1.Effects of Electroacupuncture at Acute Phase on Cognitive Dysfunction at Later Stage of MCAO Rats

Objective To explore the effects and potential mechanism of electroacupuncture at acute phase on cognitive dysfunction at later stage in rats with middle cerebral artery occlusion(MCAO).Methods Totally 39 SD rats were randomly divided into sham-operation group,model group and electroacupuncture group,with 13 rats in each group.MCAO model was established by Zea Longa thread embolism method in model group and electroacupuncture group.The sham-operation group only separated blood vessels.Electroacupuncture group was treated with electroacupuncture at"Baihui"and"Shenting"acupoints at 5 min and 6 h after model establishment respectively,for 30 min.The body mass,modified neuropathy severity scale(mNSS),and percentage of stiff immobility time of rats were monitored weekly,Nissl staining was used to observe the morphology of hippocampal tissue on the 28 d,immunofluorescence staining was used to detect the expression of Ki67,dual cortin(DCX)and neuronal differentiation factor 1(NeuroD1)in dentate gyrus of the hippocampus,the expression of synaptic vesicle protein 2A(SV2A)protein in dentate gyrus of hippocampus was detected by Western blot.Results Compared with the sham-operation group,on the first day after operation,the body mass of the model group rats decreased,then gradually increased,and on the 28th day,the body mass was still lower than that of the sham-operation group(P<0.05);on the first day after operation,the mNSS score significantly increased,gradually decreased thereafter,and remained higher than the sham-operation group until the 28th day(P<0.01);the percentage of stiff immobility time significantly decreased on the first day after operation,gradually stabilized,and remained lower than the sham-operation group on the 28th day(P<0.05);the hippocampal tissue structure was significantly damaged,and the density of Ki67+DCX+NeuroD1 positive cells in the dentate gyrus of hippocampus was significantly increased(P<0.05),the expression of SV2A protein in the dentate gyrus of hippocampal was significantly decreased(P<0.05).Compared with the model group,the body mass of rats in the electroacupuncture group showed an increasing trend(P<0.05),the mNSS score decreased(P<0.01),and the percentage of stiff immobility time increased(P<0.05);the ischemic injury area was significantly reduced,the structure was clearer,and the disordered and irregularly cells arrangement were reduced,the density of Ki67+DCX+NeuroD1 positive cells in the dentate gyrus of hippocampus significantly increased(P<0.05),and the expression of SV2A protein in the dentate gyrus of hippocampal significantly increased(P<0.05).Conclusion Electroacupuncture at acute phase at"Baihui"and"Shenting"can promote the recovery of cognitive dysfunction in MCAO rats in the later stage,and its mechanism may be related to enhancing the proliferation and differentiation of neural precursor cells in the dentate gyrus of hippocampus,promoting neurogenesis.
2.Application of metagenomic next-generation sequencing in pulmonary tuberculosis complicated with pulmonary nontuberculous mycobacterial lung disease

Objective:To explore the application value of metagenomic next-generation sequencing (mNGS) in the diagnosis of pulmonary tuberculosis (PTB) combined with non-tuberculous mycobacterial pulmonary disease (NTM-PD), compare it with conventional pathogen detection methods, and analyze its clinical characteristics.Methods:A retrospective analysis was conducted on the clinical data of 72 patients with PTB complicated by NTM-PD who were admitted to Nanjing Hospital Affiliated to Nanjing University of Chinese Medicine (Nanjing Second Hospital) from November 2021 to November 2023. All patients were diagnosed using bronchoalveolar lavage fluid (BALF) mNGS and compared with traditional culture methods. Data on patient demographics, clinical manifestations, imaging features, and treatment outcomes were collected to analyze the advantages and clinical significance of mNGS in diagnosis.Results:The positive detection rate of Mycobacterium tuberculosis by mNGS was 100% (72/72), significantly higher than that of traditional culture 27.8%(20/72). The diagnostic time of mNGS was significantly shorter (2 d vs. 45 - 60 d, P<0.001). The majority of patients were elderly (mean age: 55.92 years) and often had underlying structural lung diseases (e.g., bronchiectasis, COPD) or immunocompromised conditions (e.g., diabetes). The main clinical manifestations were cough (68.06%) and sputum production (52.78%). Imaging features included bronchiectasis (63.89%), nodular shadows (47.22%), and tree-in-bud signs (25.00%). After 6 months of follow-up, 73.8% of patients showed significant improvement in lung lesions, while 26.2% had no significant change or disease progression. Conclusions:mNGS demonstrates rapid and accurate diagnostic advantages for PTB combined with NTM-PD, significantly outperforming traditional culture methods, and provides critical support for precise clinical treatment.

Objective To analyze the relationship between clinical drug utilization and the risk of nosocomial infections among hospitalized patients,and provide evidence for the prevention and control of nosocomial infections.Methods This study adopted a retrospective case-control design.The case group included 209 patients with nosocomial infection reported from January 2023 to December 2023 in a tertiary hospital.The control group included 209 patients without nosocomial infection during the same period.The patients in the control group were selected by stratified sampling based on Charlson Comorbidity Index(CCI).Results Univariate analysis showed that proton pump inhibitors,antacids,immunosuppressants and prior antimicrobial combination therapy increased the risk of nosocomial infection(P<0.05).Multivariate log-binomial regression analysis showed that proton pump inhibitors,immunosuppressive drugs,and prior antimicrobial combination therapy were correlated with nosocomial infection.The corresponding relative risk(RR)was 1.31(95%CI:1.07-1.60),1.40(95%CI:1.02-1.91),and 1.66(95%CI:1.01-2.74),respectively.Further analysis indicated that the patients with nosocomial infection had longer time in use of proton pump inhibitors and prior antimicrobial combination therapy than the patients in the control group(Z=-6.331,P<0.001;Z=-2.667,P=0.008).The trend Chi-square test showed that there was a dose-response relationship for proton pump inhibitors(x2=73.869,P<0.001),immunosuppressive drugs(x2=16.530,P<0.001),and prior antimicrobial combination therapy(x2=35.107,P<0.001).Conclusions The use of immunosuppressants,proton pump inhibitors and antimicrobial combination therapy increases the risk of nosocomial infections in hospitalized patients.The prolonged use of these drugs will further increase the risk of nosocomial infection.

7.ADAMDEC1 regulates growth and migration of pancreatic carcinoma cells through Wnt/β-catenin signaling pathway

AIM:To investigate the effect of a disintegrin and metalloproteinase(ADAM)domain-like decy-sin 1(ADAMDEC1)knockdown on the proliferation,migration and invasion of pancreatic carcinoma cells.METHODS:Expression levels of ADAMDEC1 in pancreatic carcinoma tissues were analyzed using the GEPIA and UALCAN online da-tabases.Western blot analysis was employed to detect the protein expression levels of ADAMDEC1 in pancreatic carcino-ma cell lines(MIA PaCa-2 and PANC-1)and pancreatic ductal cell line(hTERT-HPNE).The effects of ADAMDEC1 knockdown on cell proliferation,migration and invasion were evaluated using CCK-8,colony formation,wound-healing and Transwell assays.Additionally,Western blot analysis was used to detect the effects of ADAMDEC1 knockdown on the expression levels of migration and invasion markers,as well as Wnt/β-catenin signaling pathway-related proteins in pancre-atic carcinoma cells.Furthermore,a recovery experiment was conducted to assess the role of Wnt/β-catenin signaling path-way agonist CHIR-99021 in ADAMDEC1 knockdown-induced inhibition of pancreatic carcinoma cell growth and migra-tion.RESULTS:(1)ADAMDEC1 was highly expressed in pancreatic carcinoma cells.(2)Knockdown of ADAMDEC1 led to a significant reduction in the proliferation,migration and invasion of pancreatic carcinoma cells.(3)Knockdown of ADAMDEC1 resulted in increased E-cadherin protein expression and decreased levels of matrix metalloproteinase 9,N-cadherin and vimentin proteins,alongside a reduction in the expression of Wnt/β-catenin signaling pathway-related pro-teins.(4)Co-treatment of pancreatic carcinoma cells with CHIR-99021 and ADAMDEC1 small interfering RNA reversed the inhibitory effects of ADAMDEC1 knockdown on cell proliferation,migration,and invasion.CONCLUSION:ADAMDEC1 is highly expressed in pancreatic carcinoma.Targeted silencing of ADAMDEC1 has the potential to inhibit the prolifera-tion,migration and invasion of pancreatic carcinoma cells by regulating the Wnt/β-catenin signaling pathway.
8.Summary of basic research on liver transplantation in China of 2023

Liver transplantation is the optimal treatment for end-stage liver disease and hepatocellular carcinoma, which can significantly improve clinical prognosis and quality of life of patients. However, multiple challenges, such as rejection, immune tolerance, shortage of donor liver, preservation of donor liver, ischemia-reperfusion injury and postoperative complications, etc., limit the efficacy of liver transplantation in clinical practice. Research teams in China have made significant contributions to the basic research related to liver transplantation by making continuous efforts and combining the development of emerging technologies, interdisciplinary integration and other emerging fields. In this article, the frontier progress in the basic research of liver transplantation in 2023 was reviewed, highlighting the progress made by Chinese research teams in the basic research of liver transplantation, aiming to provide reference for promoting the integration of Chinese characteristics into the research of liver transplantation, accelerate the integration of Chinese liver transplantation research with international community, and promote further advancement of liver transplantation in China.
9.Clinical values of morphology of hemophagocytes in infection associated hemophagocytic lymphohistiocytosis

Objective:To study the morphology of hemophagocytosis (HPC) in bone marrow smears of patients with infection-associated hemophagocytic lymphohistiocytosis (IAHLH), and further analyse if there were differences in the clinical and laboratory features, the cytokines level and prognosisMethods:24 patients newly diagnosed with IAHLH from 2016-Dec-1 to 2021-Dec-31 in Beijing Friendship Hospital were included as study group, and 20 patients with infectious disease as non-HLH control group. In IAHLH group, mean age was 34±13 years, including 17(71%) males and7(29%) females. In Non-HLH group, mean agewas 43±16 years, including 14 (70%) males and6 (30%) females. Depending on re-checking phagocytic cell type on the initial bone marrow smear, the HPCs were divided into HPC-1, phagocytizing non-nucleated cells (mature erythrocyte or platelets), and HPC-2, phagocytizing nucleated cells. The differences in clinical presentations covered in HLH-2004 criteria, cytokines value(IL-6, IL-10, IL-18, IFN-γ) recommended in HLH-2022-China guideline, and the mortality within 1 year of diagnosis, were compared between IAHLH and non-HLH groups, between patients with or without HPC, and between patients with HPC-2 or only with HPC-1. For categorical variables, two groups were compared with the use of either the chi-square test or Fisher′s exact test. For non-normal distribution continuous variables, the difference between two groups variation was performed by using Mann-Whitney U test, and for normal distribution continuous variables, the difference was by the Independent Samples t-test.Results:The positive rates of fever, hepatomegaly and splenomegalyand the motrtality in IAHLH were 100% (24/24), 63% (15/24), 92% (22/24) and 46% (11/24), respectivelyin non-HLH were 55%(11/20),0(0/20),25% (5/20),0(0/20),and the differences between two groups were all statistically significant( P<0.01), but thedifferences between groups with and without HPC and between IAHLH patients with HPC-2 or only with HPC-1 were no statistically significanlly, ( P>0.05).In IAHLH group, IFN-γ in patients with HPC-2 was 400(246, 532)ng/L, significantly higher than 146(38, 180)ng/L in patients only with HPC-1 [ P=0.02, 95% CI was 233(75.8 to 397)], andthe other test parameters and cytokines level showed no obvious differences ( P>0.05).
10.Construction and application of an intelligent injury monitoring system in Yinzhou District

Injury monitoring is an important approach for injury control and is an important part of comprehensive disease monitoring. With the development of medical digitalization, an intelligent injury monitoring system has been created in Yinzhou District, Ningbo City, Zhejiang Province using artificial intelligence techniques based on Ningbo Municipal Health Information Platform and has been applied across the district since 2019. The manual card-reporting mode has been transformed to intelligent card-reporting mode in this system, which achieves functions of epidemiological analyses of the monitoring data, early warning of high-incidence injuries, classified management of injury and quality control of report cards. Nearly 300 thousand cards have been automatically reported since the system was online available since November 2022, and the epidemiological characteristics of injury were preliminarily understood, which provide data supports to early earning and interventions of injury. The intelligent injury monitoring system greatly improves the injury monitoring efficiency and card-reporting quality, and saves a large number of manpower and material resources, which provides a powerful technical support to widespread injury monitoring.
