1.From tradition to modernity: the integrated development of medical humanities and narrative medicine

Medical humanities consistently run through the entire process of medical development and educational reform. However, with the increasingly prominent dominance of evidence-based medicine in clinical practice, the medical humanities have gradually been weakened in both medical education and clinical practice. Narrative medicine, through telling and listening to patients’ stories, enhances healthcare professionals’ empathy, fosters doctor–patient communication, and facilitates a return to the humanistic essence of medical education and clinical practice. By sorting out and reviewing related literature and developmental trends both at home and abroad, this paper pointed out the existing structural problem of an imbalance between technological priority and humanistic care in medical education, focusing on how to achieve an effective integration of medical humanities and narrative medicine in medical education. This paper also systematically analyzed the significance of both medical humanities and narrative medicine in the medical education system and proposed promoting the deep embedding of narrative medicine in medical education from three entry points, namely, curriculum integration, interdisciplinary collaboration, and the construction of teaching evaluation systems. The aim was to provide theoretical support and practical experience for medical education reform, foster the coordinated development of professional competence and humanistic spirit among medical talents, and truly achieve the goal of cultivating well-rounded medical talents.
2.Effects and mechanisms of oliceridine fumarate on postoperative cognitive dysfunction in elderly rats through TLR4/NF-κB pathway

OBJECTIVE To investigate the effects and mechanisms of oliceridine fumarate (TRV130) in improving postoperative cognitive dysfunction (POCD) in elderly rats based on the Toll-like receptor 4 (TLR4)/nuclear factor-κB (NF-κB) pathway. METHODS Rats were randomly divided into the control group (normal saline), the model group (normal saline), the TRV130 group (2.8 mg/kg), the TLR4/NF-κB pathway inhibitor (TAK-242) group (3 mg/kg), the β -arrestin inhibitor (Barbadin) group (3 mg/kg), and the traditional opioid drug (morphine) group (2.8 mg/kg), with 15 rats in each group. Except for the control group, POCD models were established in all other groups. From the first day after surgery, drugs/normal saline were administered via caudal vein injection once daily for 3 consecutive days. After the last administration, the pathological damage of hippocampal tissue was observed; the cognitive function, serum inflammatory factor levels, hippocampal neurons apoptosis rate, and the expression of ionized calcium-binding adapter molecule 1 (Iba-1), glial fibrillary acidic protein (GFAP), and TLR4/NF-κB pathway-related mRNA and protein in hippocampal tissue were detected. RESULTS In the model group, the neurons in the CA1 region of the hippocampus were disordered and sparse, with decreased number, pyknotic and fragmented nuclei accompanied by inflammatory cell infiltration. Compared with the control group, the escape latency, serum levels of tumor necrosis factor α (TNF-α), interleukin-6 (IL-6), and IL-1β, hippocampal neurons apoptosis rate, average fluorescence intensities of Iba-1 and GFAP, mRNA expression levels of TLR4 and NF-κB p65, and their protein expression/phosphorylation levels in hippocampal tissue were significantly increased/elevated in the model group ( P <0.05); the time spent in the target quadrant and the number of platform crossings were significantly shortened/decreased ( P <0.05). Compared with the model group, the cognitive function, pathological, inflammatory, and apoptosis-related indicators were significantly improved in the TRV130 group, TAK-242 group, and Barbadin group ( P <0.05); the mRNA expression levels of TLR4 and NF-κB p65 and their protein expression/phosphorylation levels were significantly decreased in the TRV130 group and TAK-242 group ( P <0.05). CONCLUSIONS TRV130 may improve POCD in elderly rats by inhibiting the TLR4/NF-κB pathway and alleviating postoperative central nervous system inflammatory responses.
3.Analysis of change trend of hepatectomy for 17 232 cases of hepatocellular carcinoma and efficacy of fluorescent laparoscopic hepatectomy: a 10-year single center study

Objective:To investigate the change trend of hepatectomy for hepatocellular carcinoma and efficacy of fluorescent laparoscopic hepatectomy in a single center from 2014?2023.Methods:The propensity score matching and retrospective cohort study was conducted. The clinicopathological data of 17 232 patients of hepatocellular carcinoma who were admitted to Zhongshan Hospital of Fudan University from January 2014 to December 2023 were collected. There were 14 404 males and 2 828 females, aged 58(range, 11?94)years. Observation indicators: (1) change trend of hepatectomy for hepatocellular carcinoma and application of fluorescent laparoscopy in 2014?2023; (2) propensity score matching between patients undergoing fluorescent and conventional laparoscopic hepatectomy; (3) intraoperative and postoperative conditions of patients in the fluorescent laparoscopy group and the conventional laparoscopy group after propensity score matching. Comparison of measurement data with skewed distribution between groups was conducted using the Mann-Whitney U test. Comparison of count data between groups was conducted using the chi-square test. Comparison of ordinal data between groups was conducted using the nonparametic rank sum test. Propensity score matching was performed using the 1∶3 nearest neighbor matching method, with the caliper value of 0.01. Results:(1) Change trend of hepatectomy for hepatocellular carcinoma and application of fluorescent laparoscopy in 2014-2023. Among 17 232 patients of hepatocellular carcinoma, 4 074 cases underwent laparoscopic surgery, and 13 158 cases underwent open surgery or conversion from laparoscopic to open surgery. From 2014 to 2023, the proportions of laparos-copic surgery in hepatectomy were 8.993%(117/1 301), 6.804%(99/1 455), 9.379%(145/1 546), 17.511%(287/1 639), 23.828%(427/1 792), 24.572%(459/1 868), 29.662%(544/1 834), 31.568%(584/1 850), 38.080%(599/1 573), and 34.246%(813/2 374), respectively. The proportions of Grade Ⅲ surgery in laparoscopic hepatectomy were 11.966%(14/117), 7.071%(7/99), 12.414%(18/145), 21.254%(61/287), 28.337%(121/427), 27.887%(128/459), 34.375%(187/544), 34.644%(214/584), 35.726%(214/599), and 42.681%(347/813), respectively. From 2016 to 2023, the proportions of the application of fluorescent laparoscopy in laparoscopic hepatectomy were 2.069%(3/145), 5.923%(17/287), 8.431%(36/427), 10.022%(46/459), 17.647%(96/544), 24.829%(145/584), 9.182%(55/599), and 12.915%(105/813), respectively. (2) Propensity score matching between patients undergoing fluorescent and conventional laparoscopic hepatectomy. Among the 4 074 patients who underwent laparoscopic liver resection, 503 cases using fluorescent laparoscopy were assigned to the fluorescent laparoscopy group, 3 571 cases using conventional laparoscopy were assigned to the conventional laparoscopy group. A total of 1 794 patients were successfully matched, with 483 cases in the fluorescent laparoscopy group and 1 311 cases in the conventional laparoscopy group. After propensity score matching, the confounding biases of the maximum tumor diameter and the year of surgery were eliminated, and the two groups were comparable. (3) Intraoperative and postoperative conditions of patients in the fluorescent laparoscopy group and the conventional laparoscopy group after propensity score matching. After propensity score matching, there was no perioperative death in either the fluorescent laparoscopy group or the conventional laparoscopy group.The operation time of patients in the fluorescent laparoscopy group was 240(150,328)minutes, the portal triad clamping time was 30(15,45)minutes, the volume of intraoperative blood loss was 50(30,100)mL, the number of patients with complication 72, respectively. For patients in the conventional laparoscopy group, the above indicators were 180(131,240)minutes, 23(15,30)minutes, 55(50,100)mL, and 248, respectively. There were significant differences in the above indicators between the two groups ( Z=?7.593, ?7.372, ?4.941, χ2=3.873, P<0.05). Conclusion:The proportion and difficulty level of laparoscopic hepatectomy for hepatocellular carcinoma in Zhongshan Hospital of Fudan University have shown an upward trend during 2014?2023. Compared with conventional laparoscopy, patients undergoing fluorescent laparoscopic hepatectomy have longer operation time and portal triad clamping time, but less intraoperative blood loss and lower iproportion of complications.
4.Incidence of postoperative pneumonia in a tertiary first-class general hos-pital in Shanghai from 2017 to 2023

Objective To analyze the occurrence of postoperative pneumonia(POP)in a tertiary general first-class hospital in Shanghai,and provide basis for strengthening the implementation of POP prevention and control meas-ures for the target population.Methods The real-time monitoring data of healthcare-associated infection(HAI)from 2017 to 2023 were retrospectively surveyed.The occurrence of POP in all surgical patients(including minimal-ly invasive interventional surgery)were analyzed.Results A total of 701 postoperative patients had POP.The inci-dence of POP was 0.30%.Incidence of POP was higher in male patients than in female patients(0.48%vs 0.15%),in 65-year-old-population than in<65-year-old-population(0.58%vs 0.17%),and in elective surgery than in emergency surgery(0.35%vs 0.27%),differences were statistically significant(all P<0.05).The inter-val from postoperative time to POP occurrence in 701 POP patients was 6(4,10)days,with emergency surgery pa-tients developing POP later than elective surgery(7[4,11]days vs 6[3,10]days),and difference was statistically significant(P<0.05).The top five departments with higher incidences of POP were neurosurgery(5.84%),cardi-ac surgery(4.01%),thoracic surgery(1.92%),abdominal surgery(0.74%),and minimally invasive intervention(0.17%).Incidence of POP in emergency neurosurgery was higher than that in elective neurosurgery(9.71%vs 2.14%),while incidences of POP in elective cardiac surgery and thoracic surgery were both higher than emergency surgery(5.09%vs 2.93%,2.46%vs 0.58%,respectively),differences were all statistically significant(all P<0.05).A total of 675 strains of pathogens were detected from 701 POP patients,with Gram-negative bacteria being the major pathogens(n=520,77.04%).The predominant detected pathogens were Acinetobacter baumannii(n=119),Pseudomonas aeruginosa(n=116),Staphylococcus aureus(n=108),Klebsiella pneumoniae(n=104),and Stenotrophomonas maltophilia(n=47).Conclusion In the prevention and control of perioperative infection,the key populations for POP prevention and control should be males and those aged ≥65 years old,and the key depart-ments should be neurosurgery,cardiac/thoracic surgery,and abdominal surgery.Minimal invasive surgery also has the risk of POP,which should be paid more attention.
5.Effect of Baofei Yiqi pill on improving immune function of patients with lung cancer

Objective To explore the effect of Baofei Yiqi pill on immune function of patients with lung cancer.Methods A total of 100 patients with lung cancer diagnosed and treated in Leping Hospital of Traditional Chinese Medicine from September 2020 to September 2023 were selected and divided into experimental group and control group according to random number table method,with 50 cases in each group.The control group was treated with docetaxel combined with cisplatin,and the experimental group was treated with Baofei Yiqi pill on the basis of control group.Both groups were treated with 3 cycles of chemotherapy.The clinical efficacy,quality of life,changes in weight,immune function and adverse reactions were compared between two groups.Results The total response rate of experimental group was significantly higher than that of control group(x2=14.620,P<0.001).After treatment,the Karnofsky performance status score and weight of experimental group were significantly higher than those of control group(P<0.05),and the levels of CD3+,CD4+and CD4+/CD8+in experimental group were significantly higher than those in control group,and the levels of CD8+was significantly lower than than in control group(P<0.05).The incidence of gastrointestinal reactions in experimental group was significantly lower than that in control group(P<0.05).There was no significant difference in the incidence of abnormal renal function and liver function between two groups(P>0.05).Conclusion Baofei Yiqi pill can effectively improve the immune function of patients with lung cancer,improve the quality of life of patients,reduce the adverse reactions of chemotherapy,and alleviate the disease of patients to a certain extent,which is worthy of promotion and application.
6.Analysis on the early postoperative home dietary care guidance needs of gastrointestinal polypectomy patients based on the Kano model

Objective:To analyze the early family diet nursing guidance needs of patients with gastrointestinal polyps resection based on Kano model, further screen the needs and optimize nursing strategies.Methods:This was a cross-sectional study. A total of 215 patients who underwent gastrointestinal polypectomy in Yingtan 184 Hospital of China Rongtong Group Healthcare Co. Ltd from January 2022 to June 2024 were prospectively selected using the purposive sampling method. The Kano model combined with Delphi expert consultation method was used to design a questionnaire on the family dietary nursing guidance needs of patients with gastrointestinal polypectomy. The Kano model needs attribute classification, Better-Worse coefficient analysis and two-dimensional matrix analysis were used to determine the information needs attributes, and the most sensitive and most needed factors were screened out for strategy optimization.Results:In this study, 232 questionnaires were distributed, and 215 valid questionnaires were returned, with an effective return rate of 92.67%. The 215 patients ranged in age from 20 to 72 years, with a mean age of 40.92 ± 10.06 years, including 163 males and 52 females. Among the 16 items in the survey questionnaire, 15 items need to be improved first. These include 1 attractive demand, 7 basic demands and 7 expected demands. The top 3 most sensitive needs were the formulation of an early home diet plan, the evaluation of the current diet structure, and the avoidance of exacerbating negative emotions, with sensitivities of 0.380, 0.370 and 0.328, respectively.Conclusions:The follow-up recommendations focus on providing support for family diet nursing, strengthening education on healthy diet, helping patients build a reasonable diet structure and habits, preventing malnutrition in the process, so as to meet the needs of early family diet nursing guidance for such patients.

8.Infectious Disease Burden and Pharmaceutical Care Optimization:A Three-Decade Cohort Analysis for China's Aging Population(1990-2021)

Objective To analyze temporal trends in infectious disease burden among Chinese elderly(≥60 years)using data from the Global Burden of Disease Study(GBD 2021),evaluate age-period-cohort effects on disease burden,predict trends through 2045,and propose evidence-based medication management strategies.Methods We conducted a threefold analysis of infectious disease burden from 1990 to 2021 using Joinpoint regression to identify temporal trends,Age-Period-Cohort(APC)modeling to disentangle epidemiological effects,and Nordpred projections for 2045 disease burden estimates.Results Over 32 years,all infectious disease categories except HIV/AIDS and sexually transmitted infections demonstrated significant declines.Enteric infections showed the most rapid reductions in mortality(AAPC=-7.85,P﹤0.001)and disability-adjusted life year rates(DALYR;AAPC=-7.18,P﹤0.001).We also found a significant decrease in the incidence of tropical diseases and malaria(AAPC=-6.77,P﹤0.001).APC analysis found that the age effect was mostly negative in terms of the annual percentage change in mortality and DALYR for each disease,except for HIV/AIDS,with an overall decline in period risk over time,and a generally higher risk of morbidity and mortality for the early birth cohort,and an overall decline in the risk of each disease as the year of birth progressed,but the risk of HIV/AIDS death and DALY period and cohort risks trended upward.While the overall risk for certain diseases was slightly higher in males compared to females,the trends were largely consistent across both sexes.In terms of projections,the absolute prevalence of respiratory infections and tuberculosis in terms of number of cases and age-standardized rates,and the growth of the disease burden of HIV/AIDS were particularly prominent,making them important health challenges for the future.The burden of disease in the elderly often results in issues such as potential polypharmacy,which must be addressed to improve medication management.Conclusions China has achieved remarkable progress in reducing infectious disease burdens among older adults,though HIV/AIDS and sexually transmitted infections present an escalating public health threat.These findings advocate for enhanced surveillance systems,age-specific prevention strategies,and precision medication protocols to optimize therapeutic outcomes in geriatric populations.

In recent decades,the incidence of human papillomavirus(HPV)-associated oropharyngeal squamous cell carcinoma(OPSCC)has shown a marked increase.Significant changes have also occurred in the OPSCC diagnosis and treatment paradigm.Deter-mining HPV status prior to treatment is now essential,and radiotherapy/chemotherapy,immunotherapy,and minimally invasive surgical techniques have progressively emerged as key modalities for managing OPSCC.However,alongside these paradigm shifts,a comprehen-sive technical consensus guiding the entire diagnostic and therapeutic process for OPSCC patients is currently lacking.Given China's large population base and the rising incidence of OPSCC,an expert panel convened to develop a clinical technical consensus on OPSCC diagno-sis and management tailored to China's specific context.This consensus aims to further enhance and standardize understanding of OPSCC management techniques among relevant healthcare professionals.
