1.Review and reflection on the practice of pre-ethical review for drug clinical trials

ObjectiveTo sort out the pre-ethical review status of drug clinical trials in a tertiary A hospital, summarize practical experience, and provide references for pre-ethical review. MethodA retrospective analysis was conducted on the ethical review of pre-ethical projects in a tertiary A hospital from 2018 to 2023. ResultsFrom 2018 to 2023, a total of 285 pre-ethical projects were reviewed in this tertiary hospital, including 79 projects where it served as the leading unit. Among these, 279 clinical trials ultimately received approval of the ethical review committee, while 6 projects were not approved due to sponsors’ refusal to modify study protocols. In terms of trial phases, phase III clinical trials constituted the largest proportion, and the oncology center was the department with the highest number of projects. In 2023, the average time for pre-ethical review projects from submission to approval was 43 calendar days, 3 days longer than for other project types. In 2023, this hospital reviewed 75 pre-ethical review projects, including 58 where it served as a participating unit. Among these, 42 projects received approval later than the leading unit, while 9 projects were approved earlier than the leading unit’s ethical approval date. Among the pre-ethical review projects applied in 2023, 70 projects obtained drug clinical trial notifications from the National Medical Products Administration, while 5 projects had unknown notification status due to the lack of ethical approval or discontinuation. Of the projects receiving approval notifications, 16 were annotated with matters requiring enhanced attention during clinical trials, and 7 necessitated protocol improvements. ConclusionThis tertiary A hospital has implemented multiple measures to optimize the management of pre-ethical review. This ethical review model does not compromise the quality of ethical review and contributes to accelerating the initiation of clinical trials. Notably, it is crucial to construct a patient-centered drug development system. Clinical trials guided by this concept align with ethical values, laying the foundation for the smooth conduct of clinical trials, assisting in the drug development process, and safeguarding patients’ medication needs.
2.Quality Control and Analysis of Treatment for Hospitalized Cancer Patients:Interview and Medical Records Study from Nine Hospitals in Beijing

Objective To analyze the current quality of treatment for hospitalized cancer patients in Bei-jing,identify major issues in treatment practices,and propose improvements.Methods Nine hospitals in Beijing were selected for examination.Expert on-site interviews and medical record sampling were conducted.The"Bei-jing Cancer Diagnosis and Treatment Quality Control Checklist"was used to assess the hardware,management,anti-cancer drug therapy,radiation therapy,and surgical treatment during cancer treatment at these hospitals from January to October 2023.The relevant problems were analyzed.Results Among the nine hospitals,two(22.2%)were equipped with laminar flow rooms,and three(33.3%)had intravenous drug preparation centers.In terms of institutional management,seven hospitals(77.8%)had standardized anti-cancer drug prescription authority management,eight(88.9%)had complete emergency plans,and five(55.6%)had oncology specialist pharmacists.Regarding anti-cancer drug therapy,the areas with higher completion rates included pathology diag-nosis support(97.6%),routine pre-treatment examinations(96.3%),adverse reaction evaluation(92.7%),discharge summaries(95.1%),and admission records(91.5%).However,the accuracy of tumor staging before treatment(70.7%)and the evaluation of therapeutic efficacy after drug treatment(76.9%)needed improvement.The oncology specialty significantly outperformed the non-oncology specialty in terms of the accuracy rate of TNM staging(86.0%vs.46.9%,P<0.001),the completeness of informed consent forms(100%vs.68.8%,P<0.001),the completeness of drug indication evaluation(96.0%vs.78.1%,P=0.025),the completeness of admission medical history records(98.0%vs.81.3%,P=0.008),the rationality of drug dosage(96.0%vs.75.0%,P=0.005),the rationality of drug infusion time(100%vs.62.5%,P<0.001),and the rationality of the order of drug infusion(100%vs.87.5%,P=0.010).Although the quality of radiation therapy was high,the subsequent evaluation of therapeutic efficacy(39.3%)requires enhancement.In surgical treatment,the preoper-ative pathology diagnosis support rate(78.1%)and the accuracy of tumor staging(37.5%)were relatively low,indicating issues with incomplete preoperative evaluation and the absence of multidisciplinary discussions.Conclusions There remains significant room for improvement in the quality of cancer treatment in China.It is recommended to standardize tumor staging assessment processes,strengthen entry assessments for non-oncology departments,promote the implementation of multidisciplinary treatment models,and establish a multi-department collaborative management model.Continuous monitoring of cancer diagnosis and treatment quality indicators is es-sential to promote ongoing improvements in cancer treatment quality.
3.Camera inversion technique in laparoscopic sphincter-preserving surgery for mid to low rectal cancer

Objective:To explore the application of the camera inversion technique in laparoscopic sphincter-preserving surgery for mid to low rectal cancer.Methods:A retrospective study with historical controls was conducted on patients with non-metastatic mid to low rectal cancer which received laparoscopic total mesorectal excision at Peking Union Medical College Hospital from January 2019 to June 2024. The experimental group (2021.7-2024.6) utilized the camera inversion technique (rotating the lens 180° to position the bevel upward and switching the system to reverse display mode for improved visualization and operative angles) during key surgical steps (such as intraoperative mobilization of the mid-to-lower rectum and anastomosis), while the control group (2019.1-2021.6) did not. Clinical data and surgical videos were collected to analyze indicators like operative time, blood loss, mesorectal integrity, surgical complications, and postoperative hospital stay.Results:A total of 624 patients with non-metastatic mid to low rectal cancer were included, including 412 males and 212 females, with an average age of 59.8 years and an average tumor distance of 5.6 cm from the anal verge. The experimental group comprised 301 patients, while the control group had 323 patients.The proportion of abdominal ISR (intersphincteric resection) was significantly higher in the experimental group [19.3% (58/301) vs. 10.2%(33/323), χ 2=10.140, P=0.001], with a reduction in operative time [(161.8±67.8) minutes vs. (150.2±68.5) minutes, t=2.134, P=0.033] and a decrease in postoperative hospital stay [(7.8±2.1) days vs. (8.3±3.4) days, t=2.003, P=0.046]. The experimental group also demonstrated advantages in intraoperative blood loss, mesorectal integrity rate, and postoperative complications such as urinary retention, though these differences were not statistically significant (all P>0.05). Conclusion:In laparoscopic surgery for mid to low rectal cancer, using camera inversion technique during distal rectum dissection and transanal anastomosis can provide better surgical field exposure, facilitate precise operations within the correct anatomical plane, and minimize collateral damage. The camera inversion technique is safe and effective.
4.Performance evaluation of AI-enabled blood cell morphology system for peripheral blood smear and application in grading screening network of primary medical care system

Objective To evaluate the recognition capability of AI-enabled Cellsee CS-BM1 automatic cell morphology analyzer for pe-ripheral blood smears and its roles in assisting manual classification,and explore the application value of AI system in the diagnosis network of tiered primary medical units.Methods The blood samples which triggered the re-examination rules were collected from six primary medical units,including the Laboratory Department of Shanghai Jiahui International Hospital,and so on,from March to No-vember 2023.The smears of peripheral blood were prepared and AI analyzer was used for pre-classification to evaluate its recognition performance in identifying the samples with abnormal WBC and RBC.The sensitivity,specificity,and accuracy of WBC classification by six junior and intermediate technicians,both with and without AI assistance,were analyzed.Additionally,the roles of the AI system in tiered diagnosis of primary medical units were also evaluated.Results The sensitivity,specificity,and accuracy of AI system in recognizing malignant primitive cells were 92.86%,95.16%,and 95.10%,respectively.The sensitivities of AI system in recognizing immature granulocytes,reactive lymphocytes,and nucleated RBCs were all greater than 90%.The sensitivity of AI system in identif-ying abnormal morphology of RBCs reached 99.59%,along with rapid quantitative analysis for various anomalous types of RBCs.In AI-assisted mode,the sensitivity of recognition for all cell types was improved to varying degrees by junior and intermediate technicians,and the sensitivity for recognizing malignant primitive cells,reactive lymphocytes,and immature granulocytes increased to 58.24%,53.39%,and 62.37%for junior technicians,and to 92.06%,83.24%,and 83.12%for intermediate technicians,respectively.The improvements for junior technicians were particularly significant,with increases of 12.46%,10.61%,and 3.71%for each cell type,respectively.Both groups achieved higher specificity and accuracy.Through AI pre-classification and manual review,a variety of pe-ripheral blood cell-related diseases were accurately diagnosed in the tiered healthcare practice of primary medical units,including 339 cases(11.13%)of red blood cell diseases,5 cases(0.16%)of platelet diseases,2 343 cases(76.90%)of infection-related disea-ses,and 28 cases(0.92%)of malignant hematological diseases.In addition,332 cases(10.90%)which lacked an obvious related cause or required further examinations were identified as well.Conclusion AI pre-classification has demonstrated strong cell recogni-tion capabilities and may assist technicians in improving the sensitivity,specificity,and accuracy of blood cell classification.AI could en-hance the disease-screening capabilities in the tiered diagnosis network of primary medical units,presenting a broad application prospect.

7.Effects of Regulation of SHH/Gli1 Signaling Pathway by Baishile Capsules on Hippocampal Neurogenesis Depression Model Rats

Objective To explore the effects and mechanism of Baishile Capsules regulating SHH/Gli1 signaling pathway on hippocampal neurogenesis of depression model rats.Methods Totally 32 SD rats were randomly divided into control group,model group,fluoxetine(5.4 mg/kg)group and Baishile Capsules(2.88 g/kg)group,with 8 rats in each group.A depression rat model was established using chronic unpredictable mild stress and single cage feeding method.The model was established and administered simultaneously for 21 consecutive days.Depression-like behavior in rats were evaluated by sucrose preference experiment and open field experiment,ELISA was used to detect brain derived neurotrophic factor(BDNF)contents in rat serum and hippocampal tissue,the number of BrdU,BrdU/DCX,BrdU/NeuN positive cells in dentate gyrus of the hippocampus was observed by immunofluorescence,immunofluorescence and Western blot were used to detect the fluorescence intensity and protein expression of SHH,Gli1,Smo,Ptch in hippocampal tissue.Results Compared with the control group,the degree of sucrose preference significantly decreased in the model group(P<0.01),the number of horizontal and vertical movements significantly decreased(P<0.01),the contents of BDNF in serum and hippocampal tissue significantly decreased(P<0.05),the number of BrdU,BrdU/DCX,BrdU/NeuN positive cells in dentate gyrus of the hippocampus significantly decreased(P<0.01),and the fluorescence intensity and protein expression of SHH,Gli1,Smo,Ptch in hippocampal tissue significantly decreased(P<0.01,P<0.05).Compared with the model group,the degree of sucrose preference and the number of horizontal and vertical movements in fluoxetine group and Baishile Capsule group increased significantly(P<0.05,P<0.01),the contents of BDNF in serum and hippocampal tissue significantly increased(P<0.05,P<0.01),and the number of BrdU,BrdU/DCX,BrdU/NeuN positive cells in dentate gyrus of the hippocampus significantly increased(P<0.01,P<0.05),the fluorescence intensity and protein expressions of SHH,Gli1,Smo,Ptch in hippocampal tissue significantly increased(P<0.01,P<0.05).Conclusion Baishile Capsule can promote the hippocampus neurogenesis in depression model rats by regulating SHH/Gli1 signaling pathway,and play an antidepressant role.
8.Value of multimodal ultrasound parameters combined with peripheral blood lymphocyte-monocyte ratio in predicting recurrence and metastasis after radical breast cancer surgery

Objective:To explore the value of multimodal ultrasound parameters combined with peripheral blood lymphocyte-monocyte ratio (LMR) in predicting recurrence and metastasis after radical breast cancer surgery.Methods:A total of 139 patients with breast cancer admitted to Northwest Women’s and Children’s Hospital from Jan. 2021 to Jan. 2022 were studied. The patients underwent postoperative multimodal ultrasonography and peripheral blood lymphocyte and monocyte examination. Patients were followed up for 2 years after surgery to evaluate postoperative recurrence and metastasis, and analyze the value of multimodal ultrasound parameters combined with LMR in predicting recurrence and metastasis of patients.Results:Among 139 patients, 32 recurrences and metastases occurred during 2-year follow-up; The peak systolic velocity (PSV), resistance index (RI), maximum lesion diameter, elasticity score, area under the curve (AUC) and peak intensity (PI) in patients with recurrent metastasis were higher than those without recurrent metastasis ( t=7.37, 4.75, 4.75, 5.46, 4.24, 5.22, P<0.05), and the LNR in patients with recurrent metastasis was lower. Pearson correlation analysis showed that PSV, RI, maximum lesion diameter, elasticity score, AUC, PI were negatively correlated with LMR ( r=-0.418, -0.502, -0.633, -0.526, -0.448, -0.482). P<0.05) ; The ROC curve showed that the AUC of PSV, RI, maximum lesion diameter, elasticity score, AUC, PI and LMR for predicting recurrence and metastasis were 0.852, 0.784, 0.791, 0.789, 0.615, 0.725 and 0.728, respectively, and the combined AUC value of each index was 0.992. Conclusions:The use of multimodal ultrasound parameters combined with LMR can improve the prediction value of recurrence and metastasis after radical breast cancer surgery.

10.Diagnostic value of four types of breast BI-RADS nodules by constructing a nomogram based on multi-modal ultrasound features combined with inflammatory complex indicators

Objective:To explore the diagnostic value of the nomogram constructed based on multi-modal ultrasound features combined with inflammatory complex indicators for benign and malignant nodules in breast imaging reporting and data system (BI-RADS) .Methods:102 female patients with breast BI-RADS type 4 nodules who were treated in Northwest Women’s and Children’s Hospital from Jan. 2021 to Mar. 2024 were included as study objects, and benign and malignant nodules were determined according to postoperative pathological examination results. Conventional ultrasound, shear-wave elastography and contrax-ultrasound were performed in each breast BI-RADS patient with Class 4 nodules using Mindray resona7 ultrasonic diagnostic instrument within 1 week before surgery to record the ultrasonic characteristics. According to the results of laboratory examination, neutrophil to lymphocyte ratio (NLR) and platelet to lymphocyte ratio (NLR), PLR, the systemic immune inflammation index (SII), and the systemic inflammatory response index (SIRI) were calculated and recorded.Results:There were 42 cases of malignant nodules and 60 cases of benign nodules. Compared with the benign group, there were significant differences in nodule boundary, internal echo, Alder blood flow grading, hard ring sign, area expansion after enhancement, peripheral radial enhancement and perfusion defect in the malignant group ( χ2 value was 5.83, 9.31, 5.20, 10.25, 6.99, 7.24, 4.90, P < 0.05). Compared with the benign group, NLR and SIRI in the malignant group were significantly increased, with statistical difference (t-value 3.13 and 4.33, P < 0.05). The results of multi-factor Logistics regression analysis show that breast BI-RADS were characterized by blurred nodule boundary (X1), Alder blood flow grade 2-3 (X3), hard ring sign (X4), enlarged area after enhancement (X5), peripheral radial enhancement (X6), NLR≥2.29 (X8), SIRI≥0.91 (X9) were independent risk factors for malignant nodules ( P < 0.05). Based on these factors, the Logistics regression model was established as Logit (P) = -3.217 + 0.291X1 + 1.051X3 + 0.354X4 + 0.360X5 + 0.524X6 + 0.513X8 + 0.726X9. The results of calibration curve and clinical decision curve evaluation showed that the model had good accuracy and high predictive value. ROC curve analysis results showed that the area under the curve of the nomogram model for predicting benign and malignant nodules of category 4 breast BI-RADS was 0.925 (0.877-0.974), the sensitivity was 88.6%, and the specificity was 86.1%. Conclusion:The nomogram model constructed by multi-modal ultrasound combined with inflammatory complex index NLR and SIRI has certain application value in the differential diagnosis of benign and malignant breast BI-RADS 4 nodules.
