1.Analysis of thermal environment and students thermal comfort in primary and secondary school classrooms in winter

Objective:
To evaluate the current situation of thermal environment in primary and secondary school classrooms during winter, and to analyze students thermal comfort needs, so as to provide a basis for improving classroom thermal environment.
Methods:
From December 16 to 26, 2024, a stratified cluster random sampling method was used to select 90 classrooms from 15 primary and secondary schools in centralized/air conditioned heating areas(Liaoning Province, Tianjin City, Shanghai City) and naturally ventilated areas(Anhui Province and Jiangxi Province)for on site environmental measurement. A questionnaire survey was conducted among 743 students. The differences between groups using the χ 2 test were compared. Based on actual measurement data, a predicted mean vote prepared percentage of dissatisfied (PMV-PPD) model for centralized/air conditioned classrooms and an adaptive model for naturally ventilated classrooms were established, and the thermal neutral temperature and comfort interval were calculated.
Results:
The average outdoor temperature during on site measurement was 4.00(0.20,7.00)℃. In classrooms with centralized or air conditioned heating systems, the measured average temperature was (19.33±2.59)℃, with a thermal comfort range of 20.35-25.35 ℃ and a thermal neutral temperature of 22.85 ℃. And 13.92% of students reported feeling cold, while 80.80% felt comfortable. In classrooms with natural ventilation, the measured average temperature was (12.26±1.83)℃, with a thermal neutral temperature of 19.67 ℃ and a thermal comfort range of 16.17-23.17 ℃. About 48.33% of students reported feeling cold, and 49.81 % felt comfortable.The results of univariate analysis showed that there were statistically significant differences in shoe thickness, temperature sensation, relative humidity sensation and wind speed sensation between centralized/air conditioned heating areas ( χ 2= 7.01 , 31.47, 13.57, 13.80,all P <0.05). There were also statistically significant differences in school stage for primary and secondary school students, body mass index, classroom location for seat, temperature sensation, relative humidity sensation and wind speed sensation between naturally ventilated areas ( χ 2=42.13, 11.13, 11.04, 60.39, 29.27, 38.46,all P <0.05).
Conclusions
There are differences in thermal environment and students subjective thermal comfort in primary and secondary schools under different ventilation modes in winter. The temperature standards for heated classrooms should be revised, and differentiated environmental regulation strategies should be adopted based on different ventilation methods to improve students health and comfort levels.
2.Therapeutic role of miR-26a on cardiorenal injury in a mice model of angiotensin-II induced chronic kidney disease through inhibition of LIMS1/ILK pathway.

BACKGROUND:
Chronic kidney disease (CKD) is associated with common pathophysiological processes, such as inflammation and fibrosis, in both the heart and the kidney. However, the underlying molecular mechanisms that drive these processes are not yet fully understood. Therefore, this study focused on the molecular mechanism of heart and kidney injury in CKD.
METHODS:
We generated an microRNA (miR)-26a knockout (KO) mouse model to investigate the role of miR-26a in angiotensin (Ang)-II-induced cardiac and renal injury. We performed Ang-II modeling in wild type (WT) mice and miR-26a KO mice, with six mice in each group. In addition, Ang-II-treated AC16 cells and HK2 cells were used as in vitro models of cardiac and renal injury in the context of CKD. Histological staining, immunohistochemistry, quantitative real-time polymerase chain reaction (PCR), and Western blotting were applied to study the regulation of miR-26a on Ang-II-induced cardiac and renal injury. Immunofluorescence reporter assays were used to detect downstream genes of miR-26a, and immunoprecipitation was employed to identify the interacting protein of LIM and senescent cell antigen-like domain 1 (LIMS1). We also used an adeno-associated virus (AAV) to supplement LIMS1 and explored the specific regulatory mechanism of miR-26a on Ang-II-induced cardiac and renal injury. Dunnett's multiple comparison and t -test were used to analyze the data.
RESULTS:
Compared with the control mice, miR-26a expression was significantly downregulated in both the kidney and the heart after Ang-II infusion. Our study identified LIMS1 as a novel target gene of miR-26a in both heart and kidney tissues. Downregulation of miR-26a activated the LIMS1/integrin-linked kinase (ILK) signaling pathway in the heart and kidney, which represents a common molecular mechanism underlying inflammation and fibrosis in heart and kidney tissues during CKD. Furthermore, knockout of miR-26a worsened inflammation and fibrosis in the heart and kidney by inhibiting the LIMS1/ILK signaling pathway; on the contrary, supplementation with exogenous miR-26a reversed all these changes.
CONCLUSIONS
Our findings suggest that miR-26a could be a promising therapeutic target for the treatment of cardiorenal injury in CKD. This is attributed to its ability to regulate the LIMS1/ILK signaling pathway, which represents a common molecular mechanism in both heart and kidney tissues.

3.Efficacy and safety of avatrombopag in the treatment of thrombocytopenia after umbilical cord blood transplantation.

BACKGROUND:
Delayed platelet engraftment is a common complication after umbilical cord blood transplantation (UCBT), and there is no standard therapy. Avatrombopag (AVA) is a second-generation thrombopoietin (TPO) receptor agonist (TPO-RA) that has shown efficacy in immune thrombocytopenia (ITP). However, few reports have focused on its efficacy in patients diagnosed with thrombocytopenia after allogeneic hematopoietic stem cell transplantation (allo-HSCT).
METHODS:
We conducted a retrospective study at the First Affiliated Hospital of the University of Science and Technology of China to evaluate the efficacy of AVA as a first-line TPO-RA in 65 patients after UCBT; these patients were compared with 118 historical controls. Response rates, platelet counts, megakaryocyte counts in bone marrow, bleeding events, adverse events and survival rates were evaluated in this study. Platelet reconstitution differences were compared between different medication groups. Multivariable analysis was used to explore the independent beneficial factors for platelet implantation.
RESULTS:
Fifty-two patients were given AVA within 30 days post-UCBT, and the treatment was continued for more than 7 days to promote platelet engraftment (AVA group); the other 13 patients were given AVA for secondary failure of platelet recovery (SFPR group). The median time to platelet engraftment was shorter in the AVA group than in the historical control group (32.5 days vs . 38.0 days, Z = 2.095, P = 0.036). Among the 52 patients in the AVA group, 46 achieved an overall response (OR) (88.5%), and the cumulative incidence of OR was 91.9%. Patients treated with AVA only had a greater 60-day cumulative incidence of platelet engraftment than patients treated with recombinant human thrombopoietin (rhTPO) only or rhTPO combined with AVA (95.2% vs . 84.5% vs . 80.6%, P <0.001). Patients suffering from SFPR had a slightly better cumulative incidence of OR (100%, P = 0.104). Patients who initiated AVA treatment within 14 days post-UCBT had a better 60-day cumulative incidence of platelet engraftment than did those who received AVA after 14 days post-UCBT (96.6% vs . 73.9%, P = 0.003).
CONCLUSION
Compared with those in the historical control group, our results indicate that AVA could effectively promote platelet engraftment and recovery after UCBT, especially when used in the early period (≤14 days post-UCBT).

4.Risk factors for excessive microbial contamination levels of repaired endoscopes before reuse

OBJECTIVE To analyze the risk factors for excessive microbial contamination level tested for repaired endoscopes before reuse and to formulate targeted management strategies,providing a reference for handling such events.METHODS A total of 54 repaired endoscopes reused in the Digestive Endoscopy Room of a hospital in Shanxi Province from Oct.2021 to Jun.2024,with a total of 105 repairs,were selected as the study subjects.Mi-crobial contamination levels were tested for all repaired endoscopes before reuse,and they were divided into a posi-tive group(colony count>20 CFU/item)and a negative group(colony count ≤20 CFU/item)based on the test results.The risk factors for excessive microbial contamination levels of repaired endoscopes before reuse were summarized.The positive rates of microbial contamination level tests for repaired endoscopes before reuse were compared between the pre-intervention period(from Oct.2021 to Jun.2024)and the post-intervention period(from Jul.2024 to Dec.2024).RESULTS The results of microbial contamination level tests of endoscopes with 105 repairs before reuse showed a positive rate of 27.62%(29/105).Shortening duration between cleaning and disin-fection before and after repair(OR=0.285)was a protective factor,scratches/grooves/leak repairs in the endo-scope tubes(OR=3.211),improper cleaning and disinfection(OR=5.257)and the less number of enzymatic brushing washes(OR=2.438)were identified as risk factors for excessive microbial contamination levels of re-paired endoscopes before reuse(P<0.05).Before the intervention,the positive rate of microbial contamination level tests for repaired endoscopes was higher than that for non-repaired endoscopes(27.62%vs.3.45%,P<0.001).After the intervention,there was no statistically significant difference in the positive rate of microbial con-tamination level tests between repaired and non-repaired endoscopes(5.26%vs.4.17%,P=0.681),but the posi-tive rate for repaired endoscopes was lower than that before the intervention(5.26%vs.27.62%,P=0.004).CONCLUSIONS The occurrence of excessive microbial contamination levels of repaired endoscopes before reuse frequemly occur.Formulating and implementing targeted management strategies may guarantee the qualified rate of cleaning and disinfection of endoscopes before the reuse,and enhance the safety for the reuse of repaired endoscopes.
5.PARylation promotes acute kidney injury via RACK1 dimerization-mediated HIF-1α degradation.

Poly(ADP-ribosyl)ation (PARylation) is a specific form of post-translational modification (PTM) predominantly triggered by the activation of poly-ADP-ribose polymerase 1 (PARP1). However, the role and mechanism of PARylation in the advancement of acute kidney injury (AKI) remain undetermined. Here, we demonstrated the significant upregulation of PARP1 and its associated PARylation in murine models of AKI, consistent with renal biopsy findings in patients with AKI. This elevation in PARP1 expression might be attributed to trimethylation of histone H3 lysine 4 (H3K4me3). Furthermore, a reduction in PARylation levels mitigated renal dysfunction in the AKI mouse models. Mechanistically, liquid chromatography-mass spectrometry indicated that PARylation mainly occurred in receptor for activated C kinase 1 (RACK1), thereby facilitating its subsequent phosphorylation. Moreover, the phosphorylation of RACK1 enhanced its dimerization and accelerated the ubiquitination-mediated hypoxia inducible factor-1α (HIF-1α) degradation, thereby exacerbating kidney injury. Additionally, we identified a PARP1 proteolysis-targeting chimera (PROTAC), A19, as a PARP1 degrader that demonstrated superior protective effects against renal injury compared with PJ34, a previously identified PARP1 inhibitor. Collectively, both genetic and drug-based inhibition of PARylation mitigated kidney injury, indicating that the PARylated RACK1/HIF-1α axis could be a promising therapeutic target for AKI treatment.
6.Role of vascular microenvironment disorder in pathological process of cervical spondylosis of vertebral artery type

Vascular homeostasis is a cornerstone for the normal operation of body tissues and organs,and it is closely related to the pathophysiological processes of cardiovascular and other systemic diseases.The maintenance of vas-cular homeostasis relies on a normal vascular microenvironment,and disturbances in this microenvironment are key factors in the development and progression of diseases.In recent years,significant progress has been made in the study of endo-thelial damage and hemodynamics in the pathogenesis of cervical spondylosis of vertebral artery type(CSA).However,these aspects represent only part of the vertebral artery microenvironment disorder and do not fully explain the pathogenesis of CSA.Therefore,this paper reviews endothelial dysfunction,abnormal vasomotor function of vascular smooth muscle,damage perception effects of the vascular adventitia,and matrix microenvironment disorders.The aim is to systematically elucidate the role of vascular microenvironment disorder in the pathological process of CSA,providing new perspectives for research into the disease's pathogenesis and therapeutic targets.
7.Percutaneous Pulmonary Artery Denervation for the Treatment of Isolated Post-Capillary Pulmonary Hypertension Associated With Heart Failure: A Report of 2 Cases

Although percutaneous pulmonary artery denervation (PADN) has demonstrated long-term benefits in combined post- and pre-capillary pulmonary hypertension, its usefulness for isolated post-capillary pulmonary hypertension (IpcPH) remains unclear. This report presents 2 cases of IpcPH due to left heart failure who underwent PADN treatment following an insufficient response to optimized conventional therapy. This case study illustrates how PADN, when combined with heart failure guideline-directed medical therapy, may represent a promising treatment option for IpcPH.
8.Meta-synthesis of qualitative studies on the experience of body quality management in overweight or obese pregnant women during pregnancy

Objective:To systematically evaluate the experience of body quality management in overweight or obese pregnant women during pregnancy, so as to provide reference for developing targeted intervention plans.Methods:Searched China National Knowledge Infrastructure, Wanfang, VIP, Chinese Biomedical Database, PubMed, Web of Science, CINAHL, Embase, Medline databases to screen the qualitative studies on the experience of body quality management of overweight or obese pregnant women during pregnancy, and integrated them with a pooled integration method. The search deadline was from database creation to December 11, 2024.Results:A total of 11 literatures were included and 32 results were extracted, which were summarized into 9 categories and integrated into 3 results including complex emotional experience, face a variety of challenges and difficulties; strategies to improve body quality management ability.Conclusions:Medical staff should pay attention to the psychological experience of body quality management in overweight or obese pregnant women during pregnancy, actively mobilize their positive emotions, constantly strengthen their social support system, and break through the dilemma of body quality management.
