1.Review and reflection on the practice of pre-ethical review for drug clinical trials

ObjectiveTo sort out the pre-ethical review status of drug clinical trials in a tertiary A hospital, summarize practical experience, and provide references for pre-ethical review. MethodA retrospective analysis was conducted on the ethical review of pre-ethical projects in a tertiary A hospital from 2018 to 2023. ResultsFrom 2018 to 2023, a total of 285 pre-ethical projects were reviewed in this tertiary hospital, including 79 projects where it served as the leading unit. Among these, 279 clinical trials ultimately received approval of the ethical review committee, while 6 projects were not approved due to sponsors’ refusal to modify study protocols. In terms of trial phases, phase III clinical trials constituted the largest proportion, and the oncology center was the department with the highest number of projects. In 2023, the average time for pre-ethical review projects from submission to approval was 43 calendar days, 3 days longer than for other project types. In 2023, this hospital reviewed 75 pre-ethical review projects, including 58 where it served as a participating unit. Among these, 42 projects received approval later than the leading unit, while 9 projects were approved earlier than the leading unit’s ethical approval date. Among the pre-ethical review projects applied in 2023, 70 projects obtained drug clinical trial notifications from the National Medical Products Administration, while 5 projects had unknown notification status due to the lack of ethical approval or discontinuation. Of the projects receiving approval notifications, 16 were annotated with matters requiring enhanced attention during clinical trials, and 7 necessitated protocol improvements. ConclusionThis tertiary A hospital has implemented multiple measures to optimize the management of pre-ethical review. This ethical review model does not compromise the quality of ethical review and contributes to accelerating the initiation of clinical trials. Notably, it is crucial to construct a patient-centered drug development system. Clinical trials guided by this concept align with ethical values, laying the foundation for the smooth conduct of clinical trials, assisting in the drug development process, and safeguarding patients’ medication needs.
2.Research Progress on the Regulation of Growth Plate Endochondral Ossification by Mechanical Loading

The endochondral ossification of the growth plate is crucial for the longitudinal growth of long bones. Mechanical loading plays an important regulatory role in endochondral ossification. Absence or improper mechanical loading can inhibit growth plate development, while moderate mechanical stimuli promote chondrocyte proliferation and differentiation, thereby facilitating long bone growth. Mechanosensitive structures, such as primary cilia, mechanosensitive ion channels, integrins, and the cytoskeleton, play key roles in the regulation of mechanical loading. This review summarizes how mechanical loading influences growth plate endochondral ossification and discusses potential underlying mechanisms, offering theoretical insights for the management and research of bone development disorders in children and adolescents.
3.Exploration of epidemiological characteristics of multidrug-resistant organisms among burn wound patients and prevention and control strategies based on worldwide database for nosocomial outbreaks

OBJECTIVE To investigate the epidemiological characteristics of hospital-associated infections caused by multidrug-resistant organisms(MDROs)among the burn wound patients so as to provide bases for taking tar-geted control measures.METHODS A systematic search was conducted in the worldwide database for nosocomial outbreaks,PubMed and CNKI databases so as to summarize and analyze the data regarding to outbreaks of MDROs hospital-associated infections among burn wound patients.RESULTS A total of 61 incidents of MDROs hospital-associated infections outbreaks among the burn wound patients were included,involving 2 293 patients from 21 countries and regions,50(81.97%)of which were reported for the infection sites or colonization sites in-volving burn wound,12(19.67%)involving the respiratory tract,10(16.39%)involving the bloodstream infec-tions.Methicillin-resistant Staphylococcus aureus(28 incidents,45.90%)was dominant among the pathogens causing the infections,followed by multidrug-resistant Acinetobacter baumannii(17 incidents,27.87%)and multidrug-resistant Pseudomonas aureus(9 incidents,14.75%).52 incidents(82.25%)of outbreaks were reported the contact as the major transmission mode.The suspected sources of the outbreaks included the patients(37 incidents,28.46%),health care workers(30 incidents,23.08%),ward environments(28 incidents,21.54%),medical equipments(19 incidents,30.56%),drainage systems(6 incidents,4.62%).The major pre-vention and control measures included environmental cleaning and disinfection,screening of colonization in patients and health care workers,isolation of patients with infections and hand hygiene;8 incidents were taken the measure of closing the ward.CONCLUSIONS The outbreaks of MDROs infections in the burn wound patients are mostly associated with the high frequently contact environments,medical equipments and hand hygiene of health care workers.In view of the peculiarities of the burn wound patients,it is feasible to take the targeted measures based on the summarized prevention and control combinations for MDROs so as to prevent the outbreak of hospital-asso-ciated infections.
4.Prevalence survey of implementation process of special campaign for enhancing pathogen detection rate before antimicrobial therapy in hospitalized patients of 31 hospitals

OBJECTIVE To understand the current management status of the special campaign for enhancing the pathogen detection rate before antimicrobial therapy in hospitalized patients in China,analyze identified issues dur-ing implementation,and propose improvement suggestions.METHOD A questionnaire survey was conducted to collect data from medical institutions participating in the special campaign within the region from May to Jul.2024,covering aspects such as quality control management,coordination mechanisms,data sources,indicator connotations and existing issues.RESULTS Variations were observed among medical institutions in the manage-ment of pathogen detection rates,primarily reflected in inconsistent usage of detection rate indicators,varying im-plementation levels of quality control measures and differences in multi-departmental participation.Additionally,discrepancies in data sources,statistical methods and interpretations of indicator connotations limited the compara-bility of data.CONCLUSIONS To address these issues,improvement measures such as strengthening informatiza-tion construction,standardizing indicator statistical methods and enhancing multi-departmental coordination mech-anisms should be implemented.These efforts will provide a scientific basis for the implementation of special cam-paign and robust support for the rational use of antimicrobial agents.
5.Effect of Simo decoction in improving low-grade inflammation of duodenum and protecting mucosal barrier in functional dyspepsia rats

Objective:To explore the effect of Simo decoction improving the low duodenal inflammation and protecting the du-odenal mucosal barrier in rats with functional dyspepsia(FD).Methods:Sixty SD rats were randomly divided into control group(10 rats)and model group(50 rats),and the modeling rats were prepared by multivariate intervention method.After successful modeling,the modeling rats were randomly divided into model group,Simo decoction high-dose,medium-dose,low-dose group and mosapride group,with 10 rats in each group.The high-dose,medium-dose,and low-dose groups of Simo decoction were ga-vage given 5.62 g/kg,2.81 g/kg,and 1.40 g/kg,respectively,and the mosapride group was gavage given 0.305 mg/kg of mosapride,and the control group and model group were gavage given the same amount of distilled water for 14 days.The body weight of rats was observed;gastric emptying rate and small bowel propulsion rate were measured;transmission electron microscopy was used to observe the morphology of duodenal epithelial cells;ELISA detected serum levels of IL-17A and IL-22;Western blot and immunohistochemis-try were used to detect the expressions of protease-activated receptor 2(PAR-2)and tight junction protein(ZO-1,claudin-1)in the duodenum.Results:Compared with the control group,the body weight,gastric emptying rate and small intestinal propulsion rate of the model group were significantly reduced(P<0.01),transmission electron microscopy showed widening of the duodenal epithelial cell space,serum IL-17A and IL-22 levels were significantly increased(P<0.01),the expression of PAR-2 in duodenal tissue was in-creased,and the expressions of ZO-1 and claudin-1 were downregulated(P<0.01).Compared with model group,the gastric emptying rate and small intestinal propulsion rate in Simo decoction high-dose,medium-dose and mosapride group were increased(P<0.05,P<0.01),the contents of IL-17A and IL-22 in serum decreased(P<0.05,P<0.01),and the expression of PAR-2 in duodenal tissues was down-regulated,the expressions of ZO-1 and claudin-1 was significantly increased(P<0.01).The low-dose group of Simo soup could improve weight loss(P<0.01),reduce IL-17A content and PAR-2 expression,and increase ZO-1 and claudin-1 expression(P<0.05,P<0.01),while the effect on other indexes was not obvious.Conclusion:Simo decoction may reduce low-grade duodenal in-flammation to repair the mucosal barrier by down-regulating the levels of IL-17A and IL-22 and the expression of PAR-2,and up-regu-lating the expression of ZO-1 and claudin-1,so as to exert the effect of FD treatment.
6.Comparative study of selective transurethral plasma kinetic resection of the prostate and transurethral holmium laser enucleation of the prostate for residual urine volume, maximum flow rate and urethral stricture in patients with diabetes benign prostatic hyperplasia

Objective:To explore the application value of selective transurethral plasma kinetic resection of the prostate (PKRP) and transurethral holmium laser enucleation of the prostate (HoLEP) in the treatment of diabetes benign prostatic hyperplasia (BPH).Methods:A total of 114 patients with diabetes BPH diagnosed and treated in Lanxi People′s Hospital from September 2020 to September 2023 were retrospectively selected and divided into resection group and enunciation group according to the operation method, with 57 cases in each group. The resection group received selective PKRP and the enucleation group received HoLEP. Preoperative and postoperative stress response indexes were compared between the two groups, as well as urodynamics indexes and complications before and 3 months after surgery.Results:The quality of prostatectomy in the enucleation group was higher than that in the resection group : (39.68 ± 3.02) g vs. (30.25 ± 2.84) g; the operation time, intraoperative blood loss, bladder irrigation time and hospital stay in the enucleation group were lower than those in the resection group: (63.84 ± 5.44) min vs. (72.58 ± 11.33) min, (62.38 ± 15.84) ml vs. (89.54 ± 17.91) ml, (1.84 ± 0.71) d vs. (2.35 ± 0.80) d, (4.98 ± 1.16) d vs.(5.64 ± 1.05) d, there were statistical differences ( P<0.05). The levels of serum cortisol (COR) and norepinephrine (NE) in the enucleation group at 1, 3 d after surgery were lower than those in the excision group [1 d after surgery: (279.76 ± 17.82) ng/L vs. (289.51 ± 18.37) ng/L, (287.44 ± 26.51) ng/L vs. (300.47 ± 28.35) ng/L; 3 d after surgery: (236.58 ± 15.98) ng/L vs. (247.46 ± 16.47) ng/L, (269.13 ± 23.51) ng/L vs. (278.76 ± 25.62) ng/L, there were statistical differences ( P<0.05). Three months after surgery, the maximum urine flow rate in the enucleation group was higher than that in the excision group : (21.51 ± 3.82) ml/s vs. (18.74 ± 4.17) ml/s, and the residual urine volume (RUV) was lower than that in the excision group : (8.47 ± 3.06) ml vs. (10.25 ± 2.43) ml, there were statistical differences ( P<0.05). There was no significant difference in the incidence of urethral stricture and urinary incontinence between the two groups ( P>0.05). Conclusions:HoLEP for the treatment of diabetes BPH can reduce stress response, promote disease recovery, improve urodynamics, and has a safety comparable to that of selective PKRP.
7.Precision Theranostics in Nuclear Medicine Targeting Gastrin-Releasing Peptide Receptor

The gastrin-releasing peptide receptor(GRPR)is highly expressed in various malignant tumors(e.g.,glioblastoma,prostate cancer,and breast cancer)and has emerged as an important molecular target for precision cancer diagnosis and therapy.In recent years,the combined application of radionuclide-la-beled GRPR ligands with positron emission tomography/computed tomography(PET/CT)has achieved ground-breaking progress in noninvasive diagnosis,accurate staging,and therapeutic monitoring of GRPR-positive tumors.Meanwhile,GRPR-targeted radionuclide therapeutics have demonstrated significant therapeutic potential and promising clinical applications in trials.This GRPR-based theranostic strategy pioneers a novel ap-proach for precision medicine in GRPR-overexpressing tumors.
8.Disorder of Cerebrospinal Fluid Circulation Induced by Mesencephalic Aqueduct Stenosis:A Computational Fluid Dynamics Analysis

Objective To study the effects from varying stenosis degrees of the mesencephalic aqueduct on intracranial cerebrospinal fluid(CSF)flow field.Methods Based on the clinical magnetic resonance image sequences of a male volunteer,a complete normal CSF circulation model was reconstructed by using semi-automated image segmentation technique.Subsequently,eight ideal models representing different stenosis degrees of the mesencephalic aqueduct were manually created.Computational fluid dynamics(CFD)was then performed to simulate the CSF flow field in the nine models.Results The stenosis degree of the mesencephalic aqueduct was positively correlated with the maximum pressure difference between the aqueduct upstream and downstream and the maximum velocity of CSF within the stenosed aqueduct.In the normal model,the maximum pressure difference was 0.84 Pa and the maximum velocity was 11.4 mm/s.While in the maximum stenosed model,the maximum pressure difference and velocity were 21.36 Pa and 60.3 mm/s,respectively.Compared to the normal model,the maximum pressure difference and velocity were approximately increased by 25 times and 5 times,respectively.Moreover,the maximum pressure difference was inversely proportional to the stenosis area square of the aqueduct,and there was a linear relationship between the pressure difference and the quadratic of the maximun CSF velocity.Conclusions The pressure difference and velocity of the stenosed mesencephalic aqueduct was not apparently increased with mild stenosis with respect to the normal aqueduct,while the great aqueductal stenosis increased the risk of hydrocephalus.This study provides a theoretical framework which contributes to understanding the development of obstructive hydrocephalus and intracranial hypertension.
9.Effect of exercise on the function and size of the liver in adolescents

Objective To evaluate the effects of physical exercise on liver function and size in adolescents.Methods A total of 361 adolescent subjects from Jining,Shandong Province were selected and divided into low,moderate,and high-intensity exercise groups according to the frequency of moderate intensity exercise.Physical ex-amination tests were conducted including morphological indicators,blood biochemical indicator measurements,liver size and the presence of non-alcoholic fatty liver disease.Independent sample t-tests and variance analysis were per-formed using SPSS 26.0,and correlation analysis was conducted using Spearman correlation analysis and multivari-ate linear regression analysis.Results Compared with the moderate and low-intensity exercise groups,the high-intensity exercise group had significantly lower diastolic blood pressure,homeostatic model assessment of insulin resistance(HOMA-IR),and triglycerides(TG),as well as significantly higher height,maximum oblique diameter of the right liver lobe,and direct bilirubin(DBIL).Correlation analysis showed that as exercise intensity in-creased,aspartate aminotransferase/alanine aminotransferase,DBIL,and the maximum oblique diameter of the right liver lobe increased significantly(P<0.05).The results of the cumulative effect of exercise type and intensity indicate that exercise type does not influence the effect of exercise intensity on liver function and size.Conclusions Regular exercise in adolescents can not only shape body,but also improve glucose and lipid metabolism and en-hance liver function.
10.Multidimensional analysis of diagnosis and treatment status of chronic hepatitis B

Objective:The aim was to analyze key indicators for the diagnosis and treatment of chronic hepatitis B(CHB),including virus detection rate,standardized treatment rate,and loss to follow-up rate,in order to provide a basis for optimizing diagnosis and treatment plans,improve the diagnosis and treatment level of CHB,and improve patient prognosis.Methods:Patients with CHB admitted to the Guangzhou Eighth People′s Hospital Affiliated to Guangzhou Medical University from January 2024 to January 2025 were enrolled. The datas were collected and organized using Excel. Statistical analysis was conducted using SPSS 26.0 software,with a focus on evaluating core indicators such as virus detection rate,standardized treatment rate,and loss to follow-up rate.Results:The positive rate of hepatitis B surface antigen(HBsAg)in non-specific patients was 28.95%,the antiviral treatment rate in specialized patients was 90.78%,and the standardized antiviral drug conversion treatment rate in low-level viremia(LLV)patients was 61.45%. The standardized antiviral drug conversion treatment rates for high-risk patients with combined kidney/bone injuries were 72.75% and 74.40%,respectively. The overall dropout rate was 10.47%,with a dropout rate of 13.80% for LLV patients.Conclusions:The antiviral treatment coverage rates in CHB patients were over 90%,but in certain groups(such as LLV patients and those with kidney or bone injuries),the standardized treatment rates were still low and loss to follow-up rates were high,suggesting the need to improve HBV screening,treatment for special populations,and patient adherence.
