Serum MDA content was significantly increased, and SOD activity and GSH content were significantly decreased. Rosiglitazone intervention could reduce the high fat-induced hepatic steatosis, inflammation and improve the serum indicators of oxidative stress. Primary hepatocytes incubated with FFA produced a large amount of ROS, whereas GW1929 pretreatment could diminish the FFA-induced ROS production, inflammation and increase the antioxidant factors, nuclear factor erythroid 2-related factor 2 (Nrf2) and its downstream target gene heme oxygenase-1 (HO-1) expression. Conclusions: High-fat environment can induce steatosis, oxidative stress and inflammation in hepatocytes in vitro and in vivo. PPARγ agonists may alleviate the high fat-induced oxidative stress injury and inflammatory response through inhibiting ROS production and activating Nrf2/HO-1 antioxidant pathway.
6.Pharmaceutical Care on a Patient with Cerebral Venous Sinus Thrombosis and Epilepsy

Objective To investigate the role of clinical pharmacist in medical therapy for a patient with cerebrovascular disease and epilepsy.Methods One patient was hospitalized due to headache and twitch.Clinical diagnosis was cerebral venous sinus thrombosis and epilepsy.During the treatment,the clinical pharmacist gave suggestion on drug selection of anti-infection and anti-epileptic therapy,assessed drug interaction between omeprazole-escitalopram and mannitoldexamethasone,monitored the adverse drug reaction of valproate,and provided medication education to the patient.Results Physician adopted clinical pharmacist's suggestion.The patient discharged with stable condition.Conclusion The participation of clinical pharmacists in the medication therapy of patients with cerebrovascular disease can reduce the risk of drug use and promote the efficacy and safety of medication.
7.Analysis of the causes and counter measures of non-responsible operation articles checking inveracity events in operation room

Objective To explore the management of non-responsible articles checking inveracity in operation room. Methods From January 2016 to January 2017,22 cases of non-responsible articles checking inveracity events were analyzed and the countermeasures were put forward. Results Twenty-two cases of non-responsible operation articles checking were the minor defects of surgical instrument. 7 cases related to incorrect operation of the surgeon and the indifference of the goods inventory consciousness. 9 cases were from the department of orthopedics, 5 cases were from the department of gynaecology,3 cases were from the department of hepatobiliary surgery,and another 5 cases were from other department. In the style of surgery, 12 cases of 22 cases were from minimally invasive surgery,10 cases were from open surgery.In the style of articles,10 cases were from one-off consumptive material,8 cases were from conventional instruments of surgery,and the rest of 4 cases were from rent instruments. Conclusions We should strengthen the occupation safety education for surgeons, enhance the orthopedics specialized surgical cooperation and management of external instruments,formulate checking rules of articles, summarize and revised the requirements of minimally invasive surgery.We should work on standards for new consumptive material and surgical instrument. Wen should work on the emergency plans of non-responsible operation articles checking inveracity events,timely write the report of accident events.
8.A verification study on the genes associated with laryngeal squamous cell carcinoma by cDNA microarray.

OBJECTIVE:
Our previous study revealed many genes differentially expressed in human laryngeal squamous cell carcinoma tissues (LSCC) and related adjacent normal tissues. We verificated the differentiated expressions of target genes, possibly related to LSCC. And these results can be foundations of further study of these target genes function.
METHOD:
SENP1, CD109, Laminin alpha 2, Laminin alpha 3 were selected according to the cDNA microarray results. The expression of these genes mRNA and protein were detected by RT-PCR and Western blot in 12 cases of LSCC and related adjacent normal tissues.
RESULT:
The mRNA expression of SENP1, CD109 and Laminin alpha 3 were significantly higher while Laminin alpha 2 were significantly lower in LSCC tissues than in corresponding adjacent normal tissues by Semiquantitative RT-PCR. Western blot analysis revealed SENP1, CD109 protein expression were significantly higher in LSCC tissues than in corresponding adjacent normal tissues.
CONCLUSION
SENP1, CD109, Laminin alpha 2 and Laminin alpha 3 may correlated with tumorigenesis and development of LSCC and can provide beneficial clue for study pathogenesis of LSCC in molecular level.

9.Study on high risk factors associated with positive margin of cervix conization in patient with cervical intraepithelial neoplasia

Objective To assess the high risk factors associated with the positive margin of conization in patients with cervical intraepithelial neoplasia (CIN). Methods From January 2000 to February 2008, 1699 consecutive patients with CIN undergoing conization was reviewed retrospectively in order to analyze the relationship between the positive margin of conization with clinical prognostic factors,such as patients age, disease grade, size of lesion, the procedure of excision and menopause. X<'2> tests was used to compare the different frequencies of factors in groups of positive and negative margin conization, then seven factors with positive margin were processed into unconditional logistic regression analysis. Results The rate of the positive margin in 1699 patients was 14.01% (238/1699). The mean age of patients with positive margins was (39±9 ) years old, while patients with negative margins was ( 39±8 ) years old, which didn't reach statistical difference(P>0.05). The rate of the positive margin was 8.63% in cold knife cone (CKC) and 18.66% in loop electrosurgical excision procedure (LEEP), which showed significant difference( P<0.01 ). Among 1699 patients, 90 patients were with CIN Ⅰ ,339 patients were with CIN Ⅱ ,1113 patients were with CIN Ⅲ [ including 972 with severe dysplasia and 141 with cancer in situ(CIS) ],87 patients were with cervical cancer stage Ⅰ al, 70 patients were with stage Ⅰ a2 or advanced stages. The rate of positive margin was 1.11% ( 1/90), 3.83% ( 13/339), 10.70% (104/972), 26.24% (37/141),35. 63% (31/87) and 74.29% (52/70),respectively. There was statistic difference among them, except CIN Ⅰ and CIN Ⅱ . When combined CIN Ⅰ with CIN Ⅱ , then compared with CIN Ⅲ, cervical cancer withⅠ al and Ⅰ a2, it also showed statistical difference (P<0.05 ) . The rate of positive margin in postmenopausal women was 21.54% (28/130), which was significantly higher than 13.38% (210/1569)in premenopausal women (P=0.010 ). The logistic regression analysis showed that the procedure of excision, grade of disease, size of lesion, surface of cervix, and menopause were high risk factors associated with the positive margin, the risk ratio were 5.147, 3.048, 1.271, 1.905 and 1.860, respectively.Conclusions High grade, the extent of CIN disease, LEEP and postmenopausal age are high-risk factors associated with positive margin in patients treated by conization. It should be warranted in those patients when designing conization treatment.
10.Investigation of Relationship Between Helicobacter Pylori Infection and Reflux Esophagitis

Objective To investigate the relationship between helicobacter pylori (H. pylori) infection and reflux esophagitis (RE). Methods H.Pylori infection rates of RE in patients with simple chronic gastritis(CG),which was confirmed by gastrospcope combined with pathological diagnosis were researched.The relationship between HP infection and RE classifications using gastroscope and pathology was explored.Results ⑴H.pylori infection was found in 29(27.1%) in 107 cases of RE and 43(40.2%) in 107 cases of simple CG patients respectively,HP infection positive in RE patients less than that of simple CG patients.RE of class Ⅰ,Ⅱand Ⅲ using gastroscopy classification were 62.1%, 10.3% and 27.6% in H. Pylori-positive cases respectively, while they were 56.4%, 6.4% and 37.2% respectively in H. Pylori-negative ones. However, mild, moderate and severe RE identified by pathohistology were 72.4%, 13.8% and 13.8% in H.Pylori-positive cases respectively, and they were 57.7%, 17.9% and 24.4% respectively in H.Pylori-negative ones.Conclusions All above results suggested that H.Pylori possibly has prevention role in some extent to pathogenesis of RE. RE more commonly was seen in H.Pylori-negative cases.RE could inhibit the H.Pylori survive,So that,the RE was occurred frequently in H.Pylori-negative patients.The inflammatory extent of RE is not serious in H.Pylori-positive patients.
