Osteoporosis (OP) is a metabolic disorder characterized by microstructural deterioration of bone and increased bone fragility due to reduced bone mass, which can cause the development of bone-related diseases. This condition imposes significant economic and psychological burdens on patients. While modern medicine has extensively researched the pathogenesis of OP, it remains incompletely understood. Current clinical management primarily relies on anti-resorptive drugs and synthetic metabolic agents. However, long-term use of some medications may yield suboptimal therapeutic outcomes and lead to severe adverse reactions. Given the necessity for prolonged or lifelong treatment for OP, there is a critical need to identify highly effective, safe, and cost-effective pharmaceutical interventions. In light of evolving disease management paradigms and recent advancements in OP research, traditional Chinese medicine has demonstrated emerging advantages in addressing this condition. Through literature review, this study delves into the pathogenesis of OP from five perspectives: hormonal dysregulation, autophagy, ferroptosis, oxidative stress, and intestinal flora alteration. Furthermore, it summarizes the therapeutic efficacy and specific mechanisms of traditional Chinese medicine monomers and compound formulas against OP through regulating hormone levels, interfering with autophagy, inhibiting ferroptosis, counteract oxidative stress, and maintain intestinal flora balance. These multifaceted insights are expected to provide theoretical reference and guide future clinical traditional Chinese medicine approaches for preventing and managing OP.

Medical simulation-based assessments have emerged as a vital method for evaluating the clinical competence of healthcare professionals. These assessments provide examinees with opportunities to practice clinical skills in a controlled environment that mirrors real-life medical scenarios. However, assessment organizers must implement specific measures to ensure that these assessments yield valid and reliable results. The reliability of simulation-based assessments is a crucial component of quality assurance. This article delineates twelve practical recommendations to enhance the reliability of medical simulation-based assessments across the three key dimensions of raters, simulation environment, and assessment tools. These recommendations include ① selecting raters with relevant professional backgrounds and extensive experience in simulation-based teaching and assessment; ②providing raters with standardized training for the assessment; ③conducting mock ratings and piloting to reach consensus among raters; ④preventing excessive work hours or cognitively overload while rating; ⑤providing standardized personnel with adequate training and re-training; ⑥implementing immediate and long-term quality assurance measures for standardized patient role performance; ⑦maintaining consistent operation of simulation equipment; ⑧guaranteeing the simulation provides adequate functional fidelity for the examinees; ⑨clearly defining each element and item in assessment tools while controlling their quantity; ⑩ensuring consistency when multiple assessment tools are used; 11using assessment tools with evidence of construct validity when possible; and 12establishing systems to ensure consistency in data (scoring) collection while safeguarding data security. These strategies aim to assist medical educators in developing structured and reliable simulation-based assessment schemes.

Objective To analyze the health economics of ultra-short wave therapy adjuvant to drug therapy for acute pharyngitis, and to provide a more cost-effective treatment option for clinical management. Methods Seventy patients with acute pharyngitis were randomly divided into a control group(35 cases)and an ultra-short wave group(35 cases). The control group received conventional drug therapy, while the ultra-short wave group received ultra-short wave therapy in addition to conventional drug therapy. The cure rate, treatment duration, direct costs, indirect costs, and total costs were compared between the two groups for clinical efficacy and health economics analysis. Results The cure rate in the ultra-short wave group was significantly higher than that in the control group(P=0.008),while the total treatment cost and treatment duration were significantly re-duced.To achieve the same therapeutic effect, the ultra-short wave group required 129.73 yuan less than the control group. Conclusion Ultra-short wave therapy adjuvant to conventional drug treatment for acute pharyngitis can more rapidly alleviate symptoms, reduce indirect costs due to work absence, without significantly increasing treatment expenses. It is a safe, effective, and economical therapy with higher therapeutic value compared to drug therapy alone, and is worthy of widespread promotion.

ORTEC P-type HPGE gamma spectrometry system was used to measure radionuclides. The measurement data were collated and analyzed. Results:The activity concentrations measured were 232Th 18.94-108.39 Bq/kg, 226Ra 14.37-79.20 Bq/kg and 40K 440.03-1 358.18 Bq/kg, in turn with the mean values of (68.22±16.32), (47.90±11.12) and (763.90±133.93) Bq/kg, respectively. The difference in activity concentrations of 232Th, 226Ra and 40K in agricultural soils in different areas was statistically significant( H=50.87, 45.14, 40.28, P<0.05). Conclusions:The study on the activity concentrations of natural radionuclides in agricultural soils provides basic information for the transfer of radionuclides to crops, which needs further investigation, monitoring and analysis.

Chinese Journal of Anesthesiology 2025;45(4):415-418
Objective:To evaluate the effect of ciprofol on intraoperative hypotension in patients undergoing bronchoscopy procedures. Methods:In this randomized controlled study, 112 adult patients of either sex, aged 18-64 yr, with a body mass index of 19.8-28.3 kg/m 2, of American Society of Anesthesiologists Physical Status classification Ⅱ or Ⅲ, with estimated operation time of ≥1 h, undergoing elective bronchoscopy procedures, were assigned to one of two groups ( n=56 each) using a random number table method: ciprofol group (C group) and propofol group (P group). All the patients received total intravenous anesthesia. The induction dose of ciprofol was 0.1-0.3 mg/kg, and the maintenance dose was 0.4-1.2 mg·kg -1·h -1 in group C. The induction dose of propofol was 1-3 mg/kg, and the maintenance dose was 4-12 mg·kg -1·h -1 in group P. The primary outcome was the incidence of intraoperative hypotension, and the secondary outcomes were the time of emergence from anesthesia, sleep quality, patients′ and surgeons′ satisfaction with anesthesia, and the incidence of complications within 30 days after surgery. Results:Compared with group P, the incidence of intraoperative hypotension was significantly decreased, and the time of emergence from anesthesia was shortened in group C ( P<0.05). There was no statistically significant difference in secondary outcomes between the two groups ( P>0.05). Conclusions:Ciprofol is superior to propofol in reducing the risk of intraoperative hypotension and facilitates a more rapid emergence from anesthesia in patients undergoing bronchoscopy procedures.

Objective To analyse the causal relationship between obstructive sleep apnea(OSA)and atrial fibrillation(AF)by Mendelian randomization(MR)method, and to explore the role of inflammatory factors, immune cells, blood metabolites and circulating proteins in it.Methods In this study,MR analysis was used to investigate the causal link between OSA and AF.Inverse variance weighting(IVW),MR-Egger and other methods were used for sensitivity analysis.Additionally, the study also explored the effects of 91 inflammatory factors,731 immune cells,1 400 blood metabolites and 4 907 circulating proteins on AF.Results MR analysis revealed a significant causal relationship between OSA and AF(OR=1.078,P<0.05).Conversely,reverse MR analysis did not find causal relationship between AF and OSA(P>0.05).Further analysis revealed that 5 inflammatory factors,19 immune cells,15 blood metabolites and 67 circulating proteins were risk factors for AF.Among them,OSA was the risk factor for one inflammatory factor, one blood metabolite and 12 circulating proteins. There was no significant causal association between OSA and 19 immune cells. Conclusion This study demonstrates that OSA increases the risk of AF.Meanwhile,it is found that inflammatory factors, immune cells, blood metabolites and circulating proteins have potential impacts on the pathogenesis of AF.
