1.Optimization of the extraction process of Shenye buyi formula

OBJECTIVE To optimize the extraction process of Shenye buyi formula. METHODS Based on the quality by design (QbD) concept, the contents of rutin, nuciferine, sinapine thiocyanate, liquiritin,and glycyrrhizic acid ammonium salt, as well as the extract yield, were selected as the critical quality attributes (CQAs) for the extraction process of Shenye buyi formula. The fishbone diagram combined with the failure model and effect analysis method was adopted to determine the water addition, extraction time and extraction times as critical process parameters (CPPs). High-performance liquid chromatography was employed to detect the contents of rutin, nuciferine, sinapine thiocyanate, liquiritin, and glycyrrhizic acid ammonium salt. The analytic hierarchy process-entropy weight method was used to comprehensively assign weights to the content of the five components including rutin and the extract yield. The Box-Behnken response surface method was employed to fit the mathematical model between CQAs and CPPs, screen the operational space of the extraction process, and validate it. RESULTS The optimal operational space of the extraction process was 9.5 to 12 times the amount of water added, the extraction time was 1 to 1.7 hours, and the extraction times were 3. Upon verification,the comprehensive scores of the optimal extraction process within the operational space were 89.79 and 84.93, respectively. CONCLUSIONS The extraction process of the Shenye buyi formula can be applied to the development and production of its granule preparations.

In recent decades,the incidence of human papillomavirus(HPV)-associated oropharyngeal squamous cell carcinoma(OPSCC)has shown a marked increase.Significant changes have also occurred in the OPSCC diagnosis and treatment paradigm.Deter-mining HPV status prior to treatment is now essential,and radiotherapy/chemotherapy,immunotherapy,and minimally invasive surgical techniques have progressively emerged as key modalities for managing OPSCC.However,alongside these paradigm shifts,a comprehen-sive technical consensus guiding the entire diagnostic and therapeutic process for OPSCC patients is currently lacking.Given China's large population base and the rising incidence of OPSCC,an expert panel convened to develop a clinical technical consensus on OPSCC diagno-sis and management tailored to China's specific context.This consensus aims to further enhance and standardize understanding of OPSCC management techniques among relevant healthcare professionals.
3.APIC risk management and application for hospital-acqueird infections in 'one hospital and multiple branches'

OBJECTIVE To explore the application of multiple hospital branches management mode in control of risk of hospital-acqueird infections in a general hospital so as to move forward the infection control threshold.METHODS The risk management closed loop,known as the infection index monitoring-risk assessment-risk re-sponse-effect evaluation-infection index monitoring,was adopted by People's Hospital of Ningxia Hui Autono-mous Region from Jan.to Jun.2024.The risk of infection at different levels of hospital zones,disciplines,clinical departments and risk indices was evaluated by risk matrix method.The correlation between the risk indices of clin-ical department and the risk scores was observed,and the dynamic monitoring was carried out for the change of risk of infection in the clinical departments.RESULTS The risk score was higher in the headquarter and Ningnan Branch than in the Xixia branch;the interquartile range(IQR)value from high to low was as follows:Xixia branch,hospital headquarter,Ningnan branch.The intensive care unit(ICU),nerve center and radiotherapy de-partment were the disciplines at extremely high risk of infection.The ICU,cardiac vascular surgery department and gastroenterology department of the hospital headquarters,the ICU,neurosurgery department and neurology department of Ningnan branch and the ICU and cardiac vascular surgery department of Xixia branch were the de-partments at extremely high risk of infection.The etiological submission rate before drug therapy,implementation of prevention measures for multidrug-resistant organisms(MDROs),cleaning and disinfection and missing report of hospital-acqueird infection cases were the major supervision indices.The absolute values of Spearman related co-efficient between clinical department risk scores and 4 indices-including the isolation rate of MDROs and the inci-dence of hospital-acqueird infection was greater than 0.5.CONCLUSION The risk management closed loop that is applied in the 'one hospital and multiple branches' medical institution may facilitate the dynamic monitoring,as-sessment and intervention the high-risk hospital branches,disciplines,departments and indices,and boost the ca-pability of risk management of infections in medical institutions.
4.Temporal-Spatial Parameters and Kinematic Characteristics of School-Aged Children with Torticollis Based on Three-Dimensional Gait Analysis

Objective To assess the motor function of school-aged children with congenital muscular torticollis(CMT),and analyze the spatiotemporal parameters of their walking motion as well as the kinematic parameters of the trunk and lower limbs,so as to explore the potential long-term impact of CMT on the growth and development of children.Methods Using three-dimensional(3D)gait analysis technology,the gait of children with CMT was evaluated in detail and compared with that of normal children.The motion angles and ranges of the trunk,pelvis,hip,knee,and ankle joints,along with spatio-temporal parameters,were analyzed.Thirty-one children with CMT aged 6-12 years were recruited into the torticollis group,and 31 normal children of the same age range were included in the control group.The data of the torticollis group were divided into the healthy side and the affected side based on the side of the torticollis.Statistical analysis was performed among the three groups to calculate the differences in kinematic and spatio-temporal parameters.Results Compared with nomal children of the same age in the control group,significant differences were observed in the affected and unaffected sides of the torticollis group in terms of stride length,gait speed,percentage of support phase time,average ankle inversion angle during the swing phase,average ankle inversion angle during the support phase,average foot orientation angle during the support phase,average ankle internal rotation angle relative to the tibia during the stance phase,average knee valgus angle during the stance phase,average shoulder joint elevation,and shoulder joint height at landing.Specifically,the torticollis group had significantly lower stride length,gait speed,ankle inversion angle,knee valgus angle,and foot eversion angle in both the affected and unaffected sides compared to the control group,while the percentage of support phase time and the internal rotation angle of the foot relative to the tibia during the stance phase were higher than those in the control group.Conclusions For school-aged children with CMT,there are still residual manifestations of overall motor development abnormalities.These abnormalities also affect the non-affected side.Children with CMT have a relatively high risk of developing uneven shoulders,their overall walking efficiency is lower than that of normal children of the same age,and they exhibit lower limb motor abnormalities such as insufficient ankle joint stability.
5.Msx2 regulates differentiation of outer enamel epithelial cells by modu-lating cytoskeleton and cell-cell interactions

AIM:To investigate the mechanism by which muscle segment homeobox 2(Msx2)regulates the differentiation of outer enamel epithelial cells in the enamel organ.METHODS:Tissue paraffin sections were prepared and subjected to hematoxylin-eosin(HE)staining to analyze the effect of Msx2 deficiency on the differentiation status of epithelial cells in the enamel organ at the morphological level.At the ultrastructural level,alterations in cell structure were analyzed.The intermediate steps mediating cell differentiation were identified.Transcriptome sequencing analysis was performed to validate the molecular mechanisms underlying the observed phenomena.RESULTS:Msx2 deficiency was innovatively found to induce severe squamous epithelial hyperplasia in outer enamel epithelial cells of enamel organ,accompanied by dynamic restructuring of the cell cytoskeleton and alterations in cell adhesion at the ultrastructure level.As a transcriptional repressor,the loss of Msx2 expression results in significant increases(P<0.05 or P<0.01)in the mRNA expression levels of integrin β2(Itgβ2),ItgαM,Itgα4,Rac family small GTPase 2(Rac2),Rac/Cdc42 guanine nucleo-tide exchange factor 6(Arhgef6)and protein tyrosine phosphatase receptor type C(Ptprc).CONCLUSION:Msx2 regu-lates cytoskeleton structure and cell-cell interaction through the Rho GTPases signaling pathway,thereby influencing the differentiation state of outer enamel epithelial cells.This study reveals the mechanism through which Msx2 regulates the differentiation of outer enamel epithelial cells,providing a theoretical foundation for the prevention and treatment of enam-el-related clinical dental diseases.

Through a comprehensive analysis of four cases involving repeated forensic clinical identification on the degree of human injury,disability grade,causal relationship of injury and disease,medical damage liability dispute,the reasons for the emergence of different appraisal opinions are explored.According to the reasons for the disputed conclusion,the deviation or error of the appraisal opinion can be classified in a variety of ways.Among them,the inherent limitations of theories and methods used as appraisal bases,the lack of operational skills or cognitive ability of appraisers,the unclear provisions of appraisal standards and the lack of adequacy and validity in the process of analysis and reasoning are the key reasons for the deviation or error of appraisal opinions.In light of these findings,it is necessary to strengthen the review of expert opinions in judicial practice and form a powerful error correction mechanism.

10.Effects of Generalised Joint Hypermobility on Walking Function in School-Age Children

Objective To investigate the gait characteristics of school-age children with generalised joint hypermobility(GJH).Methods The kinematic and kinetic data of lower extremity joints in 56 children with GJH and 56 healthy children were collected by 3D motion capture system and 3D force plates,and the differences between the two groups were compared and analyzed.Results Kinematics parameters:the mean ankle inversion angle(P=0.000),maximum hip adduction angle(P=0.002)and maximum foot clearance angle(P=0.004)in stance phase decreased in the GJH children group,while the mean foot progression angle(P=0.000),the maximum foot internal rotation angle(P=0.000)in stance phase,and the knee flexion angle(P=0.032)in the middle stance phase increased.Kinetics parameters:the maximum power of knee(P=0.005)and hip(P=0.000)joints and the maximum inversion moment of ankle joints(P=0.009)in stance phase decreased in GJH children group.Temporal and spatial parameters:the step length(P=0.001),walking velocity(P=0.000),and cadence(P=0.000)decreased in GJH children group,while the stance phase percentage(P=0.000),and step width(P=0.000)increased.Conclusions There is a decrease in walking efficiency and balance stability of children with GJH.The line alignment is abnormal in ankle joint.The knee function needs a long-term attention.
