1.Ultrasonography for evaluating morphological changes of levator hiatus in patients with gestational diabetes mellitus after spontaneous delivery

Objective To observe the morphological changes of levator hiatus in patients with gestational diabetes mellitus(GDM)after spontaneous delivery with ultrasonography.Methods A total of 302 pregnant women in the first trimester(6-8 weeks)were prospectively observed,and the parameters of pelvic floor muscle hiatus were measured with ultrasound during the first trimester,also 12 weeks,6 months and 1 year after delivery at resting-state(resting period),anal contraction state(systole period)and Valsalva maneuver(tension period),respectively.Blood glucose was measured at 28 weeks of gestation,GDM was diagnosed,and the pregnant women enrolled were divided into GDM group and non-GDM group.The ultrasonic parameters and postpartum pelvic floor muscle tension were compared between groups.Spearman correlation analysis was used to evaluate the correlations of pelvic floor muscle tension grade and anal levator hiatus parameters in GDM group.Results Totally 153 pregnant women were enrolled and assigned into GDM group(n=51)and non-GDM group(n=102).Transversal diameter of levator hiatus(LH-TD),anteroposterior diameter of levator hiatus(LH-APD)and levator hiatus area(LHA)in different periods 12 weeks postpartum in both groups were higher than those in early pregnancy(all P<0.05).Six months and 1 year postpartum,in GDM group,LH-APD and LHA in systole period,also LH-TD,LH-APD and LHA in tension period were higher than those in early pregnancy(all P<0.05),while in non-GDM group,LH-APD and LHA in tension period were higher than those of early pregnancy(all P<0.05).One year after delivery,LH-APD and LHA in systolic period,as well as LH-TD and LHA in tension period in GDM group were all higher than those in non-GDM group(all P<0.05),whereas the proportion of pelvic floor muscle tension of grade Ⅱ-Ⅲ was higher,of grade Ⅳ-Ⅴ was lower in GDM group than those in non-GDM group(P<0.05).One year after delivery,pelvic floor muscle tone grade in GDM group was negatively correlated with LH-TD,LH-APD and LHA in resting,systole and tension period(all P<0.05).Conclusion The morphology of levator hiatus changed greatly in GDM patients after spontaneous delivery,and rehabilitation training should be carried out early.
2.Sinicization and reliability and validity test of Questionnaire Assessing the Childbirth Experience

Objective:To translate the English version of the Questionnaire Assessing the Childbirth Experience (QACE) into Chinese and test its reliability and validity, enriching the evaluation tools used for delivery experience in China, providing guidance for medical personnel to evaluate the delivery experience of mothers, and providing reference for improving the delivery experience of mothers in the future.Methods:Based on Beaton ′s cross-cultural debugging guide, the Chinese version of QACE was developed, including correct translation, comprehensive, back translation, expert consultation and pre-test. The reliability and validity of the questionnaire survey was carried out on 235 participants who visited the postpartum clinic in West China Second Hospital, Sichuan University and Mianyang Central Hospital. Results:In 235 participants, aged 20-29 years old with 79 cases, aged 30-39 years old with 152 years old, aged 40-43 years old with 4 cases. The Chinese version of QACE consisted of 4 dimensions (emotional state, relationship with medical staff, first time spent with newborns, and feelings after 1 month postpartum) and 13 items. The Cronbach α coefficient of the scale was 0.812, the Cronbach α coefficient of each dimension was 0.661-0.844, and the broken half reliability was 0.527. The scale level content validity index was 0.95. Four common factors were extracted by exploratory factor analysis, and the cumulative variance contribution rate was 67.682%. Conclusions:The Chinese version of QACE has significant reliability and validity, and can be used to evaluate maternal delivery experiences.
3.Analysis of influential factors of allergic diseases in 90 infants and young children

Objective:To analyze the factors influencing the occurrence of allergic diseases in infants and children aged 0-3 years.Methods:A case-control study was conducted to retrospectively analyze the clinical data of 90 infants and young children diagnosed with allergic diseases (observation group) admitted to Jinhua Maternal & Child Health Care Hospital from January 2019 to June 2023 and 80 healthy infants and young children undergoing routine health check-ups during the same period (control group). The clinical characteristics of the two groups were compared, and multivariate logistic regression analysis was performed to identify the factors influencing the occurrence of allergic diseases in infants and young children aged 0 to 3 years.Results:In the observation group, the proportions of infants with a family history of allergic diseases, exposure to smoking environments, maternal smoking exposure during pregnancy, maternal consumption of docosahexaenoic acid during pregnancy, and antibiotic use in infants were 77.8% (70/90), 55.6% (50/90), 66.7% (60/90), 61.1% (55/90), and 55.6% (50/90), respectively. All of these were significantly higher than those in the control group [31.3% (25/80), 25.0% (20/80), 25.0% (20/80), 25.0% (20/80), 25.0% (20/80), χ2 = 37.19, 16.33, 29.51, 22.40, 16.33, all P < 0.05]. The proportion of breastfeeding in the observation group was 44.4% (40/90), which was significantly lower than that in the control group [75.0% (60/80), χ2 = 16.33, P < 0.05]. Multivariate logistic regression analysis results revealed that family history of allergic diseases, non-breastfeeding, exposure to smoking environments, maternal smoking exposure during pregnancy, maternal consumption of docosahexaenoic acid during pregnancy, and antibiotic use in infants were risk factors for the occurrence of allergic diseases in infants and young children aged 0 to 3 years ( OR = 3.511, 3.688, 3.377, 3.728, 3.912, 4.023, all P < 0.05). Conclusion:A family history of allergic diseases, non-breastfeeding, exposure to smoking environments, maternal smoking exposure during pregnancy, maternal consumption of docosahexaenoic acid during pregnancy, and antibiotic use in infants are risk factors for the occurrence of allergic diseases in infants and young children aged 0 to 3 years. Clinically, these factors can be used to assess the likelihood of allergic diseases in infants.
4.Summary of best evidence for prevention and management of radiation esophagitis in patients with chest tumors

Objective:To summarize the best evidence for the prevention and management of radiation esophagitis in patients with chest tumors, so as to provide basis for the prevention and management of radiation esophagitis in patients.Methods:The research question was proposed based on the population, intervention, professional, outcome, setting and type of evidence (PIPOST) model, and guidelines, expert consensus, clinical decision-making, evidence summary, systematic review, and randomized controlled trials on the prevention and management of radiation esophagitis in chest tumor patients were systematically searched on UpToDate, BMJ Best Practice, Cochrane Library, PubMed, American Society of Clinical Oncology, China National Knowledge Infrastructure, WanFang Data, and other databases and websites based on the "6S" model. The search period was from database establishment to August 1, 2023. Two researchers independently evaluated the quality of the literature and extracted and integrated evidence from the literature that met the quality standards.Results:A total of ten articles were included, including three guidelines, one expert consensus, one systematic review, two systematic evaluations, and three randomized controlled trials. Twenty-three pieces of evidence were summarized from six dimensions of building multidisciplinary teams, screening and evaluation, nutritional support, drug and non-drug prevention, integrated traditional Chinese and western medicine treatment, and health education.Conclusions:The included evidence can provide a theoretical basis for the prevention and management of radiation esophagitis in patients undergoing chest tumor radiotherapy. Clinical medical and nursing personnel can provide personalized interventions to patients based on clinical practice.
5.Analysis of quality difference of different specifications and different origins of Citri Grandis Exocarpium

OBJECTIVE To study the quality difference of different specifications of Citri Grandis Exocarpium from different origins,and to provide reference for the orderly development of Citri Grandis Exocarpium industry. METHODS Different specifications [ Citrus grandis ‘Tomentos’young fruit ,Citrus grandis (L.)Osbeck young fruits ,exocarp] of 93 batches of Citri Grandis Exocarpium medicinal materials (decoction pieces )from different origins [ Citrus grandis ‘Tomentosa’or Citrus grandis (L.)Osbeck] were taken as samples. The contents of naringin and rhoifolin in samples were determined by HPLC. Through pheatmap parameters of R language ,heatmap was drawn for the contents of naringin and rhoifolin according to origins and specifications (young fruit and exocarp ). RESULTS Of 93 batches of samples ,the contents of naringin and rhoifolin were 16.52-214.64 and 1.03-10.96 mg/g,respectively. Among different specifications ,the contents of naringin and rhoifolin in the young fruit were the highest (their average contents were 108.96 and 6.30 mg/g respectively ). Heatmap analysis of R language content showed that the contents of naringin and rhoifolin in Citri Grandis Exocarpium from origin of C. grandis ‘Tomentosa’were generally higher than those from origin of C. grandis (L.)Osbeck. Of different specifications of Citri Grandis Exocarpium from origins,the contents of naringin and rhoifolin were higher in KTP young fruit relatively. CONCLUSIONS The quality of Citri Grandis Exocarpium from origin of C. grandis ‘Tomentosa’with the young fruit as specification is the best.

7.Study on Improvement Effects and Mechanism of Guizhi Fuling Pills on Phlegm-dampness Type PCOS Model Rats

OBJECTIVE:To stu dy the improvem ent effect and mechanism of Guizhi fuling pill (GZFLP) on rats with phlegm-dampness type polycystic ovary syndrome (PCOS).METHODS:Totally fifty female SD rats with regular estrous cycle were randomly divided into blank control group (n=10) and model group (n=40). The model group received letrozole intragastrically [1 mg/(kg·d),for 21 d ] to induce phlegm-damp PCOS rat model. Forty model rats were randomly divided into model group ,metformin group [positive control ,0.135 g/(kg·d)] and GZFLP low-dose ,high-dose groups [4.49,17.96 g/(kg· d)],with 10 rats in each group. Each medicine group was given intragastric administration ,once a day ,for 30 consecutive days. Blank control and model group were given intragastric administration of equal volume normal saline.After last medication ,oral glucose tolerance test (OGTT)was used to detect the level of blood glucose in rats at 0,30,60 and 120 min. ELISA assay was used to detect the levels of follicle-stimulating hormone (FSH),luteinizing hormone (LH),estradiol(E2)and testosterone (T); body weight ,the weight of uterus ,bilateral ovaries and paragonadal fat were weighed. HE staining was used to observe morphological changes of ovarian tissue sections. RT-PCR method and Western blotting assay were used to detect mRNA and protein expression of CYP 19a1 in ovary and Glut 4 in uterine. RESULTS :Compared with blank control group ,the levels of blood glucose 30,60,120 min after intragastric administration of glucose solution as well as serum levels of LH ,E2 and T were increased significantly in model group (P<0.05);the body weight ,the weight of uterus ,bilateral ovaries and paragonadal fat were increased significantly (P<0.05);the volume of thecal cells increased significantly and showed spherical shape ;the mRNA and protein expression of CYP 19a1 in ovarian tissue and Glut 4 in uterine tissue were decreased significantly (P<0.05 or P<0.01). Compared with model group ,above indexes of administration groups were improved significantly (P<0.05 or P<0.01);the thickness of theca cell layer and the volume of theca cells in GZFLP low-dose and high-dose groups were also improved. CONCLUSIONS:GZFLP can up-regulate the expression of CYP19a1 in ovary and Glut4 in uterus ,so as to improve the abnormal glucose metabolism in phlegm-dampness type PCOS model rats ,reduce insulin resistance ,lower serum estrogen levels ,and reduce the degree of polycystic changes in ovary ,thereby playing a role in the treatment of PCOS.
8.Application of dexmedetomidine combined with propofol and remifentanil in fiberbronchoscopy in elderly patients with chronic obstructive pulmonary disease

Objective:To investigate the application effect of dexmedetomidine combined with propofol and remifentanil in fiberoptic bronchoscopy in elderly patients with chronic obstructive pulmonary disease(COPD).Methods:From October 2018 to November 2019, 112 elderly patients with COPD who were examined by fiberoptic bronchoscopy in the Affiliated Hospital of Medical College of Ningbo University were divided into control group and observation group, with 56 cases in each group.Propofol and remifentanil were used for anesthesia induction in both two groups.Dexmedetomidine (0.5μg/kg) was injected intravenously into observation group, and later 0.5μg·kg -1·h -1 continuous infusion, the control group was given the same dose of 0.9% sodium chloride injection.The OAA/S scores, propofol dosage, remifentanil dosage, wake-up time, heart rate, oxygen saturation, mean arterial pressure, pulmonary function changes and adverse reactions were compared between the two groups. Results:At the fiberbronchoscope entered immediately and 10 min after examination, the OAA/S scores in the observation group were (1.03±0.35)points, (4.42±0.28)points, respectively, which in the control group were (1.22±0.22)points, (3.01±0.22)points, respectively, the differences between the two groups were statistically significant( t=3.439, 29.631, all P<0.05). The dosages of propofol and remifentanil in the observation group were (3.00±0.28)μg/kg and (1.65±0.29)μg/kg, respectively, which were less than those in the control group [(3.70±0.39)μg/kg, (2.42±0.45)μg/kg], the recovery time was (8.40±1.58)min, which was shorter than that in the control group [(13.31±2.02)min], the differences were statistically significant( t=10.911, 10.763, 14.327, all P<0.05). The heart rate, oxygen saturation, mean arterial pressure, pulmonary function(FEV 1, MVV) in the observation group were significantly better than those in the control group( t=9.757, 11.981, 11.486, 11.913, 40.189, all P<0.05). The incidence of adverse reactions in the observation group was 7.14%(4/56), which was lower than 19.64%(11/56) in the control group(χ 2=6.737, P<0.05). Conclusion:Dexmedetomidine(0.5μg/kg, followed by 0.5μg·kg -1·h -1 continuous infusion) and target controlled infusion of propofol and remifentanil are safe for elderly patients with COPD, and the use of anesthetic drugs is small, which can shorten the recovery time after operation, and will not have a great impact on respiratory circulation.

10.Effect of EP4 gene silencing on the growth and migration of papillary thyroid carcinoma K1 cells

Objective To investigate the effect of EP4 gene silencing on the growth and migration of K1 cells. Methods K1 cells with stable knockdown of EP4 were constructed with lentiviral vector. QRT-PCR and western blot analysis were used to detect the expression of EP4 mRNA and protein in K1 cells. CCK8 assay and flow cytometry were employed to measure cell viability and apoptosis. Transwell assay was applied to detect cell migration. Results Compared with the negative control group,the mRNA and protein expression of EP4 were sig-nificantly decreased in K1 cells with stable knockdown of EP4. Furthermore,shRNA-mediated silencing of EP4 gene remarkably suppressed cell viability and induced apoptosis of K1 cells.The migration of K1 cells with knock-down of EP4 was decreased compared with the negative control group. Conclusions EP4 gene silencing can in-hibit growth and induce apoptosis of K1 cells.Downregulation of EP4 can significantly reduce migration of K1 cells.
